고형암의 뇌전이시 방사선치료 효과 (The Role of Radiotherapy in Patients with Brain Metastasis)

  • 배경 및 목적 : 고형암의 뇌전이는 20$\~$40$\%$의 암환자에서 발생하고, 뇌전이 후 1년 생존율은 15$\%$정도로 예후가 불량하며 고식적 방사선치료 등에 의하여 70$\~$90$\%$의 환자에서 증상이 호전되나 대부분의 환자가 결국은 뇌전이로 인하여 사망하게 된다. 이에 고형암의 뇌전이로 방사선치료를 받은 환자를 후향적으로 조사하여 임상양상과 생존기간을 분석하고 예후인자를 파악하였다. 방 법 : 대상환자는 1987년 1월부터 1998년 1월까지 이화여자대학교 부속병원에서 고형암의 뇌전이로 고식적 전뇌 방사선치료를 받은 71예이었다 대상 환자의 중앙연령은 63세(24$\~$89세)였고 남자 50예, 여자 21예이었다. 고식적 치료 후 신경증상의 호전정도를 평가하고, 연령, 종양의 원발병소, 진단당시 뇌전이 동반 유무, 타장기 전이 유무, 뇌전이 병변 수, 치료방법에 따라 생존기간을 비교하였다. 결 과 : 뇌전이시 동반된 증상은 두통이 34예로 가장 많았으며 근력 약화 29예, 오심 구토 및 의식 변화 각 14예, 시력장애 8예, 구음장애 7예 순이었고 치료 후 신경증상의 호전은 64.9$\%$에서 관찰되었다. 뇌전이의 치료로 전체환자 중 7예에서는 뇌전이의 수술적 제거 후 전뇌 방사선치료를 받았고 나머지 64예는 전뇌 방사선치료만을 받았다. 전체환자의 중앙생존기간은 16주였고 1년 생존율은 IS.0%, 2년 생존율은 5.1$\%$였다. 타장기 전이가 없이 뇌전이 단독인 군(n=27)의 중앙생존기간은 33주로 타장기 전이가 있는 군의 10주에 비해 길었다(p=0.0018). 뇌전이 수가 단일 병소인 37예의 환자에서는 수술적 제거 후 방사선치료를 한 군(n=7)의 중앙생존기간이 40주로 방사선치료만 시행한 군의 16주에 비해 길었다(p=0.0438). 이 외에 연령, 원발암의 종류, 전신수행 상태, 진단당시 뇌전이 동반 유무, 뇌전이 병변 수에 따른 생존기간의 차이는 없었다. 결 론 : 고형암에서 뇌전이는 나쁜 예후인자이나 뇌로의 단독전이는 타장기 전이 동반에 비해 예후가 양호하므로 수술적 절제, 방사선치료 등의 다양한 방법을 이용한 보다 적극적인 치료로 생존율 및 삶의 질을 향상시킬 수 있을 것이다.

Clinical application of RapidArc volumetric modulated arc therapy as a component in whole brain radiation therapy for poor prognostic, four or more multiple brain metastases

  • Purpose: To determine feasibility of RapidArc in sequential or simultaneous integrated tumor boost in whole brain radiation therapy (WBRT) for poor prognostic patients with four or more brain metastases. Materials and Methods: Nine patients with multiple (${\geq}4$) brain metastases were analyzed. Three patients were classified as class II in recursive partitioning analysis and 6 were class III. The class III patients presented with hemiparesis, cognitive deficit, or apraxia. The ratio of tumor to whole brain volume was 0.8-7.9%. Six patients received 2-dimensional bilateral WBRT, (30 Gy/10-12 fractions), followed by sequential RapidArc tumor boost (15-30 Gy/4-10 fractions). Three patients received RapidArc WBRT with simultaneous integrated boost to tumors (48-50 Gy) in 10-20 fractions. Results: The median biologically effective dose to metastatic tumors was 68.1 $Gy_{10}$ and 67.2 $Gy_{10}$ and the median brain volume irradiated more than 100 $Gy_3$ were 1.9% (24 $cm^3$) and 0.8% (13 $cm^3$) for each group. With less than 3 minutes of treatment time, RapidArc was easily applied to the patients with poor performance status. The follow-up period was 0.3-16.5 months. Tumor responses among the 6 patients who underwent follow-up magnetic resonance imaging were partial and stable in 3 and 3, respectively. Overall survival at 6 and 12 months were 66.7% and 41.7%, respectively. The local progression-free survival at 6 and 12 months were 100% and 62.5%, respectively. Conclusion: RapidArc as a component in whole brain radiation therapy for poor prognostic, multiple brain metastases is an effective and safe modality with easy application.

