The purpose of this study is to improve pain and injury through bicycle fitting and to propose rehabilitation methods. Pain and injuries caused by bicycles can be largely divided into knee and ankle pain, wrist, shoulder, neck, and back pain, and hip pain. The cause of these pains and injuries stems from incorrect bicycle fitting and posture. In order to improve these pain and injuries and prevent recurrence, appropriate bicycle fitting and rehabilitation exercise are needed. Pain and injury are divided into kinetic pain and pain by a fixed posture. In the case of kinetic pain, rehabilitation exercise is required along with inflammatory treatment, and pain by a fixed posture should understand the exact characteristics of the body and learn and practice bicycle posture according to the body's characteristics. Through this method, it can contribute to preventing the recurrence of bicycle pain and injuries and further creating an environment where a safe bicycle culture can be established.

Objective: To determine the energy consumed and muscle use during dance compared to different standard exercise devices. Design: Longitudinal study. Methods: Fifteen female subjects were evaluated to assess the energy cost and muscle activity during a 20 minute dance video compared to treadmill, elliptical track and bicycle ergometry. The later 3 forms of exercise were accomplished in four, 5 minute bouts at different intensities of exercise. Subjects were in the age range of 22-24 years old, were free of cardiovascular disease and did not have any neurological injuries. They were not sedentary and exercised at least twice a week. During the exercise, muscle activity was measured by the electromyogram recorded by surface electrodes on 6 muscle groups. A Cosmed metabolic cart was used to measure oxygen consumption during the exercise. Results: The aerobic dance video that was tested here was equivalent to a hard workout on any of the 3 exercise modalities. The dance routine was equivalent in terms of energy consumed to running at 225 watts of work or running for 20 minutes at a speed of 2 meters per second (4.47 miles per hour). Compared to the bicycle, it was equivalent to cycling at 112 watts for 20 minutes (2.25 kpm), and for the elliptical trainer, dance was equivalent to 435 watts. Concerning muscle use, the dance routine was the most balanced for upper, core and lower body muscles. Although the elliptical trainer was close, it required muscle less muscle use. Conclusion: A good dance video can be more effective than standard exercise equipment.

The aim of this study was to evaluate the effect of exercise-based cardiac rehabilitation on pro- and anti-inflammatory markers in patients with acute coronary syndrome (ACS). ACS patients who underwent percutaneous coronary intervention (PCI) and took medicine during phase II of rehabilitation were recruited for study. Subjects were divided into two groups; exercise group (EX, n=21) and a non-exercise group (non-EX, n=13). Supervised exercise program in hospital consisted of treadmill and bicycle exercise was performed three times per week for 6 weeks. Patients of EX received individual counseling, including knowledge of heart disease, risk factor modification, and physical training. Cardiopulmonary fitness, body composition, and biochemical blood factors were analyzed before and after experiment. There was no significant difference in serum levels of hs-CRP and TGF-${\beta}1$ between groups, and between time intervals. But there was a significant decrease in serum levels of IL-18 (P<.001). And there was a significant increase in ratio of IL-18 to IL-10 (P<.01) and serum levels of IL-10 (P<.001). After cardiac rehabilitation, there was significant increase in exercise duration (P<.001), maximal oxygen uptake ($VO_{2peak}$; P<.001) and decrease in submaximal rate-pressure product (sRPP; P<.05) in EX. In conclusion, exercise-based cardiac rehabilitation during phase II in patients with ACS after PCI decreased serum IL-18 (pro-inflammatory) content and ratio of IL-18 to IL-10 in serum (highly related with disease recurrence), and increased serum IL-10 (anti-inflammatory) content. In addition, it led to improved cardiopulmonary fitness.
