이소플라본의 투여가 골질량이 감소된 저체중과 정상체중 여대생의 골밀도 및 골대사 지표에 미치는 영향에 관한 연구 (A Study of Soy Isoflavone Supplementation Effect on Bone Mineral Density and Bone Metabolism Markers in Female College Students with Low Bone Mass)

  • To investigate the effects of isoflavone supplementation on both bone mineral density and hormone variation in premenopausal women who had decreased bone mass, the 24 subjects were divided into two groups: one was the underweight group, consisting of 13 subjects, and the other was the normal weight group consisting of 11 subjects. For each group, we investigated the effects of isoflavone supplementation of 90 mg/day on both bone mineral density and hormone variation during 3 menstrual cycles. Anthropometric measurements, dietary recall, and analyses of blood and urine were assessed from baseline to post-treatment. The results were as follows: The average age of the underweight group was 21.8 years old and that of the normal weight group was 23.2 years old. The comparative results for the two groups at baseline were as follows: Onset of menarche, menstrual cycle, and menstrual length were not significantly different between the groups. Serum protein, total, HDL-, LDL-cholesterol, triglyceride, Ca, P, Mg, Cu, and Zn level were not significantly different between the groups. Serum estradiol, SHBG, LH, and FSH level were also not significantly different between the groups. Lumbar spine BMD by T scores of the underweight group was significantly lower than that of the normal weight group. Serum osteocalcin, urinary DPD, and urinary pH were not significantly different between the groups. The comparative results for the two groups at post-treatment were as follows: From baseline to post-treatment, the intake of energy, nutrients and isoflavone in food did not significantly change in either group. Serum protein, total cholesterol, HDL-, LDL-cholesterol, and triglyceride levels did not significantly change in either group. Serum Ca, Cu, and Zn levels were significantly lower in both groups and serum Mg level significantly decreased only in the underweight group. Serum estradiol levels were significantly lower in both groups, but serum SHBG, LH, and FSH levels did not significantly change in either group. Lumbar spine BMD by T score of the underweight group significantly increased to 15%, but that of the normal weight group did not significantly change. Serum osteocalcin of the underweight group significantly increased to 28%, while that of the normal weight group significantly increased to 40%. Urinary DPD of the normal weight group significantly increased to 12%. The results show that the BMD of the underweight group was lower than that of the normal weight group. Therefore, the underweight group had a disadvantage in obtaining maximum bone mineral density. The results also show that isoflavone supplementation during 3 menstrual cycles was effective in increasing the bone mineral density of the lumbar spine and affected bone metabolism markers in premenopausal underweight women. Therefore, it can be concluded that sufficient intake of isoflavone could be helpful in preventing decreases in bone mass among premenopausal women, especially underweight women.

일개 농촌지역 주민의 체구성, 운동 및 체력상태와 골밀도와의 관계 (A Study on the Relationship between Body Composition, Exercise Status, Fitness Status and Bone Mineral Density in Some Rural Residents)

  • 본 논문은 일개 농촌주민들을 대상으로 신체구성, 운동(과거 운동여부), 체력상태, 골밀도를 조사하고, 골밀도와의 관련된 요인을 파악하여 농촌 주민들의 골다공증 예방을 위한 기초자료를 제시하고자 143명을 대상으로 연구하였고, 결과로서 조사대상자들 연령이 증가할수록, 교육수준이 낮을수록 골밀도가 낮았으며(p<0.05), 만성질환이 있는 경우는 없는 경우보다 골밀도가 낮았다(p<0.05). 그리고, 체중 BMI 체지방량 제지방량이 많이 나가는 경우가 낮은 경우보다 골밀도가 유의하게 높았으며(p<0.01), 과거 운동을 했던 경우와 기초 대사량이 높은 경우, 근육량이 많은 경우 골밀도가 높았다(p<0.01). 또한, 악력 윗몸일으키기 팔굽혀펴기 등 체력이 좋은 경우 골밀도가 높았으며(p<0.01), 골밀도 수치를 종속변수로 한 다중 회귀분석결과 연령, 만성질환 유무, 과거 운동유무가 유의한 변수로 나타났다.

