노인의 음주문제 발달궤적의 예측요인 : 다수준 성장곡선 모형의 적용 (Trajectories of Drinking problems of the elderly: A Longitudinal Multi-level Growth Curve Model for Change)

  • 본 연구는 노인들의 음주문제 발달궤적과 심리사회적 예측요인을 분석하는 데 그 목적이 있다. 이를 위해 한국복지패널 1차 조사에서 3차 조사까지에 해당하는 2006년부터 2008년까지의 자료에서 노인 연령에 해당하는 만 65세 이상의 남녀를 대상으로 연구를 진행하였다. 자료의 분석은 다수준 성장곡선 모델을 적용하여, 음주문제의 발달 궤적과 음주문제의 초기 값(intercept) 및 변화율(slope)의 예측요인을 밝히고자 하였다. 주요 연구결과는 다음과 같다. 첫째, 무조건적 다수준 성장곡선 모델을 수행한 결과, 노인의 음주문제는 첫 번째 측정시기인 2006년에서 세 번째 측정시기인 2008년까지 평균적으로 매년 조금씩 감소하고 있음을 알 수 있었다. 둘째, 조건형 다수준 성장곡선 모델을 수행한 결과, 연령이 낮을수록, 남성일수록, 결혼한 상태일수록, 종교가 없을수록, 빈곤층일수록, 스스로를 건강하지 않다고 인식할수록, 사회적 관계에 만족하지 않을수록 음주문제의 초기 값이 높은 것을 알 수 있다. 또한, 사회적 관계 만족도는 음주문제 변화율에 유의미한 예측요인으로 나타나 사회적 관계 만족도에 따라 노인의 음주문제 변화추이가 달라짐을 알 수 있다. 연구 결과를 바탕으로 다음과 같은 시사점을 얻을 수 있다. 첫째, 지역사회의 광범위한 사회복지현장에서 2차 예방차원으로 노인의 음주문제를 다룰 필요성이 있으며, 심리 사회적 위험요인들을 완화시킬 수 있는 다양한 프로그램의 개발이 필요하다. 둘째, 노인 문제음주자의 회복을 위해서 건강한 사회적 지지망의 유지 및 형성을 촉진시키는 것이 중요한 개입으로 필요함을 알 수 있다.

자녀를 둔 어머니의 출산과정 경험 (An Ethnographic Research Study on Childbearing Process of Mother with Children in Korea)

  • The childbearing process is a sociocultural phenomenon of a woman who gives birth to a child as well as a biological phenomenon. The purpose of this ethnographic research study was to explore the experience of childbearing process of mothers with children from pregnancy to the 3 months postpartum in Korea and to understand deeply the perspectives of childbearing women reflected on Korean sociocultural values. A convenient sample of 10 childbearing women were observed from January to October 2000 through field work in Seoul, Korea. Data analysis was accomplished under ongoing process. The results of this study were as follows : The mothers with children experienced self-reflection, family relation, and physical adaptation during pregnancy. In self-reflection, all mothers experienced universality and diversity in their self-discovering process. The universal experiences were maturation, life with family and priority on maternal value between being a mother and a woman. The diverse experiences were taking a dual role of working mother, emotional drift of a resigned mother, and disheartened life of a mother who has two daughters. In family relation, the foundation of the new marital relationship were attained during childbearing process and sexual life were changed for the benefit of a healthy mother and a healthy baby. All mothers established friendly relations with their mothers, but established friendly or conflicting or constraining relations with their mother-in-laws due to husband based family culture. In physical adaptation, the informants endured well the physical discomfort and recognized general appearance change. Also maternal-fetal interaction occurred and mothers realistically felt motherhood and accepted themselves as mother-to-be. The mothers prepared for the best delivery, look for a safe childbirth center, newborn goods, endorsed family coping during hospitalization and responded labor pain to make it more endurable, less painful, fast passed owing to labor recognition of the natural process to be a mother. After childbirth, they felt emancipation, satisfaction, accomplishment, more easiness, actually feeling as mother-to-be, emptiness, and showed response to the sex of newborn. Their Sanhujori practice was different according to the Sanhujori environment including provider, place, time in postpartum and reflected on Sanhubyung. The mothers felt actually mother-to-be and happiness during lactation regardless of feeding pattern. These mothers had a different maternal image about rearing subjecthood through their child-rearing experience. But all mothers felt need for family support and social support. The universal rearing response were actual feeling of mother-to-be, a strenuous experience, a pride on child-rearing, confusion, reflecting marital relationship, and wondering rivalry among children. In conclusion, mother of all with children went through self-discovery, self-reflection and made connections with the family as a mother and as a woman simultaneously during the childbearing process. Therefore it is suggested when harmony and balance between a mother and a woman is accomplished, the woman will lead a healthy and high quality of life. Also, this study sought to confirm the sociocultural factors affecting the childbearing process from the perspectives of the women with children. Therefore health care providers must understand deeply the childbearing women with children based on this finding of and try a integrative approach with new ideology of maternity with biocultural perspectives in a clinical setting.

