Prognostic Factors and Clinical Outcomes of Acute Intracerebral Hemorrhage in Patients with Chronic Kidney Disease

  • Objective : We conducted a retrospective study examining the outcomes of intracerebral hemorrhage (ICH) in patients with chronic kidney disease (CKD) to identify parameters associated with prognosis. Methods : From January 2001 to June 2008, we treated 32 ICH patients (21 men, 11 women; mean age, 62 years) with CKD. We surveyed patients age, sex, underlying disease, neurological status using Glasgow Coma Scale (GCS), ICH volume, hematoma location, accompanying intraventricular hemorrhage, anti-platelet agents, initial and 3rd day systolic blood pressure (SBP), clinical outcome using the modified Rankin Scale (mRS) and complications. The severity of renal functions was categorized using a modified glomerular filtration rate (mGFR). Multifactorial effects were identified by regression analysis. Results : The mean GCS score on admission was $9.4{\pm}4.4$ and the mean mRS was $4.3{\pm}1.8$. The overall clinical outcomes showed a significant relationship on initial neurological status, hematoma volume, and mGFR. Also, the outcomes of patients with a severe renal dysfunction were significantly different from those with mild/moderate renal dysfunction (p<0.05). Particularly, initial hematoma volume and sBP on the 3rd day after ICH onset were related with mortality (p<0.05). However, the other factors showed no correlation with clinical outcome. Conclusion : Neurological outcome was based on initial neurological status, renal function and the volume of the hematoma. In addition, hematoma volume and uncontrolled blood pressure were significantly related to mortality. Hence, the severity of renal function, initial neurological status, hematoma volume, and uncontrolled blood pressure emerged as significant prognostic factors in ICH patients with CKD.

울릉도 마가목의 다양한 부위의 항혈전 활성 (In-vitro Antithrombosis Activity of Different Parts of Sorbus commixta from Ulleung Island)

  • 마가목은 울릉도를 대표하는 약용식물 중 하나이다. 본 연구에서는 마가목을 이용한 혈행개선 기능성을 가진 고부가가치 음료 제조를 위해 마가목의 미숙과, 성숙과, 줄기 및 잎의 열수 추출물을 제조하고, 이를 대상으로 기존에 보고된 바 없는 항혈전 활성 및 항산화 활성을 검토하였다. 마가목 줄기의 경우 추출효율은 3.99%, pH 5.5이었으며, 열수 추출물은 총 폴리페놀 89.2 mg/g, 총 플라보노이드 28.3 mg/g의 함량을 나타내었다. 다양한 부위의 마가목 추출물들은 0.5 mg/ml 농도까지 인간 적혈구 용혈활성을 나타나지 않았다. 혈액응고 저해활성 평가결과, 줄기 추출물에서만 강력한 트롬빈, 프로트롬빈, 혈액응고인자 저해가 확인되었으며, 인간 혈소판 응집저해활성은 인정되지 않았다. 한편 혈전 생성억제와 관련된 항산화 활성 평가 결과, 줄기 추출물에서 강력한 DPPH 음이온, ABTS 음이온 소거능, nitrite 소거능을 확인하였으며, 각각의 RC50는 각각 119.7, 53.3, 및 117.5 μg/ml으로 계산되었다. 항산화제인 vitamin C의 DPPH 음이온, ABTS 양이온 및 nitrite 소거능에 대한 RC50가, 각각 13.7, 5.2, 및 14.9 μg/ml임을 고려할 때, 마가목 줄기 추출물은 정제되지 않은 상태에서도 우수한 항산화 활성을 나타내었으며, 환원력 역시 다른 부위에 비해 강력하였다. 본 연구결과는 강력한 항응고 활성과 항산화 활성을 나타내는 마가목 줄기 추출물이 신규의 항혈전 천연물 소재로 이용가능 함을 제시하고 있다.

동애등에의 항혈전 활성 (Antithrombotic Activity of Hermetia illucens (Black Soldier Fly))

  • 최근, 환경오염의 심화, 식량·에너지 부족, 지구온난화 문제에 대한 대안으로 곤충산업이 주목받고 있다. 본 연구에서는 환경정화 곤충 및 사료용 곤충으로 각광받고 있는 동애등에(Black soldier fly, Hermetia illicens)의 고부가 가치화를 위해, 생육시기별로 유충(2령~6령), 번데기, 용각 및 성충을 회수하여 이의 에탄올 추출물을 조제하고, 이의 유용성분 함량, 혈액응고저해 및 혈소판 응집저해 활성을 평가하였다. 동애등에 유충은 15일 사육(2령~6령) 동안 25배 무게가 증가하였으며, 흰색-미색-흑갈색으로 변화하였으며, 에탄올 추출수율은 생육시기에 따라 1.0~18.5%로 다양하였다. 추출물의 총 폴리페놀 함량은 용각(17.2 mg/g)에서 가장 높았으며, 번데기, 성충, 2령 순으로 나타났으며, 총 플라보노이드 함량은 번데기(3.4 mg/g), 용각, 6령 순으로 나타났다. 반면 총당 함량은 6령(37.6 mg/g)에서 특이하게 높게 나타났으며, 용각, 번데기, 5령순으로 나타났다. 혈액응고 저해 활성 평가결과, 용각 추출물에서 가장 우수한 트롬빈저해, 내인성 및 외인성 응고 저해를 나타내었으며, 6령 및 번데기 추출물에서도 우수한 항응고 활성을 확인하였다. 혈소판 응집저해 활성 평가에서는 3령, 용각 및 성충 추출물에서 우수한 응집저해능이 나타났으며, 4령 및 5령 추출물은 오히려 혈소판 응집을 촉진함을 확인하였다. 현재까지 동애등에의 항혈전 활성은 보고된 바 없으며, 특히 생육시기별 활성 평가도 보고된 바 없다. 본 연구결과는 동애등애 번데기 및 용각을 이용한 항혈전 소재 개발이 가능함을 제시하고 있다.