An evolving integrative physiology: skeleton and energy metabolism

  • The adipocyte-derived hormone leptin regulates appetite and bone mass. Recent research demonstrates that reciprocally, osteoblasts have a role in controlling energy metabolism. Several genes expressed in osteoblasts are involved in this process, and one of them is the Esp gene. The remaining genes regulate Esp gene expression. OST-PTP, the protein name of Esp, regulates the carboxylation of osteocalcin secreted from osteoblasts, thus affecting insulin sensitivity and insulin secretion. This review provides evidence for a novel interpretation of the connection between bone and energy metabolism and expands our understanding of the novel physiology of bone beyond its classical functions.

Primary Osteolytic Intraosseous Atypical Meningioma with Soft Tissue and Dural Invasion : Report of a Case and Review of Literatures

  • Primary intraosseous meningioma is a rare tumor, and atypical pathologic components both osteolytic lesion and dura and soft tissue invasion is extremely rare. A 65-year-old woman presented with a 5-month history of a soft mass on the right frontal area. MR imaging revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the right frontal bone, and CT showed a destructive skull lesion. The mass was adhered tightly to the scalp and dura mater, and it extended to some part of the outer and inner dural layers without brain invasion. The extradural mass and soft tissue mass were totally removed simultaneously and we reconstructed the calvarial defect with artificial bone material. The pathological study revealed an atypical meningioma as World Health Organization grade II. Six months after the operation, brain MR imaging showed that not found recurrence in both cranial and spinal lesion. Here, we report a case of primary osteolytic intraosseous atypical meningioma with soft tissue and dural invasion.

노인의 골다공증과 체질량지수의 관계에 관한 이론적 고찰 (Relationship Between Osteoporosis and Body Mass Index among the Elderly: A Theoretical Review)

  • Purpose: The purpose of this study was to review the literature to explore the relationship between body mass index (BMI) and bone mineral density (BMD) in elderly using an integrative review. Methods: The keywords 'osteoporosis,' 'body mass index,' and 'bone mineral density' were used to search peer-reviewed publications through four databases. Among 1,389 searched articles, eight articles were selected after excluding those that did not meet the inclusion criteria. Results: Seven articles stated that BMI was positively associated with BMD among elderly. In the elderly, the prevalence of osteoporosis was 25-55.7% in women, and 11-13% in men. Conclusion: This study found that high BMI may be helpful to increase BMD among elderly although the mechanism was not clear. It is necessary to identify BMD and fractures in elderly according to body composition in future research.

말티즈 견의 두개골에서 발생한 다엽성 종양 (Multilobular Tumor of Skull in a Maltese Dog)

  • A 10-year-old spayed female Maltese dog weighting 3.4 kg was referred with growing firm mass at the parietal bone region from 2 weeks ago. A firm, partially calcified mass ($1.9cm{\times}4.4cm$) was palpated in the region of the frontal and parietal skulls but had no neurologic signs. Computed tomography (CT) characteristics of mass were round to oval shape, fine granular appearance, and well defined margins. Mass involving the calvarial bones had invasion into the cranial vault with a significant intracranial portion. Histologically, the tumor was characterized by the presence of multiple lobules containing osteoid or cartilage in the center that were separated by anastomosing fibrous septae. This is case report described the clinical and histopathological features of multilobular tumor of skull in a Maltese dog.

설부 종물로 나타난 어류골편이물 1예 (A Case of Fish Bone Foreign Body Presenting as Tongue Mass)

  • Foreign bodies in the oral cavity and pharyngolarynx are frequently observed accidental cases in the otolaryngological fields. Most foreign bodies can be recognized and removable with the manipulation of endoscopes and various instruments. However, foreign bodies that penetrate the oral cavity and oropharynx to appear as a tongue mass are rare. Therefore, such cases easily can be misconceived as tongue tumor at first, so it must involve a more thorough search utilizing such aids as computed tomography. Depending on their location and size, their removal may involve surgical intervention. The authors experienced such a rare case of an elderly male patient, presenting as tongue mass. We hereby report this case along with the relevant literature.

