The Effect of Passive Joint Mobilization and Massge on subacute Lateral Ankle Ligament Injuroes (아급성기의 발목 외측 염좌 환자에게 수동적 관절가동기법과 마사지가 미치는 영향)

  • Physiotherapyists frequently use manipulative therapy technique to treat dysfunctionand pain resulting from ankle sprain. Despite the high prevalence of lateral ankle ligament injuries, few studies are available indicating any physical associations with the development of lateral ankle ligament injuries, or information of treatment for lateral ankle ligament injuries. To investigate the effect of passive joint mobilization, the anteroposterior glide on the talus, on increasing weight-bearing dorsiflexion, single support time and VAS. Sixty lateral ankle ligament injuries (grade I and grade II) aged between 17 and 27 years (mean age 21) were recruited. Subjects were randomly assigned to 1 of 2 treatment groups. The control group received a protocol of rest, ice, compression, and elevation (RICE) and massage. The experimental group received the anteroposterior mobilization, using a force that avoided incurring any increase in pain, in addition to the RICE protocol. Subjects in both groups were treated every second day for a maximum of 2 weeks or until the discharge criteria were met, and all subjects were given a home program of continued RICE application. Outcomes were measured after each treatment. The results showed that the experimental group than the control group. Weight-bearing dorsiflexion (F=7.640, P<0.05), single support time (F=85.532, P<0.05) and VAS (F=10.610, P<0.050). Between-groups differences were observed as; experimental group is increased weight-bearing dorsiflexion, single support time and reduced VAS.

  • Lisfranc joint injuries are rare and account for 0.2% of all injuries. The pattern of Lisfranc injuries varies from low-energy ligamentous sprain to high-energy fractures or crushing injuries. Early diagnosis and appropriate treatment of Lisfranc injuries are important to prevent chronic foot pain and dysfunction.

Current Trends in the Treatment of Ankle Ligament Injuries: Analysis of the Korean Foot and Ankle Society (KFAS) Member Survey (족관절 인대 손상 치료 동향: 대한족부족관절학회 회원 설문조사 분석)

  • Purpose: Despite continuous updates of standard treatment guidelines for acute ankle sprain and chronic ankle instability (CAI), in practice preferred treatment protocols vary widely. Based on a Korean Foot and Ankle Society (KFAS) member survey, this study reports current trends in the management of ankle ligament injuries. Materials and Methods: A web-based questionnaire containing 34 questions was sent to all KFAS members in September 2021. Questions mainly addressed clinical experience and preferences for the diagnosis and treatment of ankle ligament injuries. Answers with a prevalence of ≥50% among respondents were considered to reflect tendencies. Results: Eighty-four of the 550 members (15.3%) responded. Answers that showed a tendency were as follows: commonest additional image study (ultrasound), conservative treatment modality (immobilization, oral medication), frequency of surgical treatment (<5 cases per annum), most important factor when deciding on surgical treatment (activity level, e.g., occupation or sport), and commonest surgical procedure (open ligament repair). Answers that showed a tendency for CAI were as follows: most important symptom (repeated sprain, giving way), radiological factors (talar tilt, osteochondral lesion, anterior talar translation), and patient factors (occupation, sports activities, recurrent instability after surgery, etc.). For decision making regarding surgical treatment and method, the most preferred surgical procedure was the modified Broström procedure, and the most common repair technique was suture anchor technique. The following were considered poor prognostic factors; generalized laxity, failed previous surgery, cavovarus, severe mechanical instability, heavy work, obesity, and dissatisfaction after surgery because of residual pain. Conclusion: This study updates information regarding current trends in the management of ankle ligament injuries in Korea, and reveals consensus opinions and variations in approaches to patients with an acute or chronic injury. The divergence of approaches identified indicates the need for further studies to determine standard guidelines and long-term results.

Study of the Effect of Blood-letting Cupping Treatment for Acute Ankle Sprain: A Randomize Controlled Trial (급성기 족관절 염좌에 있어 자락발관법의 효과에 대한 연구:무작위 대조 실험)

  • Objective : This study was performed to investigate the effect of blood-letting cupping treatment for acute ankle sprain because there are few studies with evidence-based approach. Method : Twenty-nine patients, who had no fracture and had injured by external force and had less than 1 week passed since onset and had not been treated, were randomly divided into control or treatment group by tossing a coin. The treatment group received blood-letting cupping. After 3rd treatment, the outcomes were assessed using Ankle-Hindfoot Scale (AHS) and Numeric Rating Scale (NRS). Result : Nineteen out of 29 patients were finally assessed. The treatment group (n=11) was significantly different from the control group (n=8) in AHS and NRS (p=0.041, 0.026, respectively). Conclusion : The blood-letting cupping has an objective effect on reduing pain for acute ankle sprain.

  • Ankle osteoarthritis is a debilitating condition that causes severe pain associated with functional impairment and decreased activity. Ankle osteoarthritis, unlike that of the knee or hip joint, is rare in primary arthritis. Most cases are traumatic arthritis that occur after ankle sprain or fractures or chronic ankle instability. Although ankle fusion has been regarded as the standard treatment of ankle osteoarthritis in the past, total ankle arthroplasty (TAA) is increasing due to the development of the implant design and surgical techniques. TAA is biomechanically superior to ankle fusion by preserving the movement of the ankle joint. In particular, it is functionally superior to ankle fusion because it enables normal joint motion during gait. In addition, there is an advantage of preserving the movement of the hindfoot and reducing the abnormal stress applied to the adjacent joints after ankle fusion to prevent the occurrence of long-term adjacent joint arthritis. Although the short-term and mid-term results of TAA have been reported to be excellent, long-term follow-up has a relatively low survival rate and high complication rate compared to total knee or hip arthroplasty. Therefore, continuous and further research is needed.

