무지외반증 환자의 근위 갈매기형 절골술에서 생체 흡수성 나사못을 이용한 고정 (Bioabsorbable Screws Used in Hallux Valgus Treatment Using Proximal Chevron Osteotomy)

  • Hallux valgus is a deformity that causes pain in the first metatarsophalangeal joint. Surgical methods are quite diverse and a range of osteotomies are used at the proximal and distal part of the metatarsal bone and proximal phalange. Fixation methods, such as plate, screw, K-wire, and others have been used in various ways. The fixation device is often removed with various side effects due to the fixation devices. In the case of instruments that are absorbed in vivo, these procedures are not necessary to remove and there is an advantage of not performing the second operation. Three patients were treated, in which a proximal chevron osteotomy was used with a bioabsorbable screw (K-$MET^{TM}$; U&I Corporation).

Three-dimensional finite element analysis of the stress distribution and displacement in different fixation methods of bilateral sagittal split ramus osteotomy

  • Objectives: This study evaluated a range of fixation methods to determine which is best for the postoperative stabilization of a mandibular osteotomy using three-dimensional finite element analysis of the stress distribution on the plate, screw and surrounding bone and displacement of the lower incisors. Materials and Methods: The model was generated using the synthetic skull scan data, and the surface model was changed to a solid model using software. Bilateral sagittal split ramus osteotomy was performed using the program, and 8 different types of fixation methods were evaluated. A vertical load of 10 N was applied to the occlusal surface of the first molar. Results: In the case of bicortical screws, von-Mises stress on the screws and screw hole and deflection of the lower central incisor were minimal in type 2 (inverted L pattern with 3 bicortical repositioning screws). In the case of plates, von-Mises stress was minimal in type 8 (fixation 5 mm above the inferior border of the mandible with 1 metal plate and 4 monocortical screws), and deflection of the lower central incisor was minimal in types 6 (fixation 5 mm below the superior border of the mandible with 1 metal plate and 4 monocortical screws) and 7 (fixation 12 mm below the superior border of the mandible with 1 metal plate and 4 monocortical screws). Conclusion: Types 2 and 6 fixation methods provide better stability than the others.

Clinical characteristics of patients with the hardware failure after surgical stabilization of rib fractures in Korea: a case series

  • Purpose: Surgical stabilization of rib fractures (SSRF) is widely used in patients with flail chests, and several studies have reported the efficacy of SSRF even in multiple rib fractures. However, few reports have discussed the hardware failure (HF) of implanted plates. We aimed to evaluate the clinical characteristics of patients with HF after SSRF and further investigate the related factors. Methods: We retrospectively reviewed the electronic medical records of patients who underwent SSRF for multiple rib fractures at a level I trauma center in Korea between January 2014 and January 2021. We defined HF as the unintentional loosening of screws, dislocation, or breakage of the implanted plates. The baseline characteristics, surgical outcomes, and types of HF were assessed. Results: During the study period, 728 patients underwent SSRF, of whom 80 (10.9%) were diagnosed with HF. The mean age of HF patients was 56.5±13.6 years, and 66 (82.5%) were men. There were 59 cases (73.8%) of screw loosening, 21 (26.3%) of plate breakage, 17 (21.3%) of screw migration, and seven (8.8%) of plate dislocation. Nine patients (11.3%) experienced wound infection, and 35 patients (43.8%) experienced chronic pain. A total of 21 patients (26.3%) underwent reoperation for plate removal. The patients in the reoperation group were significantly younger, had fewer fractures and plates, underwent costal fixation, and had a longer follow-up. There were no significant differences in subjective chest symptoms or lung capacity. Conclusions: HF after SSRF occurred in 10.9% of the cases, and screw loosening was the most common. Further longitudinal studies are needed to identify risk factors for SSRF failure.

골연부 종양에서 저온 열처리한 자가골을 이용한 재건술 (Autogenous Low Heat Treated Bone Graft for Bone Reconstruction in Bone and Soft Tissue Tumors)

  • Although autoclaved autogenous bone reconstruction is one of the established procedures, it may have some problems in bone regeneration and mechanical property. The purpose of this study is to evaluate the efficacy of more biologic and anatomical reconstruction where allograft is not readily available. From Aug.1991 to Feb. 1996 the authors analyzed 32 cases of reconstruction with autogenous low heat treated bone. Autogenous graft sites were humerus 4, tibia 4, pelvis 9, and 15 femur. Average follow-up period was 23(range;12-51) months. There were 49 graft-host junctional sites. Diaphysis was 22, metaphysis 10, and flat bone 17. Average duration of healing for the 38 united sites was 7 months. Average union time for each anatomical area 8 months in 19 diaphysis, 12 months in 7 metaphysis, and 12.7 months in 12 flat bone(pelvis). Eleven nonunion sites consisted of 3 diaphysis(3/22), 3 metaphysis(3/10), and 5 flat bone(5/17). Complications other than nonunion were local recurrence(4), bone resorption(3), graft fracture(2), osteomyelitis(1), metal failure(2), and wound infection(1). Initial bone quality and stable fixation technique was important for union rate. Plate and screw is a good method for diaphyseal lesion. Metaphyseal and flat bone are weak area for rigid fixation and one stage augmentation with iliac bone graft can be a salvage procedure.

