Purpose: Rectal cancers with high microsatellite-instable have clinical and pathological features that differentiate them from microsatellite-stable or low-frequency carcinomas, which was studied rarely in stage II rectal cancer, promoting the present investigation of the usefulness of microsatellite-instability status as a predictor of the benefit of adjuvant chemotherapy with fluorouracil in stage II rectal cancer. Patients and Methods: Data of 460 patients who underwent primary anterior resection with a double stapling technique for rectal carcinoma at a single institution from 2008 to 2012 were retrospectively collected. All patients experienced a total mesorectal excision (TME) operation. Survival analysis were analyzed using the Cox regression method. Results: Five-year rate of disease-free survival (DFS) was noted in 390 (84.8%) of 460 patients with stage II rectal cancer. Of 460 tissue specimens, 97 (21.1%) exhibited high-frequency microsatellite instability. Median age of the patients was 65 (50-71) and 185 (40.2%) were male. After univariate and multivariate analysis, microsatellite instability (p= 0.001), female sex (p<0.05) and fluorouracil-based adjuvant chemotherapy (p<0.001), the 3 factors were attributed to a favorable survival status independently. Among 201 patients who did not receive adjuvant chemotherapy, those cancers displaying high-frequency microsatellite instability had a better 5-year rate of DFS than tumors exhibiting microsatellite stability or low-frequency instability (HR, 13.61 [95% CI, 1.88 to 99.28]; p= 0.010), while in 259 patients who received adjuvant chemotherapy, there was no DFS difference between the two groups (p= 0.145). Furthermore, patients exhibiting microsatellite stability or low-frequency instability who received adjuvant chemotherapy had a better 5-year rate of DFS than patients did not (HR, 5.16 [95% CI, 2.90 to 9.18]; p<0.001), while patients exhibiting high-frequency microsatellite instability were not connected with increased DFS (p= 0.696). It was implied that female patients had better survival than male. Conclusion: Survival status after anterior resection of rectal carcinoma is related to the microsatellite instability status, adjuvant chemotherapy and gender. Fluorouracil-based adjuvant chemotherapy benefits patients of stage II rectal cancer with microsatellite-stable or low microsatellite-instable, but not those with high microsatellite-instable. Additionally, free of adjuvant chemotherapy, carcinomas with high microsatellite-instable have a better 5-year rate of DFS than those with microsatellite-stable or low microsatellite-instable, and female patients have a better survival as well.

The BMD difference depending on QCT and QUS, whose usages are recently being increased for the test of BMD, was accessed for 106 subjects of a general health examination who were aged between 24~69 year-old and results were derived as follows. The measured value of BMD by QCT and QUS showed significant correlation in general (p>0.05). In terms of the difference generated between the measurement methods depending on sexes, the female subjects showed significantly low T-score in the test made through QUS (p<0.05). Depending on ages, there was T-score difference among the subjects aged more than 40 year-old within significant range (p<0.05). When it came to the effect depending on heights and on weights, no group showed significant difference and in a group with less than 22.9 of BMI value, the T-score was measured significantly low when testing it through QCT while other groups were not affected by any testing methods. Likewise, it may require a different measurement method when it comes to the measurement of bone mineral density depending on sex, age, height, and BMI among patients. Thus, it suggests obtaining accurate data by conducting various testing methods in case of a special occasion.

Background: Adequate exposure of the mitral valve is a prerequisite for mitral procedures including the repair. An extended transseptal approach in mitral valvular operations is known to have certain technical advantages in recent years because of the anatomic posterior location of the mitral valve, especially in reoperations and in the presence of the small atrium in spite of the possibility of arrhythmia due to injury of sinus nodal artery. Material and Method: We compared the preoperative status, operative, and postoperative factors among patients in two study groups, transseptal only (Group I, n=10) and extended transseptal approach(Group II, n=25). Result: There were no differences in age, sex, NYHA functional class, left atrial size, and left ventricular function. The incidence of the redo-operation was high and early postoperative arrhythmia, which was improved later, appeared in 3 patients in Group II, but not in Group I. Conclusion: We believe that atrial septal incision could be extended up to the atrial roof whenever exposure of the mitral valve during a transseptal approach is inadequate because the late results were similar.

