  • Purpose ; This Investigation was aimed to find out the Acupuncture therapy of verbal disturbance after stroke in Oriental Medicine. Methods ; We surveyed the oriental medical books which are from to recent published books that have articles about verbal disturbance. Results ; It is used on Acupuncture points: Yomchon(CV23), Amun(GV15), Chondol(CV22), Tongni(HT5), Pungnyung(ST40), Pungbu(GV16), Crugu(TE6), Yongchon(KI1), Hapkok(LI4), Paekoe(GV20), Kokchi(LI11), Puryu(KI7), Shinsu(BL23), Kumjin-okaek, Chohae(KI6). Conclusions ; The concept of phonation organ in Neiching is same as that of modern physiology. Verbal disturbance includes aphasia, dysarthrosis, aphonia, etc. The acupuncture treatment of verbal disturbance after stroke could be effective. Verbal disturbance of central nervous disease is relative with Sul-um(舌?) and that of periperal nervous disease is relative with Hu-um(侯?) in oriental medicine. From the above research, we could conclude that the acupuncture therapy on verbal disturbance should start from finding out the proper acu-points and theories in ancient oriental books and recent books and we will apply those acu-points and theories in clinic. We also concluded that the more research on clinical part should be performed.

  • A continuous electroencephalography (cEEG) can be helpful in detecting vasospasm and delayed cerebral ischemia in aneurysmal subarachnoid hemorrhage (SAH). We describe a patient with an aneurysmal SAH whose symptomatic vasospasm was detected promptly by using a real-time cEEG. Patient was immediately treated by intraarterial vasodilator therapy. A 50-year-old woman without any significant medical history presented with a severe bifrontal headache due to acute SAH with a ruptured aneurysm on the anterior communicating artery (Fisher grade 3). On bleed day 6, she developed a sudden onset of global aphasia and left hemiparesis preceded by cEEG changes consistent with vasospasm. A stat chemical dilator therapy was performed and she recovered without significant neurological deficits. A real-time and protocol-based cEEG can be utilized in order to avoid any delay in detection of vasospasm in aneurysmal SAH and thereby improve clinical outcomes.

  • There are few case reports on the treatment of churg strauss syndrome by oriental medicine. We experienced one case of a female patient, 47 ages, who had symptoms; Rt. hemiplegia, aphasia, M/S dull, Rt. facial palsy. We used acupuncture, physical therapy, herbal medication and taping therapy. After 6 months of Acupuncture therapy, Herbal medication, physical therapy and taping therapy, a remarkable improvement was made for cerebral infarction patient who accompany churg strauss syndrome. The patient could take self-ambulation after treatment and many symptoms have improved. Cerebral infarction patient who accompany churg strauss syndrome is not common ,but in case the treatment by korean traditional medicine, there was improvement in patient's state. We think that it need the further study and clinical practice for Churg Strauss Syndrome.

  • After initial recovery from acute carbon monoxide(CO) intoxication, some patients occasionally undergo severe neuropsychiatric deterioration, which is called postanomic delayed encephalopathy(sequelae). This is the clinical study about one patient, a 53-year-old woman, diagnosed with delayed encephalopathy after acute CO intoxication. The patient's symptoms were mental dysfunction including memory impairment and disorientation, aphasia, atrophy and weakness throughtout the body. She had completely recovered after an anomic episode, but the neurological symptoms that developed were preceded by an interval of apparent nomality.(the 'lucid interval'). She was characterized as suffering deficiency syndrome of the heart(心虛) and was prescribed for her an Ansinschungnoi-tang(安神淸腦湯), and thereafter her symptoms improved remarkably. For the evaluation of clinical improvement, we use the Modified Barthel Index(MBI).

  • Using diffusion tensor tractography (DTT), we demonstrated injury of the arcuate fasciculus (AF) in the nondominant hemisphere in two patients who showed subfalcine herniation after intracerebral hemorrhage (ICH) in the dominant hemisphere. Two patients (patient 1 and patient 2) with ICH and six age-matched control patients who have ICH on the left corona radiata and basal ganglia without subfalcine herniation were recruited for this study. DTT was performed at one month after onset in patient 1 and patient 2. AFs of both hemispheres in both patients were disrupted between Wernicke's and Broca's areas. The fractional anisotropy value and tract numbers of the right AFs in both patients were found to be more than two standard deviations lower than those of control patients. In contrast, the apparent diffusion coefficient value was more than two standard deviations higher than those of control patients. Using the configuration and parameters of DTT, we confirmed injury of the AF in the nondominant hemisphere in two patients with subfalcine herniation following ICH in the dominant hemisphere. Therefore, DTT would be a useful tool for detection of underlying injury of the AF in the nondominant hemisphere in patients with subfalcine herniation.

  • Objective: There were few reports on the treatment of Hypoxic ischemic encephalopathy. We treated a hypoxic ischemic encephalopathic patient after accident with Oriental medical approach, and get a significant result. this treatment shows the possibility of healing Hypoxic ischemic encephalopathy, So we are reporting that case. Method : The acupuncture, herb medication. rehabilitation therapy was applied for treting patient's chife symptom(involuntary movement, dystonia, aphasia, dysuria, constipation) Results : 1. Generally patients with Hypoxic ischemic encephalopathy show language impairment and cognition disorder for several weeks to months. After coma stage, first they recover consciousness but have various degree of confusional mentality, visual agnosia, extrapyramidal stiffness and motor disturbance. 2. Consciousness loss in acute stage of Hypoxic ischemic encephalopathy can be considered as Mental Confusion due to Phlegm(Dammisimgyu) in veiw point of Oriental medicine. 3. After oriental medical treatment, patient's chief symptoms were improved. Conclusion: We treated a hypoxic ischemic encephalopathic patient with Oriental medical approach and patient's chief symptoms were improved. this treatment shows the possibility of healing Hypoxic ischemic encephalopathy.

  • Anomia, word finding difficulty, is one of the most common feature in aphasia. Previous studies support that the process of picture naming consists of three stages, in the order of the object recognition, semantic, and phonological output stages. Anomic patients have many symptoms and it means that anomia can be sub-divided into several symptom groups. Our anomia assessment battery consists of several parts: (1) picture naming set, (2) picture-word matching task, (3) lexical decision task for mental lexicon damage, (4) naming task for phonological lexicon damage, and (5) semantic decision task. Pictures and words were selected on the basis of usage frequency, semantic category, and word length. We administered this anomia evaluation battery to many anomic aphasics and we subdivided patients into several groups. We hope that our anomia evaluation set is useful and helpful for evaluation anomic aphasics

  • Few studies have reported the effects of traditional Korean medical (TKM) therapy in patients with traumatic brain hemorrhage. The patient was a 93-year-old female who was unconscious after a traumatic intracerebral hemorrhage (ICH) and was not a candidate for any surgical treatment because of her age. We treated her with acupuncture and herbal medicine. After 28 days of treatment, we observed an improvement in consciousness on the Glasgow Coma Scale (GCS) and in other symptoms (aphasia, weakness in both legs, and headache).
