The purpose of this study was to investigate the reason of interruption and the knowledge of breastfeeding. The samples of 117 mothers who have the babies between age from 1 month to 12 months were selected from one university hospital and one public health care center in J City. The data were collected with the questionnaire during the period from the January. 5. 2002 through January. 30. 2002. Analysis was done by using frequency. percentage. t-test. ANOVA. The results of this study can be summarized as follows. 1. The comparison of the method of feeding by general characteristics. income state(p= .019), rearing supporters(p=.026) and infant age(p=.03) have shown significant difference as the variables affecting breastfeeding practice rate. Breastfeeding rate were 24.8%. mixed- feeding type, 14.5%. milkfeeding type. 60.7%. 2. The major reason which breastfeeding be done was 'for health of baby'. In the period of breastfeeding, a large number of mothers answered' up to. 6 months'. The major reason of breasttfeeding interruption was 'insufficient quantity of breastmilk'. 3. The mean of knowledge on breastfeeding by feeding patterns were the highest in the breastfeeding group(F= 10.228, p= .000). 4. In a grade of each item on knowledge of breastfeeding, over 80% of mothers wrongly answered in two items; components comparison between breastmilk and cow's milk and when the mother fell sick. whether she have to be feed breastmilk. or not. In the conclusion. this study revealed that knowledge of breastfeeding promoted practice of breastfeeding. However we suggest that systemic and supportive breastfeeding program for promoting the knowledge and the practice rate of breastfeeding have to be provided by nurses.
Purpose: To investigate factors that may affect the method of feeding among preterm infants at 4 weeks after discharge. Methods: This study included 222 mother-infant dyads born before a gestational age of 37 weeks. The feeding method and general medical characteristics of the participants were assessed at 4 weeks after discharge using a structured questionnaire. Multinomial logistic regression analysis was used to examine which factors were associated with breastfeeding at home. Results: Of the 222 infants who qualified for the study, 71 (32.9%) continued to receive breastmilk at 4 weeks post-discharge. Multinomial logistic regression analysis showed that breastfeeding at 4 weeks post-discharge was associated with higher breastfeeding self-efficacy, vaginal delivery (experience), direct breastfeeding in the neonatal intensive care unit (NICU), gestational age between 30 and 34 weeks, and breastmilk consumption in the NICU. The following factors were associated with mixed feeding at 4 weeks post-discharge: being employed, having higher breastfeeding self-efficacy, and direct breastfeeding in the NICU. Conclusion: NICU nurses should provide opportunities for direct breastfeeding during hospitalization and support breastfeeding to enhance breastfeeding self-efficacy. These factors may help to ensure the continuation of breastfeeding after discharge. Moreover, factors that affect breastfeeding should be considered when providing interventions.
Omega-3 fatty acid, docosahexaenoic acid(DHA) is found in a high proportion in the structural lipids of cell membranes, in particular those of the central nervous system and the retina. Diet-induced changes in fatty acid composition in these tissues may affect physiochemical functions. This study was conducted to investigate whether supplements of DHA in infant formula has an effect on the composition of fatty acids in erythrocytes with regard to brain development. Experimental groups were breastmilk group(n=21), placebo formula group(n=15), and DHA supplemented formula (0.26%) group(n=16). Infants were selected by mothers who deliverecdd at Kyung Hee medical center from February to April, 1996. Infant body weight, length, and head circumference were similar among the experimental groups at 16 weeks of age. The levels of DHA in breastmilk, placebo formula, and DHA supplemented formula were 0.56, 0, and 0.26% of total fatty acids, respectively. There was a significant correlation between dietary DHA intake and erythrocyte DHA levels. The levels of arachidonic acid did not differ among the three expermental groups. The result of flash visual evoke potential(VEP) test was correlated with the erythrocyte levels and dietary DHA levels at 16 weeks of age. No other fatty acid was correlated with VEP test results. No differences were found in Bayley Mental and Psychomotor Development Index scores among the three groups at 20 weeks of age. DHA seems to be an essential nutrient for optimum growth and maturation of term infants. Relatively small amounts of dietary DHA supplementation significantly elevate DHA supplementation significantly elevate DHA content in erythrocytes, which in turn has an implication for better scores for infant's VEP test. Whether supplementation of formula-fed infants with DHA has long-term benefits remains to be elucidated.