Evaluation of response to stereotactic radiosurgery in patients with radioresistant brain metastases

  • Purpose: Renal cell carcinoma (RCC) and melanoma have been considered 'radioresistant' due to the fact that they do not respond to conventionally fractionated radiation therapy. Stereotactic radiosurgery (SRS) provides high-dose radiation to a defined target volume and a limited number of studies have suggested the potential effectiveness of SRS in radioresistant histologies. We sought to determine the effectiveness of SRS for the treatment of patients with radioresistant brain metastases. Materials and Methods: We performed a retrospective review of our institutional database to identify patients with RCC or melanoma brain metastases treated with SRS. Treatment response were determined in accordance with the Response Evaluation Criteria in Solid Tumors. Results: We identified 53 radioresistant brain metastases (28% RCC and 72% melanoma) treated in 18 patients. The mean target volume and coverage was 6.2 ± 9.5 mL and 95.5% ± 2.9%, respectively. The mean prescription dose was 20 ± 4.9 Gy. Forty lesions (75%) demonstrated a complete/partial response and 13 lesions (24%) with progressive/stable disease. Smaller target volume (p < 0.001), larger SRS dose (p < 0.001), and coverage (p = 0.008) were found to be positive predictors of complete response to SRS. Conclusion: SRS is an effective management option with up to 75% response rate for radioresistant brain metastases. Tumor volume and radiation dose are predictors of response and can be used to guide the decision-making for patients with radioresistant brain metastases.

Radiosurgery for Recurrent Brain Metastases after Whole-Brain Radiotherapy : Factors Affecting Radiation-Induced Neurological Dysfunction

  • Objective : We retrospectively analyzed survival, local control rate, and incidence of radiation toxicities after radiosurgery for recurrent metastatic brain lesions whose initial metastases were treated with whole-brain radiotherapy. Various radiotherapeutical indices were examined to suggest predictors of radiation-related neurological dysfunction. Methods : In 46 patients, total 100 of recurrent metastases (mean 2.2, ranged 1-10) were treated by CyberKnife radiosurgery at average dose of 23.1 Gy in 1 to 3 fractions. The median prior radiation dose was 32.7 Gy, the median time since radiation was 5.0 months, and the mean tumor volume was $12.4cm^3$. Side effects were expressed in terms of radiation therapy oncology group (RTOG) neurotoxicity criteria. Results : Mass reduction was observed in 30 patients (65%) on MRI. After the salvage treatment, one-year progression-free survival rate was 57% and median survival was 10 months. Age(<60 years) and tumor volume affected survival rate(p=0.03, each). Acute (${\leq}$1 month) toxicity was observed in 22% of patients, subacute and chronic (>6 months) toxicity occurred in 21 %, respectively. Less acute toxicity was observed with small tumors (<$10cm^3$. p=0.03), and less chronic toxicity occurred at lower cumulative doses (<100 Gy, p=0.004). "Radiation toxicity factor" (cumulative dose times tumor volume of <1,000 Gy${\times}cm^3$) was a significant predictor of both acute and chronic CNS toxicities. Conclusion: Salvage CyberKnife radiosurgery is effective for recurrent brain metastases in previously irradiated patients, but careful evaluation is advised in patients with large tumors and high cumulative radiation doses to avoid toxicity.

