The Relating Factors of Elder Abuse among Community-dwelling Elderly with Stroke (지역사회에 거주하는 노인 뇌졸중 환자의 학대 관련 요인)

  • Purpose: The purpose of this secondary analysis study was to examine the incidence of abuse and the contributing factors for that abuse among the Korean elderly with stroke. Methods: Data were derived from the 2008 Korean National Survey on Older Adults. The participants were 934 community-dwelling elderly with stroke. Data were analyzed using the Rao-Scott $x^2$-test, simple logistic regression and multiple logistic regression with the PASW 18.0 complex samples analysis module. Results: The prevalence of elder abuse was 13.5%. Emotional abuse (10.0%) was the most frequently reported abuse, followed by financial neglect (3.8%), caring neglect (3.3%), financial abuse (2.1%), and physical abuse (1.9%). 44.6% of the perpetrators of emotional abuse were coresidential family members. Participants without spouses (odds ratio=2.05, 95% CI=1.14~3.68), those without diabetes (odds ratio=2.24, 95% CI=1.15~4.39), and depressed participants (odds ratio=2.72, 95% CI=1.34~5.52) were more likely to be abused. Conclusion: Emotional abuse was the most frequently reported type of abuse while physical abuse was least frequent among the elderly with stroke. Nurses caring for the elderly with stroke should be aware of the types of abuse and risk factors such as lack of spouse and incidents of depression.

Correlation of Berg Balance Scale and Functional Reach Test

  • The purposes of this study were to provide the basic data and investigate the reliability of functional reach test and identify correlation of Berg balance scale (BBS) and functional reach test (FRT). The subjects were twenty healthy young adults and forty-five over 65 years old in order to compare balance ability. These data were analyzed by independent t-test and Pearson's correlation test using SPSS WIN 10.0. The results were as follows. Intrarater reliability coefficients of FRT was .976 and interrater was .942. FRT was significantly correlated with age, height, and BBS (p<.05). There were no significant differences in FRT and BBS by sex. There was significant difference in reach distance between below 74 elderly and above in FRT. FRT is very reliable test for balance and significantly correlated with BBS. Therefore, it is suggested that FRT is a clinically useful tool to substitute for BBS measuring balance ability in the elderly.

Effects of the Integrative Cognitive Training Program on Elderly Cognitive Function (통합적 인지훈련프로그램의 노인 인지기능에 미치는 효과)

  • The purpose of this study is to investigate the Effects of the Integrative Cognitive Training Program on Cognitive Function of the community elderly. Nonequivalent control group pretest-posttest design was used. 28 subjects were assigned to each of the group. 24 sessions of the integrative Cognitive Training Program were applied to the experimental group from September to December. The program was provided twice per week for 12weeks, total 24 sessons to the experimental group. Cognition scale for older adults(CSOA) was used to measure cognitive function of the groups before and after the Intervention. The score of Cognition Scale for Older Adults was significantly increased in experimental group than in control group(p< .05). The results of the study that Integrative Cognitive Training Program can be vey useful to improve the cognitive ability for the community elderly.

Attitudes to Use and Non-use of Information Technology among Elderly Women in Seoul (서울시 여성노인의 정보기술 이용 및 비이용 태도 연구)

  • The study was an exploratory research on the awareness of informatization and attitudes to information technology (IT) and its use among elderly women in a metropolitan area. The questionnaire survey was conducted to collect data on IT use or non-use of elderly women aged 60 or older in senior welfare centers in Seoul, Korea. The data on 343 cases were analysed using SPSS 21.0. As a result, the rate of internet use was relatively high and different according to socio-demographic factors, such as age. The elderly women's attitudes like the levels of awareness and expectation of informatization were positive. Elderly women learned to use IT for psychological, emotional reasons as well as for practical ones, and the satisfaction level of IT use was relatively high. Finally, most IT non-users were indifferent and ignorant about IT use. In addition, the dissatisfaction level of IT non-use was normal, but elderly women had a relatively positive expectation of IT use in the future. This study may contribute to understanding the diverse pictures of IT use and the digital divide in old age, and assist in the development of practical programs and policies for the informatization and welfare for older adults.