Purpose : Benign childhood epilepsy with centrotemporal spikes(BECT) is characterized by an excellent prognosis. Therefore, the necessity for the use of antiepileptic drugs is controversial. The object of this study is to know about the long-term follow-up of BECT, comparing daily treatment with antiepileptic drug(AED) versus no medication. Methods : We retrospectively studied 56 cases of BECTs, examined at Pediatric Neurology Clinic, Korea University Medical Center as reference study time, August 2002. Thirty-eight patients didn't have mediation(group I), while eighteen patients received one or two AED(group II). We evaluated demographics, clinical manifestations at diagnosis, EEG findings and clinical courses in groups I & II. Results : There was no significant differences of sex, age, seizure frequency, seizure type, seizure onset time, family history and EEG findings between group I & II. The reasons to start AED were : parents' anxiety(three cases), frequent seizure(12 cases) and daytime seizure(three cases). There was no evolution of status epilepticus and no difficulty in school performance. Conclusion : AED treatment was of no advantage and long-term prognosis was good regardless of treatment strategy. So it is advisable not to choose AED as the first line of therapy, provided that the patient himself/herself or the family understands the benign nature of BECT.
This study is in the purpose for the revitalization of traditional market as comparing to the relevancy between the central characteristics of a floating population going around for buying something or eating food and lots of small-sized businesses comprising of the commercial districts. The several traditional markets such as Cheonan station, Dujeong-dong, Sinbu-dong in Cheon-An city has been investigated repeatedly almost every two or three years by the Small Enterprise development Agency(SEDA) since 2001. By analyzing the raw data of those commercial districts made by SEDA, we can calculate the number of firms andthe ratio of business type of each commercial districts. In this research, the type of each business is classified into four groups such as restaurant, service, retail and the rest. Moreover, the central character of the floating population is derived from the raw data, which means the customer information about sex, age structure or the most populous time zones. From these characteristics, one commercial districts has his own specific features distinguishing from the others. The most important differences of past researches are firstly the dynamic viewpoint rather than a static one. Secondly it suggests that the relation between the central characteristics of districts and the floating population would exist. Lastly, it suggests that the interaction between both of them have a significant effect on the growth or decline of the districts and the rates of business type, other adjacent commercial districts as well. Eventually, this study provides several meaningful points for the revitalization of commercial districts to government or stakeholder such as management organization, business owners and new starter etc.
Objectives : The characteristics of depressive symptoms in patients with cancer is different from those in control group. However, few research has focused on the association between depressive symptoms in cancer patients. The aim of this study was to compare the network structure of depressive symptoms between patients with cancer and normal control. Methods : This study was based on cohort data from Korea National Health and Nutrition Examination Survey in 2016-2018. The Patient health Quetionnaire-9 (PHQ-9) was used to assess depressive symptoms in 599 patients with cancer and 599 age-sex matched controls. We estimated network structure of depressive symptom using Isingfit model. Results : There was no significant difference of each PHQ-9 item score. There were strong associations between symptoms were concentration problem-psychomotor activity, anhedonia-depressed mood, and depressed mood-suicidal ideation in both groups. Strength centrality of worthlessness was significantly higher in patients with cancer. Conclusions : These results suggest that worthless is associated with other depressive symptoms more tightly in patients with cancer. Worthless can serve as important treatment targets for intervention of depression in patients with cancer.
Purpose : Juvenile rheumatoid arthritis(JRA) is one of the most common rheumatic diseases of childhood and is an important cause of short- and long-term disability. The purpose of this study was to determine the disease course and outcome in childhood patients with JRA. Methods : Fifty nine patients with JRA who were diagnosed and treated in the Department of Pediatrics, Asan Medical Center from August 1990 to November 2004 were enrolled in this study. Sex, age, type, affected joints, extra-articular manifestations, laboratory and radiologic findings, treatments, and outcomes of JRA patients were reviewed retrospectively. Results : Among JRA patients, 32.2 percent had pauciarticular type, 30.5 percent had polyarticular type and 37.3 percent had systemic type. The ratio of boys to girls was 1.7 : 1 and the mean age at onset was $9.3{\pm}3.7$(1.3-15.9) years. The most commonly affected joints were knee, ankle and wrist. The extra-articular manifestations observed were fever, rash, myalgia and lymph node enlargement, etc. The main laboratory findings observed were leukocytosis, anemia, thrombocytosis, elevated ESR, and elevated CRP. Rheumatoid factor and antinuclear antibody(ANA) were positive in 5.3 percent and 18.0 percent. Nonsteroid anti-inflammatory drugs(NSAID) were used most frequently and methotrexate with or without steroids was added in 27.1 percent of patients unresponsive to NSAID. 88.1 percent of patients were cured without functional disability and only one patient was in functional status IV. One patient, who had pulmonary involvement, died. Conclusion : Our results showed an even distribution in type of onset, male predominance, older age of onset, low incidence of iridocyclitis, and low positivity of ANA in JRA patients; this differs from occidental data. This study may suggest regional differences and variability in disease groups of JRA among different racies, but further multi-center trials and large scale epidemiological studies are needed to confirm our conclusion.
