백부자의 추출물이 자외선 B조사에 의한 기니피그 피부의 tyrosinase-related proteins발현에 미치는 영향 (Effects of Aqueous Extract from Aconitum Koreanum on the Expression of Tyrosinase-related Proteins by Ultraviolet B Irradiation in Guinea Pig Skin)

  • To investigate whether aqueous extract from Aconitum koreanum (AEAK) effects in the process of melanin synthesis, the expression of tyrosinase-related proteins (TRPs) by immunohistochemical methods were performed in ultraviolet B (UVB) irradiated skin of guinea pig. The irradiation of UVB (60 mJ/day) was performed for 3 days and treated with AEAK for 15 days. About the color evaluation, the visual scores of UV B irradiated guinea pig with AEAK treatment were slightly lower than those in the UV B alone irradiated ones. At day 15 after UVB exposure, immunohistochemical analysis for TRPs expression were performed. The intensive expression of tyrosinase was mainly observed over epidermis with skin appendage and in the cells of dermis. Slight increase of these reaction was induced in response to UVB in the spinous and granular layer of epidermis, but similar expression in the AEAK treated guinea pig as normal one. The TRP-1 and TRP-2 expression were not detected in the skin of normal guinea pig. But intensive expression for TRP-1 and TRP-2, especially TRP-2, induced by UV B irradiation in the cells of dermis. These expressions were decreased in the AEAK treated guniea pig. Collectively, these results suggest that AEAK has a potential to inhibit synthesis through regulation of TRPs expression in the skin of guinea pig, but better understanding the function of AEAK, more research should be done in the effects of AEAK on the function of TRPs in melanogesis.

개심술 후 심폐소생술 실패환아에서의 체외막산소화 치험 1례 (Extracorporeal Membrane Oxygenation in the Patient with Cardiopulmonary Resuscitation Failure after Open Heart Surgery. - A case report -)

  • 저자들은 심장수술 후 발생한 심정지로 심폐소생술을 시행한 신생아에서 성공적인 체외막산소화 장치(ECMO)사용 1례를 경험하였다. 환자는 울혈성 심부전과 폐동맥 고혈압을 가졌던 4.4kg의 35일된 남아로 술중에 특별한 문제없이 심실중격결손증을 첨포봉합법으로 봉합하였다. 심장 중환자실에서의 술후 경과는 junctional ectopic tachycardia (JET)가 나타나기 전까지 약 5시간 동안은 특별한 문제가 없었다. junctional ectopic tachycardia (JET)가 나타난 후 50분경과한 뒤 갑작스러운 서맥이 나타나면서 수축기 혈압이 50mmHg로 하강하여 곧 흉골절개 봉합부(sternotomy incision)를 열고 심폐소생술을 바로 시행하였으며 심폐소생술을 시행한 지 4시간 후에 상행대동맥에 동맥관을 그리고 정맥관은 우심방이에 삽관하여 체외막산소화 장치를 시작하였다. 환자의 혈액동력학은 체외막산소화 동안 안정적이었으며, 시작 후 38.5시간 만에 중지하였다. 흉골지연봉합을 시도하였고, 환자는 수술 후 7일째 인공호흡기를 제거했으며, 신경학적 합병증없이 수술 후 21일째 퇴원하였다.

가슴 통증을 동반한 선천성 부분 심낭 결손증 (Congenital Partial Pericardial Defect Presenting as Chest Pain)

  • 심낭 결손증은 드문 선천성 기형으로 대부분의 경우 증상이 없이 우연히 발견되지만, 심장의 일부가 탈장되어 흉통 등의 증상을 나타내는 경우도 있다. 본 증례는 14세 여자 환자가 내원 3개월 전부터 발생한 운동 시 호흡곤란과 가슴 통증을 주소로 내원하여 부분 심낭 결손을 통한 좌심방 부속지의 탈장(herniation)이 진단되어 수술적 치료를 시행하여 보고하는 바이다.

