Neck mass is a common manifestation from head and neck malignancy, most of which come from mucosal squamous cell carcinomas in the upper aero-digestive tract. However, once aspiration cytology suggests atypical malignant cells in the neck mass rather than metastatic squamous cell carcinomas, it is confusing to decide the adequate diagnostic work-ups and treatment planning. Here, we report a 29-year-old woman presenting with a growing neck mass mimicking malignancy of unknown origin, which was finally diagnosed as primary lymphoepithelial carcinoma in the parotid gland with multiple metastases to the lymph nodes. The patient underwent comprehensive neck dissection and total parotidectomy and the adjuvant radiation treatment was given. Our report highlight that the primary salivary gland cancer should be considered as the potential tumor origin in case of malignancy of unknown origin in the head and neck region and neck mass suggestive of atypical carcinomas.
1995년 겨울 경북 구룡포에 있는 축제식 양식장에서 사육중이던 넙치(평균 체장 : 31cm, 평균체중 : 300g)에 백점병이 발생하여 대량 폐사를 일으켰다. 감염 넙치로부터 병리조직학적, 원인충의 형태학적 및 생물학적 특성을 조사한 결과, 아가미 및 지느러미의 병리조직학적 검사에서 많은 섬모충들이 상피 하층으로 뚫고 들어감으로써 기생 부위의 상피 세포와 점액 세포가 증식을 일으킨 것을 볼 수 있었고 병어의 지느러미, 체표 및 아가미에서 작은 백점을 나타낸 충체는 기생성 섬모충 Cryptocaryon irritans의 특징을 갖추고 있었다. 그러나 이 충의 증식 방법은 다분열증식법 이외에도 출아와 다분열을 혼합한 또하나의 증식법(혼합 증식법)을 가진 것으로 조사되었으며 수온 $16^{\circ}C$에서는 위와 같은 두가지의 증식 방법이 약 3대 2의 비율로 나타났고 다분열 증식형의 충체는 8~14일만에, 혼합 증식형의 충체는 13~15일 만에 각각 자충을 방출하였다. 수온 $12\sim24^{\circ}C$의 조건에서 염분 농도 범위 $0\sim35\%_{\circ}$를 $5\%_{\circ}$ 간격으로 조절하여 충체의 생존능력을 살펴본 결과 다분열 증식형의 총체는 $12^{\circ}C$에서 $30\%_{\circ}$이하, $16^{\circ}C$에서 $20\%_{\circ}$ 이하, $20^{\circ}C$에서 $15\%_{\circ}$이하 그리고 $24^{\circ}C$에서 $25\%_{\circ}$ 이하에서 각각 폐사된 반면 혼합 증식형의 충체는 $12^{\circ}C$에서 $20\%_{\circ}$ 이하, $16\sim20^{\circ}C$에서 $15\%_{\circ}$ 이하 그리고 $24^{\circ}C$에서 $25\%_{\circ}$ 이하에서 각각 폐사를 나타내어 저수온($12\sim16^{\circ}C$)에서 혼합 증식형의 충체가 다분열 증식형의 충체보다 다소 낮은 염분 농도에서 생존이 가능함을 보여주었다. 수온 $12\sim24^{\circ}C$의 조건에서 탈낭 소요 시간 및 탈낭률은 다분열 증식형의 총체가 $12^{\circ}C$에서 8일 및 30%, $16^{\circ}C$에서 6.5일 및 50%, $20^{\circ}C$에서 5.5일 및 75% 그리고 $24^{\circ}C$에서 7일 및 10%를 나타낸 반면 혼합 증식형의 충체는 $12^{\circ}C$에서 15.5일, $16^{\circ}C$에서 14일 및 76.6%, $20^{\circ}C$에서 12일 및 72.2% 그리고 $24^{\circ}C$에서 10일 및 31.6%를 나타내어 모든 수온 조건에서 혼합 증식형의 충체가 다분열 증식형의 충체 보다 탈낭 소요 시간이 길며 수온 $16^{\circ}C$에서 최고의 탈낭율을 나타내어 저수온에 다소 안정적인 것을 보여주었다.
