고유수용성신경근촉진법 운동이 만성허리통증환자의 통증과 기능장애지수에 미치는 영향 (The Effects of Proprioceptive Neuromuscular Facilitation Exercise on the Pain and Functional Disability Index of Patients with Chronic Lower Back Pain)

  • Purpose: This study attempts to identify the effects of stretching and core exercise using proprioceptive neuromuscular facilitation (PNF) on the pain and functional disability index of patients with chronic lower back pain. Methods: A total of 20 patients with chronic lower back pain were randomly divided into either the experimental group (n=10), who received PNF stretching and core exercise, or the control group (n=10), who received conventional physiotherapy. Both interventions were applied three times a week for six weeks. The visible analogue scale (VAS) was measured in order to determine the level of pain, while the Oswestry Disability Index (ODI) was used to measure the change in the functional disability index. We conducted a paired t-test to compare the within-group change before and after the intervention. To compare the between-group difference, we used an independent t-test. The statistical significance level was set at ${\alpha}=0.05$ for all the variables. Results: The experimental group showed a significant within-group change in both the VAS and the ODI (p<0.01). The control group also showed a significant change (p<0.01). A significant difference was observed between the experimental group and the control group with regard to the change in both the VAS and the ODI after the interventions (p<0.01). Conclusion: In this study, the application of stretching and core exercise using PNF for subjects who complain of chronic lower back pain proved effective in reducing both pain and functional disability. We therefore expect that this intervention can be applied in the future as a useful program for patients with chronic lower back pain.

  • Objective: The Purpose of this study is the factive and descriptive research to identify the factors affecting the functional disability of the chronic low back pain patients. We investigated the effect of ODI by the Questionnaire and questioned an 70 patients who were back pain unit at G university hospital in D city from March to May, 2007. The functional disorder in this study were based on the Oswestry Disability Index(ODI) lower back pain disability questionnaire by fairbank(1980), and these data were proceeded by using SPSS/WIN version 10.0. Result: 1. The functional disorder in accordance with age, gender, academic background, occupation, diagnosis, pain cause, pain experience, treatment duration had no significantly similar difference statistically(p>0.05). 2. The functional disorder in accordance with duration of painful generation had statistically significance(P<0.05). Conclusion: we could verify that duration of painful generation affect on the factors affecting the functional disability of the low back pain patients.

다중운동 프로그램이 만성 요통 환자의 통증 강도, 몸통 근력 및 오스웨스트리 장애지수에 미치는 영향 (Effects of a Multi-modal Exercise Program on Pain Intensity, Trunk Muscle Strength, and Oswestry Disability Index in Patients with Chronic Low Back Pain)

  • Background: The purpose of this study was to examine the effects of a multi-modal exercise program for patients with chronic low back with respect to pain intensity, trunk muscle strength and Oswestry disability index. Methods: Thirty patients with chronic low back pain were recruited and divided equally into two groups. The multi-modal training program comprised a series of exercises such as warm-up, stabilization exercises, stretching, endurance exercises, and cool down whereas the control group performed only stabilization exercises. The both group spent an equal amount of time performing 60 minutes per day, three times per week, for five weeks. Results: The experimental group demonstrated statistically significant improvements in range of motion, trunk muscle strength, the visual analogue scale, and the Oswestry Disability Index (p<.05). Intergroup comparison showed a statistically significant difference in the range of motion of the lumbar spine and the degree of disability in the experimental group. Muscle strength and pain were statistically significant in both groups. Conclusion: The multi-modal exercise program is effective for patients with chronic low back pain, as it reduces lower back pain, increases trunk muscles strength, and decrease the potential for becoming disabled.

