Effect of attitude and practice of PSM and clinical practice satisfaction in nursing students. (간호대학생의 환자안전문화 태도와 환자안전관리 실천이 임상실습 만족도에 미치는 효과)

  • The purpose of this study was to assess the correlation between attitude of Patient Safety Management(PSM) and practice of PSM and clinical practice satisfaction of nursing students, also identified how these factors attitude and practice of PSM influence on clinical practice satisfaction, eventually aimed to develop education programs to strengthen practice of PSM in nursing students. A descriptive design was used in the study. The multiple regression was used for the study to prove the factors how the attitude and practice of PSM were affected to the clinical practice satisfaction. The study subjects were 226 nursing students. The results show that attitude of PSM mean was 3.71, practice of PSM mean 3.97, the clinical practice satisfaction mean 3.31. Among the general characteristics, clinical practice satisfaction showed significant differences in satisfaction of major(F=5.10, p=.001), the existence of role model(t=2.64, p=.009). Significant positive correlation was found between attitude of PSM, practice of PSM and the clinical practice satisfaction. Practice of PSM(${\beta}=.39$) was shown as the most important factor to affect on the Satisfaction on clinical practice, also the explanation power of value for this study was found 41.1%. Consequently, this study indicates to develop the education program to strengthen practice of PSM. Also process the additional study for the nursing students in other areas.

The Factors Related to the Intention of Hospital Pharmacists for taking the Role of a Preceptor (병원약국 실무실습 교육 제공 의도에 대한 영향요인)

  • Background: Pharmacy schools newly adopted a 6-year program strengthening clinical knowledge since 2011 in Korea. The clinical training under the guidance of preceptors at hospital sites is a requisite for pharmacy students during the last year of undergraduate course. It has been rarely studied on the hospital pharmacists' perspective regarding being a preceptor or teaching pharmacy students. Objectives: This study aimed to examine the hospital pharmacists' intention toward student training and to identify the relevant factors among the individual pharmacists' characteristics and working environment within the theoretical frame of the Theory of Planned Behavior (TPB). Method: A mail-survey was conducted for pharmacists working in tertiary hospitals in Seoul and Incheon. The survey questionnaire consisting of 131 questions with a 5-likert scale was developed for investigating pharmacists' attitude, subjective norm, perceived behavioral control and the intention to teach pharmacy students as well as other demographic variables. To estimate the construct validity of components, factor analysis was conducted and Cronbach's alpha was calculated to estimate the reliability of the observed variables. Statistical analyses of one-way variance analysis and multiple regression analysis were performed using SPSS 18.0. Results: The survey response rate was 53% (116/210) and the three constructs of attitude (r = .519), subjective norm (r = .233) and perceived control (r = .392) have appropriate correlations with the intention, proving the appropriateness of using the TPB model. Pharmacists working in inpatient (mean = 3.45) and outpatient clinics (mean = 3.34) generally showed positive intention for teaching. The attitude (${\beta}=.432$, p < 0.01) and perceived control (${\beta}=.270$, p < .01) constructs were significant predictors of the intention. Both age (r = 0.246, p = 0.017) and length of career (r = 0.310, p = 0.002) were positively related with the perceived control. Conclusion: Hospital pharmacists showed generally positive intention to provide student training in spite of the concern on their limited perceived behavioral control. Future research to find the actual barriers pharmacists faced in educating students need to be conducted.

Performance of a Bayesian Design Compared to Some Optimal Designs for Linear Calibration (선형 캘리브레이션에서 베이지안 실험계획과 기존의 최적실험계획과의 효과비교)