In this paper, a new rehabilitation training system was developed to improve equilibrium sense by combining virtual reality technology with a fixed exercise bicycle. The subjects consisted of two groups. A group of young people, was compared against a group of elderly. We measured three different running modes of virtual bicycle system with two successive sets. The parameters measured were running time, velocity, the weight movement, the degree of the deviation from the road, and the variables about the center of pressure. The repeated training, our results showed that the running capability of the elderly improve compared, In addition, it was found out that the ability of postural control and the equilibrium sense was improved with the presentation of the visual feedback information of the distribution of weight. From the results of this experiment, we showed that our newly developed system might be useful in the diagnosis of equilibrium sense or in the improvement of the sense of sight and, somatic, and vestibular sense of the elderly in the field of rehabilitation training.

The purpose of this study is developing a virtual bicycle system for improving the ability of postural balance control for adults in various age groups. The system consists of an exercise bicycle that allows tilt in accordance with the postural balance of the subject in the system, a visual display that shows virtual road, and a visual feedback system. The rider of the system tries to maintain balance on the bicycle with a visual feedback of a virtual road while the pedaling speed, the heading direction, and various weight distribution information are updated to the subject as visual feedbacks in the display. A series of experiments were performed with various subjects to find the factors related to postural balance control in the system. The related parameters obtained were weight shift, magnitude of the deviation from the center of the virtual road, and variables related to the movement of the center of pressure. The results found that the ability to control postural balance in the system improved with the presentation of visual feedback information of the distribution of weight. It was also found that the general performance of the subject on balance in the system improved after ten days long training. The results show that the newly developed system can be used for the diagnosis of postural balance as well as for the stimulation of various senses such as vision and somatic sense in the field of rehabilitation training.
This is a review article about range of motion, stretching, and aerobic exercise in accelerated rehabilitation of knee and shoulder. If the joint was immobilized for a long time after injury, it would cause stiffness and atrophy. Therefore, this program includes various exercise techniques; range of motion for joint stiffness, and stretching for muscle relaxation, and cardiovascular training (e.g., swimming, upper body extremity, stationary bicycle) for prevention of cardiopulmonary function decrease. In accelerated rehabilitation, It is very important factor to make interaction between clinical exercise specialist and patients. Also, we recommend that they should discuss with sports medicine doctor as a team members the following; pain, adaptation of exercise, fitness level, and progression of program.

Background : It is known that pulmonary rehabilitation improves dyspnea and exercise tolerance in patient with chronic lung disease, although it does not improve pulmonary function. But there is a controversy whether this improvement after pulmonary rehabilitation is due to increased aerobic exercise capacity. We performed this study to evaluate the effect of pulmonary rehabilitation for 6 weeks on the pulmonary function, gas exchange, exercise tolerance and aerobic exercise capacity in patients with chronic lung disease. Methods : Pulmonary rehabilitations including education, muscle strengthening exercise and symptom-Umited aerobic exercise for six weeks, were performed in fourteen patients with chronic lung disease (COPD 11, bronchiectasis 1, IPF 1, sarcoidosis 1 ; mean age $57{\pm}4$ years; male 12, female 2). Pre- and post-rehabilitaion pulmonary function and exercise capacity were compared. Results: 1) Before the rehabilitation, FVC, $FEV_1$ and $FEF_{25-75%}$ of the patients were $71.5{\pm}6.4%$. $40.6{\pm}3.4%$ and $19.3{\pm}3.8%$ of predicted value respectively. TLC, FRC and RV were $130.3{\pm}9.3%$, $157.3{\pm}13.2%$ and $211.1{\pm}23.9%$ predicted respectively. Diffusing capacity and MVV were $59.1{\pm}1.1%$ and $48.6{\pm}6.2%$. These pulmonary functions did not change after pulmonary rehabilitation. 2) In the incremental exercise test using bicycle ergometer, maximum work rale ($57.7{\pm}4.9$) watts vs. $64.8{\pm}6.0$ watts, P=0.036), maximum oxygen consumption ($0.81{\pm}0.07$ L/min vs. $0.96{\mu}0.08$ L/min, P=0.009) and anaerobic threshold ($0.60{\pm}0.06$ L/min vs. $0.76{\mu}0.06$ L/min, P=0.009) were significantly increased after pulmonary rehabilitation. There was no improvement in gas exchange after rehabilitation. 3) Exercise endurances of upper ($4.5{\pm}0.7$ joule vs. $14.8{\pm}2.4$ joule, P<0.001) and lower extremity ($25.4{\pm}5.7$ joule vs. $42.6{\pm}7.7$ joule, P<0.001), and 6 minute walking distance ($392{\pm}35$ meter vs. $459{\pm}33$ meter, P<0.001) were significantly increased after rehabilitation. Maximum inspiratory pressure was also increased after rehabilitation ($68.5{\pm}5.4$$CmH_2O$ VS. $80.4{\pm}6.4$$CmH_2O$, P<0.001). Conclusion: The pulmonary rehabilitation for 6 weeks can improve exercise performance in patients with chronic lung disease.
