Dynamic Salivary Gland Scintigraphy in Clinical Sicca Syndrome: Comparison with Static images (구내 건조증을 호소하는 환자에서 역동적 타액선 신티그라피: 정적영상과의 비교)

  • Purpose: In this study, we compared the quantitative characteristics of dynamic salivary gland scintigraphy with static scintigraphy in patients with clinical sicca syndrome using Tc-99m pertechnetate. Materials and Methods: Fifty-two parotid glands and 52 submandibular glands out of 26 patients with clinical sicca syndrome were studied by dynamic and static salivary gland scintigraphy. Ten normal volunteers were also studied as a control group for comparison of scintigraphic parameters. Ten minutes after injection of 370 MBq Tc-99m pertechnetate, we obtained pre-stimulus static images for a few minutes. Then dynamic salivary gland scintigraphy with lemon juice stimulation was performed for 20 minutes. Finally we obtained post-stimulus static images after dynamic images. On dynamic study, functional parameters such as uptake rate, secretion rate and re-uptake rate were calculated. The results of dynamic study and static images were compared. Results: On dynamic study, we could obtain functional parameters of salivary glands successfully. On dynamic study, 22 parotid glands and 22 submandibular glands out of each of 52 glands are abnormal. The static images demonstrated somewhat different results, of which reasons we could assume via dynamic study. Conclusion: Dynamic salivary gland scintigraphy using Tc-99m perechnetate were more functional than static images and might be useful in the assessment of the functional change of the salivary gland in patients with clinical sicca syndrome.

Inhibition of Inflammation by Popillia flavosellata Ethanol Extract in LPSinduced RAW264.7 Macrophages (LPS로 염증 유도된 RAW 264.7세포에 대한 참콩풍뎅이(Popillia flavosellata) 에탄올 추출물의 항염증 효과)

  • The beetle Popillia flavosellata has been no reported its functional effects. In this study, we investigated the anti-inflammatory effect of P. flavosellata ethanol extract (PFE) on RAW 264.7 mouse macrophage cells treated with lipopolysaccharide (LPS) for the induction of inflammation. First, we examined the cytotoxicity of PFE in the RAW 264.7 cells at a concentration of 2,000 μg/ml or less. To evaluate the anti-inflammatory effects of PFE, we investigated the expression levels of proinflammatory cytokines, such as tumor necrosis factor (TNF)-α and interleukin (IL)-6, and proinflammatory enzymes, such as inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) in LPS-induced RAW 264.7 cells. In addition, we examined whether PFE inhibited the translocation of nuclear factor kappa B (NF-κB) p65 into the nucleus in the LPS-induced RAW 264.7 cells. We found that the protein levels of TNF-α and IL-6 were decreased in the LPS-induced RAW 264.7 cells after the treatment with PFE in a dose-dependent manner. In addition, we confirmed that PFE inhibited the translocation of NF-κB p65 into the nucleus, as well as the protein expression levels of iNOS and COX-2. Accordingly, we propose that PFE exerts an anti-inflammatory effect through the down-regulation of NF-κB p65, TNF-α, IL-6, iNOS, and COX-2 via the toll like receptor (TLR)-4 inflammatory signaling pathway.

The Effect of Heating Rate by Ohmic Heating on Rheological Property of Corn Starch Suspension (Ohmic Heating에 의한 가열속도 변화가 옥수수전분의 물성특성에 미치는 영향)

