BACKGROUND/OBJECTIVES: Arterial stiffness and endothelial dysfunction are 2 of the independent predictors for cardiovascular disease, while Acanthopanax senticosus Harms (ASH) is a traditional medicinal plant that can improve cardiovascular health. This study aimed to investigate the efficacy of the fruit of ASH on vascular function in apparently healthy subjects. SUBJECTS/METHODS: A 12-week, randomized, double-blind, placebo-controlled design, consisting of healthy adults with at least 2 of the following 3 conditions: borderline high blood pressure (BP; 120 mmHg ≤ systolic BP ≤ 160 mmHg or 80 mmHg ≤ diastolic BP ≤ 100 mmHg), smoking (≥10 cigarettes/day), and borderline blood lipid levels (220 ≤ total cholesterol ≤ 240, 130 ≤ low density lipoprotein cholesterol ≤ 165, or 150 ≤ triglyceride ≤ 220 mg/dL). Randomly assigned 76 subjects who received a placebo or 2 doses of ASH fruit (low, 500 mg/day; high, 1,000 mg/day) completed the intervention. Brachial-ankle pulse wave velocity (baPWV), flow-mediated dilation, carotid intima-media thickness, and BP were measured both at baseline and following the 12-week intervention. Endothelial nitric oxide synthase (eNOS) phosphorylation was assessed by western blotting. RESULTS: Compared with the placebo group, the low-dose group showed more significant changes after the 12-week intervention period in terms of systolic BP (0.1 vs. -7.7 mmHg; P = 0.044), baPWV (31.3 vs. -98.7 cm/s; P = 0.007), and the ratio of phospho-eNOS/eNOS (0.8 vs. 1.22; P = 0.037). CONCLUSIONS: These results suggest that ASH fruit extract at 500 mg/day has the potential to improve BP and arterial stiffness via endothelial eNOS activation in healthy adults with smoking and the tendency of having elevated BP or blood lipid parameters.
노화로 인한 신체조성과 혈관탄성의 변화는 심혈관질환에 위험을 증가시킨다. 하지만 리듬운동은 노인들이 쉽고 재미있게 할 수 있는 유산소 운동으로 신체조성과 혈관탄성에 긍정적인 영향을 미칠 수 있을 것이다. 그리하여 본 연구는 고령여성 (>65 years) 대상으로 12주간 리듬운동을 실시한 후 신체조성 및 혈관탄성에 어떠한 효과를 미치는지에 관한 연구이다. 실험 참가자들 (n=20)은 무선할당 방식으로 운동집단 (n=10)과 통제집단 (n=10)으로 나누었다. 12주간에 리듬운동은 주3회 한 회당 60분으로 구성되었고 운동강도는 Borg의 운동자각도(6 - 20)에서 11-14로 설정되었다. 신체조성(근육량과 체지방량), 혈관탄성(맥파속도 (Pulse wave velocity (PWV))은 훈련 전과 후에 측정이 되었다. 연구 결과는 신체조성에서 운동집단의 골격근량 (p=0.04)이 유의하게 증가하였고 혈관탄성인 PWV에 오른쪽 (p=0.002)과 왼쪽 (p=0.02)모두 유의하게 감소하였지만 통제집단에서 골격근량과 맥파속도 (오른쪽과 왼쪽)에 유의한 변화가 없었다. 이러한 변화를 분석한 결과를 통하여 볼 때 여성노인들이 흥미를 가지고 쉽게 따라할 수 있는 리듬운동은 근육량을 증가시키고 혈관탄성에 긍정적인 효과를 주어서 근감소증의 예방과 지연 그리고 심혈관 질환의 위험성을 낮출 수 있는 매우 효과적인 운동이라 할 수 있다.
에이니그마타이트($Na_4(Fe^{2+},Ti,Fe^{3+}){_{12}}(Fe^{3+},Si){_{12}}O_{40}$)는 Na가 풍부한 알칼리 화성암에 흔히 산출되는 광물이며, 분지 개방형 단쇄형 규산염광물로 분류된다. 3개의 산출이 다른 자연산 에이니그마타이트를 대상으로 뫼스바우어 분광분석을 실시하였으며 상세한 결정화학적 연구가 수행되었다. 뫼스바우어 분광분석 결과 9개의 흡수선이 분리되었는데, 3쌍의 흡수선은 팔면체 자리를 점유하는 $Fe^{2+}$의 피크이며, 저속도 구간의 독립적인 2개의 흡수선과 대응하는 한 개의 중첩된 흡수선은 각각 사면체와 팔면체 자리의 $Fe^{3+}$의 피크로 밝혀졌다. $Fe^{2+}$와 $Fe^{3+}$의 정확한 함량비에 의해 화학분석치가 재계산되었으며, Fe 양이온의 사면체 자리와 팔면체 자리에 대한 정확한 자리점유율로 상세한 결정화학적 정보를 제공하였다. 상당한 함량의 $Fe^{3+}/tet$는 사면체 자리점유에 있어 $Fe^{3+}$가 $Al^{3+}$보다 우위를 보여준다. 결정화학적로 규명된 에이니그마타이트(AEN1)의 상세한 화학조성은 $(Na_{3.97}Ca_{0.03})(Ca_{0.11}Mn_{0.59}Fe^{2+}{_{8.07}}Ti_{2.07}Mg_{0.70}Fe^{3+}{_{0.43}}Al_{0.04})(Fe^{3+}{_{0.56}}Al_{0.18}Si_{11.26})O_{40}$으로 밝혀졌다.
