Intracranial Metastases of Cervical Intramedullary Low-Grade Astrocytoma without Malignant Transformation in Adult

  • The first case of intracranial metastases of a cervical intramedullary low-grade astrocytoma without malignant transformation in adult is presented in this report. Seven years ago, a 45 year-old male patient underwent biopsy to confirm pathologic characteristics and received craniocervical radiation and chemotherapy for a grade II astrocytoma in the cervical spinal cord. Two years later, posterior fusion was necessary for progressive kyphosis in the cervical spine. He was well for approximately 7 years after the primary surgery. Two months ago, he presented with partial weakness and incoordination with gait difficulty. MRI Scan demonstrated multiple small lesions in the cerebellar vermis and left hemisphere. After suboccipital craniectomy and posterior cervical exposure, the small masses in the cerebellar vermis and hemispheres were excised to a large extent by guidance of an intraoperative navigation system. The tumor at the cervical and brain lesions was classified as an astrocytoma (WHO grade II). When a patient with low-grade astrocytoma in the spinal cord has new cranial symptoms after surgery, radiaton, and chemotherapy, the possibility of its metastasis should be suspected because it can spread to the intracranial cavity even without malignant transformation as shown in this case.

방사선 재치료를 시행한 뇌전이환자의 결과 분석 (Outcomes after Reirradiation for Brain Metastases)

  • 방사선 재치료를 받은 뇌전이 환자를 대상으로 치료결과와 결과에 영향을 미치는 인자를 알아보고자 하였다. 2008년 6월부터 2012년 12월까지 완화목적의 뇌 방사선치료를 받은후 방사선 재치료를 받은 23명의 환자의 결과를 분석하였다. 첫 번째 전뇌 방사선치료 선량의 중앙값은 30 Gy (23.4~30 Gy) 이었다. 삼차원입체치료의 선량은 하루에 3 Gy로 총 30 Gy를 분할조사하였다. 정위적 방사선수술의 선량은 16 Gy를 1회 조사하였다. 방사선 재치료에서 전뇌 방사선치료, 삼차원 입체조형 치료의 중앙값은 각각 27.5 Gy와 30 Gy 이었다. 정위적 방사선수술의 재치료 선량은 16 Gy를 1회 조사하였다. 완화목적의 뇌 방사선치료 이후 뇌전이로 인한 증상 악화를 동반하고 영상학적으로 병변 진행이 확인되는 환자에서 방사선 재치료를 시행하였다. 경과 관찰기간의 중앙값은 25개월 이었다. 증상 관해율은 47.8%였고, 증상 완화의 효과 비율은 증상 유지 환자를 포함하여 82.6% 이었다. 재치료후 생존기간의 중앙값은 3.2개월이었다. 방사선 재치료 이후의 중앙값은 60으로 재치료 이전의 KPS 중앙값 30에 비해 향상되었다. 뇌전이 환자에서 방사선 재치료의 증상 완화 효과는 치료이전 KPS값이 60이상(: ECOG 0~2)의 전신수행상태가 좋은 환자에서 적합한 것으로 결과를 나타내었다.

Immune Checkpoint Inhibitors for Non-Small-Cell Lung Cancer with Brain Metastasis : The Role of Gamma Knife Radiosurgery

  • Objective : Immune checkpoint inhibitors (ICIs) are approved for treating non-small-cell lung cancer (NSCLC); however, the safety and efficacy of combined ICI and Gamma Knife radiosurgery (GKS) treatment remain undefined. In this study, we retrospectively analyzed patients treated with ICIs with or without GKS at our institute to manage patients with brain metastases from NSCLC. Methods : We retrospectively reviewed medical records of patients with brain metastases from NSCLC treated with ICIs between January 2015 and December 2017. Of 134 patients, 77 were assessable for brain responses and categorized into three groups as follows : group A, ICI alone (n=26); group B, ICI with concurrent GKS within 14 days (n=24); and group C, ICI with non-concurrent GKS (n=27). Results : The median follow-up duration after brain metastasis diagnosis was 19.1 months (range, 1-77). At the last follow-up, 53 patients (68.8%) died, 20 were alive, and four were lost to follow-up. The estimated median overall survival (OS) of all patients from the date of brain metastasis diagnosis was 20.0 months (95% confidence interval, 12.5-27.7) (10.0, 22.5, and 42.1 months in groups A, B, and C, respectively). The OS was shorter in group A than in group C (p=0.001). The intracranial disease progression-free survival (p=0.569), local progression-free survival (p=0.457), and complication rates did not significantly differ among the groups. Twelve patients showed leptomeningeal seeding (LMS) during follow-up. The 1-year LMS-free rate in treated with ICI alone group (69.1%) was significantly lower than that in treated with GKS before ICI treatment or within 14 days group (93.2%) (p=0.004). Conclusion : GKS with ICI showed no favorable OS outcome in treating brain metastasis from NSCLC. However, GKS with ICI did not increase the risk of complications. Furthermore, compared with ICI alone, GKS with ICI may be associated with a reduced incidence of LMS. Further understanding of the mechanism, which remains unknown, may help improve the quality of life of patients with brain metastasis.

