진행성 반안면위축환자의 치험례 (A Case Report of Progressive Hemifacial Atrophy)

  • A progressive hemifacial atrophy is characterized by progressive atrophy of subcutaneous fat and rarely muscle and bone. Its contour follows the underlying muscle. Unilateral involvement is common. The treatment goal has been focused on the augmentation of the soft tissue. Many materials such as implants, collagen, fat graft, fat injection, dermal fat graft, filler and vascualized autogenous graft have been used. Although these materials have been used, the best treatment hasn't been achived. In severe cases underlying soft tissue, muscle and bone may be atrophied and massive soft tissue graft, implant and orthognathic surgery must be used. The author used the dermal-fat tissue for the pupose of soft tissue augmentation. We can get the massive soft tissue by the dermolipectomy procedure through the mini-abdominoplsty. The facial augmentation was done by augmentation of the dermal-fat tissue. The progressive hemifacial atrophy is hard to treat by only one procedure and many modalites must be considered.

  • Object : This study was designed to evaluate the effects of oriental medicine therapy on a renal atrophy Patient. Methods : The patient was hospitalized from July 31. 2009 to Aug 27. 2009. Patient was treated with herbal medicine, acupuncture and moxa therapy. Result & Conclusion : After the treatment, the symptoms and IPSS, DITI results improved. From These results suggest that oriental medicine is an effective treatment for renal atrophy Patient. But more clinical case reports are needed.

  • Objectives This case is about a Soyangin cerebellar atrophy patient. In this study, we report the effect of Sasang Constitutional treatment to this patient. Methods This patient was treated by Soyangin's constitutional medications and acupuncture according to the result of Sasang Constitutional diagnosis. We used Visual Analogue Scale(VAS) and Equilibrium Function Test. Results and Conclusions This patient's chief complaints were dizziness. By using Hyeungbangjihwang-tang, she showed positive response about her symptom. This study shows that Hyeungbangjihwang-tang has effect to cerebellar atrophy in Soyangin.

p38 MAPK Participates in Muscle-Specific RING Finger 1-Mediated Atrophy in Cast-Immobilized Rat Gastrocnemius Muscle

  • Skeletal muscle atrophy is a common phenomenon during the prolonged muscle disuse caused by cast immobilization, extended aging states, bed rest, space flight, or other factors. However, the cellular mechanisms of the atrophic process are poorly understood. In this study, we investigated the involvement of mitogen-activated protein kinase (MAPK) in the expression of muscle-specific RING finger 1 (MuRF1) during atrophy of the rat gastrocnemius muscle. Histological analysis revealed that cast immobilization induced the atrophy of the gastrocnemius muscle, with diminution of muscle weight and cross-sectional area after 14 days. Cast immobilization significantly elevated the expression of MuRF1 and the phosphorylation of p38 MAPK. The starvation of L6 rat skeletal myoblasts under serum-free conditions induced the phosphorylation of p38 MAPK and the characteristics typical of cast-immobilized gastrocnemius muscle. The expression of MuRF1 was also elevated in serum-starved L6 myoblasts, but was significantly attenuated by SB203580, an inhibitor of p38 MAPK. Changes in the sizes of L6 myoblasts in response to starvation were also reversed by their transfection with MuRF1 small interfering RNA or treatment with SB203580. From these results, we suggest that the expression of MuRF1 in cast-immobilized atrophy is regulated by p38 MAPK in rat gastrocnemius muscles.

새로운 뇌 위축 동물 모델과 그 모델에서의 고려인삼의 보호 효과 (Novel animal model for brain atrophy and protective effects of Korean ginseng)

  • Objectives: Anti-oxidants are known to prevent neuronal diseases with pathological and physiological changes such as the brain atrophy and cognitive impairment. This study was designed to investigate the protective effects of Korean ginseng on the oxidative stress induced pathologic changes, and develop new animal model for the brain atrophy. Korean ginseng has anti-oxidant, anti-aging, and protective effects on the brain ischemia. Methods : The intracerebroventricular (ICV) hydrogen peroxide ($H_2O_2$) injection into mice was conducted to generate oxidative stress. Results : The ICV $H_2O_2$ (1 M, $5\;{\mu}l$ injection did not induce either convulsion or death in the acute phase. At the end of second week, cognitive impairment and pathologic change of the brain were observed. The massive brain atrophy was found in the $H_2O_2-injected$ mice, especially in the hippocampus and thalamus. Treatment with Korean ginseng showed a protective effect against the brain atrophy. The $H_2O_2$ injected mice revealed cognitive impairment in the passive avoidance test, and Korean ginseng alleviated cognitive impairment. Conclusion : The results indicate that Korean ginseng has a protective effect on the oxidative stress-induced neuronal damages.

