Outcomes into Adulthood of Survivors Born Either Extremely Low Birthweight or Extremely Preterm

  • We need to understand the outcomes into adulthood for survivors born either extremely low birthweight (ELBW; <1,000 g) or extremely preterm (EP; <28 weeks' gestational age), particularly their blood pressure and cardiovascular metabolic status,respiratory function, growth, psychological and mental health performance, and functional outcomes. Blood pressure is higher in late adolescence and early adulthood in ELBW/EP survivors compared with controls. In some studies, expreterm survivors have higher insulin and blood lipid concentrations than controls, which may also increase their risk for later cardiovascular disease. ELBW/EP survivors have more expiratory airflow obstruction than do controls. Those who had bronchopulmonary dysplasia (BPD) in the newborn period have even worse lung function than those who did not have BPD. As a group, they are unlikely to achieve their full lung growth potential, which means that more of them are likely to develop chronic obstructive airway disease in later life. Although they are smaller than term born controls, their weight gradually rises and ultimately reaches a mean z-score close to zero in late adolescence, and they ultimately attain a height z-score close to their mid-parental height z-score. On average, ex-preterm survivors have intelligence quotient (IQ) scores and performance on tests of academic achievement approximately 2/3 SD lower than do controls, and they also perform less well on tests of attention and executive function. They have similar high rates of anxiety and depression symptoms in late adolescence as do controls. They are, however, over-represented in population registries for rarer disorders such as schizophrenia and Autism Spectrum Disorder. In cohort studies, ex-preterm survivors mostly report good quality of life and participation in daily activities, and they report good levels of self-esteem. In population studies, they require higher levels of economic assistance, such as disability pensions, they do not achieve education levels as high as controls, fewer are married, and their rates of reproduction are lower, at least in early adulthood. Survivors born ELBW/EP will present more and more to health carers in adulthood, as they survive in larger numbers.

  • The objective of this review is to establish practice parameters for pharmacological treatment of children and adolescents with pervasive developmental disorders. We performed a detailed review of the literature, including a wide range of controlled clinical trials, open trials, case reports, and side-effect profiles of related drugs. Few medications have a treatment indication for pervasive developmental disorders, and few studies with well-controlled methodology are available for evaluating treatment results. Pharmacological treatments focus on associated target symptoms because symptom reduction may improve educational and social ability and enhance quality of life. Well-controlled trials have been conducted for some SSRI(selective serotonin reuptake inhibitor) antidepressants, risperidone, and methylphenidate, and showed reduction of some target symptoms. Since the medications are not specific to autism and do not treat core symptoms of the disorder, their potential side effects should be carefully considered. Family education is necessary to give proper information on target symptoms, limitation of drug treatments, and risks.

성인 발병성 스틸병(Adult-onset Still's disease)으로 오인된 전신홍반루푸스(Systemic Lupus Erythematosus) 1예 (A Case of Systemic Lupus Erythematosus Misdiagnosed as Adult-onset Still's Disease)

  • Adult-onset Still's disease (AOSD) is an inflammatory disorder that's characterized by daily, spiking high fever, arthritis and an evanescent, salmon-pink rash. AOSD is diagnosed purely on the basis of the typical clinical features of the illness. The symptoms commonly include swelling of the lymph nodes, enlargement of the spleen and liver, and a sore throat. AOSD is difficult to differentiate from systemic lupus erythematosus (SLE) due to the similar clinical manifestations. We report here on a case of a 16-year-old female patient with autism and epilepsy and who complained of daily spiking fever for 20 days. The patient had maculopapular skin rashes on the face and whole body and lymphadenopathy. The liver function tests were elevated mildly. The initial rheumatoid factor (RF) and antinuclear antibody (ANA) tests were negative. We diagnosed her as having adult-onset Still's disease according to the criteria of Yamaguchi. We successfully treated her with oral prednisolone. But her antinuclear antibody test was changed to positive after discharge. So we finally diagnosed her as having SLE.

