Metastasis of Rhabdomyosarcoma to the Male Breast: a Case Report with Magnetic Resonance Imaging Findings

  • Metastasis of rhabdomysarcoma to the breast is a very rare manifestation in adult males. Herein, we report a case of metastasis from embryonal rhabdomyosarcoma in the left hypothenar muscle that presented as a breast mass in a 38-year-old man, who four months later expired because of multiple bone metastases related to pancytopenia. We describe the various imaging findings, including mammograms, ultrasonography, computerized tomography (CT), positron emission tomography-computed tomography (PET-CT), and magnetic resonance imaging (MRI) of this rare disease. The various imaging findings of this lesion could be helpful for future diagnosis of male breast lesions.

유방암 환자에서 종양조직내 Collagenase 활성도와 골스캔과의 비교 (Comparison of Bone Scan Findings with Collagenase Activities in Patients with Breast Cancer)

  • 기저막 성분인 type IV collagen을 분해하는 type IV collagenase의 농도와 종양의 전이사이에 상관관계가 있다는 동물 실험보고가 있다. 저자들은 유방암환자의 종양조직내 type IV collagenase의 농도와 골스캔상 골전이 소견과 비교하여 그 의의를 알아보고자 하였다. 원발성 및 전이성 유방암 환자의 종양조직에서 92kDa 및 72kDa type IV collagenase에 대한 면역조직화학염색을 각각 57명, 56명 환자에서 시행하여 각 효소 농도를 평가하고 골스캔상 골전이 소견을 관찰하고 등급을 부여하였다. 면역조직화학적으로 평가한 각 효소의 농도는 원발성 유방암과 전이성 유방암 환자 사이에 큰 차이가 있었으며, 골스캔 소견과 효소농도를 상호 비교한 결과 각 효소의 농도가 170이하일 경우에는 골스캔상 활동적인 골전이 소견을 볼 수 없었으나 효소의 농도가 200이상일 경우 골스캔 소견은 정상에서 골전이 소견까지 매우 다양하게 분포하였다. 결론적으로, 면역 조직화학적으로 측정한 92kDa 및 72kDa collagenase의 농도가 170이하일 때는 골스캔상 대부분 정상소견을 보여 골전이의 확률이 낮았다. 반면에 각 효소치의 농도가 200이상일 경우에는 골전이의 확진과 병소의 위치를 확인하고 추적검사를 위해서는 골스캔이 필요하다고 사료된다.

  • Objective : The purpose of this paper is to report the improvement of the patient with paraplegia after acupuncture, moxibustion, and herbal medicine. Methods : We treated the patient with acupuncture, moxibustion and herbal medication. Results : We treated one case of paraplegia. This patient improved significantly through acupuncture, moxibustion, herbal medicine, and western medicine. Conclusion : Through a collaboration of Western and Korean medicine, we were able to achieve meaningful treatment results.

진행성 간암 환자에서 편도 전이에 FDG 섭취증가를 보인 예 (A Case of Metastatic Tonsillar Lesion Showing Increased FDG Uptake in the Patient with Advanced Hepatocellular Carcinoma)

  • A 65 year-old man with hepatocellulcar carcinoma (HCC) admitted to treat left lower leg swelling and pus discharge suspecting osteomyelitis. MRI of his lower leg revealed the bone metastasis. Whole body FDG PET/CT additionally detected left shoulder and right ilium metastasis. Hematemesis suddenly developed in this patient after 3 weeks. Metastasis of right tonsil was histologically proven. When we reviewed his FDG PET/CT, there was asymmetric mild hypermetabolism in the right tonsil. When focal hypermetabolism is shown in the organ physiologically taking glucose up such as tonsil, we should cautiously assess whole body PET/CT in the examination of distant metastasis. We present a patient with multiple distant metastasis including tonsil from HCC showing increased FDG uptake with the literature review.

  • Glioblastoma multiforme(GBM) is the most aggressive intracranial tumor and it commonly spreads by direct extension and infiltration into the adjacent brain tissue and along the white matter tract. The metastatic spread of GBM outside of the central nervous system (CNS) is rare. The possible mechanisms of extraneural metastasis of the GBM have been suggested. They include the lymphatic spread, the venous invasion and the direct invasion through dura and bone. We experienced a 46-year-old man who had extraneural metastasis of the G8M on his left neck. The patient was treated with surgery for 5 times, radiotherapy and chemotherapy. He had survived 6 years since first diagnosed. Although the exact mechanism of the extraneural metastasis is not well understood, this present case shows the possibility of extraneural metastasis of the G8M, especially in patients with long survival.

