A Study on Rheumatoid Arthritis Treatment (류마티스 관절염 치료에 대한 고찰)

  • For the perfect cure of rheumatoid arthritis, the ultimate hope of patients and therapist, it is necessary to find a patient early and treat him properly and to teach him what rheumatoid arthritis is. 1. Although rheumatoid arthritis is a chronic disease and its cause is uncertain matke the patient understand that there are many ways to reduce symptoms and to prevent deformation. 2. Explain the peculiarity and the procedure of rheumatoid arthritis to the patient and encourage him. 3. Teach the patient how to protect his arthrosises by showing him concrete movement. 4. Teach the patient or his family the ways to relieve rheumatoid arthritis easily in his house for continuous remedy. In order to make that remedy most effective for the patient, a complex and balanced treatment, considering medication, surgical teatment and rehabilitative treatment, has to be applied, which needs team work. Team work occurs when doctors, physiotherapists, nurses, nutritionists, clinical psychologists, prosthesis manufacturers, social workers, employment agents and the family of the patient work together. The members of the tern have to onderstand the procedure of the treatment of rheumatoid arthritis and apply proper remedies according to the condition of the patient.

A Case Report of Systemic Type Juvenile Rheumatoid Arthritis with Fever and Eruption (고열, 발진을 동반하는 전신형(systemic) 소아기 류마티스 관절염(juvenile rheumatoid arthritis; JRA) 환아 증례 1례 보고)

  • Objectives This study is to report a case that has an important meaning as a result of treating Juvenile Rheumatoid Arthritis. We investigated a patient who had to maintain his life with western medicines such as DMARDs, NSAIDs and steroids for a long time. The patient has recovered from all symptoms and his ESR, CRP has been back to normal range with oriental medicine treatment. Methods The patient had fever, especially repeated fever during the afternoon, pain and swelling of joints, generalized skin eruption, anorexia, delayed growth, weight loss, fatigue. So we treated him with herbal medicine and reduced his western medicine. The aim of treatment was recovery from Juvenile Rheumatoid Arthritis after discontinuance of all western medicine. Results The symptoms of systemic type Juvenile Rheumatoid Arthritis was vanished and the patient maintains his condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. His ESR and CRP levels were back to the normal range. After this treatment the patient's height and weight has been increased which showed a significant meaning in growth to the child. Conclusions This study showed that oriental medicine can elevate the Juvenile Rheumatoid Arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.

The Study of Pain, Depression and Quality of Life According to the Coping Strategy in Chronic Arthritis Patients (만성 관절염 환자의 대응양상에 따른 통증, 우울 및 삶의 질)

  • Purpose: The aim of this study was to investigate the difference of in the level of pain, depression, and quality of life by coping strategy in the chronic arthritis patients. Method: These data were collected by means of a self-reported questionnaire from 90 chronic arthritis patients undergoing treatment at a university hospital in Daegu city from June 10, 2000 to July 15, 2000. The instruments of study were coping strategy scale by Jeon(1985), pain scale by Lee and Song(1987), depression scale by Radloff(1977) and quality of life scale by Cho(1987). Data analysis was done by SPSS Win 10.0 program and Pearson correlation, t-test, Cronbach's-${\alpha}$ were used. Result: The results of this study were as follows: 1. The item mean score of pain for chronic arthritis patient was 6.54. The item mean score of depression for chronic arthritis patient was 2.18. The item mean score of quality of life for chronic arthritis patient was 5.69. 2. The chronic arthritis patients with problem-focused coping experienced pain higher than those with emotion-focused coping(t=o.400, p=o.o45). The chronic arthritis patients with emotion-focused coping experienced depression higher than those with problem-focused coping(t=-2.564, p=0.012). The chronic arthritis patients with problem-focused coping experienced quality of life higher than emotion-focused coping(t=-1.682, p=0.046). Conclusion: In conclusion this study showed that any coping strategy can control the arthritis patient's problem. Therfore, The development of a coping strategy to reduce pain and depression and improve the quality of life in the chronic arthritis patient will be needed.

