The Association of Blood Pressures and Blood Biochemical Properties with BMI in Health Checkup Examinees (종합건강검진 수검자들의 비만수준과 혈압, 혈액학적 및 혈액생화학적 지표와의 관련성)

  • The purpose of this study was to investigate the association of blood pressures, hematological and blood biochemical properties according to the degree of obesity based on differential combinations of BMI in health checkup examinees. Study subjects were 3,731 adults of 20 years and over (2,312 males, 1,419 females), not recognized as taking medicines for or having cardiovascular diseases, who underwent health package check-up at the Korea Association of Health Promotion, Daejeon Branch from Jan. 2007 to Dec. 2009. As a results, the mean values of SBP, DBP, TG, TC, LDL-C, ALT, AST, GGT, ALP were significantly increased with higher level of BMI, but the HDL-C was significantly decreased with higher level of BMI. On correlation of BMI with various factors, the BMI was positively correlated with SBP, DBP, TG, TC, LDL-C and GGT, while the HDL-C was negatively correlated. Based on the result of ordinal logistic regression analysis, the risk ratio to the abnormal level of SBP, DBP, TG, ALT and GGT were significantly increased as BMI increased.

Experimental Study on Cavo-Pulmonary Anastomosis (상공정맥-우폐동맥 문합에 관한 실험적 연구)

  • Superior vena cava to pulmonary arterial shunting operation was made between the superior vena cava and the right pulmonary artery in the fashion of end-to-end anastomosis in 20 mongrel dogs. The experimental animals were divided into three group and blood flow in the superior vena cava was occluded for 20, 30 and 60 minutes respectively, and observations were made for the changes in caval pressure and cerebrospinal fluid pressure. And pathologic examinations were also performed. On occluding the caval blood flow, the superior vena caval pressure was sharply and immediately elevated from $103.5{\pm}19.8mmH_2O$ at thoracotomy to $556.4{\pm}86.lmmH_2O$ within 2 minutes to make its plateau thereafter, and the cerebrospinal fluid pressure followed closely the changes of the superior vena caval pressure in its level and pattern being elevated from $102.0{\pm}19.9mmH_2O$ to $490.5{\pm}79.9mmH_2O$. The drops of both the caval and cerebrospinal fluid pressures were definite and marked on opening the shunt flow through the anastomosis, but these postoperative pressures retained still higher ones above their levels measured at thoracotomy. The pathological examinations of the brain and the spinal cord were also performed in six animals. Characteristic changes uniformly seen in all area and in all animals were the findings of capillary congestion and perivascular edema. On the other hand, ischemic nerve cell changes were rather evident, revealing their degrees and extents being related to the prolongation of the time of caval occlusion which has followed by the sustained high pressures in both the superior vena and the cerebrospinal fluid. The experiment suggests the safety of this surgical procedure with minimal, if any, permanent damage as long as the occlusion of the caval blood flow is not prolonged beyond the expected.

Preference and intake frequency of high sodium foods and dishes and their correlations with anthropometric measurements among Malaysian subjects

  • This study investigated the preference and intake frequency of a list of 15 commonly available high sodium Malaysian foods/dishes, dis cretionary salt use, and their possible association with demographics, blood pressures and anthropometric measurements among 300 Malaysian university students (114 males, 186 females; 259 ethnic Chinese, 41 Indians; 220 lean, 80 overweight). French fries and instant soup noodle were found to be the most preferred and most frequently consumed salty food, respectively, while salted fish was least preferred and least frequently consumed. Males had a significantly higher intake frequency of at least 6 of the salty foods, but the preference of most salty foods was not significantly different between genders. Ethnic Chinese significantly preferred more and took more frequently traditional and conventional Malaysian foods like asam laksa (a Malaysian salty-sour-spicy noodle in fish stock), salted biscuits and salted vegetable, while Indians have more affinity and frequency towards eating salty Western foods. Body Mass Index was significantly negatively correlated with the intake frequency of canned/packet soup and salted fish while waist circumference was significantly positively correlated with the preference of instant noodle. Also, an increased preference of potato chips and intake frequency of salted biscuits seemed to lead to a decreased WHR. Other than these, all the other overweight/obesity indicators did not seem to fully correlate with the salty food preference and intake frequency. Nevertheless, the preference and intake frequency of asam laksa seemed to be significant negative predictors for blood pressures. Finally, increased preference and intake frequency of high sodium shrimp paste (belacan)-based foods like asam laksa and belacan fried rice seemed to discourage discretionary salt use. In conclusion, the preference and intake frequency of the high sodium belacan-based dish asam laksa seems to be a good predictor for ethnic difference, discretionary salt use and blood pressures.