아파트 경비원의 건강상태와 관련요인 (Health of Apartment Guards and Associated Factors)

  • 아파트 경비원들의 교대근무, 좁은 공간에서의 근무 등에 기인하는 여러 증상 및 이에 영향을 미치는 요인을 분석하기 위해 182명의 아파트 경비원을 대상으로 한 연구 결과는 다음과 갈다. 1) 조사된 아파트 경비원 중 97명(53.3%)에서 소화기장애 증상을 보였으며, 평균 1개의 소화기장애 증상을 가지고 있었다. 2) 연구대상자의 83.5%에서 한 가지 이상의 신체화 증상을 보였으며, 평균 3.1개의 신체화 증상을 가지고 있었다. 3) 연구대상자의 79.7%에서 한 가지 이상의 우울증상을 보였으며, 평균 3.7개의 우울증상을 가지고 있었다. 4) 연령 및 배우자 유무에 따라서는 소화기 장애점수, 신체화 장애점수, 우울점수 모두 차이가 없었으나 학력에 따라서는 소화기 장애점수는 차이가 있었다. 5) 건강관련 행태 중 음주 및 흡연여부, 운동유무에 따라서는 유의한 차이를 보이지 않았으나, 규칙적 식사유무와 주관적 건강상태에 따라 소화기 장애접수, 신체화 장애점수, 우울점수 모두 유의한 차이가 있었다. 6) 업무와 관련된 특성 중 교대주기, 종사기간, 관리가구 수에 따라서는 차이가 없었으나, 업무만족도에 따라 신체화 장애점수, 우울점수는 유의한 차이가 있었다. 추후 추적조사 및 심층면담을 통한 조사를 통하여 건강근로자효과(healthy worker effect)에 의한 영향 및 다른 교대업무와의 차이점에 대한 조사가 필요하다.

주관적 건강인식수준에 따른 한방의료기관 이용환자의 특성 비교 - 2011년 한방의료이용 및 한약소비실태조사(보건복지부)를 중심으로 - (Characteristics of Patients' Self-Perceived Health in Traditional Korean Medical Facilities - Based on the Ministry of Health and Welfares Report on Usage and Consumption of Korean Medicine in 2011 -)

  • Objective : The purpose of this study was to identify characteristics of patients' self-perceived health in traditional Korean medical facilities. Method : This research was conducted based on the survey on patients whom have visited traditional Korean medical facilities in 2011 by the Ministry of Health and Welfares and Korean Institute for Health and Social Affairs. Using a sample of 3,931 (1,180 male and 2,751 female) outpatients' self-perceived health based on the data from usage and consumption of Korean Medicine. 'Healthy', 'Fair', and 'Poor Health' were used to measure patients' self-perceived health status. The data was analyzed by frequency, t-test, cross correlation analysis and multiple logistics regression analysis using the SPSS program package. Results : Sex(P<0.001), age(P<0.001), marital status(P<0.001), education(P<0.001), employment status(P<0.001), occupation(P<0.001), health insurance(P<0.001) and income level(P<0.0001) showed statistical significance. Main Treatment Facilities(P<0.001), experience of taking Korean medicine(P=0.032), experience of receiving acupuncture treatment(P<0.001), number of visits(P<0.001), medical expense (P=0.005), and subjective health status after the treatments showed statistical significance for Korean herbal medicine(P=0.038), acupuncture (P=0.001), cupping therapy(P=0.006), oriental physiotherapy(P=0.003), and treatment satisfaction(P<0.001). For subjective health status based on suffering disorders in the past three months, the response of poor health was higher in the group suffering recent illnesses. Statistical significance was seen in hypertension (P=0.002), arthritis(P<0.001), lumbar pain(P<0.001), diabetes mellitus(P=0.001), stroke(P<0.001), hwa-byung (P=0.001), gastric disorders(P=0.021), common cold(P<0.001), ankle sprain(P<0.001), muscular injury(P<0.001), lumbar sprain(p=0.009) and fracture(P=0.03). Also the number of diseases during the past three months showed statistical significance(P<0.001). Statistical significance was also seen in Level of knowledge(P<0.001), route of information(P<0.001), reliability of Korean medicine(P=0.003), insurance coverage(P=0.005), medical costs(P<0.001), and future willingness to use Korean medicine(P<0.001). As a result of the multiple logistics regression analysis, risks of subjective poor health statistically increased in female population, elderlies, medicaid beneficiaries, less educated, higher medical expense, and more disorders during the past three months. Conclusion : Patients' self-perceived health status has significant differences with each variables such as sex, age, marital status, education, health insurance, medical expense, number of diseases.