갈근(葛根)이 좌골신경 손상 흰쥐의 후지 근육위축에 미치는 영향 (Effect of Puerariae Radix on Hind Limb Muscle Atrophy of Sciatic Nerve Transectioned Rats)

  • This study evaluated the effects of Puerariae Radix on the skeletal muscle atrophy, Muscle atrophy was induced by the sciatic nerve transection in Sprague-Dawley rats, then aqueous-extract of Puerariae Radix was administered for 12 days, Muscle wet weight was measured in soleus, plantaris, and medial gastrocnemius. Muscle fiber type was classified by MHCf immunohistochemistry. Muscle fiber type proportion and cross section area of muscle fiber also was observed in medial gastrocnemius. Bax and Bcl-2 expressions in medial gastrocnemius of the damaged hind limb were evaluated with immunohistochemistry. The results are as follows; Puerariae Radix attenuated muscle atrophy in soleus of the sciatic nerve transectioned rats, but there was statistic significance. Puerariae Radix attenuated significantly atrophy in plantaris at 12 days and in medial gastrocnemius at 8 days and 12 days. Puerariae Radix improved histology of the atrophic changes and increased significantly cross section areas of type-I and type-II muscle fibers in medial gastrocnemius of the sciatic nerve transectioned rats. Puerariae Radix did not affect to muscle fiber type proportion in medial gastrocnemius of the sciatic nerve transectioned rats. Puerariae Radix attenuated significantly Bax positive nuclei but did not affect to Bcl-2 positive muscle fibers in medial gastrocnemius of the sciatic nerve transectioned rats.According to above results, Puerariae Radix may have an anti-atrophy effect on the denervated skeletal muscle through anti-apoptotic effects on muscle fibers.

Dexamethasone으로 유도된 근위축 세포모델에서 glucoraphanin의 효과 (Effects of glucoraphanin in dexamethasone-induced skeletal muscle atrophy in vitro model)

  • Objectives : Glucoraphanin is one of the well-known natural glucosinolates found in cruciferous plants. In the present study, we investigated the effects and molecular mechanism of glucoraphanin in dexamethasone-induced skeletal muscle atrophy in vitro model. Methods : The cytotoxic effects of glucoraphanin on C2C12 myoblasts or myotubes were evaluated by MTT assay. The glucoraphanin was evaluated effects in dexamethasone-induced skeletal muscle atrophy in C2C12 myotubes using a real-time PCR, western blots analysis, and immunofluorescence staining of myosin heavy chain. Result : Glucoraphanin had no cytotoxicity on both C2C12 myoblasts or myotubes. Dexamethasone markedly induced muscle atrophy by up-regulating muscle-specific ubiquitin E3 ligase markers, atrogin-1 and MuRF1, and down-regulating MyoD, a myogenic regulatory factor whereas co-treatment of glucoraphanin and dexamethasone dose-dependently inhibited it. Furthermore, decreased expressions of p-Akt, p-FOXO1, and p-FOXO3a induced by dexamethasone were reversed by co-treatment with glucoraphanin and dexamethasone. In addition, dexamethasone obviously reduced myotube diameters, while co-treatment of glucoraphanin and dexamethasone increased those to a similar level as control. Conclusions : These results show that glucoraphanin suppresses dexamethasone-induced muscle atrophy in C2C12 myotubes through activation of Akt/FOXO signaling pathway.

Schisandrae Fructus: A Potential Candidate Functional Food Against Muscle Atrophy and Osteoarthritis Prevention