The experimental study for hemodynamic changes in the heart-lung preparatio by autoperfusion (자가관류법에 의한 체외심폐의 혈역학적 변동에 관한 실험적 연구)

  • The experimental study for extracorporeal preservation of the heart-lung preparation by autoperfusion system was performed in 10 dogs. Under intravenous Pentothal endotracheal anesthesia bilateral thoracotomies were performed. A 24F cannula connected to a plastic reservoir bag located 100 cm above the level of the heart was introduced into the aortic arch. Left subclavian, innominate artery, and descending aorta were ligated and divided. Both vena cavae were ligated and divided after the bag was half filled with blood. A 24F catheter inserted into right atrium and connected to the plastic bag in order to keep constant the preload. The thoracic trachea was intubated and the lungs were ventilated. The heart-lung preparations were removed en bloc and floated in a $34^{\circ}C$ bath of Hartmann solution. The preparations were observed for from 2 hours to 8 hours, with the average of 5.2 hours. Hemodynamic and hematologic variables were measured during preharvest and autoperfusion. The pH revealed severe respiratory alkalosis due to very low $PaCO_2$ during autoperfusion ; $PaO_2$ remained constant for 130-140 mmHg; $A-aDO_2$ increased markedly. The static inspiratory pressure [SIP] at late autoperfusion [6hr] increased significantly as compared with at early autoperfusion [2hr]. There was no difference between white blood cell counts from right atrium and those of left atrium. Heart rates remained constant for 110-120/min; cardiac outputs maintained to approximately 0.6L/min; mean aortic pressures, 75 mmHg; mean pulmonary arterial pressures, 15-18 mmHg; mean right atrial pressures, 9-13 mmHg; mean left atrial pressures, 12 mmHg lower than those of right atrium. Serum Na maintained with normal range during autoperfusion; K increased significantly; Ca decreased progressively. Hemoglobin and hematocrit decreased significantly during autoperfusion. The study demonstrated that stable hemodynamics could be maintained throughout the experiment and the preparation of the lung seemed to be inadequate, especially after 3-4 hours, such as high $A-aDO_2$, increased SIP, and scattered atelectasis and edema in their gross appearances.

Acute Effects of Dokha Smoking on the Cardiovascular and Respiratory Systems among UAE Male University Students

  • Background: In the United Arab Emirates (UAE) tobacco use is rampant. A less reported, yet widely used form of smoking native to UAE is midwakh or dhokha. The aim of the study is to assess the acute effects of smoking dokha (Arabian pipe) on the cardiovascular and respiratory systems among male university students in the UAE. Method: A quasi-experimental study was conducted among 97 male volunteers aged more than 17 years. Blood pressure, heart rate and respiratory rate of each participant, were measured before and immediately after smoking. A self administered questionnaire was used to collect personal details and data about smoking pattern. Results: Mean increases in systolic blood pressures ($12{\pm}1$ mmHg), heart rates ($20{\pm}2$ bpm) and respiratory rates ($4{\pm}1$ breaths/min) were observed (p < 0.001). A mean decrease in diastolic blood pressures ($1{\pm}1$ mmHg) was observed (p = 0.483). Conclusion: Smoking dokha has a significant acute effect on systolic blood pressure, heart rate and respiratory rate. Anti smoking campaigns must address the ill effects of this form of smoking. Results from the study warrant further research into this method of smoking which is becoming more popular.

The effect of sensory stimulation on different sites of the body on arterial blood pressures and heart rates (혈압과 심박동에 대한 경혈자극 및 피부 분절 자극 효과의 비교연구)

  • Objective : The aim of the present study is to investigate the effect of stimulation on different sites of the body on MBP(mean blood pressure) and HR(heart rate). Methods : Six healthy men have participated in this study. Before and after 10 min, exercise on a running machine of l0km/hr, acupressure, plain acupuncture and electroacupuncture(50Hz) stimulation was practiced on GV20, LI4, ST36, BL40 and non-acupoints on T4 and T10 respectively for 20 min. and in a control group without any treatment. The changes of MBP and HR after exercise have been observed for 20 min. at 5 minute intervals. Results : Compared with control, no significant difference was observed in research of the blood pressure measurement regardless of methods nor sites of stimulation. But there were trends of reduction in the heart rates in all experimental groups. Especially, in the group of acupressure on T10 before exercise, GV20, T10 after exercise and electroacupuncture on GV20, LI4, BL40 after exercise there was statistically significant decreases in heart rates. Conclusion : From the present experiment, it is concluded that somatic stimulation has effect on the heart rates but not on the blood pressures, and the presence of effective sites on the decreases of heart rates suggest that this effect may depend on sites of stimulation.

