  • Background: Neer type II distal clavicle fractures have the drawback of coracoclavicular instability and insufficient distal bony fragment, thereby making it difficult to achieve adequate fixation. Although various surgical treatments have been described for Neer type II fracture, the optimal treatment remains controversial. This study reports the clinical results and usefulness of anatomical locking plate with additional K-wire fixation. Methods: A totally of 21 patients with type II distal clavicle fracture were included in the study. The surgical procedure reduced the fracture temporarily; it included insertion of one or two K-wire from the lateral margin of the distal fragment to the proximal fragment through the fracture site, followed by application and fixation of the locking plate. The bony union and migration of K-wire was evaluated in the follow-up radiography. The coracoclavicular distance and acromioclavicular joint arthrosis were assessed at the final follow-up. The Constant Score (CS) and Korean Shoulder Score (KSS) were evaluated for clinical scoring. Results: Bone union was achieved in all cases. At the final follow-up, coracoclavicular distance of the injured shoulder was increased, as compared to the intact shoulder (p=0.002), with no accompanying clinical symptoms. No K-wire migration was observed. At the final follow-up, K-wire irritation was observed in two cases and acromioclavicular arthrosis in one case, with no other adverse effects. Pain visual analogue scale, CS, and KSS were improved in all cases. Conclusions: The method of anatomical locking plate with additional K-wire fixation could be useful in achieving beneficial clinical results.

  • Ilsan peat-bed site, newly constructed city area, is located at the northern part of Seoul. This site is situated near the mouth of Han river, which flows through central Korea. Many fossil woods were excavated from this site, and surveyed the wood anatomical features. Totally 90 specimens were identified into 10 species including two roots and five barks which were not identified in detail. Alnus japonica and A. maxmowiczii are the most common and occupied 26. 7%, 34.4% of the total specimen number respectively, and then other species which excavated from Ilsan area are follows : Fraxinus rhynchophylla(18.9%), Lepido-balanus spp.(3.3%), Maackia amurensis(2.2%), Diospyros spp.(2.2%), Juglans mandshrica(1.1%), Prunophora spp.(1.1%), Malus or Pyrus spp.(1.1%), Rhamnus spp.(1.1%). Among the identified species there are fairly some species growing in the central - or northern temperate zone, such as A. maxmowiczii, F. rhynchophylla, Lepidobalanus spp., M. amurnsis, J. mandshurica, Rhumnus spp. Therefore, it can be estimated that the former temperature of Ilsan area was same or slightly cold than today. The species bearing fruit for food as .J, mandshunca, Diospyros spp., Prunophora spp., Mulus or Pyrus spp. were regarded as artificially planting trees, because they do not grow together in the same site.

  • In this work, a method is presented to find an anatomical site of a current source crudely in a standard brain using grand average of MEG data. Minimum norm estimation algorithm and truncated singular value decomposition were applied to calculate the distributed sources that can reproduce the measured signals. Grand average over all subjects was obtained from the transformed signals, which would be detected in a standard sensor plane by the obtained distributed current sources. In the simulation study, it was shown that the localized dipole using the grand average is consistent with the mean location of localized dipoles of all subjects within several mm even with large inter-individual differences of sensor positions. This result suggests that the mean location of low level signal source can be estimated as a dipole source in grand average and it was confirmed in the localization of the current source of N100m. when the localized dipole is registered on a standard brain. This result also suggests that the activity region obtained from grand average can be crudely estimated on a standard brain using the source location of the N100m as a reference point.

  • Animal bones that were excavated with earthenware at the Jongdal-ri shell mound archaeological site IV on Jeju Island were investigated morpholgically and osteometrically. This archaeological site is estimated to date from the late Tamra period to the unified Shilla Dynasty (A.D. 7C) based on the type of earthenware. The number of animal bones was 3,430 pieces(Quarter B : 758, Quarter C : 2,672). The animal species consisted of Cervus spp., Sus spp., Bos spp and Canivore. The animal bone pieces excavated at Quarter B consisted of those of Cervus(53.3%), Bos(2.8%) and Sus(13.7%). The bones of Cervus(53.3%), Bos(2.8%), Sus(13.7%) and Canivore(4.6%) were found at Quarter C. The greatest number of heads was unearthed as a whole, and the classification according to animal species showed that the heads were more visible than other bones. Forelimbs and hindlimbs were distributed evenly on the part of Cervus. The skeletons from Bos were identified in the order of heads, forelimbs and hindlimbs, while nearly all the Sus bones were found to be heads. These data suggests that Cervus, Sus and Bos are major fauna at the estimated period.

  • Outflow cannulation site of left ventricular assist device(LVAD) chosen by considering anatomical structure of thoracic cavity and vascular system. Though outflow cannulation site influences blood perfusion at each branch, there is no standard rule or quantitative data. In this study, we computed the amount of blood perfusion at each arterial branch numerically according to outflow cannulation sites(ascending aorta, aortic arch, descending aorta). We generated computational meshes to the three-dimensionally reconstructed arterial system. Clinically measured arterial pressure were used for inlet boundary condition, porous media were applied to mimic blood flow resistance. Blood perfusion through left common carotid artery was 2.5 times higher than other cases, and that through right common carotid artery was 1.1 times higher than other branches. Although this is simulation study, will be useful reference data for the clinical study of LVAD which considers blood perfusion efficiency.

