  • Human cases of echinostomiasis have been sporadically diagnosed by extracting worms in the endoscopy in Korea and Japan. Most of these were caused by Echinostoma hortense infection. However, in the present study, we detected 2 live worms of Echinostoma cinetorchis in the ascending colon of a Korean man (68-year old) admitted to the Gyeongsang National University Hospital with complaint of intermittent right lower quadrant abdominal pain for 5 days. Under colonoscopy, 1 worm was found attached on the edematous and hyperemic mucosal surface of the proximal ascending colon and the other was detected on the mid-ascending colon. Both worms were removed from the mucosal surface with a grasping forceps, and morphologically identified as E. cinetorchis by the characteristic head crown with total 37 collar spines including 5 end-group ones on both sides, disappearance of testes, and eggs of $108{\times}60{\mu}m$ with abopercular wrinkles. The infection source of this case seems to be the raw frogs eaten 2 months ago. This is the first case of endoscopy-diagnosed E. cinetorchis infection in Korea.

  • 1. Objects This paper was written in order to understand the formative process of Soyangin pharmacology. 2. Methods We analysis Soyangin pharmacology through pathology and new prescription in ${\ulcorner}$Gabobon(甲午本)${\lrcorner}$ and ${\ulcorner}$Sinchukbon(辛丑本)${\lrcorner}$ of ${\ulcorner}$Dongyi Suse Bowon${\lrcorner}$. 3. Results and Conclusions New prescription of Soyangin in ${\ulcorner}$Gabobon${\lrcorner}$ is same or similar to that in ${\ulcorner}$Chobonkwon${\lrcorner}$. So we can recognize that almost Soyangin pharmacology of ${\ulcorner}$Gabobon${\lrcorner}$ is made when Dongmu write ${\ulcorner}$Chobonkwon${\lrcorner}$. And the maintenance of Yin-Qi is key point of Soyangin's pharmacology New prescription of Soyangin is made by regular rules in ${\ulcorner}$Sinchukbon${\lrcorner}$. Dongmu say that Hyungkae(荊芥) Bangpung(防風) Kanghwal(羌活) Dokhwal(獨活) is herbs for descending-Yin, and Sukjihwang(熟地黃) Sansuyoo(山茱萸) Bokryung Taeksa(澤瀉) or Saengjihwang(生池黃) Sukgo(石膏) Jimo(知母) is herbs for ascending-Yang And all new prescription except Sukjihwangkosam-tang(熟地黃苦蔘湯) is composed of asneding-Yang and descending-Yin's herbs. New prescription of the exterior disease contained all descending-Yin's herbs and that of the interior disease contained all ascending-Yang's herbs completly. Namely when ${\ulcorner}$Gabobon${\lrcorner}$ was written Soyangin pharmacology is the maintenance of Yin-Qi by selecting prescription of cold character. And when ${\ulcorner}$Sinchukbon${\lrcorner}$ was written, base sprit of Soyangin pharmacology was not changed. Pharmacology was added ascending-Yang and descending-Yin at a time.

  • Purpose: This study was conducted to develop a linear Proportional-Integral-Derivative (PID) control algorithm for the ascending and descending system of a rice transplanter and to analyze its response characteristics. Methods: A hydraulic model using a single-acting actuator, proportional valve and a PID control algorithm were developed for the ascending and descending system. The PID coefficients are tuned using the Ziegler-Nichols (Z-N) method and the characteristics of unit step response are analyzed to select the PID coefficients at various pump speeds. Results: Results showed that the performance of the PID controller was superior in any condition. It was found that the highest settling time and maximum overshoot were less than 0.210 s and 5%, respectively at all pump speed. It was determined that the steady state errors were 0% in all the cases. The lowest overshoot and settling time were calculated to be nearly 2.56% and 0.205 s, respectively at the pump rated speed (2650 rpm). Conclusions: The results indicated that the developed PID control algorithm would be feasible for the ascending and descending system of a rice transplanter. Finally, it would be helpful to plant the seedlings uniformly and improve the performance of the rice transplanter.

  • This study is aimed to investigate the effects of intravenous Cultivated Wild Ginseng Pharmacopuncture(CWGP) and the FOLFIRI chemotherapy combination on recurred and metastatic ascending colon cancer patient. A 42-years-old man was diagnosed as ascending colon adenocarcinoma on 9th Mar. 2011. After performing right hemicolectomy and 12 cycles of FOLFOX chemotherapy recurrence at hemicolectomy site and metastases in liver, spleen and lungs were found on 7th Feb. 2012. Intravenous CWGP were performed during total 12 cycles of FOLFIRI chemotherapy from 3rd Mar. 2012 to 27th Sep. 2012. The effects and toxicities of CWGP and FOLFIRI chemotherapy combination were evaluated with PET torso(AA) and National Cancer Institute-Common Toxicity Criteria. The tumor mass in the splenic hilum and liver S4 was disappeared and multiple pulmonic lymph nodules were decreased in size. The recurred lesion on the site of right hemicolectomy showed no changes. During the treatment the patient had no toxicity over grade 1.