Objective: Cervix cancer is one of the most common gynecological cancer types that cause cancer deaths among women. This study was planned based on a descriptive method in order to evaluate the results of PAP smear screening during pregnancy for prevention of cancer. Materials and Methods: The research involved 110 pregnant women registered at the Obstetrics and Gynecology Polyclinic of Bagcilar Training and Research Hospital and 86 non-pregnant women of the same ages as a control group. As criteria for acceptance were conditions such as not being in coitus within the last 48 hours, not using vaginal ovule, and not performing vaginal lavage. A survey consisting of 33 questions was conducted and the results were processed using Bethesda. Results: The average ages were $27.1{\pm}4.70$ for the pregnant women and $28.8{\pm}4.24$ for the control group. 60.7% of cases had previously heard of a PAP smear test, 49% were aware of why PAP smear tests were conducted, 26.4% of pregnant participants and 27.3% of non-pregnant participants had previously undergone a smear test. In this study, smear results of all cases were 95.4% sufficient. 18.2% of pregnant cases had an infection, 54.5% had reactive cellular change, and 0.9% had atypical squamous cells of undetermined significance (ASC-US). 16.3% of non-pregnant cases had an infection, 58.1% had reactive cellular change, 3.5% had atypical squamous cells of undetermined significance (ASC-US), and 1.2% had low-grade squamous intraepithelial lesions (LGSIL). Conclusion: PAP smear test is a good opportunity to identify pre-invasive lesions in early phases of pregnancy.
Background: Detection of cervical high grade lesions in patients with atypical squamous cells of undetermined significance (ASCUS) is still a challenge. Our study tested the efficacy of the paired boxed gene 1 (PAX1) methylation analysis by methylation-sensitive high-resolution melting (MS-HRM) in the detection of high grade lesions in ASCUS and compared performance with the hybrid capture 2 (HC2) human papillomavirus (HPV) test. Materials and Methods: A total of 463 consecutive ASCUS women from primary screening were selected. Their cervical scrapings were collected and assessed by PAX1 methylation analysis (MS-HRM) and high-risk HPV-DNA test (HC2). All patients with ASCUS were admitted to colposcopy and cervical biopsies. The Chisquare test was used to test the differences of PAX1 methylation or HPV infection between groups. Results: The specificity, sensitivity, and accuracy for detecting CIN2 + lesions were: 95.6%, 82.4%, and 94.6%, respectively, for the PAX1 MS-HRM test; and 59.7%, 64.7%, and 60.0% for the HC2 HPV test. Conclusions: The PAX1 methylation analysis by MS-HRM demonstrated a better performance than the high-risk HPV-DNA test for the detection of high grade lesions (CIN2 +) in ASCUS cases. This approach could screen out the majority of low grade cases of ASCUS, and thus reduce the referral rate to colposcopy.
A Pap smear is the most important screening test for the diagnosis of cervical cancer. However, subjective judgment by the operator cannot be excluded, and replicability may greatly be reduced if uncertain specimens are examined. Examiners often experience difficulties in differentiating atrophy with inflammatory changes and ASCUS when diagnosing squamous epithelial lesions from a pap smear. Reports often vary between cytologists and pathologists, and misdiagnosis may result in delayed follow-ups and advanced diseases. Hence, auxiliary examinations are necessary when confusing results between atrophy and ASCUS are obtained. The importance of p16INK4a activation due to HPV infection, which is an important factor in the outbreak of cervical cancer, has been highlighted. Recent studies have reported that p16INK4a immunocytochemical staining and HPV high-risk type tests using liquid-based cervical specimens are effective to detect the presence of lesions of grade HSIL or higher in patients with ASC-H. However, no research exists on the utility of HPV and p16INK4a tests on the differential diagnosis of atrophy and ASCUS. This study focused on whether p16INK4a immunocytochemical staining and HPV tests can help diagnose borderline lesions between atrophy and ASCUS. The results reported that p16INK4a activation can significantly (P<0.001) differentiate atrophy from ASCUS in atrophic lesions infected with High risk-HPV. Therefore, it may be concluded that p16INK4a immunocytochemical staining is an effective auxiliary test in lesions infected with HR-HPV when atrophic lesions are difficult to differentiate by morphology. Such results are expected to help decide on adequate follow-up and treatment.
