자율신경 기능 이상이 동반된 류마티즘 환자에 대한 온열-척추 마사지 치료 효과 : 증례 연구 (Effects of Thermo-spinal massage treatment in a Patient with Rheumatism patient with Autonomic nervous system Dysfunction: A Case Report)

  • 만성통증을 동반한 류마티즘 환자에서 자율신경계 이상(autonomic nervous system dysfunction)이 동반되기도 한다. 특히 만성통증 환자에서 자율신경계 이상은 국소 통증 강도 증가 및 통증 역치 감소를 발생하여 만성 근골격계 통증에 악영향을 미친다고 확인되었다. 이런 만성 근골격계 통증 환자에 온열-척추 마사지 치료를 실시한 실험에서 통증경감과 자율신경계 회복이 되었다고 보고되고 있다. 그래서 우리는 만성 통증과 자율신경계 이상이 동반된 류마티즘 환자에 온열-척추 마사지 치료를 적용하였고 자율신경기능의 회복과 통증의 감소를 경험한 사례가 있어 보고하고자 한다.

가미온담탕(加味溫膽湯)으로 호전된 자율신경실조증 환자 1례 (The Case Report of 1 case of Patient with Dysfunction of Autonomic Nervous System by oriental medical therapy)

  • Objectives: The purpose of this study is to show a case of patient with dysfunction of autonomic nervous system improved by oriental medical treatment. Methods: This patient was 66 year-old woman who complained pantalgia including abdominal pain and headache, cold sweating, anxiety, palpitation and fatigue. She was treated by herbal medicine(加味溫膽湯 $ji\bar{a}w\grave{e}iw\bar{e}nd\breve{a}nt\bar{a}ng$), acupuncture, moxa and aroma therapy. We used VAS(Visual analogue scale) to measure the progress of symptoms. Results : The symptoms of dysfunction of autonomic nervous system was improved after oriental medical therapy. Conclusions : This study suggests that oriental medicine therapy is effective in the trearment of dysfunction of autonomic nervous system.

  • 심전도에 의한 R-R 간격변동은 자율신경계의 기능을 검사하는데 매우 유용하고 또한 교감 신경계와 부교감신경계의 가능을 정량적으로 알아낼 수 있을 것으로 사료되었다. 특히, 당뇨병질환에 있어서 자율신경계의 dysfunction현상을 고찰하는데 매우 유용할 것으로 기대된다(Fig.5 참조). 그러나 임상에 직접 적용시켜온 바로는 기립시, 심호흡시에 발생되는 근전도에 의한 잡음이 간혹 발생되는 경우가 있는데 이것은 전극접착법과 무선송신기에 의해 제거될 것으로 기대되며 향후의 과제로 남아있다.

Relationships of autonomic dysfunction with disease severity and neuropathic pain features in fibromyalgia: is it really a sympathetically maintained neuropathic pain?

  • Background: The pathophysiology of fibromyalgia (FM) involves many mechanisms including central nervous system sensitization theory, autonomic nervous system (ANS) dysfunction, and recently small fiber neuropathy. While the small fiber neuropathy itself can cause ANS dysfunction and neuropathic pain (NP), it is still unknown whether ANS problems have an association with severity of disease and NP in patients with FM. The aim of this study was to evaluate ANS dysfunction in FM patients and to explore possible associations of ANS dysfunction with disease severity and NP. Methods: Twenty-nine FM patients and 20 healthy controls were included in this cross-sectional study. Participants were tested using sympathetic skin responses (SSR) and R-R interval variation analyses for sympathetic and parasympathetic ANS dysfunction, respectively. Disease severity and somatic symptoms of patients with FM were evaluated using the ACR-2010 scales and Fibromyalgia Impact Questionnaire, and NP symptoms were evaluated using the Pain Detect Questionnaire and Douleur Neuropathique questionnaire. Results: FM patients were found to have ANS dysfunction characterized by increased sympathetic response and decreased parasympathetic response. SSR amplitudes were found to be correlated with a more severe disease. Although nonsignificant, NP severity tended to be associated with a decrease in sympathetic and parasympathetic activities. Conclusions: ANS dysfunction may play a role in the pathophysiology of FM. The trend of decreased ANS functions in FM patients exhibiting NP contradicts the notion that FM is a sympathetically maintained NP and may be explained with small fiber involvement.

  • Autonomic dysfunction occurs frequently in multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD). Patients with either condition may present with autonomic symptoms such as bladder, sexual, cardiovascular, thermoregulatory, and gastrointestinal dysfunction, and fatigue, but autonomic symptoms that affect quality of life are underrecognized in clinical practice. The immunopathogenesis of MS has been considered to be associated with autonomic dysfunction. Applying appropriate treatment strategies for autonomic dysfunction is important to improve the quality of life of patients. Here we review autonomic dysfunction and how this is managed in patients with MS and NMOSD.

  • Tetanus is an infectious disease of the nervous system caused by Clostridium tetani, and is characterized by tonic muscle contractions, painful spasms, and autonomic dysfunction. Severe autonomic dysfunction associated with tetanus can be life-threatening. We present a 62-year-old female who experienced lockjaw after an ankle fracture. The patient was diagnosed with tetanus and received tetanus immunoglobulin and a vaccination. The patient subsequently experienced labile hypertension. This case highlights the challenge and importance of managing cardiovascular instability.