본 연구는 슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향을 알아보고자 실시하였다. 대상자는 무릎 관절 전치환술을 받고 입원 중인 환자 20명이며, 수술 후에 CPM (Continuous passive motion) 만을 적용하는 집단 (CPM group; CG)과 CPM과 슬링을 이용한 능동 운동프로그램을 병행하는 집단 (CPM with active exercise program using sling group; CSG)으로 10명씩 무작위로 배치되었다. CG는 CPM을 주 5일, CSG는 CPM을 주 2일, 슬링을 이용한 능동 운동프로그램을 주 3일 실시하였고, 각 중재는 하루 40분씩 총 4주간 실시하였다. 통증은 VAS(Visual analog scale)을 이용하여 평가하였고, 균형은 BT4 (Balance training 4)를 이용하여 C90 area, Trace length, Sway average velocity를 눈을 뜬 상태와 감은 상태로 측정하였다. 그 결과, 두 집단 모두 통증에서 집단 내에서 유의하게 감소하였고, 집단 간의 변화량에서도 유의한 차이가 있었다. 균형에서 CG의 C90을 제외한 모든 변인이 중재 후 유의한 변화가 있었으며, 집단 간 변화량에서는 눈을 감은 상태의 C90과 Vel에서 유의한 차이가 있었다. 따라서 CPM과 슬링을 이용한 능동 운동프로그램을 함께 시행하는 것은 무릎 관절 전치환술 환자의 통증 감소와 균형 향상에 효과적인 중재라고 생각한다.
The purpose of this study was to determine the effect of gait initiation training on gait and center of pressure (CoP) during gait initiation in stroke patients. Twenty-three subjects were randomly assigned to either an experimental group (EG) or a control group (CG). The EG received gait initiation training with increased CoP posterior distances the maximum the rear on gait training. The CG received general gait training. Both groups received training three times a week over a period of four consecutive weeks. The figures for CoP distances the maximum the rear, CoP distances time the mover the maximum the rear, the Tinetti Performance-Oriented Mobility Assessment (POMA), and gait velocity were recorded both before and after the training sessions for both groups. The EG's results for CoP distances the maximum the rear, CoP distances time the mover the maximum the rear, and POMA improved after training (p<.05). In terms of the rate of change of CoP distances the maximum the rear, the EG demonstrated a significantly higher increase (p<.05) than did the CG. The results of this study suggest that increased CoP distances the maximum the rear affect the gait initiation and gait performance of stroke patients. Further studies with a larger sample size are necessary to verify the accuracy of the results of this study.
PURPOSE: Gait disturbances in patients with hemiplegic stroke involve asymmetry of stance time. Step box training is used to supplement the limitations of stair walking training and increasing the torque value of the paralyzed lower leg's strength. This study aimed to investigate whether step-up training on unstable support could change walking ability in patients with chronic stroke. METHODS: Thirty stroke patients were randomly assigned to the step-up training group (experimental group), that performed training on an unstable surface, and the control group, that performed training on a stable surface. Walking speed, step length, and cadence were measured before and after training. Paired t-tests were used to compare pre- and post-intervention data, while the independent samples t-test was used to determine intergroup differences. Values of p < .05 were considered statistically significant. RESULTS: There was a significant difference in walking ability before versus after the intervention in both groups, although the experimental group showed greater differences than the control group (walking velocity by 8.1%; step length of the non-paralyzed side by 6.9%, respectively; p<.05). CONCLUSION: Step-up training might be more effective on an unstable surface than on a stable surface for increasing walking speed and step length of the non-paralyzed side.
Objective: Robot assisted gait training is implemented as part of therapy for the recovery of gait patterns in recent clinical fields, and the scope of implications are continuously increasing. However clear therapy protocols of robot assisted gait training are insufficent. The purpose of this study was to investigate the effects of robot-assisted gait training applied with guidance force on balance and gait performance in persons with hemiparetic stroke. Design: Two group pre-test post-test design. Methods: Nineteen persons were diagnosed with hemiparesis following stroke participated in this study. The participants were randomly assigned to the unilateral guidance group or bilateral guidance group to conduct robot-assisted gait training. All participants underwent robot-assisted gait training for twelve sessions (30 min/d, 3 d/wk for 4 weeks). They were assessed with gait parameters (gait velocity, cadence, step length, stance phase, and swing phase) using Optogait. This study also measured the dynamic gait index (DGI), the Berg balance scale (BBS) score, and timed up and go (TUG). Results: After training, BBS scores were was significantly increased in the bilateral training group than in the unilateral guidance group (p<0.05). Spatiotemporal parameters were significantly changed in the bilateral training group (gait speed, swing phase ratio, and stance phase ratio) compared to the unilateral training group (p<0.05). Conclusions: The results of this study suggest that robot-assisted gait training show feasibility in facilitating improvements in balance and gait performance for subacute hemiparetic stroke patients.
