Associations of Demographic and Socioeconomic Factors with Stage at Diagnosis of Breast Cancer

  • Background: Stage at diagnosis is one of the most important prognostic factors of breast cancer survival. Because in the breast cancer case this may vary with socioeconomic characteristics, this study was performed to recognize the relationship between demographic and socioeconomic factors with stage at diagnosis in Iran. Materials and Methods: This cross-sectional, descriptive study conducted on 526 patients suffering from breast cancer and registered in Cancer Research Center of Shahid Beheshti University of Medical Sciences from 2008 to 2013. A reliable and valid questionnaire about family levels of socioeconomic status filled in by interviewing the patients via phone. For analyzing the data, Multinomial logistic regression, Kendal tau-b correlation coefficient and Contingency Coefficient tests were executed by SPSS22. Economic status, educational attainment of patient and household head and/or a combination of these were considered as parameters for socioeconomic status. First, the relationship between stage at diagnosis and demographic and socioeconomic status was assessed in univariate analysis then these relationships assessed in two different models of multinomial logistic regression. Results: The mean age of the patients was 48.3 (SD=11.4). According to the results of this study, there were significant relationships between stage at diagnosis of breast cancer with patient education (p=0.011), living place (p=0.044) and combined socioeconomic status (p=0.024). These relationships persisted in multiple multinomial logistic regressions. Other variables, however, had no significant correlation. Conclusions: Patient education, combined socioeconomic status and living place are important variables in stage at diagnosis of breast cancer in Iranian women. Interventions have to be applied with the aim of raising women's accessibility to diagnostic and medical facilities and also awareness in order to reducing delay in referring. In addition, covering breast cancer screening services by insurance is recommended.

남성 유방암 폐전이 환자의 폐절제술 후 한의기반 통합암치료로 증상 호전에 대한 증례보고 (A Case Report of Symptom Improvement after lobectomy in Male Breast Cancer with Lung Metastasis Patient Treated with Korean Medicine based Integrative Cancer Treatment)

  • Objective: The purpose of this study is to report improvement of symptoms after lobectomy of male breast cancer lung metastasis treated with Korean Medicine based Integrative Cancer Treatment (ICT). Methods: A male left breast cancer patient diagnosed with metastasis on lung at July 2019. After Video assisted thoracic surgery (VATS) left lower lobe (LLL) lobectomy and En bloc wedge resection the patient visited the Daejeon korean medicine hospital of Daejeon university East West Cancer Center (EWCC) to treat operation-site (op-site) pain, dysphagia, anorexia with Korean Medicine Treatment. The patient was treated with Korean Medicine based ICT for an approximately 20 days. The clinical outcomes were measured by National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Numeral rating scale (NRS) and Eastern Cooperative Oncology Group (ECOG). The safety of treatment was verified by blood tests. Results: After treatment, op-site pain was improved from NRS 9 to 6, dysphagia and anorexia were relieved from NRS 9 to 2. And ECOG score of the patient was improved from grade 2 to 1. Conclusion: This case study suggests that Korean Medicine based ICT may help to improve post operative sequelae in metastatic lung cancer patient.

뉴럴네트워크 기반의 유방암 조기 진단을 위한 분류 (Classification for early diagnosis for breast cancer base on Neural Network)

  • 유방암은 전체 여성의 암환자 중 두 번째로 많으며, 여성의 암으로 인한 사망 원인으로 가장 높은 것으로 나타났다. 유방암은 조기 발견 경우 완치율이 92%에 이른다. 하지만, 조기 발견을 하지 못할 경우 유방암은 전이율이 매우 높다. 암세포의 전이는 암의 진행이 많이 될수록 다른 장기로의 전이가 더욱 잘 되는 것으로 나타났다. 암의 조기 진단은 삶의 질을 높일 수 있는 중요한 요소이다. 유방암을 검사하는 방법으로는 맘모그래피(Mammography), 초음파, 맘모톰(momotome) 등이 있다. 그 중 맘모그래피는 검사자에게 통증이 적을 뿐 아니라, 쉽게 접근할 수 있어 유방암 검사에 유용하게 사용된다. 본 논문에서는 유방암 진단 데이터로 맘모그래프 데이터를 사용하였다. 본 논문에서는 뉴럴네트워크인 NEWFM(Neural network with weighted fuzzy membership function)를 사용하여 암 조기 진단을 위한 클래스를 분류하였다. NEWFM을 이용하여 데이터를 학습시킨 후 유방암 데이터 분류 결과 정확도가 84.4391%가 나타났다.