  • Objectives The aim of this study was to report the improvement of delirium and performance status in the small-cell lung cancer patient who had multiple brain metastases and pericardial effusion after Sasang constitutional treatment. Methods We retrospectively reviewed the medical records, medical laboratory and image scans of 71-year-old male patient diagnosed as small-cell lung cancer. Results The small-cell lung cancer with multiple brain metastases patient sometimes talked deliriously even after the whole brain radiation therapy. During the hospitalization period, he showed delirium. We treated him with Gihwangbaekho-tang and Dojeokgangki-tang as a main therapy. After treatment, he didn't show delirium and performance status was improved. Conclusions A small-cell lung cancer with multiple brain metastases patient showed the improvement of symptoms (delirium, poor performance status, constipation and poor oral intake) with the treatment of Gihwangbaekho-tang, Yanggyuksanhwa-tang and Dojeokgangki-tang.

Brain metastasis in human epidermal growth factor receptor 2-positive breast cancer: from biology to treatment

  • Overexpression of human epidermal growth factor receptor 2 (HER2) is found in about 20% of breast cancer patients. With treatment using trastuzumab, an anti-HER2 monoclonal antibody, systemic control is improved. Nonetheless, the incidence of brain metastasis does not be improved, rather seems to be increased in HER2-positive breast cancer. The mainstay treatment for brain metastases is radiotherapy. According to the number of metastatic lesions and performance status of patients, radiosurgery or whole brain radiotherapy can be performed. The concurrent use of a radiosensitizer further improves intracranial control. Due to its large molecular weight, trastuzumab has a limited ability to cross the blood-brain barrier. However, small tyrosine kinase inhibitors such as lapatinib, has been noted to be a promising agent that can be used as a radiosensitizer to affect HER2-positive breast cancer. This review will outline general management of brain metastases and will focus on preclinical findings regarding the radiosensitizing effect of small molecule HER2 targeting agents.

위암의 근치적 절제 후 발생한 뇌 전이 (Brain Metastasis after a Gastrectomy for Gastric Cancer)

  • Purpose: The common features of brain metastases from gastric cancer are unknown because brain metastasis is an uncommon pattern of metastasis. The purpose of this study was to investigate the clinical features of and the prognosis for patients with brain metastases after a curative resection for gastric cancer. Materials and Methods: Twenty-one (21) cases of patients with brain metastases of gastric cancer, who had been treated at the Department of Surgery, Yonsei University College of Medicine, were assessed retrospectively. Results: The mean age was $55.8\pm9.6$ years (range: $34\~70$ years), and the male-to-female ratio was 2.5 : .1. The most common neurologic symptom was headache ($38.5\%$), and no patient was free from the neurologic symptoms. The incidence of parenchymal metastasis (PM: $76.2\%$) was higher than that of leptomeningeal metastasis (LM: $19.0\%$). Patients with gastric cancer and brain metastasis showed high rates of blood and lymphatic vessel invasion (lymphatic vessel invasion: $85.7\%$; blood vessel invasion: $80.9\%$). According to Lauren's classification, the incidence of intestinal types was 14/21 ($66.7\%$), that of diffuse types was 3/21 ($14.3\%$) and that of mixed types was 4/21 ($19.0\%$). The mean interval between the gastrectomy and the diagnosis of brain metastasis was $24.7\pm4.0$ months (PM: 26.8 months; LM: 20.3 months). The median period of survival after diagnosis of brain metastasis was 2 months for paren chymal metastasis and 0 months for leptomeningeal metastasis. Conclusion:.. During a follow-up period, patients with neurologic symptoms should be suspected of having brain metastasis. Early diagnosis and treatment is the only hope to prolong survival in such patients.

Malignant Peripheral Nerve Sheath Tumor of Non-Neurofibromatosis Type I Metastasized to the Cerebrospinal Axis

  • A malignant peripheral nerve sheath tumor (MPNST) is a type of sarcoma that arises from peripheral nerves or cells of the associated nerve sheath. This tumor most commonly metastasizes to the lung and metastases to the spinal cord and brain are very rare. We describe a case of young patient with spinal cord and brain metastases resulting from MPNST. An 18-year-old man presented with a 6-month history of low back pain and radiating pain to his anterior thigh. Magnetic resonance imaging showed a paraspinal mass that extended from the central space of L2 to right psoas muscle through the right L2-3 foraminal space. The patient underwent surgery and the result of the histopathologic study was diagnostic for MPNST. Six months after surgery, follow-up images revealed multiple spinal cord and brain metastases. The patient was managed with chemotherapy, but died several months later. Despite complete surgical excision, the MPNST progressed rapidly and aggressively. Thus, patients with MPNST should be followed carefully to identify local recurrence or metastasis as early as possible.