Younghung-myun is located on a small island under the jurisdiction of Gyunggi province. The island occupying a total of approximately 40sq km is populated by 5,850 people. Two areas within the island were selected for this survey; 1) Nae 4-ri in Younghung-myun (mainly engaged in farming) 2) Nae 5-ri in Younghung-myun (mainly engaged in fishery) The survey was conducted for the period of 5-12 august 1978. Intake of foods and nutrients of all members in the households surveyed by means of questionnaire was computed based on number of meals a day. On the other hand, daily intake of nutrients by an individual was calculated by sex, age and type of work based on the conversion rate of RDA (Korean Recommended Dietary Allowances for Adult). The results obtained are summarized; 1. Status of food intake Average daily food intake of an individual in the surveyed areas was 1,018gm. Out of 1,018gm above, 490gm which represent 48% of the total intake were from cereals, grains or other vegetative products. Ratio of animal-origin foods was determined as only 16% of the total intake. These findings led us to the conclusion that people in the surveyed areas live heavily on vegetative foods. 2. Status of nutrients intake 1) Overall average intake of calorie by an individual in the surveyed areas was 2026.3Kcal per day. On the other hand, average calorie intake by adult persons was 2,491.3 Kcal per day, which is lower than 2,700 Kcal recommended by RDA by approximately 7.7%. Furthermore, it was observed that most of 2,491.3 Kcal obtained by adult persons was from such vegetative foods as cereals, grains, ect. 2) Overall average intake of protein by an individual in the areas surveyed was 52.2gm per dar. On the other hand, average protein intake among adult persons was 67.7gm a day, which indicates lower intake of protein than 80gm of RDA. It was also found that 84.3% of the protein intake by an adult person was of plant origin. 3) Overall average of fat intake by an individual in the areas surveyed was 18.9gm per day, whereas average fat intake among adult persons was 23. 2gm a day. It was brought to our attention that the average fat intake by adult persons is much lower than 36gm recom mended by RDA. 4) Average calcium intake among adult persons in the areas surveyed was 457.1mg per day, that is somewhat lower than 500mg in RDA. However, average iron intake among adult persons in the areas was slightly higher with 10.5mg a day than 10mg of RDA. 5) With exception with vitamin C, intake of all other vitamin groups, such as vit. A, vit. $B_1$, vit. $B_2$ and niacin, were lower among the inhabitants than recommended by RDA. 3. Economic status Average schooling years of food handlers was 6.5 years with average monthly income of \89,000 in the areas surveyed. Petroleum was used by majority of the households for fuel.
Changes in the public welfare attitudes are very important to the welfare politics and policy. Considering that variability of public welfare attitudes has been ignored in previous studies, we analyzed welfare attitudes changes at individual level in Korea. Using the longitudinal data from the Additional Survey of 2010/2013/2016 Korea Welfare Panel Survey, we analyzed the changes in public welfare attitudes during 2010th-2016th years. Major findings are as follows. First, one of the findings from analysis is that the public welfare attitudes are very active and dynamic in Korea. This finding is contradiction to the results of the previous researches that are point to the stability or inertia of the public welfare attitudes in western welfare states and also in Korea. Second, occurrences of the welfare attitudes change are related with various individual characteristics; income status, occupation status, age, region, sex, political ideology, etc. These results from this study on the welfare attitudes change suggest that dynamics of the Korea welfare politics in recent years reflected the dynamic changes in public welfare attitudes. And, they suggest that Korea welfare politics are transformed from the traditional classes welfare politics to the new welfare politics which are influenced by various interests of the socio-demographic groups.

In order to prevent unnecessary re-admission, it is necessary to intensively manage the groups with high probability of re-admission. For this, it is necessary to develop a re-admission prediction model. Two - year discharge summary data of one university hospital were collected from 2016 to 2017 to develop a predictive model of re-admission. In this case, the re-admitted patients were defined as those who were discharged more than once during the study period. We conducted descriptive statistics and crosstab analysis to identify the characteristics of rehospitalized patients. The re-admission prediction model was developed using logistic regression, neural network, and decision tree. AUC (Area Under Curve) was used for model evaluation. The logistic regression model was selected as the final re-admission predictive model because the AUC was the best at 0.81. The main variables affecting the selected rehospitalization in the logistic regression model were Residental regions, Age, CCS, Charlson Index Score, Discharge Dept., Via ER, LOS, Operation, Sex, Total payment, and Insurance. The model developed in this study was limited to generalization because it was two years data of one hospital. It is necessary to develop a model that can collect and generalize long-term data from various hospitals in the future. Furthermore, it is necessary to develop a model that can predict the re-admission that was not planned.