  • Angiosarcoma of the brain, either primary or metastatic is extremely rare. Moreover, angiosarcoma metastazing to the brain is also highly unlike to occur when comparing with metastases to the other organs. Thus, an ideal treatment strategy has not been established. A 67-year-old man with past surgical history of a scalp angiosarcoma underwent surgical resection of intracranial invasion. Because of wide scalp flap excision and resultant poor vascularity of the scalp flap, additional radiation was not provided. Because adjuvant therapy is impossible due to poor scalp condition, more careful but ample resection of the primary lesion is essential to conduct initial operation.

두경부에 다중전이된 신세포암 1례 (A Case of Multiple Head and Neck Metastases of Renal Cell Carcinoma)

  • Renal cell carcinoma is the third most common metastatic tumor to the bone and soft tissues of the head and neck. The common sites of metastatic renal cell carcinoma in head and neck region are nasal cavity, paranasal sinuses, oral mucosa, gingiva, tongue, palate, lip as the favored site. The present paper deals with one patient with metachronous oral tongue, nasal cavity and suspicious brain metastases after 2 years of renal cell carcinoma nephrectomy. Also, the patient had history of total thyroidectomy for thyroid follicular carcinoma. Total excision of nasal cavity and tongue mass were performed. Therapeutic aspects are briefly reviewed in literature.

  • Objective: A Case Study of a Patient with Cancer Fatigue Syndrome and Cancer Metastases who Received Ortho-Cellular Nutrition Therapy (OCNT) Methods: The patient is a Korean woman aged 61 years. 13 years ago, she was diagnosed with stage 1 breast cancer and underwent surgery, chemotherapy, and radiation therapy. owever, 13 years later, the disease spread to the lungs and brain. Results: Following nutritional therapy, cancer fatigue syndrome improved. Conclusion: Nutritional therapy can be beneficial for cancer fatigue syndrome, a condition that remains undiagnosed and untreated.

척수내 전이암 - 2례 보고 및 문헌고찰 - (Intramedullary Spinal Cord Metastasis : A Report of Two Cases and a Review of the Literature)

  • 척수내 전이암은 암환자에서 매우 드문 합병증으로 증상이 있는 척수 전이암 중 $3.4\%$정도를 차지한다고 보고된 바 있다. 척수내 전이암 환자의 생존 기간은 매우 짧아서 2개월 이내인 경우가 대부분인데 그 이유는 신경학적으로 빨리 악화되고 뇌전이를 포함하여 여러 곳에 전이암이 동반되기 때문이다. 국내 문헌상 척수내 전이암에 대한 보고가 거의 없고 저자들이 경험한 두 증례는 서로 다른 임상 경과를 보였기에 문헌 고찰과 함께 보고하는 바이다. 원발암이 접형동암인 환자에서 척수내 전이암으로 인해 이차성 척수공동증을 동반한 증례는 세계 문헌상 세번째 증례이다. 한 증례는 증상이 서서히 진행되었고 척수에 방사선치료를 시행한 후 생존기간이 26개월이었고, 다른 증례는 증상이 매우 빠르게 진행되었고 생존기간이 훨씬 짧았다. 척수내 전이암도 신경학적 증상이 가역적인 상태에서 조기 진단이 내려진다면 더욱 효과적인 증상완화를 기대할 수도 있다.

