Nocardia farcinia, an aerobic, gram-positive bacilli actinomycetes of the genus Nocardia, is an uncommon pathogen found in humans. The most common Nocardia infection sites are the lung, central nervous system, and skin. Even though hematogenous dissemination can occur, isolation of the organism from blood cultures is very rare. We report a case of Nocardia infection that was isolated on blood cultures. A 59-year-old male with a medical history that includes a liver transplantation 6-years prior due to hepatocellular carcinoma secondary to chronic hepatitis B, developed pneumonia and was transferred to Severance Hospital. At the time of admission, the patient's initial exam showed hypothermia, tachypnea, and hypotension. His chest radiograph showed severe pneumonia and a large abscess on left upper lobe. Under the presumptive diagnosis of bacterial pneumonia or other opportunistic infection, we started broad spectrum antibiotics. However, he developed Nocardia sepsis, rapidly deteriorated, and subsequently died.
Background: High concentrations of particulate matter less than 2.5 ㎛ in diameter (PM2.5) in poultry houses is an important cause of respiratory disease in animals and humans. Pseudomonas aeruginosa is an opportunistic pathogen that can induce severe respiratory disease in animals under stress or with abnormal immune functions. When excessively high concentrations of PM2.5 in poultry houses damage the respiratory system and impair host immunity, secondary infections with P. aeruginosa can occur and produce a more intense inflammatory response, resulting in more severe lung injury. Objectives: In this study, we focused on the synergistic induction of inflammatory injury in the respiratory system and the related molecular mechanisms induced by PM2.5 and P. aeruginosa in poultry houses. Methods: High-throughput 16S rDNA sequence analysis was used for characterizing the bacterial diversity and relative abundance of the PM2.5 samples, and the effects of PM2.5 and P. aeruginosa stimulation on inflammation were detected by in vitro and in vivo. Results: Sequencing results indicated that the PM2.5 in poultry houses contained a high abundance of potentially pathogenic genera, such as Pseudomonas (2.94%). The lung tissues of mice had more significant pathological damage when co-stimulated by PM2.5 and P. aeruginosa, and it can increase the expression levels of interleukin (IL)-6, IL-8, and tumor necrosis factor-α through nuclear factor (NF)-κB pathway in vivo and in vitro. Conclusions: The results confirmed that poultry house PM2.5 in combination with P. aeruginosa could aggravate the inflammatory response and cause more severe respiratory system injuries through a process closely related to the activation of the NF-κB pathway.
실험용 마우스(ICR)에 프레드니솔론을 주어 인공적으로 면역억제시키면서 Cryptosporidium 발현 여부를 7주 동안 관찰한 바 다음과 같은 결과를 얻었다. 면역 억제 1주 후부터 마우스 대변에서 Cryptosporidium의 Oocyst (크기 440 ㎛)가 검출되었고 2주 후부터 마우스가 사망하기 시작하였다. 대변에서 oocyst가 검출된 마우스의 소장 절편을 H&E 염색하여 광학현미경으로 관찰한 결과 Cryptosporisium은 공장 하부에서 호발하였으며 장 융모의 점막 상피세포 표면에 점같이 붙은 모습으로 무수히 발견되었고 호염기성이었으며 크기는 직경 2∼4 ㎛정도 되었다. 이들의 여러 발육 단계에 대해 주사 전자현미경 및 투과 전자현미경으로 입체구조를 관찰하였으며 그 결과 충체의 종은 C. Parvum으로 동정하였다. 항균제(tetracycline, trimethoprim-sulfamethoxazole)를 면역억제제와 함께 투여한 실험군에서도 Cryptosporidium이 발현되었으나 면역 억제제만을 투여한 실험군의 마우스보다 수명을 다소 연장시킬 수 있었다. 이 실험 결과 우리 나라에도 Cryptosporidium(C. Parvum)이 존재하고 있음을 확인하였으며 인체 감염례가 있을 가능성을 짐작할 수 있었다.
Klebsiella pneumoniae is an opportunistic and clinically significant emerging pathogen. We investigated the relative roles of Toll-like receptor (TLR) 2 and TLR4 in initiating host defenses against K. pneumoniae. TLR2 knockout (KO), TLR4 KO, TLR2/4 double KO (DKO), and wild-type (WT) mice were inoculated with K. pneumoniae. Mice in each group were sacrificed after either 12 or 24h, and the lungs, liver, and blood were harvested to enumerate bacterial colony-forming units (CFU). Cytokine and chemokine levels were analyzed using enzyme-linked immunosorbent assay and real-time PCR, and pneumonia severity was determined by histopathological analysis. Survival was significantly shortened in TLR4 KO and TLR2/4 DKO mice compared with that of WT mice after infection with $5{\times}10^3CFU$. TLR2 KO mice were more susceptible to infection than WT mice after exposure to a higher infectious dose. Bacterial burdens in the lungs and liver were significantly higher in TLR2/4 DKO mice than in WT mice. Serum $TNF-{\alpha}$, MCP-1, MIP-2, and nitric oxide levels were significantly decreased in TLR2/4 DKO mice relative to those in WT mice, and TLR2/4 DKO mice showed significantly decreased levels of $TNF-{\alpha}$, IL-6, MCP-1, and inducible nitric oxide synthase mRNA in the lung compared with those in WT mice. Collectively, these data indicate that TLR2/4 DKO mice were more susceptible to K. pneumoniae infection than single TLR2 KO and TLR4 KO mice. These results suggest that TLR2 and TLR4 play cooperative roles in lung innate immune responses and bacterial dissemination, resulting in systemic inflammation during K. pneumoniae infection.