  • PET detects only less than 50% of early gastric cancer and 62-98% of advanced gastric cancer. Therefore, mass screening programs are recommended for all adults over the age of 40 for early detection and early treatment of gastric cancer through endoscopy or various radiological tests. The most important step after being diagnosed with gastric cancer is accurate staging, which mainly evaluates tumor resectability to avoid unnecessary surgery. Important factors that affect tumor resectability are whether the tumor can be separated from adjacent organs or important blood vessels, the extent of lymph node metastasis, presence of peritoneal metastasis, or distant organ metastasis. To evaluate the extent of local tumor invasion, anatomical imaging that has superior spatial resolution is essential. There are a few studies on prognostic significance of FDG uptake with inconsistent results between them. In spite of lower sensitivities for lymph node staging, the specificities of CT and PET are very high, and the specificity for PET tends to be higher than that for CT. Limited data published so far show that PET seems less useful in the detection of lung and bone metastasis. In the evaluation of pleural or peritoneal metastasis, PET seems very specific but insensitive as well. When FDG uptake of the primary tumor is low, the distant metastasis is also known to show low FDG uptake reducing its detection. There are only a few data available in the evaluation of recurrence detection and treatment response using FDG PET.

Metastatic intestinal adenocarcinoma with osseous metaplasia in two Domestic Korean Shorthair cats

  • Two Domestic Korean Shorthair cats presented with dyschezia and vomiting. Computed tomography revealed a colonic mass with calcification and lymph node metastasis in case 1, and a small intestinal mass with disseminated mesenteric metastasis and calcification in case 2. Histopathology revealed intestinal adenocarcinoma with osseous metaplasia. Case 1 died two months after surgery from distant metastasis; and case 2 showed no metastasis for five months but presented with anorexia, euthanized seven months after diagnosis. Metastatic intestinal adenocarcinoma with bone formation should be considered as differential diagnosis for calcification on imaging, and lymph node metastasis at diagnosis may indicate poor prognosis.

Nodular scalp mass as the first presentation of pulmonary large cell neuroendocrine carcinoma: a case report

  • Metastasis of lung cancer to the skin is uncommon, presenting in 0.22% to 12% of lung cancer patients, and it is extremely rare for skin metastasis to be the first clinical manifestation of lung cancer. In the few cases where skin metastasis has been reported as the first sign of lung cancer, the patients were typically heavy smokers or had preexisting respiratory diseases and symptoms. This prompted clinicians to consider skin metastasis of a pulmonary malignancy. Large cell neuroendocrine carcinoma (LCNEC) is a rare type of lung cancer that accounts for approximately 3% of lung cancers. LCNEC mainly metastasizes to visceral organs, such as the liver, bone, and brain, and it only shows metastasis to the skin in very rare cases. Herein, we report an unusual case of a metastatic skin lesion as the first sign of primary pulmonary LCNEC, in a 63-year-old woman with no pulmonary symptoms or personal history of smoking or pulmonary disease.

The Effectiveness of Osteoplasty System (Vertebroplasty using Large Cannula-low Pressure Delivery System) in Compression Fracture and other Spinal Pathology

  • Objective : The vertebroplasty is an excellent procedure in spine compression fracture, but there are some problems such as cement leakage and difficulties in bone biopsy. Recently, the osteoplasty system is developed to solve these problems, so we will report the usefulness of the osteoplasty system. Methods : From January 2003 to November 2003, there are 9patients with simple osteoporotic spine compression fracture, 2 compression fracture patients combined with suspicious spinal metastasis, 1patient with suspicious primary bone tumor, and 2patient with infection on spine. All patients were treated using the osteoplasty system. Results : All 11 compression fracture patients were relived the back pain after the osteoplasty and there is no complication. The bone biopsies in 3 suspicious cancer patients were also effectively performed using the osteoplasty system; negative result in 2patients and positive result in 1patient. The culture result of spontaneous discitis was no growth for 48hours. The spine tuberculosis was confirm using the osteoplasty system. Conclusion : The osteoplasty system has distinguished advantages in comparison with the vertebroplasty. That is, the risk of cement leakage is lower than vertebroplasty because of low pressure delivery system. And we can obtain the specimen effectively in bone biopsy because of large cannula. In conclusion, we emphasize that the osteoplasty system is a more useful procedure in spine compression fracture especially in the patient needed bone biopsy for diagnosis.

  • 골육종에서 드물게 발생하는 도약전이 병소는 불량한 예후 인자로서 중요한 연관성이 있기 때문에 골육종 환자의 광범위 절제술 시에 절제범위에 포함되어야 한다. 따라서 수술 전 검사에서 골육종의 골수 내 침범에 대한 정밀한 평가가 필수적이며, 골스캔 검사는 도약전이의 발견에 있어서는 높은 위음성률을 보일수 있으며 자기공명영상 검사는 종양이 위치한 골 전체를 촬영하지 않는 경우 임상적인 증거가 없으면 간과하기가 쉽다는 단점이 있다. 이에 저자들은 주 종양 부위만을 포함한 국소 자기공명영상 촬영과 골스캔에서 발견되지 않았으나 FDG-PET/CT에서 도약 전이가 진단된 대퇴 원위부의 골육종 증례에 대해 문헌 고찰과 함께 보고하고자 한다.