Survival of Brain Metastatic Patients in Yazd, Iran

  • Background: Brain metastasis occurs when cancerous cells come from a known (or sometimes an unknown) primary tumor to the brain and implant and grow there. This event is potentially lethal and causes neurologic symptoms and signs. These patients are treated in order to decrease their neurologic problems, increase quality of life and overall survival. Materials and Methods: In this study we evaluated clinical characteristics of 206 patients with brain metastases referred to our center from 2004 to 2011. Results: The mean age was 53.6 years. The primary tumors were breast cancer (32%), lung cancer (24.8%), lymphoma (4.4%), sarcoma (3.9%), melanoma (2.9%), colorectal cancer (2.4%) and renal cell carcinoma (1.5%). In 16.5% of the patients, brain metastasis was the first presenting symptom and the primary site was unknown. Forty two (20.4%) patients had a single brain metastasis, 18 patients (8.7%) had two or three lesions, 87 (42.2%) patients had more than three lesions. Leptomeningeal involvement was seen in 49 (23.8%) patients. Thirty five (17%) had undergone surgical resection. Whole brain radiation therapy was performed for all of the patients. Overall survival was 10.1 months (95%CI; 8.65-11.63). One and two year survival was 27% and 12% respectively. Conclusions: Overall survival of patients who were treated by combination of surgery and whole brain radiation therapy was significantly better than those who were treated with whole brain radiation therapy only [13.8 vs 9.3 months (p=0.03)]. Age, sex, primary site and the number of brain lesions did not show significant relationships with overall survival.

Cisplatin Plus Gemcitabine for Treatment of Breast Cancer Patients with Brain Metastases: a Preferential Option for Triple Negative Patients?

  • Background: To assess the efficacy and tolerability of Cisplatin plus Gemcitabine combination in patients with brain metastases (BM) from breast cancer (BC). Materials and Methods: Eighteen BC patients with BM who were treated with Cisplatin plus Gemcitabine regimen between 2003-2011 were evaluated. Results: A median of 6 cycles of this regimen were received, in fifteen patients (83.3%) as first-line chemotherapy, in 2 as second-line and in 1 as third-line after diagnosis of BM. Dose reduction was performed in 11 (61.1%) patients; major reasons were neutropenia and leukopenia. Grade III neutropenia and Grade II trombocytopenia rates were 33.3% and 16.7% respectively. Overall response rate (ORR; complete+partial response rate) was 33.4% (n=6) for the entire study population; triple negative patients achieved an 66.6% ORR while hormone receptor (HR) positive patients had 25% and HER2 positive patients 12.5%. Median progression-free survival was 5.6 months (2.4-8.8 months, 95%CI) and longer in patients with triple negative breast cancer (TNBC) (median 7.4 months, 95%CI, 2.4-12.3 months) than the patients with other subtypes (median 5 months for HER2 positive and 3.6 months for HR positive patients). Median PFS of the patients with TNBC who received this regimen as first-line was 9.2 months (5.2-13.2 months, 95%CI). Conclusions: Cisplatin plus Gemcitabine may be a treatment option for patients with BM from breast cancer. Longer PFS and higher response rates are results that support the usage of this regimen especially for the triple negative subtype. However, further prospective and randomized trials are clearly required to provide more exact information.

A Systematic Review of Risk Factors for Brain Metastases and Value of Prophylactic Cranial Irradiation in Non-Small Cell Lung Cancer

  • Background: The incidence of brain metastases (BM) varies in patients with non-small cell lung cancer (NSCLC), calls into question the value of prophylactic cranial irradiation (PCI). It is possible that clinicopathologic characteristics are associated with the development of BM, but these have yet to be identified in detail. Thus, we conducted the present meta-analysis on risk factors for BM and the value of PCI in patients with NSCLC. Methods: Eligible data were extracted and the risk factors for BM and the value of PCI in patients with NSCLC were analyzed by calculating the pooled odds ratio (OR). Heterogeneity was detected using Q and I-squared statistics, and publication bias was tested by funnel plots and Egger's test. Results: Six randomized controlled trials with a focus on the value of PCI and 13 eligible studies with a focus on risk factors for BM were included. PCI significantly reduced the incidence of BM in patients with NSCLC (p=0.000, pooled OR=0.34, 95% confidence interval = 0.37-0.59). Compared with non-squamous cell carcinoma, squamous cell carcinoma was associated with a low incidence of BM in patients with NSCLC (p=0.000, pooled OR=0.47, 95% confidence interval =0.34-0.65). The funnel plot and Egger's test suggested that there was no publication bias in the current meta-analysis. Conclusions: This meta-analysis provides statistical evidence that compared with non-squamous cell carcinoma, squamous cell carcinoma can be used as a predictor for BM in patients with NSCLC, and PCI might reduce the incidence of BM in patients with NSCLC, but does not provide a survival benefit.