No Association of the Human Y Chromosome with Blood Pressure in Korean Male Population

  • It has been reported that the genetic variations in the Y chromosome has influence the blood pressure in some Caucasian male populations, but the effect in non-Caucasian population is unclear. In the present study, we examined the relationship between blood pressure and a HindIII RFLP of Y chromosome in 152 unrelated male individuals of ethnically homogeneous Korean origin. There were no significant differences in systolic and diastolic blood pressures between genotype groups, respectively. However, the frequency of A genotype in Korean population was much higher than those of Caucasian populations (P<0.05). Therefore, the results of this study will con-tribute the better understanding the genetic characteristics of Y chromosome in Korean population.

Effects of ATP2B1 Variants on the Systolic and Diastolic Blood Pressure according to the Degree of Obesity in the South Korean Population (한국인에게서 ATP2B1 유전 변이가 비만 정도에 따른 수축기 혈압과 이완기 혈압에 미치는 영향)

  • Hypertension often leads to cardiovascular disease and kidney disease, and hypertention is an important worldwide problem. Body mass index (BMI) has an important role for raising blood pressure. Further, hypertension can be affected by both environmental factors and genetic factors. Many single nucleotide polymorphisms have been associated with hypertension. Genome wide association study (GWAS) is a method of confirming a new locus of increasing the risk of disease, and GWAS has confirmed several single nucleotide polymorphisms (SNPs) that are associated with high blood pressure. This study analyzed the relationship between systolic blood pressure, diastolic blood pressure and SNP of the ATP2B1 gene in 994 Koreans. SNPs that showed the highest statistical significance with systolic and diastolic blood pressures were selected on the multiple linear regression analysis. One-way analysis of variance for systolic and diastolic blood pressures was performed, and multiple logistic regression analysis was performed on the risk of hypertension. The P values were two-tailed, and P<0.05 was considered significant. Four SNPs were associated with systolic blood pressure and six SNPs were associated with diastolic blood pressure. In addition, a genotype-based analysis showed significant odds ratios for the risk of hypertension in older men (adjusted OR, 5.743; 95% CI, 1.173~28.121; P=0.031). This study suggests that the ATP2B1 variants affect both the systolic and diastolic blood pressure.

Relationship between atrial pressures and the interventricular pressure in the moving actuator type total artificial heart (심실간 공간 압력을 이용한 이동작동기형 완전이식 인공심장에서의 좌, 우심방압 추정)

  • The right and left atrial pressures are important parameters in automatic control of a total artificial heart (TAH) within normal physiological ranges. Our TAH is composed of a moving actuator, right and left ventricles and the interventricular space enclosed by a semi-rigid housing. During operation of the TAH, the jnterventpicular space's volume is changed dynamically by the difference between the ejection volume of one ventricle and the inflow volume of the other. Therefore, the changes in pressure of the interventricular space is related to both atrial pressures. We measured the interventricular pressure (IVP) waveform using a pressure sensor and attempted to indirectly estimate the changes of atrial pressures. This method has an advantage that the sensor does not contact the blood directly. Furthermore, the IVP waveforms have its zero baseline in each pump cycle, thus the pressure measurements are free from the transducer drift problems by measuring the peak pressure from these baseline values. From the In vitro experiments, we found that the IVP waveform contained several useful parameters such as negative peak, dP/dT on the initial break, the area enclosed by the profile in each stroke, which are associated with atrial pressures and the filling conditions of the ventricles. The measured atrial pressures were linearly related to the negative peak of the interventricular pressure.

Effects of Capsici fructus on Blood Figures and Blood Pressure in Spontaneously Hypertensive Rats (자발성(自發性) 고혈압백서(高血壓白鼠)에서 고추(Capsici fructus)의 장기투여(長期投與)가 혈액상(血液像) 및 혈압(血壓)에 미치는 영향(影響))

  • The present study was undertaken to investigate effect of feeding hot pepper on the blood pressure and blood figures in SHR. Also studied were effect of hot pepper on plasma electrolyte concentration as well as blood sugar level. Twentytwo SHR at age of 15 weeks were divided into control and CF administered group. And in experimental group hot pepper (1 gm/kg/day) was administered into stomach of each animal by gastric intubation for 8 weeks. The results obtained are as follows: 1) On a day before starting experiment blood pressure(systolic) of the control and CF group were respectively $196.7{\pm}4.7\;mmHg$ and $197.4{\pm}3.9\;mmHg$ while it changed to $207.4{\pm}4.8\;mmHg$ and $227.5{\pm}4.2\;mmHg$ respectively on the last day of experiment. Comparing blood pressures of two groups a significant, though not remarkable, difference was observed. 2) No significant differences in blood figures(RBC, WBC, Hct, and Hb level), concentration of plasma electrolytes($Na^+$ and $K^+$)and blood sugar level were found between control and experimental group.