  • 레트 증후군(Rett syndrome)은 1983년 Andreas Rett에 의해 처음으로 보고되었다. 여성에서 주로 호발하며 다양한 민족에서 발견되는 신경장애 중의 하나이다. 레트 증후군의 가장 특징적인 장애의 양상으로 실행증을 들 수 있으며 성장에 따라 집중해서 보는 듯한 눈을 보이기도 한다. 대부분의 레트 증후군은 자폐, 뇌성마비, 규정되지 않은 발달지체로 오진된다. 레트증후군의 원인은 유전적 요소가 있는 것으로 보이나 아직까지 확실히 밝혀지지 않았다. 본 증례에서 레트 증후군 환아는 과개교합으로 인한 상악 구개부의 손상을 주소로 내원하였다. 상악 구개부의 손상을 감소시키기 위해 교합판을 제작하여 만족할 만한 결과를 얻었기에 보고하는 바이다.

Prognosis following dental implant treatment under general anesthesia in patients with special needs

  • Background: This study retrospectively investigated outcomes following dental implantation in patients with special needs who required general anesthesia to enable treatment. Method: Patients underwent implant treatment under general anesthesia at the Clinic for the Disabled in Seoul National University Dental Hospital between January 2004 and June 2017. The study analyzed medical records and radiographs. Implant survival rates were calculated by applying criteria for success or failure. Results: Of 19 patients in the study, 8 were males and 11 were females, with a mean age of 32.9 years. The patients included 11 with mental retardation, 3 with autism, 2 with cerebral palsy, 2 with schizophrenia, and 1 with a brain disorder; 2 patients also had seizure disorders. All were incapable of oral self-care due to serious cognitive impairment and could not cooperate with normal dental treatment. A total of 27 rounds of general anesthesia and 1 round of intravenous sedation were performed for implant surgery. Implant placement was performed in 3 patients whose prosthesis records could not be found, while 3 other patients had less than 1 year of follow-up after prosthetic treatment. When the criteria for implant success or failure were applied in 13 remaining patients, 3 implant failures occurred in 59 total treatments. The cumulative survival rate of implants over an average of 43.3 months (15-116 months) was 94.9%. Conclusion: For patients with severe cognitive impairment who are incapable of oral self-care, implant treatment under general anesthesia showed a favorable prognosis.

정신적 장애인의 경제수준, 직업능력 및 취업욕구관련 분석 (Employment Needs of People with Mental Disabilities - Centering on Economic Status and Occupational Ability Variables)

  • 본 연구는 정신적 장애인의 경제수준과 직업능력에 따른 취업욕구를 살펴보고 그에 따른 직업재활 활성화 방안을 제안하고자 하였다. 분석자료는 2008년부터 2010년 까지 장애인 고용패널조사에 3년간 참여한 정신적 장애인 298명(가중빈도적용:154,348명)을 대상으로 하였다. 연구결과, 직업교육 참여희망은 남성일 때, 중졸 이상일 때, 자격증을 보유했을 때, 그리고 장애유형중 지적장애일 때 직업교육 참여희망이 높았다. 한편, 취업의사 관련요인은 남성일 때, 가구주일 때, 중등교육 이상 이수하였을 때, 자격증을 보유하고 있을 때, 육체활동 능력이 높아질 때, 그리고 장애유형 중 자폐장애일 때 취업의사가 높았다. 이러한 결과를 토대로 정신적 장애인의 경제활동 참여를 높이기 위하여 정신적 장애인의 의사를 반영한 직업재활 서비스제공 및 사업주 주도적 직업활동 등을 제안하였다.

  • 감성 컴퓨팅은 인간의 상호 작용에서 중요한 역할을 하기 때문에 인간을 인식하는 인공 지능을 통해 감정을 이해하고 식별한다. 우울증, 자폐증, 주의력 결핍 과잉 행동 장애 및 게임 중독과 같은 정신 질환을 잘 이해함으로써 감정과 관련된 문제들을 잘 관리할 수 있을 것이다. 이러한 문제들을 해결하기 위해 감정 인식을 위한 다양한 연구가 수행되었는데 기계학습을 적용하는데 있어서는 알고리즘의 복잡성을 줄이고 정확도를 향상시키기 위한 노력이 필요하다. 본 논문에서는 이러한 노력중의 하나로 Stack AutoEncoder (SAE)를 이용하여 차원을 감소하는 방법과 Long-Short-Term-Memory/Recurrent Neural Networks (LSTM / RNN) 분류를 이용한 감성 분류에 대해 연구한 결과를 제시한다. 제안된 방법은 모델의 복잡성을 줄이고 분류기의 성능을 크게 향상시킨 결과를 가져왔다.