To study the differentiating cytomorphological features of Hodgkin lymphoma (HL) and anaplastic large cell lymphoma (ALCL) using fine needle aspiration cytology (FNAC), cytomorphological features of 16 patients with HL (n=8) or ALCL (n=8) were analyzed. In the initial cytological diagnosis prior to biopsy, HLs were properly diagnosed in 4 out of 8 cases (4 HL, 2 atypical, 2 benign), whereas all ALCL were diagnosed as malignancies. However, correct diagnosis of non-Hodgkin lymphoma (NHL) was made in only two ALCL patients (2 NHL, 1 HL, 1 sarcoma, 4 malignancy without specific type). Overall, the percentage of large abnormal cells ranged from 30% to 90% in ALCL except for one case, whereas it was less than 5% in all 8 HL. A spectrum of atypical cells was more characteristic of ALCL. In contrast, HL showed an sharp difference between reactive lymphoid cells and neoplastic ones (bimorphic pattern). Moreover, the emergence of kidney-shaped abnormal cells or wreath-like multinucleated cells was helpful in diagnosing ALCL. The combination of thesefeatures would be useful in differentiating HL and ALCL. Nevertheless, these two types of lymphomas cannot be definitely distinguished based on cytomorphological features alone. Therefore, the aim of FNAC would be to suggest a specific diagnosis and indicate the need for a biopsy.
Apocrine carcinoma is a rare form of breast malignancy and is composed of entirely or predominantly of apocrine type epithelial cells. Apocrine metaplastic cells are frequently noted in fine needle aspiration cytology(FNAC) of breast lesions, especially fibrocystic disease. These apocrine cells may occasionally be atypical, to make a diagnostic difficulty. Two cases of apocrine carcinoma of the breast diagnosed by FNAC are described, and differential cytologic points between apocrine metaplasia and apocrine carcinoma are discussed. The first case is a right breast mass of a 37-year-old woman for 20 days. The smears show many single or sheets of large cells on bloody background. Each cell has a large vesicular nuclei with multiple macronucleoli and abundant eosinophilic granular cytoplasm. The second case is a left breast mass of a 35-year-old woman for one month. The smears show similar findings as seen in the first one. Histopathologic findings of both cases are typical of apocrine carcinoma. Electron microscopy demonstrates variable numbers of large osmiophilic granules in diameter of $200\sim600nm$ in both cases.
The effect of Roentgen rays on carcinoma of the cervix has long been of great interest to both radiologists and gynecologists. Since most cervical carcinomas are treated by irradiation, any additional knowledge either concerning the radiosensitivity of cervical tumors or their ultimate prognosis would be of value. The vaginal smear is considered to be one of convenient and rapid methods to study the effects of radiation on cervical malignancy. We observed morphologic changes in 297 cytologic preparations obtained from 60 patients who had underwent irradiation for cancer of the cervix. With the morphologic parameters such as cytoplasmic vacuolization, cytoplasmic basophilia, multinucleated giant cell formation, polymorphonuclear leucocytes (PMNL) sticking and postradiation dysplasia, we analyzed the findings in relation to the follow up time interval. The most common effect was the cytoplasmic vacuolization with basophilia of basaloid cells, which were noted in more than 90% of followed patients. The multinucleated giant ceil formation and PMNL stickering were noted in 38 cases(63%) and 48 cases(80% ) respectively. The differential diagnosis of postradiation dysplasia from recurrent or persistent carcinoma, reparative atypical cells, and regressing tumor cells was difficult and further study seems to be needed to clarify the more accurate morphologic features and biologic behavior.
The cytologic findings of endodermal sinus tumor of the ovary are reported. The cytologic preperations were obtained from ascitic specimens. The findings on Papanicolaou-stained smears included a clean background and poorly preserved atypical cells loosely arranged in irregular or papillary groups. At high magnification, the cells had ill-defined and microvacuolated cytoplasms, with an increased nuclear-cytoplasmic ratio and prominent nucleoli. Hyaline globules characteristic of the alpha-fetoprotein (AFP)-synthesizing cells of endodermal sinus tumor were observed within the cells with periodic-acid-Schiff(PAS) stain. The presence of PAS-positive hyaline globules can be regarded, therefore, as a diagnostic clue to endodermal sinus tumor in an appropriate clinical setting and in the presence of AFP production. The cytologic findings in a cell block is important, in addition to smears, to obtain more diagnostic clues. A preliminary cytomorphologic diagnosis of this highly malignant tumor is valuable as a guide in planning further immunocytochemical and serologic studies.
A case of multiple myeloma with massive pleural effusion is reported. A 53 year-old previous known multiple myeloma patient vistited our hospital complaining of cough with sputum. Radiologic study revealed multiple osteolytic bony lesions and left side pleural effusion. The effusion were bloody exudates containing numerous atypical plasma cells. The tumor cells showed pleomorphism, eccentric nuclei, prominent nucleoli, perinuclear halo, multincleation, and chromatin patterns of occasional cart-wheel appearance. The cytological examination of pleural fluid established the malignant nature of the effusion with multiple myeloma.