  • Muscle atrophy, known as a sarcopenia, is defined as a loss of muscle mass resulting from a reduction in muscle fiber area or density due to a decrease in muscle protein synthesis and an increase in protein breakdown. Many conditions are associated with muscle atrophy, such as aging, denervation, disuse, starvation, severe injury and inflammation, prolonged bed rest, glucocorticoid treatment, sepsis, cancer, and other cachectic diseases. On the other hand, osteoarthritis (OA) is the most common form of joint disease and is wide spread in the elderly population and is characterized by erosion of articular cartilage, osteophyte formation, and subchondral bone sclerosis. The cytokine network plays an important role in the development and progression of OA with the inflammatory cytokine. Schisandrae Fructus (SF) derived from the ripe fruit of Schisandra chinensis (Turcz.) Baill. (Magnoliaceae) has been extensively used in traditional herbal medicines in Asia. It was originally used as a tonic and has been traditionally used for the treatment of many uncomfortable symptoms, such as cough, dyspnea, dysentery, insomnia, and amnesia for a long time. Previous reports have shown that SF and its related compounds possess various biological activities such as antioxidant, anti-inflammatory, anticancer, anti-microbial, antiseptic, anti-aging, hepatoprotective and immunostimulating effects. However, the therapeutic effects of SF on muscle atrophy and OA has not yet been evaluated. In the present study, we aimed to determine whether extracts of SF, the dried fruit of S. chinensis, mitigates the development of muscle atrophy and OA.

자기공명영상 (Magnetic Resonance Image)을 통한 요통 환자의 다열근 위축에 대한 후향적 연구 (The Retrospective Study on the Correlation between the Multifidus Muscle Atrophy on Low Back Pain Patients and the Magnetic Resonance Images)

  • Objectives : In the assessment of the lumbar spine by magnetic resonance imaging (hereinafter, "MRI"), changes in the paraspinal muscles are overlooked. The purpose of our study is to examine the correlation between the multifidus muscle atrophy on MRI findings and the clinical findings in low back pain (hereinafter, "LBP") patients. Methods : The retrospective study on 38 LBP patients, presenting either with or without associated leg pains, was undertaken. The MRI findings on the patients were visually analysed to find out a lumbar multifidus muscle atrophy, disc herniation, disc degeneration, spinal stenosis and nerve root compressions. The clinical history in each case was obtained from their case notes and pain drawing charts. Results : The lumbar multifidus muscle atrophy has occurred from more than 80% of the patients with LBP. Most of lumbar multifidus muscle atrophies have increased from lower level of lumbar spine. It was bilateral in the majority of the cases. In addition, multifidus muscle atrophy was correlated to the patient's age, disc degenerations and spinal stenosis. On the contrary, gender, the duration of LBP, referred leg pain, disc herniation and nerve root compressions had no relevance to multifidus muscle atrophies. Therefore, when assessing the MRIs of the lumbar spine, we should have more attetion on multifidus muscle, because it has lot's of information about spinal neuropathy problems. Conclusions : Therefore, the examination of multifidus muscle atrophies should be considered when assessing the MRIs of the lumbar spine. In addition, it helps to evaluate and plan the treatment modalities of LBP. Moreover, it prevents from LBP by discovering the importance between the multifidus muscle and the spine stabilization exercise.

Analysis of location and prevalence of maxillary sinus septa

  • Purpose: The sinus lift procedure requires detailed knowledge of maxillary sinus anatomy and the possible anatomical variations. This study evaluated the location and prevalence of maxillary sinus septa using computed tomography (CT). Methods: This study was based on the analysis of CT images for posterior maxilla which were obtained from patients who visited Chonbuk National University Dental Hospital during the period of June 2007 to December 2008. With the exclusion of cases presenting any pathological changes, 236 maxillary sinuses in 204 patients were retrospectively analyzed. The average age of the patients was 50.9. The cases were divided into two groups, an atrophy/edentulous segment and a non-atrophy/dentate segment, and maxillary sinus septa of less than 2.5 mm were not taken in-to consideration. The location of septa was also divided for analysis into 3 regions: the anterior (1st and 2nd premolar), middle (1st and 2nd molar) and posterior (behind 2ndmolar) regions. Results: In 54 (20.9%) of the 204 patients there were pathologic findings, and those patients were excluded from the analysis. Sinus septa were present in 58 (24.6%) of the 236 maxillary sinuses and in 55 (27%) of the 204 total patients. In the atrophy/ edentulous ridge group (148 maxillary sinuses), 41 cases (27.7%) were found, and 17 cases (19.3%) were found in the non-atrophy/ dentulous ridge group (88 maxillary sinuses). In terms of location, septa were found in 18 cases (27.3%) in the anterior, in 33 cases (50%) in the middle and in 15 cases (22.7%) in the posterior regions. Conclusions: In the posterior maxilla, regardless of type of ridge (atrophy/edentulous or non-atrophy/dentate), the anatomical variation of sinus septa is diverse in its prevalence and location. Thus, accurate information on the maxillary sinus of thepatient is essential and should be clearly understood by the surgeon to prevent possible complications during sinus lifting.