Applicability of Oncoplastic Breast Conserving Surgery in Asian Breast Cancer Patients

  • Background: There are limited studies on oncoplastic breast conserving surgery in Asian women. We aimed to determine the applicability and safety of oncoplastic surgery, highlighting the specific circumstances when it will be most useful and compare our preferred technique with the worldwide practice of oncoplastic approaches. Materials and Methods: Breast cancer patients who underwent oncoplastic breast conserving surgery at a single institution from 1st May 2014-31st March 2015 were included. Data on patient demographics, tumor characteristics and the type of oncoplastic surgery performed were collected. Results: Nineteen breast cancer patients were identified. 42.1% of patients had grade I ptosis. The indications for surgery included a large tumor to breast size ratio (52.6%), multifocal/multicentric lesions (36.8%) and asymmetric breasts (10.6%), averting a mastectomy in 89.4%. Round block was the commonest technique in 63% of patients, in contrast to the inverted T pattern most frequently used in renowned institutions in the West. Mean and median tumor size and weight of specimen were 29.4/25mm (11 - 75mm) and 77g/64g (10 - 246g) respectively. Re-excision rate was 10.5%. Complete mastectomy was performed for one patient. One patient developed wound dehiscence which was treated conservatively. Cosmetic outcome was rated as excellent/good by 94.7% of patients. The patients remained clinically well after a median 16 months follow up. Conclusions: Oncoplastic breast conserving surgery is useful in a specific subgroup of Asian patients with a relatively small breast volume and minimal ptosis. Round block was the commonest technique in our series, in contrast to the worldwide utility of oncoplastic techniques. It is oncologically safe and has good cosmetic outcomes.

Delayed Presentation of Self-discovered Breast Cancer Symptoms in Iranian Women: A Qualitative Study

  • Background: Delayed presentation of symptomatic breast cancer is a public health issue in Iran, making a major contribution to low survival. Despite the importance of this problem, current knowledge is insufficient to inform interventions to shorten patient delay. The aim of this study was to explore factors influencing patient delay in Iranian women with self-discovered breast cancer symptom. Materials and Methods: This qualitative study was conducted during 2012-2013. Purposeful sampling was used to recruit 20 Iranian women with self-discovered symptoms of breast cancer who attended the Cancer Institute of Tehran University of Medical Sciences, Tehran, Iran. Data were collected through semi-structured in-depth audiotaped interviews, which were transcribed and analyzed using conventional content analysis with MAXqda software version 10. Findings: Content analysis of the data revealed four main themes related to the delay in seeking medical help including: 1) attributing symptoms to the benign conditions; 2) conditional health behavior; 3) inhibiting emotional expression; and 4) barriers to access to health care systems. Conclusions: These results suggest that patient delay is influenced by complex and multiple factors. Effective intervention to reduce patient delay for breast cancer should be developed by focusing on improvement of women's medical knowledge, managing patients' emotional expression and reform of the referral system.

자음강화탕 투여 후 상열감의 호전을 보인 타목시펜 복용 중인 유방암 환자 1례 (Case Study of a Breast Cancer Patient Accompanying with Hot Flush by Tamoxifen Whose Condition Was Improved by Jayeumganghwa-tang)

  • Objective : To observe the effectiveness of Jayeumganghwa-tang on hot flush of a breast cancer patient who takes tamoxifen. Methods : For six weeks a dosage witch contained 13.92g of Jayeumganghwa-tang was administered 3 times (30 mins after meal) per day, for improvement of hot flush. No other therapies like hormone therapy or substituted therapy were done. Results : As a result of 6 weeks of Jayeumganghwa-tang therapy, the patient who suffered hot flush on taking tamoxifen showed improvement on several indices, including frequency, intensity and point of hot flush, Beck Depression Inventory-K (BDI-K) which shows uneasiness and sadness, and also Functional Assessment Cancer Therapy - Breast (FACT-B) which is an index of life quality of breast cancer patients. But no changes on female hormones like follicle-stimulating hormone (FSH) or luteinizing hormone (LH) were observed. Conclusion : This case demonstrates 6 weeks of Jayeumganghwa-tang taking had no effect on female hormones but can reduce the hot flush by tamoxifen. This shows minimum evidence of safety and efficacy of Jayeumganghwa-tang on hot flush of breast cancer patient. However this is a single case study so further case-series research should be compiled.