  • Aortic valve sparing operations were developed to preserve the native aortic valve during surgery for aortic root aneurysm as well as surgery for ascending aortic aneurysms with associated aortic insufficiency. There are basically two types of aortic valve sparing oprations: remodeling of the aortic root and reimplantation of the aortic valve. These operations have been performed for over two decades and the clinical outcomes have been excellent in experienced hands. Although remodeling of the aortic root is physiologically superior to reimplantation of the aortic valve, long-term follow-up suggests that the latter is associated with lower risk of developing aortic insufficiency. Failure of remodeling of the aortic root is often due to dilatation of the aortic annulus. Thus, this type of aortic valve sparing should be reserved for older patients with ascending aortic aneurysm and normal aortic annulus whereas reimplantation of the aortic valve is more appropriate for young patients with inherited disorders that cause aortic root aneurysms. This article summarizes the published experience with these two operations. They are no longer experimental procedures and should be part of the surgical armamentarium to treat patients with aortic root aneurysm and ascending aortic aneurysms with associated aortic insufficiency.

  • The baseline wander of ECG makes difficult to detect P-, Q-, R- and S-wave. This paper presents and compares two method to remove the baseline wander of ECG using slope traceing waves. For the first method that had been proposed already[1], the ascending slope tracing method is used to eliminate baseline wander. And the second method employs the contour of the approximated S-wave to remove baseline wander. These two methods are common to use the slope tracing wave. However, the first one is to get difference signals between the ascending tracing waves and ECG signals, and the other is to remove baseline wander using the contour of the approximated S-wave obtained from descending slope waves. To evaluate these methods, we apply to MIT/BIH database.

  • We report on changes in the ascending reticular activating system (ARAS) concurrent with the recovery of impaired consciousness following rehabilitation and cranioplasty in a patient with traumatic brain injury (TBI), which were demonstrated on diffusion tensor tractography (DTT). A 34-year-old male patient was diagnosed with a traumatic intracerebral hemorrhage after falling from a height of approximately 7 m and underwent a right frontoparietotemporal decompressive craniectomy and hematoma removal. At 5 months after onset, when starting rehabilitation, the patient showed impaired consciousness, with a Glasgow Coma Scale (GCS) score of 4. Comprehensive rehabilitative therapy was provided until 14 months after onset, and his GCS score improved to 8. Cranioplasty was performed using auto-bone at 14 months after onset. One month after cranioplasty, his GCS score improved to 12. On the 15-month DTT, the deviated lower dorsal ARAS was restored on both sides, and the right side had become thicker. The right lower ventral ARAS was reconstructed, and increased neural connectivity of the upper ARAS was detected in both the prefrontal cortices. Thus, changes in the ARAS were demonstrated in a patient with TBI during recovery of consciousness following rehabilitation and cranioplasty.

  • Powered prosthesis is used to assist walking of people with an amputated lower limb and/or weak leg strength. The accurate gait phase classification is indispensable in smooth movement control of the powered prosthesis. In previous gait phase classification using physical sensors, there is limitation that powered prosthesis should be simulated as same as the speed of training process. Therefore, we propose EMG signal based gait phase recognition method to classify stairs ascending and stairs descending into four steps without using physical sensors, respectively. RMS, VAR, MAV, SSC, ZC, WAMP features are extracted from EMG signal data and LDA(Linear Discriminant Analysis) classifier is used. In the training process, the AHRS sensor produces various ranges of walking steps according to the change of knee angles. The experimental results show that the average accuracies of the proposed method are about 85.6% in stairs ascending and 69.5% in stairs descending whereas those of preliminary studies are about 58.5% in stairs ascending and 35.3% in stairs descending. In addition, we can analyze the average recognition ratio of each gait step with respect to the individual muscle.

  • Pseudoaneurysm of the ascending aorta following cardiac surgery is very unusual and it is poten- tially fatal. We report here a fourteen year-old female patient with pseudoaneurysm of the ascending aorta following a repair of a congenital ventricular septal defect at other hospital 50 months ago. Although she had a mild superficial wound infection postoperatively, she enjoyed uneventful. life until she visited our hospital for a generalized weakness and exertional dyspnea which developed a month ago. Chest CT and echocardiogram showed partially calcified pseudoaneurysm of the ascending aorta. Two aortic defects were located on the anterolateral ascending aortic wall wkere it was suspected as a previous sites of aortic and cardioplegic cannulation. The internal wall of the pseudoaneurysm was covered with neoendothelium and intervened by septal tissue. Two defects on he aortic wall were oval in shape and about 1.5cm in the greatest diameter The defects were trimmed to make a one large de- fect and it was reconstructed with patch designed from 22mm collagen impregnated double velour Dacron graft. The postoperative course was uneventful.
