뇌졸중 환자의 신체기능, 균형, 인지 관계에서 재활동기의 조절효과에 관한 연구 (The moderator effect of rehabilitation motivation on physical function, balance, and cognition of stroke patients)

  • Background: This study evaluates physical function, balance, and cognition rehabilitation motivation. Design: Cross-sectional study. Methods: The subjects include 67 stroke patients to measure physical function, balance, and cognition rehabilitation motivation. For evaluation of physical function, Manual Muscle Test, Range of Motion, Modified Ashworth Scale for spasticity, grasping power, and balance was measured using Functional reach test (FRT), Berg Balance Scale (BBS). For evaluating Cognition, Korean-Mini Mental State Examination, motivation for rehabilitation scale was used to measure for motivation. Results: The results of this study that the moderating effect of rehabilitation motivation was significant in the effect of physical function on BBS (p<0.01), The moderating effect of rehabilitation motivation was significant in the effect of FRT on physical function (p<0.01), The moderating effect of external motivation was also significant in the effect of FRT on physical function (p<0.01). However, the results of the rehabilitation motivations generally divided into three groups (Lower, Average, Upper) and all three results were statistically significant only in groups of average and lower. Conclusion: This study confirms that the rehabilitation motive has demonstrated significant adjustment effects between the body function and balance. In this study, physical function, balance and cognition were significantly correlated with each other, and rehabilitation motivation proved a significant moderating effect between physical function and balance.

PNF기법을 적용한 슬링운동이 만성요통환자의 통증과 균형능력에 미치는 영향 (Effect of Sling Exercise with PNF Basic Procedure for Pain and Balance Ability of Patients with Chronic Low Back Pain)

  • Purpose : The purpose of this study was to investigate the effect of sling exercise with PNF basic procedure in patients with chronic low back pain. Methods : This study included 14 patients with chronic low back pain, who were performed sling exercise combined PNF basic procedure. The exercise program comprised 3 sessions of 30 minutes per week for 8 weeks. The VAS(Visual analogue Scale) and BBS(Berg Balance Scale) were evaluated before and after training. All data were analyzed using SPSS 12.0. Results : Significant differences were observed the chronic low back pain patient for VAS, BBS. Chronic low back pain patient improved all test. Conclusion : Sling exercise with PNF basic procedure about chronic low back pain patient is very useful and effective. It is effective in clinical practice.

편마비 환자의 발목관절 전략 운동이 균형 조절 능력에 미치는 영향 (The Effects of Ankle Strategy Exercise on Balance of Patients with Hemiplegia)

  • PURPOSE: PURPOSE: The purpose of this study was to analyze the effect and examine the feasibility of an intervention of ankle strategy exercise on balance of patients with hemiplegia. METHODS: The subject were randomly allocated to three groups: ankle strategy exercise (group A), balance exercise (group B) and control (group C). Group A was received the conventional physical therapy plus ankle strategy exercises for 20 minutes in one session. Group B was received the same conventional physical therapy plus balance exercises for 20 minutes in one session. Two active groups were performed in the session 3 times a week, for a total of 6 weeks. and Group C was only received the same conventional physical therapy. Balance test was assessed using center of pressure (COP) in the anteroposterior (A-P), mediolateral (M-L) direction, Berg balance scale (BBS) and Timed Up and Go Test (TUG). RESULTS: All groups showed improvements in balance parameters. In especial, the Group A was statistically significant differences in almost part evaluation items and showed more improvements in BBS and TUG parameters than Group B and Group C. and moving distance of M-L COP was more improvements than Group B. CONCLUSION: Ankle strategy exercises had more influence on balance than balance exercises and neurodevelopmental treatment.

  • Background : The purpose of this study was to investigate on the ankle pain, Range of Motion(ROM) and balance ability with old man have fall down experience used to ankle strengthening exercise for 6 weeks. We introduced ankle strengthening exercise in this study in order to recover the pain, ankle ROM and balance ability. Methods : Twenty-four old mans have fall down experience were included for the study. Among them, twelve old mans(experimental group) were ankle strengthening exercise and physical science methods(H/P, TENS, U/S), another twelve old mans(control group) were treated by physical science methods(H/P, TENS, U/S). Each group made use of Visual Analogue Scale(VAS) to pain and goniometer to ankle ROM and Berg Balance Test(BBS) to balance ability to compare the possible changes of pain, ROM and balance ability in two groups after treatment. Results : In experimental group, pain reduced, ankle ROM has increased in dorsi-flexion, plantar-flexion and balance ability has increased for 6 weeks. In control group, pain reduced, ankle ROM has not changed in dorsi-flextion, plantar-flextion and balance ability has not changed for 6 weeks. Conclusion : According to the results, we concluded that ankle strengthening exercise for 6 weeks is effective for reducing pain and increasing ankle ROM and balance ability. Therefore, we thought the old man have fall dawn experience need ankle strengthening exercise for prevention repeatedly fall down.