치과기공사의 전문직 자아개념 연구 - 서울지역 근무자에 한하여 - (A Study on the Dental Technicians' Professional Self-Concept - Centering on the Dental Technicians in Seoul Area -)

  • The aim of this study was to make dental technicians recognize the importance of their professional self-concept, and to provide basic data to devise the educational programs and policies of establishing their positive professional self-concept. In addition, the investigator tried to present some improvement measures for dental technicians' actual services by examining their job satisfaction. Study findings are as follows: First, the score of study subjects' self-concept is 3.41($\pm$.58) (perfect score=5), middle or high in rank. As for the mean score of professional self-concept by realms, the self-concept of communication is 3.59($\pm$.67), the highest; the self-concept of leadership in the realm of professional service is 3.54($\pm$.75); the self-concept of skills is 3.50($\pm$.75); the self-concept of adaptability is 3.47($\pm$.69); and the recognizant level of satisfactory self-concept is 2.95($\pm$.81), below the normal. Second, as for the difference of professional self-concept according to general characteristics, such variables as age, marital states, and the length of one's service have a significant influence upon the recognition of self-concept. First, the self-concept of those whose age is between 31 and 35 is the highest (3.64$\pm$.49), and it is followed by that of those whose age is 36 and above (3.57$\pm$.77) and then that of those whose age is between 26 and 30 (3.31$\pm$.56)). And that of those whose age is below 25 is the lowest. The obove results show us that the professional self-concept of older study subjects is higher than that of younger ones in general(p<.01). As for marital states, that of married persons(3.54$\pm$.64) is higher than that of unmarried ones(3.35$\pm$.55)(p<.05). As for the length of one's service, that of longtime employees is higher than that of short-period workers(p<.01). In relation to positions, a manager's professional self-concept is the highest(3.89$\pm$.55) and that of an assistant technician is the lowest(3.17$\pm$.58). Study findings show that the professional self-concept of higher ranking technicians is higher than that of lower ones in general (p<.001). Based on the study findings, the investigator suggests followings: First, studies on the professional self-concept that is appropriate to the characteristics of dental technicians must be carried out with a new point of view. Second, a tool must be researched and developed in order for the professional self-concept of dental technicians to be measured. Third, if a new professional self-concept is established and a measuring tool is developed, a study on the relationship between dental technicians' professional self-concept and practice performance can be carried out again.

EuroQoL-5 Dimension 한국 가중치 모형의 적용 연구 (A Study on the Application of the Korean Valuation Weights for EuroQoL-5 Dimension)

  • Objectives: This study was conducted to estimate the health-related quality of life (HRQOL) using EuroQoL-5 Dimension (EQ-5D) and to identify its related factors among urban-dwelling adults. Methods: The data for this study were obtained from 1,134 subjects aged $20\sim91$, who participated in 'Survey on the health status and demand for health' in two cities of Korea (Dong-gu, Gwangju and Suncheon-si, Jeollanamdo). The HRQOL was measured using the EQ-5D instrument and EQ-5D index scores were calculated by two Korean valuation study model using time trade-off method. Results: The mean EQ-5D index scores for all subjects were $0.865{\pm}0.218$ (model A), and $0.921{\pm}0.170$ (model B). The EQ-5D index score was significantly different according to demographic and socioeconomic characteristics (gender, age, marital status, education, occupation, income, and health security system), self-rated health condition, health-related psychological assessments (enough sleep, fatigue rate, stress rate, and degree of satisfaction on the residence). The results of multiple linear regression showed that age, marital status, income, coverage of medical insurance, self-rated health condition, and fatigue rate were significantly related common statistical factors of HRQOL in two Korean valuation study model. Conclusion: Among the adults residing in urban environment, the HRQOL was significantly lower on the subjects with following conditions: higher age, being alone without a spouse as a result of death, divorce or separation, low income, medical aid program, poor self-rated health condition, and chronic fatigue. In order to improve the urban adults' quality of life, healthcare policy and health promotion program must be developed with considerations to factors related to the HRQOL.

