만성 상악동염이 중이기능에 미치는 영향 (IMPEDANCE AUDIOMETRY IN RELATION TO CHRONIC MAXILLARY SINUSITIS)

  • 만성 상악동염은 동점막의 복잡한 변화를 일으키고, 만성증으로 지속되는 비분비물 및 비점막의 염증이 이관에 파급되어 중이에 영향을 미치며 그 치유경과를 늦추게 한다. 저자는 만성 상악동염에서 동점막의 비후정도가 이관 및 중이에 미치는 영향을 알아 보고자, 1986년 1월부터 3월까지 이화여자대학교 의과대학 부속병원 이비인후과에 비폐색 및 비루를 주소로하여 내원한 환자중 만성 상악동염으로 진단된 16세 이상의 성인 환자를 대상으로 검사하였다. 동점막 비후정도에 따라 비정상군을 경도(2~3mm), 중등도(4mm이상), 고도(상악동 전체혼탁) 3군으로나누어 각각 40예를 조사군으로 하고 동점막의 두께가 0~1mm인 경우를 정상대조군으로 하여 40예를 GSI 28 AUTO TYMP를 사용하여 screening audiometry, tympanometry, acoustic reflex threshold (ipsilateral and contralateral)를 시행하여 다음과 같은 결과를 얻었다. 1) 남녀의 비는 1 : 2.6이고 21세에서 30세군이 53예(33.0%)로 가장 많았다. 2) 증상발현기간은 6개월 이내가 42.5%로 가장 많았다. 3) Tympanogram의 분포에서 비정상적 type으로 나오는 비율이 정상군과 비정상군중에서 경도군이 각각 22.5%이었고 중등도군이 30.0%, 고도군이 50.0% 순이었다. 4) 등골근반사역치는 정상군과 비정상군 사이에 차이가 없었다. 5) 등골근무반사율 역시 정상군과 비정상군 사이에 차이가 없었다.

Auditory Recognition of Digit-in-Noise under Unaided and Aided Conditions in Moderate and Severe Sensorineural Hearing Loss

  • Background and Objectives: The speech-in-noise test is typically performed using an audiometer. The results of the digit-in-noise recognition (DIN) test may be influenced by the flat frequency response of free-field audiometry and frequency of the hearing aid fit based on fitting rationale. This study aims to investigate the DIN test in unaided and aided conditions. Subjects and Methods: Thirty four adults with moderate and severe sensorineural hearing loss (SNHL) participated in the study. The signal-to-noise ratio (SNR) for 50% of the DIN test was obtained in the following two conditions: 1) the unaided condition, performed using an audiometer in a free field; and 2) aided condition, performed using a hearing aid with an unvented individual earmold that was fitted based on NAL-NL2. Results: There was a statistically significant elevation in the mean SNR for the severe SNHL group in both test conditions when compared with that of the moderate SNHL group. In both groups, the SNR for the aided condition was significantly lower than that of the unaided condition. Conclusions: Speech recognition in hearing-impaired patients can be realized by fitting hearing aids based on evidence-based fitting rationale rather than by measuring it using free-field audiometry measurement that is utilized in a routine clinic setup.

동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례 (Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.

어음청력점사방법과 한국어음청력검사표에 관한 연구 (The Study on Method of Speech Audiometry and the Korean Word Lists Part 1 : Normal Hearing Group)

