The Lived Experience of the Burnout of Nurses Working in Oncology Wards (종양병동 간호사의 소진 체험)

  • Purpose: This study was done to understand the meanings and nature of experiences of the burnout of nurses working in oncology wards. Methods: The hermeneutic phenomenology developed by van-Manen was used. The period for data collection was from November, 2006 to May, 2007. This study took place in 3 university hospitals in B & U cities. Nine nurses working in oncology wards participated. Data collection was done through individual in-depth interviews. Results: The eight most common themes demonstrated by participants from this study were 'being fed up with endless demands', 'collapsing in front of deaths', 'gasping for breath due to overwhelming tasks', 'uncontrollable suffering of body', 'gradually loosing self-esteem', 'frustration from the work world', 'beleaguered relationships', and 'desire to escape'. Conclusion: The results of this study indicate that there is a need to develop a preventive burnout program for nurses working in oncology wards.

  • The stress is an unavoidable result and a complex phenomenon in modern society. too, Human activity changes under control of the stress. The stress is a reaction to adrenalin or other stress hormones. It provides the power for fighting and the energy for going away from dangers. Because of the stress, our bodies increase the pulse, the blood pressure and the breath for more blood and more oxygen. So, in this research, we are measuring those which was increased by the stress. Also we will examine the change of HRV and APG on music. After that, in order to reduce the stress, we will design matching system that is based on the sense for music by using Zigbee.

  • Today, many people are sitting on chair in their life. If a sitting posture is not correct, there is some trouble with the waist. And if it goes on long time, sometimes it cause the hurts of waist or the deformded spinal column. A crouched posture is an obstacle to breath and it give rise to drowsiness because the lack of oxygen. Sitting posture is a habit so people can't feel it oneself and look over some kind of risks. It analyzes a condition of muscles by measuring EMG of spinal both side of spin al-bones. It can have a right sitting posture by the analyzing that increasing of muscles tention in one or the other side of muscles when the posture inclines on one side or ahead.

  • We have developed the spirometer system; many patients having weak respiration have difficulties in using conventional spirometers in hospitals, because their weak breath can be affected by the errors of inertia and instrumental pressure. We have developed a new ultrasound spirometer using an ARM 920T processor. To detect weak respiratory signals, we add a comparator circuit, which can afford more information of respiration characteristics such as respiration volume, directions, and velocities. We have also developed Gill for graphical display of respiration characteristics. Through the pilot test, it has been verified that the developed spirometer is operating reliably.

A Comparative Study on the Effect of Smoking Cessation Education between CAI(Computer Assisted Instruction) and Lecture - Focused on Vocational High School Male Students - (CAI 개별 학습 프로그램을 적용한 금연 교육과 강의식 금연 교육의 효과 비교 - 실업계 남자 고등학생을 대상으로 -)

  • The purpose of this study was to compare the effect of education between CAI(Computer Assisted Instruction) and lectures for smoking cessation among male students who attended vocational high schools. Conducted from February 24th to April 26th, 2003, the study design was quasi-experimental with nonequivalent control group pretest-posttest design. The study subjects were 60 male students in K vocational high school in Daegu city, who were present smokers and had more than 7.0 ppm concentration level of carbon monoxide. Thirty students were randomly chosen as the experimental group which applied CAI education method for smoking cessation. The other 30 students served as the control group which received lecture education method of 40 minutes on four consecutive days. CAI education for smoking cessation was composed of ready-made individual learning contents, counseling by using cyber-communication, writing a letter to stop smoking, and writing a written agreement for smoking cessation. Lecture education for smoking cessation was composed of a ready-prepared lecture for the group, writing a letter to stop smoking, and writing a written agreement for smoking cessation. To measure smoking related knowledge, Jeong Ree Roh(1996)'s smoking related knowledge scale$(Cronbach's\;{\alpha}=0.84)$ was modified and used by the researcher. To measure smoking related attitude, Jeong Ree Roh(1996)'s smoking related attitude scale$(Cronbach's\;{\alpha}=0.91)$ was modified and used by the researcher. Smoking related knowledge scale's Cronbach's $\alpha$ was 0.83 in the pilot study and 0.93 in this study. Smoking related attitude scale's Cronbach's a was 0.80 in the pilot study and 0.98 in this study. To determine the smoking amount, the number of cigarettes smoked per day was checked. The concentration level of CO in the exhaled breath was measured (Micro CO Cat. No. MCO2, UK). Data was analyzed by $x^2-test$, t-test, repeated measures ANOVA. simple main effects, and time contrast test with SPSS/Win 11.0 program. The results of this study were as follows: 1. The first hypothesis. that 'Smoking-related knowledge score in the experimental group by using CAI education for smoking cessation will be higher than that in the control group by using lecture education for smoking cessation', was not supported. 2. The second hypothesis, that 'Smoking-related attitude in the experimental group by using CAI education for smoking cessation will be higher than that in the control group by using lecture education for smoking cessation'. was supported(F=6490.79. p=0.000). 3. The third hypothesis. that 'Smoking amount in the experimental group by using CAI education for smoking cessation will be less than that in the control group by using lecture education for smoking cessation'. was supported. 1) The third-1st sub-hypothesis. that 'The number of cigarettes smoked per day in the experimental group by using CAI education for smoking cessation will be less than that in the control group by using lecture education for smoking cessation'. was supported(F=134.19. p=0.000). 2) The third-2nd sub-hypothesis. that 'The concentration level of CO by ppm per one exhaled breath in the experimental group by using CAI education for smoking cessation will be lower than that in the control group by using lecture education for smoking cessation"' was supported(F=268.55. p=0.000). From the above results. CAI education can be an effective intervention to improve smoking-related knowledge and attitude. and to reduce the number of cigarettes smoked per day and the concentration level of CO by ppm per one exhaled breath. Lecture education can be effective to improve smoking-related knowledge. In the future, when CAI education and lecture education for smoking cessation are applied on the school nursing field. the students can gain a comprehensive understanding of smoking cessation, changes in smoking-related knowledge. smoking-related attitude and reducing smoking amount. Furthermore, CAI education for smoking cessation could be developed as an individual self initiative program and could give a guideline to apply CAI education for smoking cessation in other field.