나무가 갖는 특유의 향인 피톤치드는 휘발성 화학물질로서, 식물이 방어를 위해 스스로 생성하여 발산하는 물질이며, 항균, 방충, 소취 등 다양한 기능을 가지고 있는 천연 물질이다. 편백나무(Chamaecyparis obtusa Sieb. et Zucc.; Japanese cypress)에서 추출한 피톤치드 정유가 구강 내 상주균주이면서 구강 캔디다증을 일으키는 Candida albicans에 미치는 효과를 알아보기 위해 편백 피톤치드와 함께 C. albicans 를 배양한 후, 성장정도, 생존력 및 형태적 변화를 관찰한 결과, 피톤치드는 C. albicans에 강한 항진균력을 보였고, C. albicans에 대한 피톤치드의 최소억제농도(MIC)는 0.25%, 최소살균농도(MFC)는 0.5%였으며, 성장 곡선 상에서, MIC에 근접한 농도의 피톤치드는 C. albicans의 성장을 억제하였다. 피톤치드를 첨가하여 배양한 C. albicans는 투과전자현미경 상에서, MIC 농도에서 대다수의 세포가 세포질 내 전자밀도가 높은 과립물질과 공포를 보이는 비전형적인 형태를 보였다. 이상의 결과로, 피톤치드는 C. albicans에 대해 살균작용에 의한 항진균효과를 갖고 있는 것으로 판단된다. 따라서, 피톤치드의 항진균력은 구강 내 상주균주이면서 특별한 경우에 구강 캔디다증을 일으키는 C. albicans에 유효하게 작용하며, 이를 이용한 구강 위생용품의 적용으로 구강 캔디다증의 예방과 치료 및 예후관리에도 매우 유용하게 사용되리라고 생각된다.
미포자충인 Enterocytozoon bieneusi는 최근에 AIDS 환자에 기회감염 되는 병원체로 부각되었다. 이 병원체는 여러 동물에서 발견되고 있으며 인수공통 기생충으로서 특별한 관심의 대상이 되고 있다. 충남, 대전, 전북 지역의 도축장으로부터 특별한 증상을 보이지 않은 건강한 한우 총 1,729 마리의 직장변을 취하여 E. bieneusi의 감염율을 조사하였다. 조사방법은 Trichrome 염색 변법과 중합효소연쇄반응(PCR)을 이용한 분자생물학적 기법을 적용하였다. Trichrome 염색변법을 적용한 광학현미경 관찰에서 194 (11.28%) 개체에서 미포자충이 관찰되었으며, 특히 3월에 가장 높은 감염율을 보였다. 한편 분자생물학적 기법을 적용한 시험에서는 79 (4.59%) 개체에서 양성 반응을 보였으며, 이 역시 3월에 가장 높은 감염율을 나타내었다.
Pseudomonas aeruginosa는 기회 감염성 병원균으로, Cystic fibrosis, 미생물 감염성 각막염,화상 부위 2차 감염 등의 다양한 질병을 초래한다. 정족수 인식(쿼럼 센싱)이라고도 알려져 있는 세포간 신호전달 기전이 이러한 감염에서 중요한 역할을 하기 때문에 P. aeruginosa의 정족수 인식 시스템들이 집중적으로 연구되어 왔다. P. aeruginosa의 정족수 인식 시스템들을 소개하는 많은 문헌들이 주로 두 개의 주요 acyl-homoserine lactone (AHL) 계열 정족수 신호물질들인 N-3-oxododecanoyl homoserine lactone (3OC12)과 N-butanoyl homoserine lactone (C4)에 초점을 맞추어 설명하고 있지만, 실제로는 몇 가지 새로운 신호물질들이 발견되어져 왔고, 그들이 P. aeruginosa의 병독성과 신호전달에 중요한 역할을 할 수 있음이 제안되어져 왔다. 그 중 하나가 PQS(Pseudomonas quinolone signal; 2-heptyl-3-hydroxy-4-quinolone)인데, 이 물질은 현재 P. aeruginosa의 잘 규명된 주요 신호물질로 인식되고 있다. 이에 더하여, 최근의 연구들은 또 다른 가능성 있는 P. aeruginosa신호물질들을 제안해 왔는데, diketopiperazines (DKPs)과 pyocyanin이 그들이다. DKPs는 환형 dipeptide로써 이를 구성하는 아미노산의 종류에 따라 다양한 구조를 가진다. P. aeruginosa의 배양액에서 검출된 몇몇 DKPs들이 기존에는 AHL에만 특이적으로 반응한다고 알려졌던 Vibrio 랸�N갸 LuxR biosensor를 활성화 시킬 수 있다는 것이 발견되어 새로운 신호물질로 제안되었다. Pyocyanin (1-hydroxy-5-methyl-phenazine)은 P. aeruginosa가 생산하는 여러 phenazine 화합물들 중의 하나로써, 특징적인 청록색을 띄는 산화-환원 활성물질이다. 이 물질도 정체 성장기 동안 일부 정족수 인식의 조절을 받는 유전자들의 발현을 증가시키는 최종 신호 인자로 최근 제안되었으며, 그 신호는 또 다른 전사 조절 인자인 SoxR에 의해 매개된다고 제안되었다. 본 논문에서는 P. aeruginosa에서 새롭게 발견, 제안되고 있는 이들 신호 전달 물질들에 대해 자세히 다루어 보기로 한다.