  • We consider a linear calibration problem, $y_i = $$\alpha + \beta (x_i - x_0) + \epsilon_i$, $i=1, 2, {\cdot}{\cdot},n$ $y_f = \alpha + \beta (x_f - x_0) + \epsilon, $ where we observe $(x_i, y_i)$'s for the controlled calibration experiments and later we make inference about $x_f$ from a new observation $y_f$. The objective of the calibration design problem is to find the optimal design $x = (x_i, \cdots, x_n$ that gives the best estimates for $x_f$. We compare Kim(1989)'s Bayesian design which minimizes the expected value of the posterior variance of $x_f$ and some optimal designs from literature. Kim suggested the Bayesian optimal design based on the analysis of the characteristics of the expected loss function and numerical must be equal to the prior mean and that the sum of squares be as large as possible. The designs to be compared are (1) Buonaccorsi(1986)'s AV optimal design that minimizes the average asymptotic variance of the classical estimators, (2) D-optimal and A-optimal design for the linear regression model that optimize some functions of $M(x) = \sum x_i x_i'$, and (3) Hunter & Lamboy (1981)'s reference design from their paper. In order to compare the designs which are optimal in some sense, we consider two criteria. First, we compare them by the expected posterior variance criterion and secondly, we perform the Monte Carlo simulation to obtain the HPD intervals and compare the lengths of them. If the prior mean of $x_f$ is at the center of the finite design interval, then the Bayesian, AV optimal, D-optimal and A-optimal designs are indentical and they are equally weighted end-point design. However if the prior mean is not at the center, then they are not expected to be identical.In this case, we demonstrate that the almost Bayesian-optimal design was slightly better than the approximate AV optimal design. We also investigate the effects of the prior variance of the parameters and solution for the case when the number of experiments is odd.

Assessment of Relationship between Fyn-related Kinase Gene Polymorphisms and Overweight/Obesity in Korean Population

  • The fyn-related kinase (FRK) belongs to the tyrosine kinase family of protein kinases. Recent studies have shown that Frk affects pancreatic beta cell number during embryogenesis and promotes beta cell cytotoxic signals in response to streptozotocin. To investigate the genetic association between FRK polymorphisms and the risk of obesity in Korean population, single nucleotide polymorphisms (SNPs) in the FRK gene region were selected and analyzed. The body mass index (BMI) was calculated, and biochemical data (systolic blood pressure, diastolic blood pressure, hemoglobin A1C, triglyceride, total cholesterol, high density lipoprotein, and low density lipoprotein) of blood sample from each subject were also measured. One hundred fifty five healthy control and 204 overweight/obesity subjects were recruited. Genotype frequencies of six SNPs [rs6568920 (+8391G>A), rs3756772 (+56780A>G), rs3798234 (+75687C>T), rs9384970 (+68506G>A), rs1933739 (+72978G>A), and rs9400883 (+75809A>G)] in the FRK gene were determined by Affymetrix Targeted Genotyping Chip data. According to the classification of Korean Society for the Study of Obesity, control (BMI 18 to < 23) and overweight/obesity (BMI$\geq$23) subjects were recruited. For the analysis of genetic data, EM algorithm, SNPStats, Haploview, HapAnalyzer, SNPAnalyzer, and Helixtree programs were used. Multiple logistic regression analysis (codominant, dominant, and recessive models) was performed. Age and gender as covariates were adjusted. For biochemical data, Student's t test was used. The mean value of BMI in the control and overweigh/obesity groups was 21.1${\pm}$1.2 (mean${\pm}$SD) and 25.6${\pm}$2.0, respectively. All biochemical data of the overweight/obesity group were statistically significance, compared with the control group. Among six SNPs, two linkage disequilibrium (LD) blocks were discovered. One block consisted of rs1933739 and rs9400883, and the other comprised rs3756772 and rs3798234. One SNP (rs9384970, +68506G>A) showed an association with overweight/obesity in the codominant model (p=0.03). Interestingly, the AA genotype distribution in the overweight/obesity group (n=7, 3.5%) was higher than those in the control group (n=1, 0.6%), which is not found in either Japanese or Chinese subjects. Therefore, the AA genotype of rs9384970 may be a risk factor for development of obesity in Korean population. The results suggest that FRK may be associated with overweight/obesity in Korean population.

Factors Associated with the Concentrations of Urinary Creatinine in Korean Children and Adolescents (한국 어린이와 청소년의 요중 크레아티닌 농도와 영향요인에 대한 연구)