남성노인의 성생활인식도와 우울과의 관계연구 (A Correlational Study on Perceived Sex life and Depression in Old Men)

  • Purpose: Sexuality is important for the quality of life in old men, but this issue has been less explored due to social prejudice. Therefore, this study was carried out to describe a correlation between sex life and depression perceived by old men. Method: The subjects were 370 old men who were aged over 60 and lived in a city and three provinces. Instruments used in this study were Perception of Sex life Scale 01 items) and Depression Scale (20 items). The data was collected from December 2001 to February 2002 by written responses of the subjects to the questionnaire as well as one-to-one interview by research assistants for subjects. Statistical analysis for the data was done using SPSS and the level of significance was tested at 95% of confidence interval. Result: 1) Mean age of the subjects was $69.35{\pm}5.70$. A total of 253 subjects (68.6%) had spouse, and 191 of the subjects (52.3%) had sexual intercourse as frequently as 3 times per month. The average score for perception of sex life was $21.25{\pm}5.06$, out of the total score of 44, and that of depression was $35.08{\pm}9.29$ out of the total score of 80. 2) Perception of sex life was statistically significant by age, education, disease, medication, living arrangement, marital status, presence of girlfriend, pattern of inter-coitus, pocket money age of last coitus, subjective health state, and relationship (P<0.05). 3) Depression was statistically significant by age, education, disease. medication, living arrangement, marital status, satisfaction for marriage life, pattern of inter-coitus, feeling for change of sex life, pocket money, subjective health state, and relationship (P<0.05). 4) The correlation between perception of sex life and depression was negative, showing that depression tended to decrease as perception of sex life increases. Conclusion: It is concluded that depression in old men is closely related to their sex life. Positive sex life may have an important effect on the quality of life in old men.

  • IMF이후 경쟁의 심화와 적대적인 경영환경에 효과적으로 대응하고자 많은 호텔기업들이 구조조정을 실시해오고 있다. 이러한 구조조정은 인력감축을 동반하기 때문에 구조조정 과정에서 공정성을 지각하는데, 인력감축에서 살아남은 생존자들이 인력감축의 절차나 실무에서 불공정성을 지각할 때 상사에 대한 신뢰감이나 조직유효성을 감소시키는 것으로 나타났다. 따라서 본 연구는 호텔기업을 대상으로 구조조정 이후 살아남은 생존자를 대상으로 정리해고의 공정성 지각이 경영진의 신뢰, 직무만족 및 조직몰입에 어떠한 영향을 미치는지를 살펴보고자 하는 것이다. 이러한 연구목적을 달성하기 위해 실증분석을 실시한 결과 잔류종업원들은 구조조정 과정에서 절차 공정성 및 분배 공정성을 높게 지각 할수록 경영진에 대한 신뢰감과 조직 몰입이 높아지는 것으로 나타났다. 그러나 구조조정 과정에서 절차 공정성을 높게 지각할수록 직무만족은 높아지는 것으로 나타났지만, 분배 공정성은 직무만족과는 인과관계가 없는 것으로 나타났다. 또한 구조조정 과정에서의 경영진에 대한 신뢰감이 높아질수록 직무만족이나 조직몰입은 높아지는 것으로 나타났다.