  • Granule swelling is essential phenomenon of starch gelatinization in excess water, and characteristic of heated starch dispersion depends largely on size and distribution of swelled starch granule. Although swelling characteristic of starch granules depends on type of starch, heating rate, and moisture content, influence of heating rate on swelling phenomenon of starch granule has not been fully discussed, because constant heating rate of starch dispersion cannot be obtained by conventional heating method. Ohmic heating, electric-resistant heat generation method, applies alternative current to food materials, through which heating rate can be easily controlled precisely and conveniently at wide range of constant heating rates. Starch dispersion heated at low heating rates below $7.5^{\circ}C/min$ showed Newtonian fluid behavior, whereas showed pseudoplastic behavior at heating rates above $16.4^{\circ}C/min$. Apparent viscosity of starch dispersion increased linearly with increasing heating rate, and yield stress was dramatically increased at heating rates above $16.4^{\circ}C/min$. Average diameter of corn starch granules during ohmic heating was dramatically increased from $30.97\;to\;37.88\;{\mu}m$ by increasing heating rate from $0.6\;to\;16.4^{\circ}C/min$ (raw corn starch: $13.7\;{\mu}m$). Hardness of starch gel prepared with 15% corn starch dispersion after heating to $90^{\circ}C$ at different heating rates decreased gradually with increasing heating rate, then showed nearly constant value from $9.4\;to\;23.2^{\circ}C/min$. Hardness increased with increase of heating rate higher than $23.2^{\circ}C/min$.

  • A numerical simulation has been performed for isothermal and reacting flows in an exisymmetric, bluff-body research combustor. The present formulation is based on the density-weighted averaged Navier-Stokes equations together with a k-epsilon. turbulence model and a modified eddy-breakup combustion model. The PISO algorithm is employed for solution of thel Navier-Stokes system. Comparison between measurements and predictions are made for a centerline axial velocities, location of stagnation points, strength of recirculation zone, and temperature profile. Even though the numerical simulation gives acceptable agreement with experimental data in many respects, the present model is defictient in predicting the recoveryt rate of a central near-wake region, the non-isotropic turbulence effects, and variation of turbulent Schmidt number. Several possible explanations for these discrepancies have been discussed.

Radiolysis Assessment of $^{18}F$-FDG According to Automatic Synthesis Module (자동합성장치에 따른 $^{18}F$-FDG의 방사선분해 평가)

  • Purpose : Among quality control items, the radiochemical impurity must be below 10% of total radioactivity. In this regard, as the recently commercialized automatic synthesis module produces a large amount of 18F-FDG, radiolysis of radiopharmaceuticals is very likely to occur. Thus, this study compared the changes in radiochemical purity regarding radiolysis of $^{18}F$-FDG according to automatic synthesis module. Materials and methods : Cyclotron (PETtrace, GE Healthcare) was used to produce $^{18}F$ and automatic synthesis module (FASTlab, Tracerlab MX, GE Healthcare) was used to achieve synthesis into FDG. For radiochemical purity, Radio-TLC Scanner (AR 2000, Bioscan), GC (Gas Chromatograph, Agilent 7890A) was used to measure the content of ethanol included in $^{18}F$-FDG. Glass board applied with silica gel ($1{\times}10cm$) was used for stationary phase while a mixed liquid formed of acetonitrile and water (ratio 19:1) was used for mobile phase. High-concentration and low-concentration $^{18}F$-FDG were produced in each synthesis module and the radiochemical purity was measured every 2 hours. Results : The purity in low-concentration (below 2.59 GBq/mL) was measured as 99.26%, 98.69%, 98.25%, 98.09% in Tracerlab MX and as 99.09%, 97.83%, 96.89%, 96.62% in FASTlab according to 0, 2, 4, 6 hours changes, respectively. The purity in high-concentration (above 3.7 GBq/mL) was measured as 99.54%, 96.08%, 93.77%, 92.54% in Tracerlab MX and as 99.53%, 95.65%, 92.39%, 89.82% in FASTlab according to 0, 2, 4, 6 hours changes, respectively. Also, ethanol was not detected in GC of $^{18}F$-FDG produced in FASTlab, while 100~300 ppm ethanol was detected in Tracerlab MX. Conclusion : Whereas the change of radiochemical purity was only 3% in low-concentration $^{18}F$-FDG, the change was rapidly increased to 10% in high-concentration. Also, higher radiolysis were observed in $^{18}F$-FDG produced in FASTlab than Tracerlab MX. This is because ethanol is included in the synthesis stage of Tracerlab MX but not in the synthesis stage of FASTlab. Thus, radiolysis is influenced by radioactivity concentration than the inclusion of ethanol, which is the radioprotector. Therefore, after producing high-concentration $^{18}F$-FDG, the content must be diluted through saline to lower concentration.