  • Objectives: The aim of this study is to determine prevalence and related factors of musculoskeletal symptoms in electronics assembly line workers. Method: A questionnaire survey was performed on 350 workers in In-chon city. The questionnaire consisted of questions on the general characteristics, work duration, working hours, hobby activity hours, housekeeping activity hours and standardized descriptions of NOISH on musculoskeletal symptoms. Result: Complaint rates of musculoskeletal symptoms by anatomical site were as follows: shoulder, 67% : neck, 40.4% : waist, 28.7% : wrist and hand, 27.1% : foot and leg, 14.4% : arm and elbow, 11.2%. Significantly higher prevalence of shoulder musculoskeletal symptom was found women than men, married than unmarried, less than 1hours housekeeping activity than over 1hours housekeeping activity. Multiple logistic regression analysis identified gender, marital status and housekeeping activity as the main affecting factors for shoulder musculoskeletal symptoms. Conclusions: The most prevalence of musculoskeletal symptoms was shoulder site. It was higher in married women and over 1hours housekeeping activity in a day. It is recommended that married women should be reduce housekeeping activity time.

  • This study describes recent trends in incidence, survival and prevalence of subgroups of esophageal and gastric cancer in Linzhou city between 2003 and 2009. Data of esophageal and gastric cancer for the period of interest were extracted from the Linzhou Cancer Registry. Using information on tumor morphology or anatomical site, data were divided into six groups; esophageal squamous cell carcinoma, esophageal adenocarcinoma, other and unspecified types of esophageal cancer, and cardia, non-cardia, and unspecified anatomical site of stomach cancer. Incidence, survival and prevalence rates for each of the six cancer groups were calculated. The majority of esophageal cancers were squamous cell carcinomas (82%). Cardiac cancer was the major gastric cancer group (64%). The incidence of esophageal squamous cell carcinoma and gastric cardiac cancer increased between 2003 and 2009. Both esophageal and gastric cancer had a higher incidence in males compared with females. Overall survival was poor in all sub-groups with 1 year survival ranging from 45.9 to 65.6% and 5 year survival ranging from 14.7 to 30.5%. Prevalence of esophageal squamous cell carcinoma and gastric cardiac cancer was high (accounting for 80% overall). An increased focus on prevention and early diagnosis, especially in esophageal squamous cell carcinoma and gastric cardiac cancer, is required.

  • Objective: To investigate the prognostic role of antigen KI-67 (Ki-67) and G1/S-specific cyclin-D1 (cyclin-D1) in patients with laryngeal squamous cell carcinoma (LSCC). Methods: Immunohistochemical staining (IHS) was used to determine the protein expression of Ki-67 and cyclin-D1 in LSCC tissues. Kaplan-Meier survival curves was calculated with reference to Ki-67 and cyclin-D1 levels. Results: Cyclin-D1 and Ki67 were expressed in the nuclei of cancer cells. Among the total of 92 cancer tissues examined by immunohistochemistry, 60 (65.22%) had cyclin-D1 overexpression and 56 (60.87%) had Ki67 overexpression. Cyclin-D1 overexpression is associated with the advanced stage of the cancer (P=0.029), but not with gender, age, stage of cancer, histological differentiation, anatomical site, smoking history and alcohol consumption history. Ki67 overexpression is not associated with the advanced stage, gender, age, histological differentiation, anatomical site, smoking history and alcohol consumption history. A statistically significant correlation was found between lymph node status and the expression of Ki67 (p = 0.025). Overexpression of cyclin-D1 was correlated to shorter relapse-free survival period (P<0.001). Conclusions: Overexpression of cyclin-D1 can be used as a marker to predict relapse in patients with LSCC after primary curative resection.

  • Background: Knowledge of the anatomical variation of the vertebral artery has clinical importance not only for the performance of interventional or surgical procedures itself but also to ensure their safety. We conducted a study of the anatomical variation by reviewing multi-detector computed tomography (MDCT) images of the cervical spine from 460 Korean patients. Methods: 16-row MDCT data from 460 patients were used in this study. We observed 920 vertebral arteries. Examination points included level of entrance of the artery into the transverse foramen of the cervical vertebra, origin site of the vertebral artery, course of a vertebral artery with aberrant entrance. Result: The vertebral artery in 2 (0.2%) cases in this study entered into the transverse foramen of the 7th cervical vertebra from the left. In 45 (4.9%) cases, the vertebral artery entered into the transverse foramen of the 5th cervical vertebra. Of these, the entrance was on the right in 15 (1.6%) and on the left in 30 (3.3%). We found 17 (1.8%) cases in which the artery entered into the transverse foramen of the 4th cervical vertebra, 10 (1.1%) on the right and 7 (0.7%) on the left side. As is commonly acknowledged, the 6th cervical vertebra was the most common site of entry; the vertebral artery entered the transverse foramen of the 6th cervical vertebra in the remaining 855 (93.0%) cases, on the right in 434 (47.2%) and on the left in 421 (45.8%). Conclusions: In conclusion, the possibility of an atypical course of the vertebral artery in segments V1 and V2 should be evaluated with magnetic resonance imaging (MRI) or CT images before carrying out procedures involving the anterior cervical vertebrae.