유방암 환자의 통함 암 치료를 통한 항암화학요법 유발 말초신경병증, 전신통 호전에 대한 증례 보고 (A Case Report on Improvement of Chemotherapy-Induced Peripheral Neuropathy and Pantalgia Side Effects with Integrated Cancer Treatment in a Breast Cancer Patient)

  • Objectives: To report the improvement of chemotherapy-induced peripheral neuropathy and pantalgia with integrative cancer treatment on adverse effects of chemotherapy in a breast cancer patient. Methods: A 63-year-old female patient who has been diagnosed with breast cancer got treated for 103 days with integrative cancer treatment including acupuncture, moxibustion, herbal medicine, physiotherapies, hand and foot bath to decrease side effects of chemotherapy. The patient was also treated Western immunotherapies like Thymosin, Viscum album. Paclitaxel, Carboplatin, Doxorubicin, Cyclophosphamide was applied and chemotherapy-induced peripheral neuropathy(CIPN), pantalgia and nausea occured. The efficacy of treatment was measured by a numeric rating scale(NRS) of symptoms, National Cancer Institute Common Terminology Criteria for Adverse Event(NCI-CTCAE) and Eastern Cooperative Oncology Group(ECOG) Performance Status Scale. Results: The NRS scroes for CIPN, pantalgia, nausea were improved. There was no adverse effects of 3 or higher assessed by the NCI-CTCAE. The ECOG grade improved from grade 2 to 1. Conclusions: This study suggests that integrative cancer treatment could improve CIPN, pantalgia after chemotherapy in breast cancer.

Association Between Alterations in the Serum 25-Hydroxyvitamin D Status During Follow-Up and Breast Cancer Patient Prognosis

  • Background: Serum vitamin D status can affect the prognosis of breast cancer patients. Our aim was to determine the association between alterations in the 25-hydroxyvitamin D [25(OH)D] status during follow-up and the prognosis of breast cancer patients. Additionally, we evaluated the association between the 25(OH)D status at the time of diagnosis and the prognosis using a detailed age and stage categorization. Materials and Methods: Four hundred and sixty-nine Korean breast cancer patients were included. We collected patient clinicopathological data, including their serum 25(OH)D concentration at diagnosis and at the annual follow-up until 4 years after diagnosis. The patients were divided according to their 25(OH)D status at diagnosis into a deficient (<20 ng/ml) and a non-deficient (${\geq}20ng/ml$) group. At follow-up, patients were categorized into the four following groups according to 25(OH)D status alterations: persistently deficient, improved, deteriorated and persistently non-deficient. Results: At diagnosis, 118 patients were classified into the deficient group and 351 into the non-deficient group. After a median follow-up period of $85.8{\pm}31.0$ months, the patients with advanced-stage disease or an older age in the non-deficient group showed a significantly better survival compared with the deficient group. Furthermore, at the 1-year follow-up of 25(OH)D status, the persistently non-deficient group and the improved group had better survival compared with the other two groups. Conclusions: Our results suggest that maintaining an optimal 25(OH)D status at diagnosis and during the 1-year follow-up period is important for improving breast cancer patient survival.

유방암 여성의 유방보존술 후 경험에 대한 임상 간호사의 인지 (Clinical Nurses' Interpretation of the Experiences of Breast Cancer Women after Breast Conservative Operation)

  • Purpose: Even though the breast is not removed, women with a breast conservative operation(BCO) are suffering from the psychosocial distress comparable with mastectomies. The purpose of this study was to identify how clinical nurses view essential nursing problems and nursing approaches to post operative breast cancer experiences. Method: To identify the clinical nurses's interpretation, they were asked to respond to a case involving a woman struggling with breast cancer. The nurses' responses were analyzed using a manifest content analysis strategy to determine themes in problem identification and nursing care strategies. Result: The clinical nurses recognized that most of the patient's experiences originated from a psycho-social problem. Fifteen nurses(83%) interpreted the breast cancer woman's experience derived from her psychosocial problem. They identified the most effective nursing approaches for breast cancer women following surgery as counseling and providing information. Conclusion: In conclusion, the results revealed that a psychosocial skill including an empathic communication skill is essential for excellent nursing in post operative patient care.

유방암 여성이 인식한 환자중심 간호의 질 (Perceptions of Quality of Patient-Centered Nursing Care among Women with Breast Cancer)

  • Purpose: This study aimed to investigate the quality of patient-centered nursing care (PCNC) among women with breast cancer at a cancer center in Seoul, Korea. Methods: In a cross-sectional survey design, 223 women with breast cancer were recruited from the oncology surgery unit. The Korean version of the oncology patients' perceptions of the quality of nursing care scale, which is conceptualized in four sub-dimensions (individualization, proficiency, responsiveness, and coordination) was used for measurement. Data were analyzed using descriptive statistics and ANOVA. Results: The participants were all women, with a mean age of 51.3 years. The mean score of PCNC was high and significantly different from each other according to age group. Breast cancer women, who had mastectomy, were satisfied highly in terms of proficiency and responsiveness care, but less satisfied with individualization and coordination care than those of other women. Conclusion: The findings of this study show the quality of cancer nursing care, especially focusing on patient-centeredness, can be measured from the patients'perspective. The individualized and coordinated nursing care is considered to be the core of quality cancer care implying patient-centeredness. Based on the findings of this study, more research is necessary to explore the patients' view of quality cancer care and to test the effects of PCNC within the context of comparative effectiveness.