Current Status and Improvements of Transfered PET/CT Data from Other Hospitals (외부 반출 PET/CT 영상 현황 및 개선점)

  • Purpose: This study was performed to find the current problems of PET/CT data from other hospitals. Materials and Methods: The subjects were acquired from 64 hospitals referred to our department for image interpretation. The formats and contents of PET/CT data were reviewed and the phone questionnaire survey about these were performed. Results: PET/CT data from 39 of 64 hospitals (61%) included all transaxial CT and PET images with DICOM (Digital Imaging Communications in Medicine) standard format which were required for authentic interpretation. PET/CT data from the others included only secondary capture images or fusion PET/CT images. Conclusion: The majority of hospitals provided limited PET/CT data which could be inadequate for accurate interpretation and clinical decision making. It is necessary to standardize the format of PET/CT data to transfer including all transaxial CT and PET images with DICOM standard format.

Usefulness of "Volumetrix Suite" with SPECT/CT (SPECT/CT 영상에서 Volumetrix Suite의 유용성)

  • Purpose: The SPECT/CT is able to acquire diagnostic information resolved the difficult problems that discriminate regions of focals by intergrating functional images and anatomical images. We introduce the usefulness of "Volumetrix Suite" which can describe 3D images by the convergence of the SPECT/CT images and reference CT images. Materials and Methods: We applied Volumetrix Suite program (Volumetrix IR, Volumetrix 3D) to patients, Bone, Venography, Parathyroid, WBC, taken diagnostic CT examination which have same regions of focal in Seoul Metropolitan Government Seoul National University Boramae Medical Center. After acquiring SPECT/CT images and reference CT images, we fused a couple of scans applying for this programs. The CT scan of Infinia Hawkeye 4 shows limitation of anatomical information. For this reason, we tried to transfer CT images that have lots of diagnostic informations as the form of Dicom file in PACS, and changed from 2D images to 3D images after image registering in Xeleris Workstaion of Hawkeye 4. Results & Conclusion: By using Volumetrix Suite program, we're able to acquire more accurate anatomical informations with 3D rendering which can distinguish both location and range of focals in Infinia Hawkeye 4. Thus, the result of utilizing this program indicate that nuclear medicine anatomical images can be improved by providing more diagnostic imformations produced by its program.

Usefulness of Hepatocellular Carcinoma by Hepatic Arterial Perfusion Scintigraphy with $^{99m}Tc$-MAA ($^{99m}Tc$-MAA를 이용한 간세포암의 간동맥 관류 스캔의 유용성)

  • Purpose: $^{99m}Tc$-macroaggregated albumin (MAA) hepatic arterial perfusion scintigraphy was known for useful method to evaluate patients receiving intraarterial chemotherapy for liver cancer. This study evaluate about usefulness of normal liver on hepatocellular carcinoma (HCC) from HCC patients. This study is to see the usefullness of Hepatic Arterial Perfusion Scintigraphy (HAPS) by measuring mass size, shape, lung shunting and tumor to normal ratio (T/N ratio) in relative blood stream of HCC patients compared with HCC on normal liver. Materials and Methods: From June 2009 to September 2009, HAPS studies were performed on 7 patients (men 6, women 1, mean 64) who were diagnosed HCC. HAPS was performed after proper hepatic artery $^{99m}Tc$-MAA of 5 mCi (185 MBq) injection by catheter. We performed anterior, posterior, both lateral view, SPECT of chest and abdomen. Then we set up ROI and calculated lung shunting, T/N ratio for each count, count/pixel (mean value). Results: Tumor and liver size analyzed by ROI of anterior, posterior view are 2.0-10.8 cm (mean 3.75 cm), 8.8-18.5 cm (mean 14.6 cm). T/N ratio analyzed by total tumor and total normal mean value are 2.41-5.76 (mean 3.8). lung shunting analyzed by total liver count is 3.14-13.92% (mean 6.77%). Conclusion: HAPS with $^{99m}Tc$-MAA can evaluate mass size, location, quantitative analysis through T/N ratio. also HAPS can evaluate detection of arteriovenous shunt through lung uptake before radioisotope therapy. Therefore HAPS with $^{99m}Tc$-MAA can be useful method in aspect of evaluation and treatment of HCC.