광릉늑대거미(Arctosa kwangreungensis) 중추신경계 발생에 관한 연구 (Development of the Central Nervous System in the Wolf Spider Arctosa kwangreungensis (Araneae: Lycosidae))

  • 배회성거미류인 광릉늑대거미(Arctosa kwangreungensis)를 실험재료로 배후발생에 따른 거미 중추신경계의 형태적 분화와 그 조직학적 변화를 관찰하였다. 식도를 중심으로 위쪽의 식도상신경절과 아래쪽의 식도하신경절이 서로 연결된 구조를 지닌 광릉늑대거미의 중추신경계는 부화가 이루어지는 후기배아 시기 이전에 확립되었고, 전대뇌(protocerebrum) 영역도 구분되었다. 후기배아 단계에서는 시각정보를 수용하는 시엽(optic lobe)의 발달이 미진하였으나 알주머니를 벗어나 본격적인 먹이활동을 시작하는 제2령 시기를 전후하여 성체와 동일한 신경계의 체제가 구축되었다. 후기배아 시기에 관찰된 식도상신경절의 미분화 세포들로부터 시각기 신경절이 발생되었고, 식도하신경절에서는 네 쌍의 부속지 신경절과 복부 신경절이 분화되었다. 또한, 발생 중인 유충의 중추신경계에서는 조직학적 특성이 다른 세 유형의 신경세포들이 관찰되었고, 발생단계에 따라 이들 세포집단의 이동과 분포상의 차이가 확인되었다.

다중 공진을 이용한 이중 부이 파력발전장치의 모형실험 (Model Test of Dual-Buoy Wave Energy Converter using Multi-resonance)

  • In this study, we proposed a new type of dual-buoy wave energy converter (WEC) exploiting multi-resonance and analyzed the experimental results from a model test in a 2-D wave flume. A dual-buoy WEC using multi-resonance has two advantages: high efficiency at the resonant frequencies and the potential to extend the frequency range available to extract wave power from the WEC. The suggested WEC was composed of an outer buoy and an inner buoy sliding vertically inside the outer buoy. As the power take-off device, a linear electric generator (LEG) consisting of permanent magnets and coils fixed at each buoy was adopted. Electricity was produced by the relative heave motion between the two buoys. To search for the optimal shape of a dual-buoy WEC, we conducted experiments on the heave motion of a two-body system in regular waves without an LEG installed. Model tests with six combinations of experimental models were conducted in order to find the motion characteristics of a dual-buoy WEC. It was found that model 2, which included a ring-shaped appendage to move the resonant frequency of the outer buoy toward a high value, showed a higher relative heave response amplitude operator (RAO) curve than model 1. In addition, the double-peak shape of the heave RAO curve shown for model 2 indicated the extension of the frequency range for extracting wave power in irregular waves.

고속선의 트림제어 장치가 항주 자세 및 저항 성능에 미치는 영향 (Experimental investigation of dynamic trim control devices in fast speed vessel)

  • The displacement Deep-V catamaran concept was developed in Newcastle University(UNEW) through development of the systematic Deep-V catamaran series. One of the most important Deep-V catamaran launched to date is Newcastle University's own multi-purpose research vessel, The Princess Royal. The vessel was launched in 2011 and enhanced the Deep-V catamaran concept further with the successful adoption of a novel anti-slamming bulbous bow and tunnel stern for improved efficiency. It was however identified that the vessel has substantial amount of dynamic trim that limited the visibility of the captain. The dynamic trim also increased the wave-making resistance thereby preventing the vessel from attaining its maximum speed in certain sea states. This paper therefore presents the application of devices such as Trim Tabs, Interceptors, Transom Wedges and Integrated Transom Wedges-Tabs to control the dynamic trim and improvement of fuel efficiency of the vessel. All of these energy saving devices were fitted into a model for tests in Newcastle University's Towing Tank. Model test verification confirmed that the optimum appendage was the interceptors, they produced a 5% power saving and 1.2 degree trim reduction at 15 knots, and investigations of full scale trials will be scheduled with and without application of device to compare the improvement of performance.

