Building Living Lab for Acquiring Behavioral Data for Early Screening of Developmental Disorders

  • 발달장애는 영유아 기부터 시작하는 뇌 신경계 발달장애들의 집합으로 언어 및 의사소통, 인지력, 사회성 등의 측면에서 이루어져야 할 발달이 심하게 지체되거나 성취되지 않은 장애를 의미한다. 이러한 발달장애 진단에는 아동의 얼굴 표정과 같은 감정표현의 의미와 맥락 등 비언어적 반응에 대한 관찰로 이루어진다. 이를 사람이 측정기에는 상당히 주관적인 판단이 개입하게 되어 객관적인 기술이 필요하다. 따라서 본 연구에서는 영유아/아동의 언어, 비언어적 행동 반응을 관찰하는 ADOS(Autism Diagnostic Observation Schedule)와 BeDevel(Behavior Development Screening for Toddler) 검사에서 검사자와 피검사자간의 상호작용이 녹화된 영상을 리빙랩 환경에서 획득하여 인공지능 기반의 비정상적/상동적 행동 인지 기술 개발에 필요한 영상 및 음성 데이터 확보를 목표로 한다.

자폐 스펙트럼 장애 아동의 섭식 문제를 위한 부모교육의 효과: 체계적 고찰 (The Effect of Parent Education on Feeding Problems in Children With Autism Spectrum Disorder: A Systematic Review)

  • 목적 : 본 연구는 자폐 스펙트럼 아동의 섭식 문제에 대한 부모교육의 효과를 알아보고자 체계적 고찰을 실시하였다. 연구방법 : 2000년 1월부터 2019년 5월까지 게재된 논문을 대상으로, Cochrane library, Embase, Ovid, Pubmed 데이터베이스를 이용하여 검색하였다. 총 6개의 논문을 최종 선택하여 연구의 근거 수준에 따라 질적 분석을 실시하였고, PICO(Patient, Intervention, Comparison, Outcome) 에 따라 정리하였다. 결과 : 최종 선정된 논문 중 4편이 근거 수준 1이고, 나머지는 근거 수준 3으로 나타났다. 부모교육의 기반이 되는 중재 접근법 중 행동치료가 기반이 되는 논문이 3편으로 가장 많았으며, 모두 근거 수준 1에 해당하였다. 결론 : 본 연구는 섭식 문제를 가진 자폐 스펙트럼 아동의 가정에 도움을 주는 다양한 방법의 부모교육을 알아봄으로써 임상에서 적용할 수 있는 부모교육의 방향을 모색하는 데 의의가 있다.

Patterns of Restricted and Repetitive Behaviors in Toddlers and Young Children with Autism Spectrum Disorder

  • Objectives: Restricted and repetitive behaviors (RRBs) are a core symptom in the diagnosis of autism spectrum disorder (ASD). The complexity of behavioral patterns has called for the creation of phenotypically homogeneous subgroups among individuals with ASD. The purpose of this study was 1) to investigate the different types of RRBs and 2) to explore whether subgroups created by RRBs would show unique levels of functioning in toddlers and young children with ASD. Methods: A total of 313 children with ASD, aged 12-42 months were included in the analysis. The Autism Diagnostic Interview-Revised was used to obtain information on the different types of RRBs by grouping 15 items into six categories. The Vineland Adaptive Behaviors Scale, a parent-reported questionnaire, was used to measure adaptive functioning. A portion of the children were analyzed separately for verbal-related RRBs based on their expressive language level. Two-step cluster analysis using RRB groups as features was used to create subgroups. Analysis of covariance while covarying for age and language was performed to explore the clinical characteristics of each cluster group. Results: Sensory-related RRBs were the most prevalent, followed by circumscribed interests, interest in objects, resistance to change, and repetitive body movements. A subset of the children was analyzed separately to explore verbal-related RRBs. Four cluster groups were created based on reported RRBs, with multiple RRBs demonstrating significant delays in adaptive functioning. Conclusion: Heterogeneity of RRBs emerges at a young age. The different patterns of RRBs can be used as valuable information to determine developmental trajectories with better implications for treatment approaches.

사회적 의사소통장애의 임상적 이해 (Clinical Implications of Social Communication Disorder)

  • Social (pragmatic) communication disorder (SCD) is a new diagnosis included under communication disorders in the neurodevelopmental disorders section of Diagnostic and Statistical Manual of Mental Disorders-5. SCD is defined as a primary deficit in the social use of nonverbal and verbal communication. SCD has very much in common with pragmatic language impairment, which is characterized by difficulties in understanding and using language in context and following the social rules of language, despite relative strengths in word knowledge and grammar. SCD and Autism Spectrum Disorder (ASD) are similar in that they both involve deficits in social communication skills, however individuals with SCD do not demonstrate restricted interests, repetitive behaviors, insistence on sameness, or sensory abnormalities. It is essential to rule out a diagnosis of ASD by verifying the lack of these additional symptoms, current or past. The criteria for SCD are qualitatively different from those of ASD and are not equivalent to those of mild ASD. It is clinically important that SCD should be differentiated from high-functioning ASD (such as Asperger syndrome) and nonverbal learning disabilities. The ultimate goals are the refinement of the conceptualization, development and validation of assessment tools and interventions, and obtaining a comprehensive understanding of the shared and unique etiologic factors for SCD in relation to those of other neurodevelopmental disorders.