온담탕이 고혈압 백서와 인간유래 혈관내피세포주(ECV 304)에 미치는 영향 (An Experimental Study of Effect on ECV 304 Cells, Platelet Rich Plasma and Rats treated with L-NAME by Ondamtang extract)

  • Nitric oxide(NO) play an important role in normal and pathophysiological cells including as a messenger molecule, neurotransmitter, microbiocidal agent, or dilator of blood vessels and artheriosclerosis, hypertension, myocardial infarction, respectively. To investigate that Ondamtang in the potential contribution of the levels of nitric oxide generated by endothelial nitric oxide synthase (eNOS) and the mechanisms of protection against L-NAME, human ECV304 cells, which normally do not express eNOS, were expressed by L-NAME. L-NAME stimulated rat or cells were found to be resistant to injury and delayed death following the Ondam-tang. Inhibition of nitric oxide synthesis abolished the protective effect against L-NAME, thrombin and collagen exposure. Interestingly, such effects have bee observed during stimulation with agents such as KCl on L-NAME mediate rats, were damaged by the NOS inhibitor NG-nitro-L-arginine methyl ester (L-NAME). Cardiovascular diseases is one of the blood vessels and renin-angiotensin system dynfunction. So we studied on herbal medicine that have a relation of vessels endothelium necrosis. In Oriental Medicine, Ondam-tang has been used for disease in relation to cardiovascular system. We studied on the protection and inhibitory effects of cardiovascular diseases in L-NAME induced rat or ECV304 cell lines through the Cell morphological pattern, Tunel assay, LDH activity, heart rate, blood pressure and immunohistochemistric analysis by Ondam-tang. As the result of this study, In group, the anti-apoptosis and necrosis in the cardiovascular system have a potential capacity for prevented, protected and treating the diseases of cardiovascular system, against the necrosis of rat and ECV304 cells with eNOS and calpain expression by L-NAME is promoted.

Risk Factors of Delayed Surgical Intervention after Conservatively Treated Acute Traumatic Subdural Hematoma

  • Objective : Acute subdural hematoma (ASDH) is generally considered a condition that should be managed surgically. However, some patients initially receive conservative treatment, a subset of whom require surgical intervention later. This study aimed to evaluate the predictors of delayed surgical intervention in ASDH patients who are initially managed conservatively. Methods : From January 2007 to December 2015, 842 patients diagnosed with ASDH were treated at our institution. Among them, 158 patients with convexity ASDH were initially treated conservatively. Patients were divided into a delayed surgery group and a conservative group. Demographic characteristics, past medication and medical histories, and radiological and laboratory data were collected by retrospective chart review. Independent risk factors were identified with univariate and multivariate analyses. Results : Twenty-eight patients (17.7%) underwent delayed surgical intervention. Their mean age was 69.0 years, and 82.1% were male. Hypertension, diabetes mellitus, and heart disease prevalence and use of anti-platelet agents did not significantly differ from the conservative group. However, age (p=0.024), previous cerebral infarction history (p=0.026), increased maximal hematoma thickness (p<0.001), midline shifting (p=0.001) and accompanying subarachnoid hemorrhage (p=0.022) on initial brain computed tomography (CT) scan, low hemoglobin level (p<0.001), high leukocyte count (p=0.004), and low glucose level (p=0.002) were significantly associated with delayed surgical intervention. In multivariate analysis, increased maximal hematoma thickness (odds ratio [OR]=1.279, 95% confidence interval [CI] 1.075-1.521; p=0.006), low hemoglobin level (OR=0.673, 95% CI 0.467-0.970; p=0.034), and high leukocyte count (OR=1.142, 95% CI 1.024-1.272; p=0.017) were independent risk factors for delayed surgical intervention. Conclusion : Due to the high likelihood of delayed surgical intervention among minimal ASDH patients with a thicker hematoma on initial brain CT, lower hemoglobin level, and higher leukocyte count, these patients should receive more careful observation.

Endothelial cell products can modulate the magnitude of a response to a vasoconstrictor, as evinced by the greater constriction after endothelium removal or NO synthesis blockade. To investigate that Boonsimgieum in the potential contribution of the levels of nitric oxide generated by endothelial nitric oxide synthase (eNOS) and the mechanisms of protection against NG-nitro-L-arginine methyl ester (L-NAME), human ECV 304 cells, which normally do not express eNOS, were expressed by L-NAME. L-NAME stimulated rat or cells were found to be resistant to injury and delayed death following the Boonsimgieum. Inhibition of nitric oxide synthesis abolished the protective effect against L-NAME, thrombin and collagen exposure. Interestingly, such effects have been observed during stimulation with agents such as phenylephrine and KCl on L-NAME mediate rats, were damaged by the NOS inhibitor L-NAME. Result : As the result of this study, In group, the anti-apoptosis and necrosis in the cardiovascular system have a potential capacity for prevented, protected and treating the diseases of cardiovascular system, against the necrosis of rat and ECV 304 cells with Caspase 3 and calpain expression by L-NAME is promoted. Conclusion : these results demonstrate neuroprotective and memory enhancing effects of ZIBU, suggesting its beneficial actions for the treatment of AD.