  • 세일링요트의 추진시 횡류를 방지하기 위하여 선체 하부에 킬을 부착하게 되는데 이는 세일의 양력에 의한 횡흐름을 멈추게 하는 역할과 힐 모멘트를 없애고 복원력을 생기게 한다. 이는 안전성에 영향을 미치기 때문에 적절한 킬의 선택은 중요하다고 할 수 있다. 또한 킬 하부에 부가물의 부착은 발라스트 중량중심을 낮춤으로서 선체의 복원력을 향상하기에 최적형상을 도출하여야 한다. 이에 본 연구에서는 저항성능 향상을 위한 동일 침수표면적의 핀킬 하부 부가물에 따른 저항특성 비교를 위하여 회류수조를 이용하여 3가지 핀킬 하부 부가물 모형킬을 모형선에 부착하여 형상별 저항특성을 비교 분석하여 저항감소 최적 형상을 도출한 것이다.

좌심실 보조장치를 이용한 전격성 심근염의 치료 - 1례 보고 - (Management of Acute Fulminant Myocarditis Using a Left Ventricular Assist Device - A case report -)

  • 17세 여자 고등학교 학생이 전흉부 동통을 주소로 입원하였다. 약물 치료와 대동맥내 풍선 펌프에도 불구하고 폐부종과 순환 허탈이 악화되었다. 급성 전격성 심근염 추정 진단 하에 좌심실 보조장치를 설치하였다. 유입구 도관은 유방하 좌전개흉술로 좌심이를 통하여 좌심방에 삽입하였고, 좌심이의 일부를 조직검사를 위해 떼어냈다. 유출구 도관은 좌측 대퇴 동맥에 PTFE도관을 간치시켜 삽입하였다. 체외순환 158시간 후 심초음파에서 좌심실의 운동성이 거의 정상으로 회복되었고, 좌심실 보조장치를 성공적으로 이탈할 수 있었다. 면역화학 검사와 중화 항체 검사에서 콕사키바이러스가 확인되었다. 환자는 입원 23일 째 아무런 심부전 증상없이 퇴원하였다.

완전방실차단을 동반한 감염성 심내막염 환자에서 판막치환술 후 관정맥동을 통해 좌심실을 조율하는 심박조율기 시술 (Implantation of a permanent pacemaker through the coronary sinus in a patient who underwent mechanical valve replacement for infective endocarditis with a complete atrioventricular block)

  • A 52-year-old man was referred to our hospital due to fever and myalgia that occurred 2 weeks earlier. He showed a complete atrioventricular block on his electrocardiogram, and his vital signs were unstable. On his transthoracic echocardiograph, the 1.5 cm vegetation in the aortic valve with severe aortic regurgitation suggested infective endocarditis. His transesophageal enchocardiograph showed abscess in his mitral-aortic intervalvular fibrosa and vegetation was suspected on his anterior mitral valve leaflet. The patient underwent an emergent operation for valve replacement with temporary epicardial pacing. Intraoperatively, the septal leaflet of his tricuspid valve was injured during the debridement of the abscess pocket that was extended to the membranous septum. The aortic, mitral, and tricuspid mechanical valves were replaced with annular reconstruction without complications. After 14 days of intravenous antibiotics, we successfully changed the epicardial pacemaker into a transvenous DDD-type permanent pacemaker by placing a left ventricular lead via the coronary sinus and an atrial lead in the right atrium appendage. The patient was discharged in a tolerable state and was examined uneventfully in our hospital's outpatient clinic for 8 months.

KRISO 대형 캐비테이션터널 시험조건의 함정 모형선 반류에 대한 수치해석적 연구 (Numerical Analysis of the Wake of a Surface Ship Model Mounted in KRISO Large Cavitation Tunnel)

  • The accurate assessment of hull-appendage interaction in the early design stage is important to control the inflow to the propeller plane, which can cause undesirable hydrodynamic effects in terms of cavitation phenomenon. This paper describes a numerical analysis for the flow around a fully appended surface ship model for which KRISO has carried out a model test in the Large Cavitation Tunnel(LCT). This numerical study was performed with the LCT model test in a complementary manner for a good reproduction of the wake distribution of surface ships. A second order accurate finite volume method provided by a commercial computational fluid dynamics(CFD) program was used to solve the governing Reynolds Averaged Navier-Stokes(RANS) equations, where the SST $k-{\omega}$ model was used for turbulence closure. The numerical results were compared to available LCT experimental data for validation. The calculations gave good predictions for the boundary layer profiles on the walls of the empty cavitation tunnel and the wake at the propeller plane of the fully appended hull model in the LCT.