만성 폐쇄성 폐질환 환자의 자율신경 장애 (Dysfunction of Autonomic Nervous System in Patients with Chronic Obstructive Pulmonary Diseases)

  • 연구목적: 호흡기에 대한 자율신경 기능도 기도 및 혈관의 평활근, 접막하선의 점액분비. 기관지 순환 혈류조절, 비만 세포의 염증 반응에 관계되는 매개물의 분비등 매우 다양하게 나타나며 이들 자율신경 장애는 기도 폐쇄의 원인으로 작용한다. 천식은 자율신경계 이상이 원인으로 작용하고 특히 부교감신경 기능 항진이 중요한 원인으로 알려져 있다. 저자들은 심혈관계와 호흡기 자율신경 지배가 공통적으로 일어난다는 사실을 기초로 비관혈적이고 안정한 심혈관 지율신경 검사를 실시하여 만성 폐쇄성 폐질환 환자에서 자율신경 장애유무를 알아보았다. 방 법: 대상은 영남대학교 의과대학 부속병원에서 만성 폐쇄성 폐질환으로 진단 받은 환자 20명과 건강진단 센터에서 건강한 것으로 판정 받은 비슷한 나이의 20명을 대조군으로 하였다. 만성 폐쇄성 폐질환은 American Thoracic Society의 정의를 따랐으며 허혈성 심장질환, 부정맥, 당뇨병, 중심성 및 말초성 신경질환 등과 같은 자율신경계에 영향을 미칠 수 있는 질환이 동반된 경우는 제외하였다. 자율신경검사는 Ewing과 Clarke의 방법을 사용하였으며 모든 대상에서 동맥혈 가스분석과 폐기능 검사를 동시에 실시하였다. 결 과: 만성 폐쇄생 폐질환 환자는 건강한 사랑에 비하여 빈번하게 자율신경 장애가 동반되어 있었으며 만성 폐쇄성 폐질환 환자에서 교감신경 장애보다 부교감신경 장애가 더 뚜렷하였다. 만성 폐쇄성 폐질환은 질병 이환 기간이 길거나, 흡연량이 많거나, 폐기능 검사상 $FEV_1$ 및 FVC 감소 정도가 심할수록 그리고 저산소혈증이 심한 경우에 자율신경 장애가 심하였으나 연령, 만성폐쇄성 폐질환의 종류 및 동맥혈 이산화탄소 분압과는 차이가 없었다. 결 론: 만성 폐쇄성 폐질환은 부교감신경 장애가 동반되어 있으며, 부교간신경 장애는 만성 폐쇄성 폐질환의 정도가 심할수록 더 뚜렷하게 나타났다. 이러한 부교감신경 장애는 만성 폐쇄성 폐질환의 원인으로 작용하기보다는 질병의 진행으로 인한 자율신경계의 기능 저하로 생각된다.

배수혈의 내장기 치료 기전에 관한 연구 (Study on the Treatment Mechanism of Back-Shu Points for Organ Dysfunction)

  • Objectives : This study aims to overview the therapeutic mechanism of back-shu points in terms of sympathetic visceral motor nervous system. Methods : Studies about autonomic nervous system, and studies and ancient texts about back-shu points were reviewed. We interpreted possible mechanism of back-shu points considering similarities of anatomical and physiological characteristics of back-shu points and visceral motor nervous system. Results : Afferent signals for organ lesions that can develop the symptoms of autonomic neurological symptoms, pain, hyperalgesia through the skin segment. Through a physical examination of the myotome and dermatome, it is possible to diagnose segmental disorders. Treatment stimulation of the thick fibers of the disorder segment skin can reduce abnormal autonomic influence over the sympathetic reflex mechanism. In addition, if spinal muscles are relaxed, the pressure on the nerve roots could be reduced and consequently the hyperactivity of the sympathetic visceral motor signal would be suppressed. Conclusions : The back-shu points treatments work through the mechanism of the sympathetic nervous reflex. Moreover, segmental acupuncture can reduce tension of the spinal muscles, thereby improving pathological conditions of the sympathetic nervous system.

Autonomic dysfunction in patients with orthostatic dizziness

  • Orthostatic dizziness is feeling dizzy or lightheaded when standing up. Hemodynamic orthostatic dizziness can be caused by autonomic dysfunction such as orthostatic hypotension or postural tachycardia syndrome. The interpretation of the autonomic function test results in patients with orthostatic dizziness is crucial for diagnosing and managing the underlying condition. The head-up tilt and Valsalva tests are especially important for evaluating adrenergic function in patients with hemodynamic orthostatic dizziness. However, it is important to note that autonomic function tests do not cover the entire diagnostic process, since their findings need to be considered along with the detailed history and physical examination results of the patient because various differential diagnoses exist for orthostatic dizziness. Ensuring appropriate treatment by interpreting the autonomic function test results can help to determine the improvement of and prevents falls from orthostatic dizziness.

태극침법(太極鍼法)의 임상 적응증에 대한 소고(小考) (Clinical Opinion of Taegeuk Acupuncture Treatment by Sasang(4-type) Constitutional Medicine)

  • Objectives : This study was undertaken in order to establish clinical application of Taegeuk acupuncture treatment by Sasang(4-type) Constitutional medicine. Methods : 1. Dr. Lee Byung-haeng suggested nine applications of Taegeuk acupuncture treatment of Soyang(lesser Yang) type men. 2. The author summarized the application of Taegeuk acupuncture treatment gathered by clinical experience. 3. The author researched the effect of Taegeuk acupuncture on patients after treatment. Results & Conclusions : 1. Taegeuk acupuncture treatment is effective against psychogenic disease. 2. Taegeuk acupuncture treatment is effective against autonomic nervous system dysfunction(For example, blood circulation disorder, etc.) and has an effect on recovery from sub-health. 3. The author conclude that Taegeuk acupuncture treatment for incurable diseases or diseases of unknown etiology needs a further clinical study in the future.