일부 요통환자들의 오스웨스터리요통장애지수 및 관련요인 (Oswestry Low Back Pain Disability Index and Related Factors in Patients with Low Back Pain)

  • Purpose: We measured the Oswestry Low Back Pain Disability Index (OLBPDI) and related factors in patients with low back pain. Methods: The sample consisted of 50 patients who received physical therapy at the physical therapy units of the Andong Seoul Sintong Clinic, St. Luke Clinic, and Yeongju Seoul Sintong Clinic in Andong and Yeongju city from October, 2007, to February, 2008. The OLBPDI questionnaire was administered by 5 physical therapists as a cross-sectional study. Student's t-test and analysis of variance (ANOVA/Tukey and Scheffe) were used to analyze OLBPDI score differences. We also used nonparametric statistic analysis (Wilcoxon rank sum test, Median test). Pearson correlation analysis (Spearman correlation analysis) was used to analyze the relationship between OLBPDI and the visual analogue scale (VAS). Multiple regression analysis was performed to determine the effects of independent variables on pain scores as defined by the OLBPDI. Results: The average patient age was 37.1 years (range: 18$\sim$78 years old), and time from onset was 21.7 months (1$\sim$180). OLBPD and VAS scores were 12.70 (3.0$\sim$28.0) and 5.14 (1$\sim$8), respectively. OLBPDI scores were 14.4 in patients taking medicine and 11.57 in those who did not. There was a statistically significant relationship between OLBPDI and VAS (r=0.54, p=0.0001; r=0.55, p=0.0001 by Spearman coefficient). Gender ($\beta$=6.14, p=0.0124), age ($\beta$=-2.01, p=0.0324), weight ($\beta$=0.31, p=0.0222), time from onset ($\beta$=1.54, p=0.0044), and VAS score ($\beta$=1.59, p=0.0004) were significantly associated with OLBPD by multiple regression analysis. Conclusion: Variables associated with OLBPD were gender, age, weight, time from onset, and VAS score. Collecting information on the pain index using OLBPDI was acceptable to patients with low back pain. Further research should explore the pain index by using larger sample sizes and longer follow-up periods.

요통 환자의 굴곡신연기법의 효과에 대한 임상적 연구 (The Clinical Study for the Effects of Flexion-Distraction Technique on Patients with Low Back Pain)

  • Objectives : The purpose of this study is to find out the effects of flexion-distraction technique on low back pain. Methods : The 40 patients were divided into 2 groups: group a was treated except flexion-distraction technique and group B was treated with flexion-distraction technique. Both groups were treated with acupuncture treatment, physical therapy and manipulation. Patients were evaluated by McGill pain questionnaire-short form(SF-MPQ), visual analogue scale(VAS), Oswestry disability index(ODI) and pain disability index(PDI). Results : 1. Both Groups were significantly decreased in VAS and ODI after 3 weeks of treatment. 2. Group B compared with Group A was significantly decreased in SF-MPQ, VAS, ODI and PDI after 3 weeks of treatment. Conclusions : We found out that flexion-distraction technique is considered to be effective and useful on low back pain.

추나치료를 병행한 임신초기 급성요통 한의 치험 1례 (A Clinical Study of Acute Low Back Pain treated by Chuna & General Oriental Therapy during Pregnancy)

  • Objectives: The object of this study is to report a clinical effect of oriental medical treatments with Chuna for acute low back pain during the early stages of pregnancy.Methods: The patient in this case, 31 years-old female was admitted for 1days (20th/June/2016-30th/June/2016) due to the acute low back pain on the 7 weeks of pregnancy. We diagnosed as 挫閃腰痛, 傷筋 (Sprain and strain of lumbar spine) and treated with Chuna therapy and other conservative treatments including acupuncture herbal mixture. And we measured Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ).Results: After treatments, Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ) were significantly improved in case.Conclusion: Oriental medical treatments with Chuna manual therapy were associated with improvement of acute low back pain during the early stages of pregnancy.

대학생의 허리통증에 대한 SNAGS(Sustained Natural Apophyseal Glides)기법이 허리통증과 오스웨스트리 장애지수(KODI)에 미치는 영향 (Effect of SNAGS Technique on the Lower Back Pain Korean Oswestry Disability Index(KODI) and Pain in Lower Back Pain of University Students)