A Case Report of Osteochondral Lesions of the Left Talus after Ankle Sprain Treated with Complex Traditional Korean Medical Care (염좌로 유발된 좌측 거골 골연골 병변 1례에 대한 한방 복합치료 증례 보고)

  • Objectives : The purpose of this report is to introduce a case of osteochondral lesions of the left talus caused by ankle sprain and suggest complex traditional Korean medical care with warm needling therapy as a possible method of conservative treatment. Methods : A 20-year-old male with osteochondral lesions of the left talus complaining of ankle pain and restricted range of movement was treated mainly with warm needling therapy and other combined Korean medicine therapy from January 5, 2015 to January 12, 2015. Improvements of symptoms were measured by American Orthopaedic Foot & Ankle Society Score (AOFAS Score), Visual Analog Scale (VAS) and Range of movement (ROM). Results : During 8 days of treatment, the patient showed gradual improvement in AOFAS score, VAS at rest and at weight bearing position, as well as ROM of the ankle. Conclusion : According to the result, warm needling therapy is a possible conservative treatment for osteochondral lesions of the talus. Further studies are needed regarding possible longterm effects.

Chronic subtalar joint instability - One case report - (거골하 관절의 만성 불안정성 - 1예 보고 -)

  • Purpose: Chronic subtalar instability is not common and similar to chronic ankle instability and the incidence and cause chronic subtalar instability are not well known. Recently we have experienced chronic subtalar instability without chronic ankle instability which was treated with modified Brostrom procedures. Materials and Methods: The patient is 46 year old man who has suffered from left ankle sprain for 30 years and recently aggravated more than twice a day. On subtalar stress view, 14 degree angulation of subtalar joint was noted and on anterior drawer view, 8 mm anterior displacement of left ankle was seen. Results: In operation, there was no anterior talofibular ligament abnormility but calcaneofibular ligament loosening was found. Ligament reconstruction was performed using modified Brostrom procedure. At 12 months after operation, the patient complains no pain and no limit of motion and no instability. Conclusion: We experienced chronic subtalar instability without ankle instability treated with modified Brostrom procedures. No instability was found after treatment without complication.

Tarsal Coalitions (족근 골 결합)

  • A tarsal coalition is an abnormal union between 2 or more tarsal bones of the hind- and midfoot, which can be congenital or acquired. The documented overall incidence of tarsal coalition is 1% or less. The resulting abnormal articulation leads to accelerated degeneration within adjacent joint. Pain is often diffuse, exacerbated by strenuous activity or following an ankle sprain. The findings on physical examination is protruded mass, diminished range of motion of the involved joint. It is possible to identify of tarsal coalitions with conventional radiography, but CT scanning necessary to evaluate of the size, location, characteristic and preoperative planning of tarsal coalitions. The initial treatment for a tarsal coalition is conservative, but tarsal coalitions unresponsive to conservative treatment, are managed by coalition resection, or arthrodesis in case of presence of degenerative changes.

  • Complex regional pain syndrome type I of vascular origin is difficult to detect unless the classic symptoms and signs exist and/or overt extremity trauma has precipitated the pain. The diagnosis is confirmed by relief of pain following a sympathetic nerve blockade. A 36-year-old woman with arterial occlusive disease of the right lower extremity presented with burning pain and hyperesthesia after sprain had occurred which was accompanied by motor weakness of right ankle. A lumbar sympathetic ganglion blockade with 2% lidocaine 10 ml and triamcinolone 80 mg produced prompt improvement of the pain and motion.

Comparison of Service Usage of Outpatients at Korean Medical Clinics by Regions and Gender (한방의료기관을 이용하는 외래환자의 지역 및 남녀별 치료질병의 우선순위 비교)

  • This research has academic significance in that it uses the data from "Research on Usage of Korean Medicine Service and Medicine" to analyze the differences in usage of Korean medicine service by regions and genders on a national scale. The items analyzed were the sociodemographic characteristics, preferences of forms of Korean medicine service by gender and province. The results showed that there were statistically significant differences by marital status, education, occupation, and income (p<.01). Men used service for back pains (23.0%), muscle injuries (20.7%), back sprain (16.5%), ankle sprain (11.2%), and arthritis (10.5%). Women used service for back pains (26.5%), arthritis (18.7%), muscle injuries (12.9%), back sprain (12.1%), and indigestion problems (10.1%). There were statistically significant differences among men and women (p<.01). In the comparison analysis by cities, back pain was the biggest reason (p<.01), but there were differences from city to city; In Gwangju, back sprains (24.4%) and ankle sprains (16.1%) were first and second reasons, while In Ulsan, back sprains were the first reason (29.8%). By province level, back pains were the biggest reason for men, but there were differences by provinces (p<.05); In Chungbuk and Jeonnam, muscle injuries were first (23.7% and 23.9%, respectively). In conclusion, there were differences by cities and provinces in usage of Korean medical service, as well as by gender.