Influence of Lead on Repetitive Behavior and Dopamine Metabolism in a Mouse Model of Iron Overload

  • Exposures to lead (Pb) are associated with neurological problems including psychiatric disorders and impaired learning and memory. Pb can be absorbed by iron transporters, which are up-regulated in hereditary hemochromatosis, an iron overload disorder in which increased iron deposition in various parenchymal organs promote metal-induced oxidative damage. While dysfunction in HFE (High Fe) gene is the major cause of hemochromatosis, the transport and toxicity of Pb in Hfe-related hemochromatosis are largely unknown. To elucidate the relationship between HFE gene dysfunction and Pb absorption, H67D knock-in Hfe-mutant and wild-type mice were given drinking water containing Pb 1.6 mg/ml ad libitum for 6 weeks and examined for behavioral phenotypes using the nestlet-shredding and marble-burying tests. Latency to nestlet-shredding in Pb-treated wild-type mice was prolonged compared with non-exposed wild-types (p < 0.001), whereas Pb exposure did not alter shredding latency in Hfe-mutant mice. In the marble-burying test, Hfe-mutant mice showed an increased number of marbles buried compared with wild-type mice (p = 0.002), indicating more repetitive behavior upon Hfe mutation. Importantly, Pb-exposed wild-type mice buried more marbles than non-exposed wild-types, whereas the number of marbles buried by Hfe-mutant mice did not change whether or not exposed to Pb. These results suggest that Hfe mutation could normalize Pb-induced behavioral alteration. To explore the mechanism of repetitive behavior caused by Pb, western blot analysis was conducted for proteins involved in brain dopamine metabolism. The levels of tyrosine hydroxylase and dopamine transporter increased upon Pb exposure in both genotypes, whereas Hfe-mutant mice displayed down-regulation of the dopamine transporter and dopamine D1 receptor with D2 receptor elevated. Taken together, our data support the idea that both Pb exposure and Hfe mutation increase repetitive behavior in mice and further suggest that these behavioral changes could be associated with altered dopaminergic neurotransmission, providing a therapeutic basis for psychiatric disorders caused by Pb toxicity.

Effects of Intraperitoneal N-methyl-D-aspartate (NMDA) Administration on Nociceptive/Repetitive Behaviors in Juvenile Mice

  • Dysregulation of excitatory neurotransmission has been implicated in the pathogenesis of neuropsychiatric disorders. Pharmacological inhibition of N-methyl-D-aspartate (NMDA) receptors is widely used to model neurobehavioral pathologies and underlying mechanisms. There is ample evidence that overstimulation of NMDA-dependent neurotransmission may induce neurobehavioral abnormalities, such as repetitive behaviors and hypersensitization to nociception and cognitive disruption, pharmacological modeling using NMDA has been limited due to the induction of neurotoxicity and blood brain barrier breakdown, especially in young animals. In this study, we examined the effects of intraperitoneal NMDA-administration on nociceptive and repetitive behaviors in ICR mice. Intraperitoneal injection of NMDA induced repetitive grooming and tail biting/licking behaviors in a dose- and age-dependent manner. Nociceptive and repetitive behaviors were more prominent in juvenile mice than adult mice. We did not observe extensive blood brain barrier breakdown or neuronal cell death after peritoneal injection of NMDA, indicating limited neurotoxic effects despite a significant increase in NMDA concentration in the cerebrospinal fluid. These findings suggest that the observed behavioral changes were not mediated by general NMDA toxicity. In the hot plate test, we found that the latency of paw licking and jumping decreased in the NMDA-exposed mice especially in the 75 mg/kg group, suggesting increased nociceptive sensitivity in NMDA-treated animals. Repetitive behaviors and increased pain sensitivity are often comorbid in psychiatric disorders (e.g., autism spectrum disorder). Therefore, the behavioral characteristics of intraperitoneal NMDA-administered mice described herein may be valuable for studying the mechanisms underlying relevant disorders and screening candidate therapeutic molecules.