Background: The aim of this randomized controlled pilot study was to determine the effect of trunk training in wheelchair on fall efficacy, fall risk and activities of daily living in acute stroke patients. Design: Randomized controlled pilot trial. Methods: The study included 18 patients with acute stroke who were randomly allocated to an experimental group (EG) (n=9) and a control group (CG) (n=9). Patients in the EG group received general rehabilitation therapy combined with trunk training in wheelchair for 20min, whereas CG group received general rehabilitation therapy combined with bicycle training for 20min. Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using Korean-Fall Efficacy Scale (K-FES), Fullerton Advanced Balance Scale (FAB) and Korean-Modified Barthel Index (K-MBI). Results: After 3 weeks of training, both groups showed significantly improved K-FES, FAB and K-MBI (p<.05 in both groups). However, the K-FES, FAB and K-MBI in the experimental group was significantly improving than in the control group (p<.05). Conclusion: These findings indicate that trunk training in wheelchair may be effective at decreasing fall risk and improving activities of daily living in acute stroke patients. Therefore, trunk training in wheelchair may be recommended as an intervention in reducing the incidence of fall risk in acute stroke patient.
Pulmonary rehabilitation has been known to improve dyspnea and exercise tolerance in patients with chronic lung disease, although it does not improve pulmonary function. The mechanism of this improvement is not clearly explained till now; however some authors suggested that the improvement in the skeletal muscle metabolism after the rehabilitation could be a possible mechanism. The metabolc changes in skeletal muscle in patients with COPD are characterized by impaired oxidative phosphorylation which causes early activation of anaerobic glycolysis and excess lactate production with exercise. In order to evaluate the change in the skeletal muscle metabolism as a possible cause of the improvement in the exercise tolerance after the rehabilitation, noninvasive $^{31}P$ magnetic resonance spectroscopy(MRS) of the forearm flexor muscle was performed before and after the exercise training in nine patients with chronic lung disease who have undertaken intensive pulmonary rehabilitation for 6 weeks. 31p MRS was studied during the sustained isometric contraction of the dominant forearm flexor muscles up to the exhaustion state and the recovery period. Maximal voluntary contraction(MVC) force of the muscle was measured before the isometric exercise, and then 30% of MVC force was constantly loaded to each patient during the isometric exercise. After the exercise training, exercise endurance of upper and lower extremities and 6 minute walking distance were significantly increased(p<0.05). There were no differences of baseline intracellular pH (pHi) and inorganic phosphate/phosphocreatine(Pi/PCr). After rehabilitation pHi at the exercise and the exhaustion state showed a significant increase($6.91{\pm}0.1$ to $6.99{\pm}0.1$ and $6.76{\pm}0.2$ to $6.84{\pm}0.2$ respectively, p<0.05). Pi/PCr at the exercise and the recovery rate of pHi and Pi/PCr did not show significant differences. These results suggest that the delayed intracellular acidosis of skeletal muscle may contribute to the improvement of exercise endurance after pulmonary rehabilitation.