BACKGROUND/OBJECTIVES: This study aimed to establish a mother and child cohort in the Chinese population, and investigate human breastmilk (HBM) composition and its relationship with maternal body mass index (BMI) and infant growth during the first 3 mon of life. SUBJECTS/METHODS: A total of 101 Chinese mother and infant pairs were included in this prospective cohort. Alterations in the milk macronutrients of Chinese mothers at 1 mon (T1), 2 mon (T2), and 3 mon (T3) lactation were analyzed. HBM fatty acid (FA) profiles were measured by gas chromatography (GC), and HBM proteomic profiling was achieved by matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry (MS). RESULTS: During the first 3 mon of lactation (P < 0.05), significant decreases were determined in the levels of total energy, fat, protein, and osteopontin (OPN), as well as ratios of long-chain saturated FA (including C16:0, C22:0 and C24:0), monounsaturated FA (including C16:1), and n-6 poly unsaturated FA (PUFA) (including C20:3n-6 and C20:4n-6, and n-6/n-3). Conversely, butyrate, C6:0 and n-3 PUFA C18:3n-3 (α-linolenic acid, ALA) were significantly increased during the first 3 mon (P < 0.05). HBM proteomic analyses distinguished compositional protein differences over time (P = 0.001). Personalized motherinfant analyses demonstrated that HBM from high BMI mothers presented increased total energy, fat, protein and OPN, and increased content of n-6 PUFA (including C18:3n-6, C20:3n-6 and n-6/n-3 ratio) as compared with low BMI mothers (P < 0.05). Furthermore, BMI of the mothers positively correlated with the head circumference (HC) of infants as well as the specific n-6 PUFA C20:3n-6 over the 3 time points examined. Infant HC was negatively associated with C18:0. CONCLUSION: This study provides additional evidence to the Chinese HBM database, and further knowledge of FA function. It also helps to establish future maternal strategies that support the healthy growth and development of Chinese infants.
Purpose: To provide basic informations for guidelines of Ginseng usage during lactation. Method: Based on a regulation of scope and preparation of herbal prescriptions by Ministry of health and welfare of Korea, we selected the formulae for postpartum care and disease in 7 Korean Medical Classics. And we searched the number of formulae including Ginseng, dosage and indications of Ginseng in formulae in those books. Results: The range of Ginseng dosage during lactation from medical classics is 1.5-18.75g/day for medical purposes. Indications of Ginseng are lethargy, excessive bleeding, asthma, fever, cold, pain, dizziness, mental disorder, spasm, digestive problem, constipation, diarrhea, urinary incontinence, edema, breast engorgement, lack of breastmilk, breast ulcer, etc. But Ginseng had not been used in the case of stroke, epistaxis, beginning of mastitis, tumor of lower abdomen. Conclusions: Ginseng should be recommended less than 1.5g/d as a nutritional supplement. If breastfeeding woman wants to take Ginseng as a medication or as a tonic, she should consult with a official specialist about proper prescription and dosage instead of risky random medication.
A survey was carried out to investigate the breastfeeding knowledge, breastfeeding rates ultra related factors among women in Seoul and the Chungbuk area, in order to obtain baseline data for the development of an education program on breastfeeding. Subjects included 671 lactating mothers who visited public health or pediatric centers, between December, 1999 and February, 2000 and were interviewed using a questionnaire. The results are as follows: In the breastfeeding knowledge, most of the subjects (93.4%) know correctly that 'breastfeeding is better for a baby's emotional development than formula feeding,'whereas only half of the subjects (51.7%) answered correctly the question on whether 'formula is more nutritious than breastmilk.'The breastfeeding rates were about 57%, 40%, 33% and 12%, respectively for baby's aged 1, 3, 6, and 12 months. These results showed that the breastfeeding rate rapidly decreased with the baby's age. Lower breastfeeding rates were associated with high educational level (college graduate and above), high income, mothers'employment, no experience of attending breastfeeding education programs, and no planning on breastfeeding prior to pregnancy. There was a significant relationship between the level of knowledge and breastfeeding practice; the higher the knowledge score, the higher the breastfeeding rate. In conclusion, an education program should be developed and offered for each low breastfeeding rate group, in order to promote breastfeeding.