  • 지금까지의 검사방법에는 순음청력검사, 어음청력검사, 자기청력검사, impedance audiometry등 여러가지 객관적청력검사방법이 있으며, 이들 가운데서 순음청력검사 및 어음청력검사가 기본검사로써 많이 이용되고 있음은 우리들이 주지하고 있는 터이지만 우리나라에 있어서의 어음청력검사에 관한 연구는 과거 20년간의 꾸준한 연구가 있었음에도 불구하고 아직까지 통일원 어음청력검사표가 없으므로써 각각의 어음청취역치와 명료도치 상호간의 정확한 비교가 되지 못하고 있을 뿐 아니라 이들의 검사방법 및 검사재료에 따른 계통적인 연구도 적었기 때문에 어때한 청사 방법과 어느 검사재료가 한국인의 어음청력검사에서 보다 적합한지를 모르고 있는 실정에 있다. 차제에 저자들은 한국에서의 통일된 어음청력검사표의 기준 및 검사법의 통일을 전제로해서 다음과 같이, 한국에서 통용되고 있으며, 대한청각학회에 제출되어 있는 5가지의 어음표를 사용하여 정상인에서 얻은 검사결과를 비교분석하여 다음과 같은 지견을 얻었다. A. 검사재료 및 방법 a. 어음검사표 1. 백준기어음표 (이하 "백 "이라고 약함) 2. 소진명어음표 (이하 "소"라고 약함) 3. 신규식어음표 (이하 "신"이라고 약함) 4. 이종담어음표 (이하 "이" 라고 약함) 5. 함태영어음표 (이하 "함"이라고 약함) b. 검사방법 1. 상행법 2. 하행법 c. 검사대상 1. 여자육성어음 2. 남자녹음어음 3. 여자녹음어음 B. 결 론 1. 상행법이 전검사에 있어서 하행법보다 검사치가 낮아 좋은 결과를 보였다. 2. 어음청취역치및 명료도치는 여자육성이 남자녹음 음에서 보다 낮았으나 여자녹음음에서 보다는 높았다. 3. 어음청취역치는 "함", "이", "소", "신", "백"순으로 수치가 낮았다. 4. 명료도치는 "신", "이", "소", "함", "백"의 순이였다. 5. 이상의 지견을 고찰할 때 우리나라에 있어서도 하루 속히 어음청력검사에 따른 기준치의 설정과 표준 어음청력검사표의 완성등 적절한 조치가 이루어져야할 것으로 촉구되었다.

이명환자의 순음청력검사에 대한 임상적 고찰 (Clinical Study for Tinnitus by Pure Tone Audiometry)

  • 이명은 청기질환의 중요한 증후로 청기질환의 조기 혹은 단독증상으로 존재할 때도 있으나 대부분의 경우에 있어서 난청을 수반하게 된다. 이명의 성립기전은 불분명하나 청기내 혹은 그 중추경로에의 이상자극에 의해 발생한다고 한다. 1978년 1월에서 1980년 12월까지 이명을 주소로 이대부속병원 이비인후과에 내원한 환자중 순음청력검사를 시행한 111명을 임상적 분석 및 고찰을 하였다. 1) 성별분포는 남자가 52.2%, 여자가 47.8%로 남자가 약간 많았고 연령은 21세에서 30세까지가 가장 많아 28.8%였다. 2) 발병기간은 1달에서 1년까지가 가장 많아 31.5%였다. 3) 이명만을 호소하였던 환자는 11명 (10.0%)였으며 대부분인 72%에서는 수반증상으로 난청을 호소하였다 4) 38.7%의 환자에서는 양측, 32.4%는 좌측, 28.9%는 우측의 이명을 호소하였다. 5) 고막소견은 48.0%에서는 정상고막소견을 보였으나 33.1%에서는 내함, 8.4%에서는 고막천공이 있었다. 6) 청력소실정도는 29.2%에서 정상이었고 22.7%에서는 중등고도의 청력소실이 있었다. 7) 청력상을 보면 33.1%에서 고음장애형이었고 수평형은 33.0%, 저음장애형은 11.9%였으며 $C_{5}$ dip은 11.0%에서 있었다. 8) 난청의 종류는 감각신경성난청이 가장 많아 46.9%, 혼합성난청이 33.1% 전도성난청이 20.1%였다.