지역사회 뇌졸중 환자를 위한 근력강화 운동과 보행훈련의 효과 (The Effect of Muscle Strengthening Exercise and Gait Training for Stroke Persons in a Community)

  • The limited walking ability after a stroke restricts a patient's independent mobility at home and in the community. It also brings about significant social handicaps. Therefore, it is necessary to improve walking ability in community-dwelling persons with stroke. The purpose of this study was to evaluate the effectiveness of gait training and muscle strengthening exercise of lower extremities in persons with chronic stroke. Nineteen community-dwelling individuals with stroke participated in this program. The exercise program lasted for seven weeks, with a 1-hour program twice per week, and it consisted of balance training, gait training, and strengthening of lower extremities. The outcome of the program was assessed by the gait speed, Wisconsin Gait Scale (WGS), Berg Balance Scale (BBS), and Stroke Impact Scale (SIS). Significant effects were found for the WGS, BBS, and mobility and hand function domain of the SIS (p<.05). It was found that short-term gait training exercise could improve quality of gait, balance, and mobility. Therefore, a more effective exercise program is required for community-based persons with stroke.

Clinical Criteria to Perform the Step through Step Gait with a Cane in Chronic Stroke Patients

  • PURPOSE: The purpose of this study was to propose clinical criteria to differentiate patients who are able to perform the step-through-step gait pattern in chronic stroke patients. METHODS: Sixty patients with chronic stroke patients participated this study. To differentiate patients who could perform the step-through-step gait pattern, age, gender, and causes of stroke were noted, a Chedoke-McMaster (CM) damage list, Fugl-Meyer (FM) assessment scales and the Berg Balance Scale (BBS) were determined. A 10 meter gait test and Timed Up and Go (TUG) test were conducted to determine the differences in gait speed and dynamic balance between patients walking with or without canes in the step-through-step gait pattern group. RESULTS: There was no significant statistical difference in age, gender, and stroke type between all subjects. There were significant differences in the CM scale for postural and lower extremities, and FM scale for lower extremities and BBS. The dynamic balance ability and gait speed showed significant differences between the subjects in the step-through-step gait pattern with or without a cane during gait. CONCLUSION: CM and FM scales for the lower extremities and postural control, as well as BBS scales, can be used as criteria to differentiate patients who are able to perform the step-through-step gait pattern. These results can also be used to provide beneficial information to patients that are walking with canes.

다감각 자극훈련이 노인의 인지기능 및 균형능력에 미치는 영향 (The Effect of Multi-Sensory Stimulation Training on Cognitive Function and Balance Skill of the Community Resident Elderly)

  • 목적 : 본 연구는 다감각 자극훈련이 노인의 인지기능 및 균형능력에 미치는 영향을 알아보고자 하였다. 연구방법 : 본 연구는 지역사회에 거주하는 65세 이상 노인 10명을 대상으로 다감각 자극훈련을 주 1회 60분씩 12주 동안 진행하였다. 다감각 자극훈련은 신체도식, 대근육, 촉각자극 영역을 포함한 총 12가지 주제에 맞는 활동으로 구성된 프로그램이다. 결과측정은 한국형-간이정신상태평가(Mini-Mental State Examination-Korean, MMSE-K)와 버그균형검사(Berg Balance Scale, BBS)를 사용하여 대상자의 사전사후 인지기능과 균형 능력을 평가하였고, 중재 이후 프로그램 만족도를 조사하였다. 측정 결과는 윌콕슨 순위 검정(Wilcoxon matched-pair signed rank test)을 사용하여 인지기능과 균형능력의 중재 전후로 차이를 비교하여 분석하였다. 결과 : 다감각 자극훈련 중재 이후 대상자들은 인지기능과 균형능력에서 유의한 향상을 보였다(p<.05). 또한 중재 이후 대상자 모두 프로그램에 대한 긍정적인 만족도를 보였다. 결론 : 지역사회 노인을 대상으로 한 다감각 자극훈련은 노인의 인지기능과 균형능력의 향상에 긍정적인 영향을 미쳤으며, 중재 이후 만족도에도 긍정적인 결과를 보였다. 다감각 자극훈련은 고령화 시대에 노인들에게 만족감을 줄 수 있는 그룹치료활동으로 효과적인 사용을 위한 추가적인 연구가 더 필요할 것으로 생각된다.