A Case of Live Leech(Hirudo Medicinalis) Therapy with Herbal Medication on Regional Rheumatoid Arthritis (한약투여 및 의료용 거머리 요법을 병행 시술 후 관절 경직 및 부종이 호전된 국소 류마티즘 환자 증례 1례)

  • Objectives : The purpose of this study is to report the effect of live leech(hirudo medicinalis) therapy with oriental medication on the patient with regional rheumatoid arthritis. The patient had resistant to DMARDs so she was in the debilitating condition and suffered from edema, pain, stiffness of joint and purpura before live leech(hirudo medicinalis) therapy. Methods : The regional rheumatoid arthritis patient had edema, pain, stiffness of joint and purpura on right foot. We treated the patient with live leech(hirudo medicinalis) therapy and oriental medicine. Results : The symptoms of regional rheumatoid arthritis was improved through live leech(hirudo medicinalis) therapy and oriental medicine. The patient maintains her condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. After this treatment the patient's walking difficulty has been improved. Conclusions : This study shows that live leech(hirudo medicinalis) therapy and oriental medicine can elevate the regional rheumatoid arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.

A Case Report of Systemic Type Juvenile Idiopathic Arthritis with Night Fever (야간 발열을 동반하는 전신형(Systemic type) 소아기 류마티스 관절염(Juvenile Idiopathic Arthritis; JIA) 환아 증례보고)

  • Objectives: This study is to report a case that has an effective result to systemic type juvenile idiopathic arthritis patient with an oriental medicine treatment. Methods: We treated the patient with an oriental medicine and tapered down with the western medicine. We followed up the laboratory blood tests every two or three months and through telephone at least six days in a week. Results: The symptoms of systemic type juvenile idiopathic arthritis were vanished and the patient maintains his condition with oriental medicine treatment after discontinued all Disease-Modifying Antirheumatic Drugs(DMARDs). His ESR, CRP levels and other blood test results were back to normal range. Now he is in clinical remission status. Conclusions: According to the result, the oriental medicine treatment is considered to be effective on the systemic juvenile idiopathic arthritis and further studies will be needed with more cases.

  • Objective : This study shows the clinical effects on Bee-venom acupuncture and Burning acupuncture therapy for a patient who was diagnosed as Rheumatoid arthritis with ankle pain. Methods : This study was carried out a patient who was diagnosed as Rheumatoid arthritis with ankle pain. The patient was treated by Bee-venom acupuncture, Burning acupuncture therapy. To evaluate the effect of treatment, measured grade of pain by Visual Analogue Scale(VAS). Result and Conclusion: The ankle pain was reduced from VAS 10 to VAS 2 and the patient could walk on foot. In this study, Bee-venom acupuncture, Burning acupuncture therapy was effective in arthralgia of Rheumatoid arthritis.

The Effect of Tai-Chi for Arthritis(TCA) Program in Osteoarthritis and Rheumatoid Arthritis Patients (골관절염환자와 류마티스관절염 환자에게 적용한 타이치 운동프로그램(TCA)의 효과 비교연구)

  • The aim of study was to investigate the effects of 12 forms of Sun-style Tai-Chi for arthritis(TCA) on physical symptom(pain, tenderness, swelling fatigue, and blood pressure), physical function(balance, flexibility, grip strength) and social-psychological health status in osteoarthritis(OA) patients and rheumatoid arthritis(RA) patients. This study was done with one group pretest-posttest design. A total 21 arthritis patients(6 OA patient and 15 RA patient) participated in 60 minute session consisting of warm-up exercise 12-main movement including qigong, and cool-down exercise twice a week for 6weeks. Physiological and social-psychological variables were measured before and after TCA 12 form. The researcher who completed a Tai Chi workshop for exercise leaders in Australia and Seoul taught the Tai Chi movement step by step until the patient felt comfort enough to perform them correctly by themselves during the first 4 weeks. At the last 2 weeks, the subject perform 10-15set of these at a session with traditional music to help patients move in a slow tempo. TCA 12 forms program showed significant improvement in fatigue(p=0.039) of rheumatoid arthritis patient and pain(p=0.006), fatigue(p=0.013), tenderness(p=0.032), flexibility(rt arm up, p=0.014, If arm up, P=0.003), grip strength(rt hand, p=0.002, If hand, P=0.003) of osteoarthritis patients. With assessment by Arthritis Impact Measurement Scale 2 questionnaire(AIMS2), physical component(0.002) and social interaction(0.025) on osteoarthritis patients were significantly improved. Evaluation of the participants about TCA exercise noted that the exercise was fairly easy(23.6%), moderately difficult(57.1%), and fairly difficult(14.3%). Considering the result of this study, TCA exercise program was more effective in pain, tenderness, grip strength, flexibility, and physical and social interaction on osteoarthritis patients than rheumatoid arthritis patients. Tai-Chi for arthritis 12 form was an appropriate intensity exercise for osteoarthritis patients.