돌발성 난청 환자의 경추부 추나요법 증례보고 (Sudden Hearing Loss Treated by Chuna Manual Therepy; A Case Report)

  • The purpose of this study is to report a clinical progress of sudden hearing loss through korean medicine. Between 26th Mar. 2014 and 19th Apr. 2014, she was treated with Chuna manual therapy every day and observed by Pure Tone Audiometry (before and after) and visual analogue pain scale (VAS). A patient was diagnosed with sudden hearing loss on 23th Dec. 2013 in Severance Hospital, she had sudden hearing loss on left side. She had 19 times treatments. At the first of treatment, her audibility was 20 dB on right side, 74 dB on left side and she had tinitus and ear fullness (VAS 8 / VAS 9). At the middle of treatment (2nd Apr. 2014), symptoms of tinitus and ear fullness started improving. State was tinitus VAS 6, ear fullness VAS 6. At the end of treatment (21th Apr. 2014), state was tinitus VAS 4, ear fullness VAS 4, and recorded 35 dB on right side, 80 dB on left side in pure tone audiometry. Chuna manual therapy is estimated to be good for sudden hearing loss especially on tinitus and ear fullness. More cases are required to develop treatment of sudden hearing loss.

Sensorineural hearing loss in patients with Kawasaki disease

  • Purpose: Kawasaki disease involves acute febrile systemic vasculitis that can cause a variety of symptoms by affecting various organs. Here, we aimed to evaluate the prevalence, causes, and prognosis of sensorineural hearing loss (SNHL) occurring in children with Kawasaki disease. Methods: Patients who were diagnosed with Kawasaki disease and received inpatient treatment in the Pediatrics Department at one of three university hospitals in Daegu city from February 2012 to September 2012 were enrolled in the study. The clinical features, hematological results, echocardiography results, audiometry results, and aspirin and salicylic acid serum levels of the patients were evaluated. Results: Of the 59 children enrolled in the study, three showed mild bilateral SNHL on audiometry tests conducted after 48 hours of defervescence; these patients demonstrated normal patterns of recovery on follow-up tests 8 weeks later. Aspirin serum levels were significantly higher in the SNHL group after 48 hours of afebrile condition with high dose aspirin intake (P=0.034). However, no significant differences were found in other laboratory tests or for fever duration (P>0.05). Upon echocardiography, coronary artery abnormality was observed in 9 cases, but none of these patients showed hearing loss. Conclusion: The results indicate that SNHL in children with Kawasaki disease might occur during treatment of the acute phase; this SNHL usually involves mild bilateral hearing loss and recovers naturally. However, this study suggests that determination of the causes and clinical implications of hearing loss in Kawasaki disease requires long-term follow-up studies with more cases.

Cortical Deafness Due to Ischaemic Strokes in Both Temporal Lobes

  • Cortical deafness is a clinical rarity whereby a patient is unresponsive to all types of sounds despite the preserved integrity of the peripheral hearing organs. In this study, we present a patient who suddenly lost his hearing following ischaemic infarcts in both temporal lobes with no other neurological deficits. The CT confirmed damage to the primary auditory cortex (Heschl's gyrus) of both hemispheres. Initially, the patient was unresponsive to all sounds, however, he regained some of the auditory abilities during 10 months follow up. Pure tone threshold improvement from complete deafness to the level of moderate hearing loss in the right ear and severe in the left was observed in pure tone audiometry. Otoacoustic emissions, auditory brainstem responses, and acoustic reflex findings showed normal results. The middle and late latency potential results confirmed objectively the improvement of the patient's hearing, however, after 10 months still, they were somewhat compromised on both sides. In speech audiometry, there was no comprehension of spoken words neither at 3 nor at 10 months. The absent mismatch negativity confirmed above mentioned comprehension deficit. The extensive auditory electrophysiological testing presented in this study contributes to the understanding of the neural and functional changes in cortical deafness. It presents the evolution of changes after ischaemic cerebrovascular event expressed as auditory evoked potentials starting from short through middle and long latency and ending with event-related potentials and supported by neuroimaging.