성견 구개부 골내고정원 장치에 가해진 즉시 교정력이 주위조직에 미치는 영향 (An effect of immediate orthodontic force on palatal endosseous appliance$(C-Palatal\;Plate^{TM})$ in beagle Dog)

  • 이 연구는 구개부 골내고정원 장치 C-Palatal $Plate^{TM}$에 가해진 즉시 교정력이 장치 주위 골조직 및 연조직의 치유 반응에 미치는 영향을 조직학적으로 관찰하고자 시행되었다. 4마리의 비글 성견을 대상으로 정중 구개부에 장치를 식립한 후, 교정력을 전혀 가하지 않고 8주 경과된 한 마리를 대조군으로 하고, 식립 직후부터 8주간 지속적인 교정력을 받은 한 마리, 18주간 지속적인 교정력을 받은 한 마리, 그리고 18주간 지속적인 교정력을 받은 후 장치가 제거되고 이후 4주간의 치유기간을 거친 한 마리, 총 세마리를 실험군으로 하여 동요도 검사, 방사선 사진 관찰, 그리고 조직학적 관찰을 시행한 결과 다음과 같은 결론을 얻었다. 1. 희생 후 조직괴의 측면 방사선 사진상에서, 대조군에 비하여 즉시 교정력을 받은 군에서는 플레이트의 수직적 변위가 발생하였으나 경미하였고, 동요도 검사시 모든 군에서 초기 안정성이 확인되었으며 실험기간 중 동요도의 증가는 없었다. 2. 상부 연조직의 치유상태는 실험군과 대조군 모두 병리적 변화가 관찰되지 않았다. 3. 골유착은 교정력을 가한 8주군과 18주군 모두에서 관찰되었고 18주 경과시 골유착의 분포가 더욱 증가되었다. 그러나 압박측과 긴장측간에 골유착 분포의 차이는 관찰되지 않았으며, 대조군이 실험군에 비하여 골유착에 불리한 것으로 나타났다. 4. 스크류 경부의 변연골은 대조군에서는 뚜렷한 변화가 없었던 반면, 8주군과 18주군에서는 압박측, 긴장측과 무관하게 부분적으로 변연골 침착 및 흡수가 일어났으나 경미하였다. 5. 비강으로 관통된 스크류에서, 8주군과 18주군에서는 스크류 표면을 따라 비강측으로 신생골 침착이 관찰되었으나, 대조군에서는 관찰되지 않았다. 6. 장치 제거 4주 후, 상피층은 긴밀하게 재생되었으나 결합조직은 성글고 불규칙한 배열을 보였으며, 장치를 둘러쌌던 결합조직피막이 잔존하였다. 스크류가 식립되었던 골부위에서는 왕성한 골개조가 진행되고 있었다. 즉시 교정력에 대하여 C-Palatal $Plate^{TM}$는 임상적으로 뿐만 아니라 조직학적으로도 충분한 고정원 가치를 지니는 것으로 확인되었다.

흡수성 고정판이 가토의 악골골절 치유에 미치는 영향 (THE EFFECTS OF RESORBABLE PLATE IN THE HEALING PROCESS OF MANDIBULAR FRACTURE OF THE RABBITS)

  • The purpose of this study is to evaluate the effects of resorbable plate in the healing process of mandibular fracture. Reduction and rigid fixation was carried out on the artificial mandibular fracture site of the rabbits, using a resorbable screw, 1.5mm in diameter and 4.0mm in length, and an absorbable plate 1.5mm in thickness(Lactosorb$^{(R)}$). En block tissue specimens with plate were taken from the rabbits at 4, 6, 8, and 10 weeks intervals and specimen were observed with light microscope under the hematoxylin-eosin staining, to observe the inflammatory reaction and tissue healing process. The following conclusions were drawn: 1. The subject displayed good healing with no signs of detachment of the fixation plate. 2. At 4 weeks, the plate was covered by the connective tissue. Then at 6 weeks, bone regeneration was discovered around the plate. 3. During the period of healing, no inflammatory reaction or foreign body reaction, as a result of using resorbable plate, were observed. 4. At 8 weeks, the initial phase of the absorption of the plate was observed. Then at 10 weeks, macrophage were observed around the plate, indicating the absorption phase was in process. From the above results, it can be determined that when the resorbable plate is applied to rabbit, full recuperation occurs naturally in the absence of inflammatory reaction and foreign body reaction. The selected study is clinically valuable in proving this procedure.