  • Background: This study investigated the effects of sustained natural apophyseal glides (SNAGS) technique on lower back pain and pain disability among university students with lower back pain. Methods: A total thirty 20~30 year old university students with lower back pain were divided into two groups. A lumbar flexibility exercise was applied to the control group (n=15). The SNAGS technique group (n=15) received L1~L4 segment mobilization using SNAGS technique. Intervention was implemented for 20 minutes, 3 times a week for 3 weeks. The SNAGS were performed using belt with flexion and extension, whereas lumbar flexibility exercise included lumbar flexion and extension. Lower back disability was measured using the Korean Oswestry disability index (KODI), and pain sensitivity was measured by pain pressure threshold (PPT). The groups were assessed for lower back pain disability and pain, before and after the intervention. Results: There were significant improvements in both the KODI and PPT of the SNAGS technique group (p<.05), while the lumbar flexibility exercise group showed no significant change (p>.05). In addition, the SNAGS technique group had a statistically significant difference in KODI and PPT compared to the lumbar flexibility exercise group (p<.05). Conclusion: SNAGS technique is more effective than lumbar spine joint flexibility exercise in the improvement of lower back pain disability and pain among university students with lower back pain.

요통을 가진 대상자에 있어 전방으로 굴곡된 자세 및 갑작스런 부하가 균형 수행력에 미치는 영향 (The Effects of Balance Performance to Forward Bending Posture and Sudden Load in Subjects with Low Back Pain)

  • The purpose of this study was to determine differences of balance performance to upright standing, forward bending posture, and sudden load during forward bending posture in subjects with and without a history of low back pain. A study was conducted on 8 subjects with low back pain and 13 healthy subjects. Dynamic Balance System was used to measure the postural sway index, left-right sway index, and anterior-posterior sway index on balance performance. There were no differences between subject groups on balance performance during upright standing. Balance performance was increased in forward bending posture and decreased in sudden load during forward bending posture in subjects with low back pain. These results suggest that subjects with low back pain demonstrated increased activity and decreased reaction times of trunk muscles.

Effects of Horse Riding Simulator on Pain, Oswestry Disability Index and Balance in Adults with Nonspecific Chronic Low Back Pain

  • PURPOSE: The purpose of this study was to investigate effects of Horse riding simulator exercise on pain, dysfunction and dynamic balance in adults with nonspecific chronic low back pain. METHODS: In this study, total 19 college students usually complain of low back pain who were randomly divided into the horse riding simulator exercise group (n=10), lumbar strengthening exercise group (n=9) were recruited. Each group carried out for 30 minutes exercise three times a week for 4 weeks. Horse riding simulator exercise group carried out 15 minutes horse riding simulator exercise and 15 minutes lumbar strengthening exercise. Lumbar strengthening exercise group carried out 30 minutes lumbar strengthening exercise. Visual analogue scale (VAS) were measured for evaluation back pain. Korean oswestry disability index (KODI) were measured for dysfunction. Limits of stability (LOS) were measured for dynamic balance. RESULTS: VAS, KODI, LOS results showed a significant change within both horse riding simulator exercise group, lumbar strengthening exercise group. CONCLUSION: Present study suggested that the horse riding simulator exercise can improve back pain, dysfunction, dynamic balance. Horse riding simulator exercise provides more convenience, interest and motivation than conversional therapy and it could be a possible approach to adults with nonspecific chronic low back pain.

Effects of Lumbar Stabilization Exercise with Kinesio Taping on Pain, Muscle Strength, and Oswestry Disability Index in Patients with Chronic Low Back Pain

  • Objective: The purpose of this study was to examine the effects lumbar stabilization exercise with kinesio taping on pain, muscle strength, and oswestry disability index (ODI) in patients with chronic low back pain. Design: Two groups pre-post randomized controlled design Methods: Thirty-two subjects were randomly divided in two groups; 1) lumbar stabilization exercise with kinesio taping group (Experimental group, n=16), 2) lumbar stabilization exercise with sham kinesio taping group (Control group, n=16). The intervention was conducted in each group for thirty minutes a day, 5 times a week, for 4 weeks. Both group did 30 minutes of lumbar stabilization application. Evaluations were performed before the commencement of training and again 4 weeks after training was initiated. Visual analog scale (VAS) was used to evaluate pain level of patients with chronic low back pain. Distal muscle test was used to evaluate muscle strength of trunk extension. In addition, ODI was used to evaluate activity daily life of low back pain. Results: After training, the VAS, muscle strength of trunk extension and ODI were significantly more improvement in Experimental group than in the control group (p<0.05). Conclusions: We confirmed that the effects of lumbar stabilization exercise with kinesio taping group on pain, muscle strength, and ODI in patients with chronic low back pain.