녹농균은 병원, 물, 토양 및 대기 등의 많은 자연과 인공환경에서 서식하는 기회주의적 병원체로, 병원 내 감염 등의 심각한 문제점을 야기할 수 있다. 본 연구에서는 신뢰높은 표준화 방법인 International Organization for Standardization (ISO)을 적용하여, 물과 토양에서 220개의 녹농균의 추정 집락을 분리하였으며, 생리생화학적 특성으로 녹농균을 동정하였다. ISO/NP 16266 방법을 적용하였을 때, 총 220개의 추정 집락 중 방류수에서 drain swab으로 채취한 오직 1개의 집락만이 최종적으로 녹농균 양성으로 확정되었다. 한편, ISO에서 2014년 새롭게 제안한 1,10 phenanthroline 저항성 실험은, 환경 중 P. aeruginosa를 검출 및 동정하기 위해 적합한 방법으로 사료된다.
Candida albicans는 면역력이 약화된 환자의 표재성에서 전신적 감염까지 다양한 부위에서 감염을 유발시킬 수 있는 기회 감염적 병원성 진균이다. C. albicans는 효모형에서 균사형으로 변환될 수 있으며 이 때 바이오필름을 형성할 수 있다. 바이오필름과 관련된 C. albicans의 감염은 일반적으로 통상적인 항진균제에 대해 내성을 보이므로 새로운 항진균제에 대한 개발이 절실하다. 대황(Rheum undulatum)은 전통적으로 한국과 중국에서 하제나 소염제로 사용되는 약용 식물이다. 본 연구의 목적은 R. undulatum이 캔디다증 환자로부터 분리한 C. albicans 바이오필름 형성 균주에 대한 바이오필름 형성 억제효과와 이에 대한 항진균 활성 기작을 알아보는 것이다. R. undulatum (0.098 mg/ml)은 12종의 바이오필름 형성 임상균주의 캔디다 바이오필름을 $49.4{\pm}6.0%$ 감소시켰고 C. albicans의 폴리스티렌 표면으로의 부착을 억제시켰다. CFDA, AM과 propidium iodide로 이중 염색한 결과 R. undulatum은 C. albicans의 세포막을 손상시켰으며 propidium iodide와 neutral red로 염색하여 공초점 레이저 현미경과 위상차 현미경으로 관찰한 결과 C. albicans의 세포용해를 야기함을 관찰할 수 있었다. Crystal violet 흡수율 실험으로 R. undulatum에 의한 세포막 투과성의 변화를 관찰하였다. 따라서 R. undulatum은 세포막의 손상과 세포막의 투과성 변화로 야기된 세포의 용해와 관련된 항진균 활성이 C. albicans의 바이오필름 형성을 억제하는 것으로 보여진다. 본 연구의 결과는 R. undulatum이 바이오필름과 관련된 캔디다의 감염을 치료하고 제거하기 위한 천연물 기반 항진균제 개발에 대한 좋은 후보물질임을 보여준다.
Although pneumococcus is one of the most frequently encountered opportunistic pathogen in the world, the mechanisms responsible for its infectiveness have not yet been fully understood. In this paper, we have attempted to characterize the effects of pneumococcal transformation on the pathogenesis of the organism. We constructed three transformation-deficient pneumococcal strains, which were designated as Nos. 1d, 2d, and 22d. The construction of these altered strains was achieved via the insertion of the inactivated gene, comE, to strains 1, 2 and 22. We then conducted a comparison between the virulence of the transformation-deficient strains and that of the wild-type strains, via an evaluation of the ability of each strain to adhere to endothelial cells, and also assessed psaA mRNA expression, and the survival of hosts after bacterial challenge. Compared to what was observed with the wild-type strains, our results indicated that the ability of all of the transformation-deficient strains to adhere to the ECV304 cells had been significantly reduced (p < 0.05), the expression of psaA mRNA was reduced significantly (p < 0.05) in strains 2d and 22d, and the median survival time of mice infected with strains Id and 2d was increased significantly after intraperitoneal bacterial challenge (p < 0.05). The results of our study also clearly indicated that transformation exerts significant effects on the virulence characteristics of S. pneumoniae, although the degree to which this effect is noted appears to depend primarily on the genetic background of the bacteria.