Correlation among Motor Function and Gait Velocity, and Explanatory Variable of Gait Velocity in Chronic Stroke Survivors

  • Objective: The purpose of this study to investigate the correlations among the motor function, balance, and gait velocity and the strength that could explain the variation of gait velocity of chronic stroke survivors. Design: This was a cross-sectional cohort study. Methods: Thirty hemiplegic stroke survivors hospitalized in an inpatient rehabilitation center were participated. The muscle tone of ankle plantarflexor and muscle strength of ankle dorsiflexor were measured respectively with modified Ashworth scale (MAS) and hand-held dynamometer. And the motor recovery and function with Fugl-Meyer assessment (FMA), balance with Berg balance scale (BBS) and timed up and go (TUG) test were measured. Gait velocity was measured with GAITRite. The correlation among motor function, muscle tone, muscle strength, balance, and gait were analyzed. In addition, the strength of the relationship between the response (gait velocity) and the explanatory variables was analyzed. Results: The gait velocity had positive correlations with FMA, muscle strength, and BBS, and negative correlation with MAS and TUG. Regression analysis showed that TUG (𝛽=-0.829) was a major explanatory variable for gait velocity. Conclusions: Our results suggest that gait velocity had correlations with muscle strength, MAS, FMA, BBS, and TUG. The tests and measurements affecting the variation of gait velocity the greatest were TUG, followed by FMA, BBS, muscle strength, and MAS. This study shows that TUG would be a possible assessment tool to determine the variation of gait velocity in stroke rehabilitation.

Effects of elastic band orthosis (aider) on balance and gait in chronic stroke patients

  • Objective: The objective of this study was to investigate the feasibility and effects of balance training using a newly developed elastic band orthosis (aider) for improvement of mobility and balance in chronic stroke patients. Design: Cross-sectional study. Methods: Ten patients with chronic hemiplegia participated in this study. There were six males and four females; two patients had right hemiplegia and eight had left hemiplegia. This study investigated the effect of the elastic band orthosis on balance and gait ability compared with bare foot condition. Gait parameters were measured using the opto-gait system for analysis of the spatial and temporal parameters of walking in stroke patients. In addition, balance ability in stroke patients was evaluated using the Timed Up and Go (TUG) and Berg Balance Scale (BBS). Results: This study investigated the effect of the elastic band orthosis on balance compared with bare foot condition. The TUG and BBS showed significant improvement with use of the elastic band orthosis (p<0.05). Use of the Elastic band orthosis resulted in significantly improved velocity, cadence, less-affected step length, less-affected stride length, and less-affected single limb support in stroke patients (p<0.05). Conclusions: We demonstrated a significant improvement in dynamic balance and gait ability in chronic stroke patients using the elastic band orthosis. This orthosis may aid in prevention of spastic foot drop, leading to improvement of walking ability.

고유수용성 조절이 만성 편마비 환자의 균형에 미치는 영향 (Effects of Proprioceptive Control on the Balance in Patients With Chronic Hemiplegia)

  • Hemiplegic patients usually present with difficulty maintaining balance. Balance retraining is the major component of rehabilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of prorpioceptive exercise program on the improvement of balance in the patients with chronic hemiplegia. Thirty subjects (mean age $57.0{\pm}9.8$) were recruited and the subjects were divided into a proprioceptive group and a visual group. The subjects for the proprioceptive group were participated in the proprioceptive exercise program for 4 weeks, and the visual group were treated visual feedback training using Balance Master. At 4 week follow-up test, Berg Balance Scale significantly improved 1.1 points (p<.01), Timed Up & Go test improved 4.2 second (p<.01), and weight distribution during sit to stand also improved 5.0% (p<.01). As a result of this study, the proprioceptive control approach improved dynamic balance in the patients with chronic hemiplegia. It is suggested that there was no benefit of visual feedback training like as Balance Master when administrated in combination with other physical therapy interventions, compared with physical therapy alone using proprioceptive control approach to hemiplegia.