  • Creatinine-adjustment is an important process in the urinary monitoring of the environmental exposure of children and adolescents. The purpose of this study was to investigate the concentrations of urinary creatinine and factors associated with them among Korean children and adolescents. We recruited 1,025 persons from 128 extracted schools. They were from three to 18 years old and supplied urine samples for measuring creatinine. The concentrations of urinary creatinine were 98.18 mg/dl (SD, 67.67) in arithmetic mean and 72.05 mg/dl (GSD 2.49) in geometric mean, were significantly higher among male children/adolescents than females in all age groups, and higher values appeared following increasing ages, heights and BMIs. The rates of the number who were below the lowest limit recommended by WHO (<30 mg/dl) were 25.57% among three to four year olds, 21.77% among five to six year olds, 20.0% among seven to eight year olds and 14.69% among nine to ten year olds, respectively. The rates of those above the highest limit (>300 mg/dl) were 0.0% among three to twelve year olds. The coefficient of determination R-square of the fitted regression model for urinary creatinine was 27.4% with general characteristic variables of sex, age, BMI and height. The significant variables among these were height (standardized beta = 0.372) and age (standardized beta = 0.129). Another coefficient of determination R-square was 15.3% with dietary habit variables of smoking, drinking, dining area, number of meals and snacks, and intake of milk food, cup-noodles, canned foods, popcorn, nachos, and hamburgers. In conclusion, the concentration of urinary creatinine was significantly lower in children than in adults, and was very significantly associated with the height of children. Therefore, children need the recommended concentrations for urinary creatinine, as distinguished from adults.

Genetic Parameters of Milk β-Hydroxybutyric Acid and Acetone and Their Genetic Association with Milk Production Traits of Holstein Cattle

  • This study was conducted to estimate the genetic parameters of ${\beta}$-hydroxybutyrate (BHBA) and acetone concentration in milk by Fourier transform infrared spectroscopy along with test-day milk production traits including fat %, protein % and milk yield based on monthly samples of milk obtained as part of a routine milk recording program in Korea. Additionally, the feasibility of using such data in the official dairy cattle breeding system for selection of cows with low susceptibility of ketosis was evaluated. A total of 57,190 monthly test-day records for parities 1, 2, and 3 of 7,895 cows with pedigree information were collected from April 2012 to August 2014 from herds enrolled in the Korea Animal Improvement Association. Multi-trait random regression models were separately applied to estimate genetic parameters of test-day records for each parity. The model included fixed herd test-day effects, calving age and season effects, and random regressions for additive genetic and permanent environmental effects. Abundance of variation of acetone may provide a more sensitive indication of ketosis than many zero observations in concentration of milk BHBA. Heritabilities of milk BHBA levels ranged from 0.04 to 0.17 with a mean of 0.09 for the interval between 4 and 305 days in milk during three lactations. The average heritabilities for milk acetone concentration were 0.29, 0.29, and 0.22 for parities 1, 2, and 3, respectively. There was no clear genetic association of the concentration of two ketone bodies with three test-day milk production traits, even if some correlations among breeding values of the test-day records in this study were observed. These results suggest that genetic selection for low susceptibility of ketosis in early lactation is possible. Further, it is desirable for the breeding scheme of dairy cattle to include the records of milk acetone rather than the records of milk BHBA.

The Effect of Nursing Professional Attitude and Employment Strategy on the Core Fundamental Nursing Education Program of Graduated Grade Nursing Students (졸업학년 간호학생의 간호전문직태도와 취업전략이 핵심기본간호술 교육프로그램에 미치는 영향)

  • This study was conducted to investigate the effects of nursing professional attitude and employment strategy on graduate nursing students core fundamental nursing education program, after applied the core fundamental nursing education program for one semester. There were 165 subjects who were surveyed from June 23 to 24, 2016. The mean score of the core fundamental nursing education program satisfaction was $3.99({\pm}0.48)$. The mean score of nursing professional attitude was $3.77({\pm}0.49)$ and that of employment strategy was $3.28({\pm}0.49)$. The nursing professional attitude related to campus life satisfaction n(p=0.004), employment preparation(p=0.002), factors affecting employment(p=0.005), and employment strategy related with motivation for entering(p=0.018), average credits(p<0.001), employment preparation(p<0.001), information route of employment(p=0.048) showed statistical significance. A significant and positive correlation was found between nursing professional attitude and employment strategy(r=0.472, p<0.001). Nursing professional attitude(${\beta}=0.462$, p<0.001) was found to be an influential variable in core fundamental nursing education programs. The regression model explained 29% of the core fundamental nursing education program(F=17.79, p<0.001). In conclusion, nursing students' nursing professional attitude was related to employment strategy. To increase the core fundamental nursing skills, it is necessary to develop a practical education program and institutional strategies to establish a nursing professional attitude and strengthen employment strategy.