전이성 뇌종양의 고식적 방사선치료 (Palliative Radiotherapy for Brain Metastases)

  • 목적 : 방사선치료는 전이성 뇌종양의 치료에 주된 역할을 해왔다. 고식적 목적으로 방사선치료를 받은 전이성 뇌종양 환자에서 치료 효과와 생존율 및 예후인자를 분석해 보고자 하였다. 대상 및 방법 : 1994년 1월부터 1997년 7월까지 충북대학교병원에서 전이성 뇌종양으로 고식적 방사선치료를 받은 42예의 환자를 대상으로 하였다. 33예의 환자에서 30Gy/10fx의 전뇌조사를 실시하였고, 이들 중 16예의 환자에서 단일전이 병소에 10Gy/5fx의 추가조사를 실시하였다. 9예의 환자들은 계획된 치료를 마치지 못했다. 결과 : 치료를 끝낸 33예의 환자에서 완전관해 4례, 부분관해 22예로 $79\%$에서 치료 효과를 나타내었다. 중앙생존기간은 4개월이었으며 단변수 분석상 치료전 신경기능적 단계(p=0.0136), 두개외 종양의 활성도(p=0.042), 치료 효과 여부(p=0.001)가 생존율에 영향을 미치는 유의한 예후인자였다. 결론 : 전이성 뇌종양 환자의 치료에서 전뇌조사의 유용성을 확인하였고, 치료전 신경기능적 단계, 두개외 종양의 활성도, 방사선치료 효과 여부가 생존율에 대한 예후인자로 분석되었다.

전이성 뇌암 환자의 호스피스 완화의료 (Hospice and Palliative Care for Cancer Patients with Brain Metastases)

  • 목적: 치료에 관계없이 뇌전이는 예후가 불량하다. 본 연구는 뇌 전이로 호스피스 완화의료를 받았던 환자를 대상으로 적절한 돌봄을 받았는지를 알아보고 임상적인 특성을 알아보고자 하였다. 방법: 2003년 3월부터 2005년 3월까지 본원의 호스피스 병동에서 뇌전이로 호스피스 완화의료를 받았던 40명의 말기 암환자를 대상으로 후향적으로 의무기록을 조사하였다. 결과: 대상자 중 남자 20명(50%), 여자 20명(50%)이며 연령의 중간값은 64세 였다. 가장 많은 암은 폐암으로 22명(55%)이고 다음으로 위암 5명(13%), 유방암 3명(8%) 순서였다. 원발암에 대한 전치료는 전혀 치료를 하지 않은 환자가 16명(40%)이고 항암치료는 20명(50%)를 받았다. 뇌전이 때의 증상은 두통 12명(30%), 의식변화 10명(25%), 국소 약화 9명(23%), 경련 4명(10%) 순서였다. 호스피스 병동의 입원 이유는 의식변화가 13명(33%)으로 가장 많았고 통증이 9명(23%)였다. 호스피스 완화의료를 받게 된 이유로 환자와 가족이 원한 경우가 14명(35%), 의사가 의뢰한 경우가 1명, 그 나머지 25명(62%)은 본원에서 보존적인 요법을 받던 중 호스피스 완화의료를 받게 된 경우이다. 호스피스 병동에서 입원기간의 중간값은 19일이었으며 호스피스 완화의료로 전원 후 생존기간의 중간값은 41일이었다. 뇌전이 진단 후 중간 생존기간은 87일이었다. 결론: 전이성 뇌암 환자의 호스피스 완화의료의 기간은 환자를 돌보기에는 충분하지 못하였다. 효과적인 호스피스 완화의료를 위해서는 의사와 환자, 가족에 대한 지속적인 호스피스 완화의료 교육과 홍보가 필요하리라고 생각된다.