Purpose: This study was to develop a breastfeeding promotion program and to test effects of the program on levels of breast discomfort, breast size, sodium in breast milk, and type of feeding in mothers with breast engorgement following cesarean birth. Methods: A non-synchronized non-equivalent control group pretest-posttest design was used in this study. The participants were 70 postpartum mothers who were admitted to a postpartum care center and experienced breast engorgement following cesarean birth. The planned nursing intervention was the breastfeeding promotion program consisting of breast massage and 1:1 breastfeeding education, counseling, and support focusing on individualized problem solving provided for 10 days. Fifty-three women completed the program (experimental group 26, control group, 27). Measurements were level of breast discomfort, breast size, sodium in breast milk and type of feeding at pre and posttest. Results: Women who participated in the program experienced lower scores for breast discomfort, greater decrease in breast size, lower levels of sodium in breast milk, and practiced breastfeeding more than those in the control group. Conclusion: Results indicate that this breastfeeding promotion program is effective in reducing breast engorgement and improving breastfeeding practices, and is therefore recommended to enhance breastfeeding promotion practice in postpartum care centers.
The purpose of this study is to investigate the effect of an educational program on parturient women's feeding behavior One hundred and twenty seven mothers who delivered in one general hospital in Seoul participated in the study, 66 mothers of whom were assigned to experimental group and 61, to control group. The data collection was done from April 20 to June 30 in 1997 and the educational program was applied to the experimental group on the third postpartum day and telephone calls were made on the fourth and the twelfth postpartum week to investigate feeding behavior after discharge. The collected data were analyzed by means of Chi-square test and descriptive statistics. The results are as follows : 1. Breast-feeding rate of the experimental group was higher than that of the control group($x^2$=6.578, p=.043). 2. Background variables affecting feeding behavior were age and feeding plan(age: $x^2$=8.660, p=.070, feeding plan : $x^2$=25.762, p=.000). 3. The insufficient milk supply was the main cause of discontinuing breast-feeding and others were mother's job, baby's jaundice and diarrhea, baby's refusal to suck breastmilk. In conclusion, the breast-feeding educational program which was applied in this study is effective in the promotion of breast-feeding.
Purpose: This study examined the effects of breastfeeding education among preterm mothers. Methods: A nonequivalent control group, repeated measure, quasi-experimental study design was used. The participants comprised 46 preterm mothers recruited from the neonatal intensive care unit of a university hospital who were divided into two groups: 23 in the control group and 22 in the experimental group. The breastfeeding education program intervention to promote preterm mothers' self-efficacy used was devised based on the self-efficacy theory. The intervention program consisted of individual breastfeeding education with demonstrations and discharge education, and telephone follow-up education within one week after being discharged. The participants were surveyed before and after the intervention, and four weeks after being discharged from the newborn intensive care unit. Results: The intervention group showed a significant increase in breastfeeding self-efficacy compared with the control group (F=6.92, p=.003) and showed a significant increase in their breastfeeding attitude score (F=3.49, p=.039). Four weeks after discharge, the breastmilk volume of the intervention group increased significantly (t=-2.87, p=.006), and the same group continued to breastfeed more compared with the control group (χ2=4.50, p=.049). Conclusion: This study suggests that the preterm breastfeeding program had a positive effect on breastfeeding self-efficacy, breastfeeding attitude, and extending the breastfeeding period among preterm mothers.
This study was carried out to investigate the future plan for infant feeding practice and its associated factors among university students so as to develop focus for nutritional education program for the promotion of breastfeeding. The subjects were 301 students (males 130, females 171). The results obtained were as follows. In this survey, the subjects had a basic knowledge of breastfeeding and lactation, especially of the ingredient of breastmilk. However they were not well informed about the physiology and method of breastfeeding and benefits of breastfeeding for mothers. The average scores far attitude toward breastfeeding and lactation were 3.09 points on a 4 point scale, which indicated a slightly positive attitudes toward breastfeeding. However the participants had a negative attitude toward its emotional and practical aspects. Their future plan for infant feeding was breastfeeding 40.3%, formula feeding 3.0% and mixed feeding 56.7%. A concern was that the students showed a high rate with regard to a mixed feeding plan, and Dey decided this based on an incorrect knowledge of breastfeeding. The group who planned to breastfeed in the future (BF) showed higher scores with respect to knowledge and attitudes than those who planned to formula feed or mixed feed (FF + MF). Therefore, to encourage mothers to breastfeed and support breastfeeding, breastfeeding nutritional education is urgently needed among university students. In addition, nutrition education programs should promote the benefits of breastfeeding for mothers and the physiology and practical method of breastfeeding. Also a practical and culturally sensitive course about food and nutrition is recommended to inform participants about breastfeeding. (Korean J Community Nutrition 8(2) : 240∼249, 2003)