The Study of Prescription Behaviors of Practicing Pharmacists with Simulated Patients of Arthritis (관절염 증상의 모의 환자를 이용한 약사의 처방 행태 연구)

  • Objectives : In Korea, pharmacists can dispense medicines without doctor's prescription. This causes the high proportion of pharmaceutical expenditures. The study shows the prescribing behaviors of practicing pharmacists with the simulated patient of arthritis. We select the arthritis as a subject of simulation, because the arthritis is one of the major health problems and the abuse of cortico-steroids is usual in treatment of arthritis patients. Methods : Twenty drug stores among the 320 drug stores in a district, Seoul, Korea were randomly selected. One of the researchers visited the drug stores and received the medicines from the pharmacists after explaining standardized scenario of arthritis. The simulated patient recorded the practice behaviors of pharmacists. Results: The mean number of prescribed drugs are four and half. Among the twenty pharmacists, the nineteen prescribed non-steroidal anti-inflammatory drugs and the seven(35%) prescribed the cortico-steroids. The antacids were prescribed by the fourteen(70%) pharmacists. The five(25%) pharmacists only recommended the simulated patients to visit the medical doctors, and the three(15%) performed physical examination to the simulated patients. The three pharmacists(15%) asked the past history of the drug adverse effects and no pharmacist explained the adverse effects of prescribed medicines. Conclusions : The research shows that the cortico-steroids are frequently prescribed and the pharmacists commonly do not give the explanations of the prescribed medicines to the arthritis patients.

  • Objective : This case report aims to show the effect of Bekhogainsam-tang on Overactive Bladder and Rheumatoid arthritis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Yangmyung-byng and has been unusual dietary habits. So she treated with Bekhogainsam-tang decoction for 75 days. We evaluated the results of treatment by Overactive Bladder Symptom Score(OABSS), Modified Overactive Bladder Validated 8-question Screener(OAB-V8), and Rheumatoid Arthritis Pain Score(RAPS). Results : After taking Bekhogainsam-tang decoction for 75 days, the patient's symptoms and OABSS was decreased from 20 to 11, OAB-V8 was from 42 to 20, and RAPS was from 130 to 63. Conclusions : This case report showed an effectiveness of using Bekhogainsam-tang according to Diagnositic system based on Shanghanlun provisions on Overactive Bladder and Rheumatoid Arthritis.

  • By process of treatment for a case which diagnosed as suppurative arthritis and admitted from the 5th, October, 1999 to the 15th, November, 1999, the results are as follows. Method & Results : In the earlier days of admission, this patient was diagnosed as damp-heat(濕熱) and medicated Cheongyeolsaseup-tang(qingrexieshi-tang). As the result, heating, pain in both knees and heating in both lmees are improved. In the later days of admission, this patient was diagnosed as impairment of the liver and kidney(肝腎虛損) and medicated Samgi-eum(sanqi-yin). As the reslut, weakness in lower limbs is improved. Conclusion : I consider that the rate of recovery for other infectious diseases including suppurative arthritis can be maximized by accumulation of clinical experiences and continuous research through both western medical treatment and oriental medical treatment.