The Influence of Health Behaviors and Sleep related Factors on Cognitive Function in the Elderly Hypertensive Patients (노인 고혈압 환자의 건강행태 및 수면 관련 요인이 인지기능에 미치는 영향)

  • This study examined effects of health behaviors and sleep related factor on cognitive function in the elderly hypertensive patients. Data were collected through a face to face interview survey with structured questionnaire form 140 elderly with hypertension ($age{\geq}65years$) from February 5 to May 1, 2013. Research instruments included Pittsburgh Sleep Quality Index(PSQI), Epworth Sleepiness Scale(ESS) and Korean version the Mini-Mental State Examination(MMSE-K). Cognitive function was negatively related to degradation in quality of sleep(r=-.29, p<.001). Sleep duration were negatively related to body mass index(r=-.18, p=.032) and degradation in quality of sleep(r=-.59, p<.001). Sleep duration was positively related to daytime sleepiness(r=.22, p=.008). Hierarchical multiple regression showed that age, education levels and living arrangement were associated with cognitive function(F=8.56, p<.001, Adjusted $R^2=.14$). After controlling for demographic characteristics and health behaviors, degradation in quality of sleep(${\beta}=-.27$, p=.008) was identified as significant predictors of cognitive function. This final model explained 17.0% of the cognitive function in the elderly hypertensive patients(F=4.09, p<.001). Therefore, as a strategy improving cognitive function of the elderly with hypertension, therapeutic intervention should be developed to improve quality of sleep considering age, education levels and living arrangement.

A Study of the Effect of Acculturative Stress on the Marital Conflicts of Immigrant Women Who Are Married to Korean Men -Focusing on the Mediating Effect of Social Support- (결혼이주여성의 문화적응스트레스가 부부갈등에 미치는 영향 -사회적 지지의 조절효과 중심으로-)

  • This study focuses on immigrant women who are married to Korean men and who live in a multicultural family situation in Gyeonggi-do. These women experience acculturative stress and marital conflict and this study aims to determine how social support seeks to mediate the effects of those stressors. The women in this study participated in activities and received services from one of Gyeonggi-do Province's civic organizations, religious organizations, or social service organizations, such as the Social Welfare Agency and the Multicultural Family Support Center. In order to verify the study's hypothesis, the researchers used the following statistical analytical methods : t-test, two-way ANOVA and multi-regression analysis. Analysis of the study's results showed that the highest degree of marital conflict was found in the sub-zones and variables that were personal. Those variable were : the difference in mindset and values (personal domain), economic problems (communal living area), a child's upbringing and education issues (third party area), and the participant's sex life (in the marital relationship). The hypothesis was tested using the research model validation and the results are summarized as follows. First, in order to analyze the relationship between the marriage migration females' acculturative stress and the impact of that stress on marriage conflict, the hierarchical regression analysis was used. It identified that a direct correlation existed between acculturative stress and marriage conflict; where a higher degree of acculturative stress was present, a higher degree of marriage conflict was found. Second, the study found a statistical significance in the correlation and relationship (${\beta}$=.208, p<0.05) between the acculturative stress of the marriage migration female and material support. In other words, the material support can be seen as having a moderating effect on the acculturative stress, which is the independent variable, and the marital conflict, which is the dependent variable.

A Study on effects of emotional labor and emotional intelligence of dental hygienist on organizational effectiveness (치과위생사의 감정노동과 감성지능이 조직유효성에 미치는 영향 연구)

  • This study was undertaken to analyze factors influencing the emotional labor and emotional intelligence of a dental hygienist on organizational effectiveness. The study was conducted between September 4, 2018 to December 7, 2018, enrolling a total of 233 dental hygienists living in the Jeollabuk-do province. The data were analyzed by independent t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficient, and stepwise multiple regression, using SPSS 18.0. The regression model obtained was F=138.917, P<0.05, with an explanatory percentage of 58.2. Emotional labor (${\beta}=0.608$) was determined to be the most influential variable among independent variables, and other factors affecting the organizational effectiveness of a dental hygienist include the highest level of education, career, and emotional intelligence. We conclude that in order to improve organizational effectiveness among dental hygienists, it is necessary to establish an effective plan for reducing emotional labor and promote emotional intelligence.