Evaluation on the Usefulness of Alternative Radiopharmaceutical by Particle size in Sentinel Lymphoscintigraphy (감시림프절 검사 시 입자크기에 따른 대체 방사성의약품의 유용성평가)

  • Purpose Sentinel lymphoscintigraphy (SLS) was using only $^{99m}Tc-phytate$. If the supply is interrupted temporarily, there is no alternative radiopharmaceuticals. The aim of this study measure the particle size of radiopharmaceuticals and look for radiopharmaceuticals which can be substituted for $^{99m}Tc-phytate$. Materials and Methods The particle size of radiopharmaceuticals were analyzed by a nano-particle analyzer. This study were selected known radiopharmaceuticals to be useful particle size for SLS. We were divided into control and experimental groups using $^{99m}Tc-DPD$, $^{99m}Tc-MAG3$, $^{99m}Tc-DMSA$ with $^{99m}Tc-phytate$. For in-vivo experiment, radiopharmaceuticals were injected intradermally at both foot to perform lymphoscintigraphy. Imaging was acquired to dynamic and delayed static image and observe the inguinal lymph nodes with the naked eye. Results Particle size was measured respectively Phytate 105~255 nm (81.9%), MAG3 91~255 nm (98.7%), DPD 105~342 nm (77.3%), DMSA 164~ 342 nm (99.2%), MAA 1281~2305 nm (90.6%), DTPA 342~1106 nm (79.4%), and HDP 295~955 nm (94%). In-vivo delayed static image, inguinal lymph nodes of all experiment groups and two control groups are visible to naked eye. however, $^{99m}Tc-MAG3$ of control groups is not visible to naked eye. Conclusion We were analyzed to the particle size of the radiopharmaceuticals that are used in in-vivo. Consequently, $^{99m}Tc-DPD$, $^{99m}Tc-DMSA $are possible in an alternative radiopharmaceuticals of emergency.

Treatment of Early Glassy Cell Carcinoma of Uterine Cervix (초기 자궁경부 유리세포암의 치료)

  • Purpose: The purpose of this study was to investigate the clinical findings, treatment, and outcome of patients with glassy cell carcinoma of cervix. Materials and Methods: We reviewed all cases of glassy cell carcinoma of the uterine cervix confirmed and treated at the Dongsan Medical Center, Keimyung University, between January 1993 and December 2005. There were 7 cases with histopathologically confirmed gassy cell carcinoma. A tumor was diagnosed as glassy cell carcinoma if over 50% of the tumor cell type displayed glassy cell features. Six patients with stage IB had radical hysterectomy and bilateral pelvic node dissection, and 2 of them received adjuvant external pelvic irradiation with concurrent chemotherapy. Remaining one patient with stage IIA had curative concurrent chemoradiotherapy with external pelvic irradiation and brachytherapy. Results: There were 7 patients diagnosed as glassy cell carcinoma among the 3,745 (0.2%) patients of carcinoma of uterine cervix. The mean age of 7 patients was 44 years with range of 35 to 53 years of age. The most frequent symptom was vaginal bleeding (86%). By the punch biopsy undertaken before treatment of 7 cases, 2 only cases could diagnose as glassy cell carcinoma of uterine cervix, but remaining of them confirmed by surgical pathological examination. The mean follow up duration was 73 months with range of 13 to 150 months. All 7 patients were alive without disease after treatment. Conclusion: Glassy cell carcinoma of the uterine cervix is a distinct clinicopathologic entity that demonstrates an aggressive biologic behavior. However for early-stage disease, we may have more favorable clinical outcome with radical surgery followed by chemoradiothery.