푸들견에서 자연발생한 유선의 양성혼합종 (Spontaneous Benign Mixed Tumor in a Poodle Dog)

  • A white-yellowish 5-7 cm sized solid mass was observed in the mammary gland of a 7 years old poodle bitch. Mass was well-defined and metastasis was not observed. In microscopic findings, mass contained hyperplasia of glandular epithelial cells, proliferation of mucin-secreting myoepithelial cells and their stroma and formation of cartilage and bone. Bone marrow with fat and hematopoietic cells was observed. Based on the microscopic findings, the mass was diagnosed as benign mixed tumor of the mammary gland in dog.

Differential effects of type 1 diabetes mellitus and subsequent osteoblastic β-catenin activation on trabecular and cortical bone in a mouse mode

  • Type 1 diabetes mellitus (T1DM) is a pathological condition associated with osteopenia. $WNT/{\beta}$-catenin signaling is implicated in this process. Trabecular and cortical bone respond differently to $WNT/{\beta}$-catenin signaling in healthy mice. We investigated whether this signaling has different effects on trabecular and cortical bone in T1DM. We first established a streptozotocin-induced T1DM mouse model and then constitutively activated ${\beta}$-catenin in osteoblasts in the setting of T1DM (T1-CA). The extent of bone loss was greater in trabecular bone than that in cortical bone in T1DM mice, and this difference was consistent with the reduction in the expression of ${\beta}$-catenin signaling in the two bone compartments. Further experiments demonstrated that in T1DM mice, trabecular bone showed lower levels of insulin-like growth factor-1 receptor (IGF-1R) than the levels in cortical bone, leading to lower $WNT/{\beta}$-catenin signaling activity through the inhibition of the IGF-1R/Akt/glycogen synthase kinase $3{\beta}$ ($GSK3{\beta}$) pathway. After ${\beta}$-catenin was activated in T1-CA mice, the bone mass and bone strength increased to substantially greater extents in trabecular bone than those in cortical bone. In addition, the cortical bone of the T1-CA mice displayed an unexpected increase in bone porosity, with increased bone resorption. The downregulated expression of WNT16 might be responsible for these cortical bone changes. In conclusion, we found that although the activation of $WNT/{\beta}$-catenin signaling increased the trabecular bone mass and bone strength in T1DM mice, it also increased the cortical bone porosity, impairing the bone strength. These findings should be considered in the future treatment of T1DM-related osteopenia.

A Comparative Study of the Iron Nutritional Status of Female College Women according to Bone Mineral Density

  • The purpose of this study was to investigate the relationship between nutritional status of iron and bone minernl density in premenopausal women. In the study, we classified the subjects into osteopenia (-2.5-I, n=29) groups according to their lumbar spine bone mineral density. Anthropometric measurements, dietary intake analysis and blood biochemistry measurements were performed on the subjects. The average ages of those in the osteopenia and normal groups were 22.2 yrs and 23.0 yrs, respectively, with no significant difference. The average body mass index (p<0.05) of those in the osteopenia group (19.6) was significantly lower than that of the normal group (21.3). The mean protein intake of those in the osteopenia group was significantly lower than that (p<0.05) the subjects in the normal group. The osteopenia group consumed a significantly lower amount of iron (p<0.05) and non-heme iron (p<0.05) compared to the normal group. The intakes of total food, vegetables and milk of those in the osteopenia group were significantly lower than those of the subjects in the normal group. The serum ferritin (p<0.001) level of those in the osteopenia group was significantly lower than those of the subjects in the normal group. In conclusion, a balance of iron status may be helpful in the prevention of bone mass loss in premenopausal young women.