A Case of Myasthenia Gravis Combined with Mediastinal Leiomyosarcoma and Stevens-Johnson Syndrome (종격 평활근육종 및 Stevens-Johnson증후군과 동반된 중증 근무력증 1예)

  • We report a case of 36-year-old woman with myasthenia gravis (MG) combined with mediastinal leiomyosarcoma (LMS) and Stevens-Johnson syndrome (SJS). She was admitted to ICU with the symptoms of acute onset headache, diplopia, ptosis, dysphagia, general weakness, and respiratory difficulty for several days. Physical examination revealed tachypnea, decreased breath sounds and dullness to percussion in right lower chest. Neurologic examination showed ptosis, diplopia, decreased gag reflexes, and generalized proximal weakness. Laboratory studies revealed increased serum acetylcholine receptor antibodies and positive Tensilon test. Chest CT showed a huge mass in the right middle mediastium but no evidence of thymic enlargement. Mediastinal LMS was diagnosed by ultrasound-guided needle biopsy. The myasthenic symptoms were fluctuated in spite og intravenous immunoglobulin, plasmapheresis, and corticosteroid. During therapy, SJS developed. She died 4 months after the onset of the myasthenic symptoms despite the chemotherapy for LMS.

Preliminary Study on Pattern Questionnaire for Damum Patterns (담음변증(痰飮辨證) 설간(設間) 개발(開發)을 위한 문헌연구(文獻硏究))

  • Objectives: In this study a pattern questionnaire for damum patterns was developed by means of literature review and statistical analysis. The individual approach of korean medicine is based on the concept of pattern identification, the objectivity and validity issues of which hold important meaning in the pratice and research. Methods: Review of literatures led to the selection of 22 items describing Dam or Um pattern. A preliminary questinnaire consisted of these items that may be scored with positive score at zero to seven. Results: Damum is all over the body syndromes. That is abnormal body conditions. Damun arouse body pain, breath disorder, digestion disorder, nerves disorder, excretion disorder. Conclusions: Review of literatures led to selection of 29 items describing Damun. A questionnaire is in preparation.

  • 일반적으로 심장자기공명영상을 이용하여 우관상동맥을 영상화 하기 위해서 3차원 데이터를 얻는다. 그렇게 하는 경우 데이터를 받는 시간이 오래 걸리고, 받은 3차원 데이터에서 차원 원하는 부분만을 2차원으로 재구성하는 작업을 해야 하는 번거러움이 있다. 반면에 호흡 정지상태에서 우관상동맥의 위치를 잘 선택하게 되면 한번 숨을 참을 수 있는 시간안에 원하는 영상을 얻을 수 있는 이점이 있다. 그렇게 하기 위해서는 우관상동맥을 찾아서 그 부분만 영상화해야 하는데, 본 논문에서는 한 영상에 같이 나타내기 어려운 대동맥에서 시작하여 심장 뒤편으로 돌아 들어가는 전체 우관상동맥을 쉽게 찾고, 한번 호흡을 참는 시간안에 영상화 하는 방법을 제안한다.

Measurements of fractional exhaled nitric oxide in pediatric asthma

  • Exhaled nitric oxide (NO) has been extensively investigated as a noninvasive marker of airway inflammation in asthma. The increased NO expression induced by inflammatory mediators in airways can be monitored easily in exhaled air from asthmatic children. Based on the relationship between the increased NO expression and eosinophilic airway inflammation, fractional exhaled nitric oxide (FeNO) measurements become an important adjunct for the evaluation of asthma. In addition, the availability of portable devices makes it possible to measure FeNO more easily and frequently in the routine pediatric practice. Despite various confounding factors affecting its levels, FeNO can be applicable in diagnosing asthma, monitoring treatment response, evaluating asthma control, and predicting asthma exacerbations. Thus, although pulmonary function tests are the standard tools for objective measurements of asthmatic control, FeNO can broaden the way of asthma monitoring and supplement standard clinical asthma care guidelines.