간호·간병통합서비스를 이용한 입원환자가 인식한 간호사의 의사소통유형, 간호서비스 만족도 및 병원재이용 의도의 관계 (The Relation of Communication Types of Nurses, Satisfaction on Nursing Services and Intent to Revisit the Hospital Perceived by Patients Admitted to Comprehensive Nursing Care Service)

  • 본 연구는 간호 통합서비스가 확대 실시되고 있는 시점에 맞추어 입원환자가 인식한 간호사의 의사소통유형과 간호서비스 만족도, 병원 재이용 의도를 파악하고, 병원 재이용 의도에 영향을 미치는 영향요인을 규명하고자 시도하였다. 연구대상자는 경상남도에 소재한 간호 간병서비스를 제공하는 100병상 이상의 6개 종합병원의 입원환자 123명을 대상으로 하였다. 자료 수집은 2017년 7월 1일부터 7월 30일까지의 기간 동안에 자가 보고형 설문조사를 하였다. 수집된 자료는 SPSS 21.0 프로그램을 이용하여 t-test와 ANOVA, Person's correlation coefficients, hierarchical regression으로 분석하였다. 연구결과는 다음과 같다. 대상자의 병원 재이용 의도 정도는 연령(F=9.83, p<.001), 성별(t=-3.82, p<.001), 배우자 유무(t=2.24, p=.028), 학력(F=11.92, p<.001), 입원기간(F=7.81, p<.001)에 따라서 통계적으로 유의한 차이가 있었다. 병원 재이용 의도는 정보 제공적(r=.609, p<.001), 친화적(r=.536, p<.001) 의사소통 유형과 간호서비스 만족도(r=.797, p<.001)간에 유의한 정적 상관관계가 있었으며, 권위적 의사소통 유형(r=-.356, p<.001)간에는 유의한 부적 상관관계가 있었다. 병원 재이용 의도에 영향을 미치는 요인은 정보 제공적 의사소통 유형(${\beta}=.38$, p=.003)과 간호서비스 만족도(${\beta}=.84$, p<.001)이었다. 따라서 간호 간병통합서비스 입원환자의 간호서비스 질과 고객만족을 향상시키기 위해서는 간호사와 환자의 효과적인 의사소통을 위한 다양한 교육 프로그램 개발과 체계적인 교육이 필요하다.

J지역 치과위생사의 직무만족과 이직의 상관관계 분석 (Analysis of the Correlation of Job Satisfaction to Turnover Among Dental Hygienists in the Region of J)

  • 본 연구는 치과위생사들의 이직요인을 찾아보고, 직무만족과 이직과의 상관관계 등을 살펴봄으로써 치과위생사들의 이직률 감소를 위한 기초자료를 제공하고자 2006년 7월 24일부터 9월 24일까지 전라북도의 구강진료기관에 근무하는 치과위생사를 180명을 대상으로 구조화된 자기기입식 설문지를 이용하였다. 분석방법으로는 SPSS 12.0을 이용하여 치과위생사의 이직 경험유무에 따른 치과위생사의 일반적 특성 및 이직 사유, 이직희망기관, 직무만족과 이직과의 상관관계 등을 분석하여 다음과 같은 결과를 얻었다. 1. 이직경험은 연령별에서 24~26세의 36.2%, 미혼 83.0%, 경력별로는 1~3년 미만의 50.0%가 이직 경험이 있는 것으로 나타났으며(p < 0.001), 월 평균 소득별에서도 100~129만원의 50.0%가 이직경험이 있는 것으로 나타나 통계적으로 유의한 차이가 있었다(p < 0.05). 2. 이직 사유는 근무환경이 28.1%로 가장 높게 나타났으며, 발전가능성은 18.0%, 상사 및 동료와의 관계는 12.4%, 일에 대한 보상 체계는 4.5%로 나타났다. 3. 이직 희망 근무기관은 개인치과의원에 근무하고 있는 치과위생사의 66.2%가 보건(지)소로 이직을 희망하고 있는 것으로 나타났으며, 최종학력에서는 대학 졸업자의 64.3%가 보건(지)소로 이직을 희망하고 있는 것으로 나타났다(p < 0.001). 4. 직무만족 요인 중 장래성요인이 이직에 가장 크게 영향을 미치는 것으로 나타났으며, 다음으로 업무량요인, 급여요인, 동료관계요인 순으로 나타났고, 이러한 모든 요인들이 음(-)의 상관관계로 나타나 만족수준이 낮을수록 이직은 높아짐을 알 수 있었으며 통계적으로 유의한 차이가 있었다. 직무만족요인들이 이직의도에 미치는 영향을 설명하는 $R^2=.254$로 나타났다.