원위 대퇴골 골절에 대한 고령 환자군의 치료 결과 비교 -임상적, 방사선학적 비교- (Clinical and Radiological Outcome of Distal Femoral Fracture in Elderly Patient Group)

  • Purpose: To report the postoperative Clinical and Radiological outcomes following distal femoral fractures in elderly patients compared with young patients. Methods: From March, 1996 to March, 2012, 83 patients who received surgical treatment for fractures of the distal femur were enrolled in this retrospective study. Ages more than 65 was named group A. Group A was 49 cases and mean age is 72.5 year(65~91year). Group B was 49 cases and mean age is 45.7 year(16~61 year). Surgical methods are retrograde IM nail, locking compression plate, cannulated screw and postoperative rehabilitation is no difference between two groups. Clinical results were evaluated using Neer scores, radiographic results and the presence of clinical complications. Results: The mean union period was 18.4(12-40) weeks in group A and 17(10-24) weeks in group B. Neer functional scores are no significant statistical difference between two groups. There are 5 cases metal breakage in group A and 1case in group B. There are 3 cases nonunion in group A and 1 case in group B. Conclusion: In the case of fractures of the distal femur in elderly patients, locking plate using minimally invasive percutaneous periosteal osteosynthesis (MIPPO) technique may be one of the most effective methods and preoperative bone stock evaluation in important.

대퇴골(Femur)고정용 골 금속판 제작 및 강도 평가 (Practicability Strength Assessment of a Bone Metallic Plate at the Femur Fixation)

  • Study was developed the metallic plate for fixation in the femur fracture and plates has a firm place in fracture treatment. This plates can be stabilized for fracture fixation as well as biological and dynamical device. The device's designation and sizing has a optimization with bending structural stiffness and strength, known meaning that is reliable regardless of the plate by the short type and long type. The bending strength of the curved metallic long plate has to evaluate a 11,000N and the bending strength of the curved metallic short plate has to evaluate a 6,525N. This see the X-ray image of bending angle made certain of 15$^{\circ}$ at number 2 and same 82.87$^{\circ}$ at number 2, 4, 5, 7, 8, 9, 10 by outside angle, and confirmed 25.26$^{\circ}$ at number 3, 3.68$^{\circ}$ at number 6, 15.64$^{\circ}$ at number 9 by inside angle. This study shows that keep up the metallic plate for fixation in the femur fracture through X-ray Image and the device can be used to support Revision case of Hip Implant and to use a case of Hip screw compression of Hip Neck Fracture. Short plate have a wrapping of femur and long plate have to preserve a pole of femur.

Proximal Interphalangeal Joint (PIPJ) Arthrodesis for Treating PIPJ Osteoarthritis in a Horse

  • A 2-year-old, 440 kg, thoroughbred gelding showed a lameness grade of 3 out of 5, as well as swelling and heating on the proximal interphalangeal joint (PIPJ). Periarticular new bones due to the arthritis were observed on performing radiography and ultrasonography. PIPJ arthrodesis was performed under general anesthesia. With the PIPJ open, the articular cartilage was removed by using curette, chisel, and mallet. Arthrodesis was performed with a PIP plate and two additional transarticular cortex screws inserted in lag fashion. A cast was placed over the limb distal from the proximal metatarsal bone. At 6 months after surgery, complete union of the PIPJ was confirmed by radiography, and the incision site was well closed. Based on the outcome, a PIP plate with two additional transarticular cortex screws inserted in lag fashion is recommended as a method for treating osteoarthritis of the PIPJ in horses.

Treatment of Nonsyndromic Craniosynostosis Using Multi-Split Osteotomy and Rigid Fixation with Absorbable Plates

  • Background: Nonsyndromic craniosynostosis is a relatively common craniofacial anomaly and various techniques were introduced to achieve its operative goals. Authors found that by using smaller bone fragments than that used in conventional cranioplasty, sufficiently rigid bone union and effective regeneration capacity could be achieved with better postoperative outcome, only if their stable fixation was ensured. Methods: Through bicoronal incisional approach, involved synostotic cranial bone together with its surrounding areas were removed. The resected bone flap was split into as many pieces as possible. The extent of this 'multi-split osteotomy' depends on the degree of dysmorphology, expectative volume increment after surgery and probable dead space caused by bony gap between bone segments. Rigid interosseous fixation was performed with variable types of absorbable plate and screw. In all cases, the pre-operational three-dimensional computed tomography (3D CT) was checked and brain CT was taken immediately after the surgery. Also about 12 months after the operation, 3D CT was checked again to see postoperative morphology improvement, bone union, regeneration and intracranial volume change. Results: The bony gaps seen in the immediate postoperative brain CT were all improved as seen in the 3D CT after 12 months from the surgery. No small bone fragment resorption was observed. Brain volume increase was found to be made gradually, leaving no case of remaining epidural dead space. Conclusion: We conclude that it is meaningful in presenting a new possibility to be applied to not only nonsyndromic craniosynostosis but also other reconstructive cranial vault surgeries.