자폐성장애 아동 어머니의 삶의 전환 과정 (Life Transition of Mothers of Children with Autism)

  • Purpose: While there are a number of studies on children with disabilities, there have been few studies on mothers of children with autism. The purpose of this study was to explore the process of life transition of mothers who have children with autism. Methods: From June 2007 to May 2009, the researcher interviewed 15 mothers of children with autism living in Seoul City, Gyeonggi or Chonbuk Provinces, and then analyzed the data gathered using the constant comparative method of grounded theory. Results: "Living together holding a string of fate" was a core category showing along the continuum of life. The basic social process of life transition encompassed 5 stages: stages of denying, wandering, devoting, mind controlling, and finally accepting. These five stages proceeded in phases, though returned back to the wandering stage occasionally. Conclusion: This study has opened the door to understanding how mothers of children with autism experienced life transition. The findings suggest that differentiated support and care at each stage should be given and there is the need to develop transition assessment tools for mothers of children with autism.

Autistic-like social deficits in hippocampal MeCP2 knockdown rat models are rescued by ketamine

  • Autism or autism spectrum disorder (ASD) is a behavioral syndrome characterized by persistent deficits in social interaction, and repetitive patterns of behavior, interests, or activities. The gene encoding Methyl-CpG binding protein 2 (MeCP2) is one of a few exceptional genes of established causal effect in ASD. Although genetically engineered mice studies may shed light on how MeCP2 loss affects synaptic activity patterns across the whole brain, such studies are not considered practical in ASD patients due to the overall level of impairment, and are technically challenging in mice. For the first time, we show that hippocampal MeCP2 knockdown produces behavioral abnormalities associated with autism-like traits in rats, providing a new strategy to investigate the efficacy of therapeutics in ASD. Ketamine, an N-Methyl-D-aspartate (NMDA) blocker, has been proposed as a possible treatment for autism. Using the MeCP2 knockdown rats in conjunction with a rat model of valproic acid (VPA)-induced ASD, we examined gene expression and ASD behaviors upon ketamine treatment. We report that the core symptoms of autism in MeCP2 knockdown rats with social impairment recovered dramatically following a single treatment with ketamine.

The association between preeclampsia and autism spectrum disorders among children: a meta-analysis

  • Purpose: In this meta-analysis, relevant case-control and cohort studies were pooled to evaluate the association between preeclampsia and the risk of autism spectrum disorders (ASDs) in children. Methods: A search for relevant studies in major databases, including Web of Science, PubMed, and Scopus, was performed up to May 2018. The odds ratios (ORs) or rate ratios (RRs) with 95% confidence intervals (CIs) were extracted from eligible studies to determine the association among studies. Results: The pooled estimates of ORs and RRs indicated a significant association between preeclampsia and ASD [(OR, 1.36; 95% CI, 1.12-1.60) and (RR, 1.30; 95% CI, 1.20-1.41)]. Conclusions: Despite existing controversy, our findings indicated that preeclampsia was associated with an increased risk of ASD among children.

수판을 이용한 자폐성 장애 학생의 수세기와 덧셈, 뺄셈의 지도 사례 (A Case Study on Lessons for Counting, Addition and Subtraction of Natural Number with Counting Board for Students with Autism Spectrum Disorder)

  • 본 연구는 자폐성 장애 학생들의 수판을 이용한 자연수의 수세기, 덧셈, 뺄셈의 지도 사례에 대한 분석을 바탕으로 장애 학생의 수와 연산 지도에 관한 시사점을 제공하고자 하였다. 이를 위해 일반학교의 특수학급에서 통합교육을 받는 4학년, 6학년의 자폐성 장애 학생을 대상으로 주당 1시간씩 30주간 수판을 사용하여 수세기, 덧셈, 뺄셈에 관한 수업을 실시하고 이를 분석하였다. 분석 결과를 바탕으로 다음과 같은 결론을 제시하였다. 자폐성 장애 학생들을 위한 수세기, 덧셈, 뺄셈의 지도에서 수의 구조가 드러나는 수판은 효과적인 교구이며, 수세기 전략과 연산 전략을 지도하는 것은 효율적인 지도 방안이 될 수 있고, 수학적 의사소통을 지도하는 것이 가능하다. 이러한 결과를 바탕으로 장애 학생의 수학 지도에 관한 시사점을 제시하였다.