본 논문은 산업화와 교통 환경의 발달로 인해 최근 들어 발생빈도가 크게 증가되고 있는 외상성 뇌손상을 대상으로 하여, 치료적 운동의 적용에 있어 수중운동의 뇌신경 가소성에 미치는 영향과 전정-운동에 대한 영향에 대해 알아보고자 한 연구이다. 수중운동이라는 운동학적 특성을 통해 기능적 독립을 위해선 반드시 전제되어야 하는 뇌신경 가소성과 전정-운동 기능의 회복에 어떠한 영향을 주는지를 알아보기 위하여 80마리의 백서를 대상으로 4군으로 나누고, 3주간 서로 다른 방식의 운동 훈련(실험군 I: 대조군, II: 수중운동군, III: 속도 및 경사 조절 트레드밀 운동군, IV: 단순 트레드밀 운동군)을 적용하여 전정-운동 기능에 미치는 영향을 Rota-Rod 검사를 통해 알아보고, 신경가소성에 대한 영향성에 대해서는 대뇌피질에서의 GAP-43 인자를 조직면역화학적 방법으로 검사해 보았다. 연구결과, 수중운동을 적용한 군에서, 다른 군들에 비해 뇌신경 가소성에 긍정적이고도 유의한 영향을 미칠 수 있음을 확인할 수 있었으며, 전정-운동 기능 향상에 있어서도 다른 군들에 비해 유의한 향상 효과를 나타내었다. 이는 물이 가지고 있는 다양한 감각적 특성과 환경적 상황이 뇌손상으로 인해 불안정한 신체의 운동에 긍정적인 영향을 미치기 때문인 것으로 사료된다.

The efficacy of highly active antiretroviral therapy (HAART) has led to an increase demand for therapeutic use, thereby necessitating investigation into drug toxicity. This study was designed to investigate the in vivo effects of HAART on sperm parameters and testicular oxidative stress in lean and obese rats. Wistar rats (males, n = 40, weighing 180~200 g) were assigned randomly into 4 groups and treated accordingly for 16 weeks as follows: Control (C): lean group fed with standard rat chow; Diet induced obesity (DIO): obese animals fed a high caloric diet; C + ART: lean animals treated with HAART; DIO + ART: obese animals treated with HAART. An antiretroviral drug combination of Tenofovir, Emtricitabine and Efavirenz at a dose of 17, 26 and 50 mg/kg/day was administered for the latter 6 weeks via jelly cube feeding. At the end of the experimental period, sperm analysis was performed on sperm collected from the caudal epididymis, while the testis was homogenized for antioxidant enzyme and lipid peroxidation assays. Results showed that HAART significantly decreased sperm motility (p < 0.05) in both lean and obese animals, and viability (p < 0.05) in the DIO group. Testicular glutathione, catalase and superoxide dismutase were significantly decreased (p < 0.05), while Thiobarbituric acid reactive substances (TBARS) levels were significantly increased (p < 0.05) when the DIO+ART group was compared to Control group. Thus, the decreased sperm qualities associated with HAART might be as a result of increased testicular oxidative stress prominent in obese animals.
본 연구는 가상현실을 이용한 재활운동을 적용하여 뇌졸중 환자의 보행시 에너지 소모의 효율성에 효과가 있는가를 알아보고자 하였다. 뇌졸중으로 6개월 이상 장애를 가진 32명이 연구에 참여하였고 가상현실군 16명과 대조군 16명으로 나누었다. 가상현실군은 가상현실을 이용한 게임장치를 이용하여 1시간씩 주 3회를 6주간 실시하였다. 대조군은 운동의 적용없이 평상시 생활을 유지하도록 하였다. 운동 전과 후에 심박동수와 보행속도를 측정하여 에너지소모지수를 산출하여 효과를 비교하였다. 통계처리 방법으로 운동 전후 차이를 검증하기 위하여 대응표본 t 검정을 실시하였고 그룹간 차이 검증을 위하여 독립표본 t 검정을 실시하였다. 모든 통계적 유의수준은 0.05로 하였다. 연구결과 대조군과 비교하여 가상현실군에서 보행시 에너지소모지수가 유의하게 감소하였다(p<.05). 결론적으로 가상현실을 이용한 재활운동이 뇌졸중 환자의 보행시 에너지 소모에 있어 효과적이며 보행능력의 향상에 효과적인 방법임을 확인하였다.