A Case Report on the Shelter for Dementia Programs based on the Primary Health Care Post (보건진료소 중심 치매 쉼터 프로그램 사례)

  • Purpose: This study developed a shelter for dementia programs based on primary health care posts; and examined its effectiveness in rural older adults. Methods: This study used a case report provided by a primary healthcare post in Jeonnam, Korea, in 2023. A single group pre-test post-test design was used; for three months, and 10 older adults participated in the shelter for dementia programs based on the primary health care post. The effectiveness of the intervention was measured immediately after entering the shelter for dementia programs. Results: There were significant differences between the pre-test and post-test on the cognitive impairment screening test (p=.005). Conclusion: The shelter for dementia programs based on primary health care posts was effective in improving cognitive impairments of rural older adults.

A Prediction Model for Stage of Change of Exercise In the Korean Elderly -Based on the Transtheoretical Model- (한국노인의 운동행위 변화단계의 예측모형구축 -범이론적 모델(Transtheoretical Model)을 기반으로-)

  • The purpose of this study was to identify causal relationships among variables of transtheoretical model for exercise in the elderly A predictivel model explaining the stage of change was constructed based on a transtheoretical model. Empirical data for testing the hypothetical model was collected from 198 old adults over 60 years old in a community setting in Seoul, Korea in April and May,1999. Data were analyzed by descriptive statistics and correlational analysis using pc-SAS program. The Linear Structural Modeling (LISREL) 8.0 program was used to find the best fit model which predicts causal relationship of variables. The fit of the hypothetical model to the data was X2=132.85. (df=22, p=.000). GFI=.88, NNFI=.35, NFI=.77, AGFI=.59 which was not favorable but the fit of modified model to the data was X2=46.90. (df=27, p=.01).GFI= .95, NNFI=.91, NFI=.92, AGFI=.87) which was more than moderate. The predictable variables of stage of change for exercise of the Korean elderly were helping relationship, self cognitive determination, conversion of negative condition in process of change and efficacy for exercise. These variables explained 68% of stage of change for exercise of the Korean elderly.

The Study on Blood Lipid Levels according to the Food Habits and Food Intake Patterns in Korean Elderly (일부 한국 노인의 식습관과 식사 섭취 패턴에 따른 혈중 지질 수준과의 관련성)

  • The purpose of this study was to analyze the eating habits and food intake patterns to elucidate the relationship between on blood lipid levels. The subjects based on the nutrition counseling data of medical health examination. The subjects were 774 adults(448 males and 326 females). Survey samples were divided by age $65\leq$ years. The subjects were 774 adults(448 males and 327 females). Survey samples were divided by age $65\leq$ years. Blood samples were analyzed for total cholesterol (TC), triglycerides(TG), LDL-cholesterol. The results showed there were significant difference frequency of overeating meal and total cholesterol(p<0.05), meal frequency, regularity of meals, frequency of meal out, eating breakfast, speed of meal et eating habits and blood lipid levels not significant difference. However, food intake patterns were significant difference in some kind of food such butter and cream bakery but there were not significant relationships between eggs, cod roe, Chinese foods, fried foods, Gal-bi-tang, Sun-ji-soup and blood lipid levels. Eating habits and food intake patterns was do not seem to be cause for blood lipid level in Korean elderly.

The Surgical Treatment of Three Young Chronic Subdural Hematoma Patients with Different Causes

  • Chronic subdural hematoma (CSDH), which rarely happens in the young, is thought to be a disease of the elderly. Whereas unspecific symptoms and insidious onset in juveniles and young adults, as a result of its relative low morbidity, CSDH is usually neglected even undertreated in the young. Through the three cases and review of the current literature on this subject, we tried to illustrate the clinical and etiopathological characteristics of this entity and find out the most appropriate treatment strategy. We report three young CSDH patients with different but similar symptoms. The present histories, tests and examinations revealed different predisposing factors accounting for the genesis of CSDH. Their preoperative symptoms were all resolved with burr hole and drainage operation. Juveniles and young adults suffering from CSDH differ from that of their elderly counterparts in their clinical and etiopathological characteristics. Although trauma is the most important risk factor in young and old CSDH patients, some other predisposing factors may exist. Burr hole and drainage surgery could resolve the problem most of the time. But further tests and examinations even specific management should be made in some cases.

Efficacy of Pneumococcal Vaccines (폐렴구균백신의 효과)

  • Streptococcus pneumonia is a very important pathogen for children and elderly people. Two types of pneumococcal vaccines are available in the market: pneumococcal polysaccharide vaccine (PPSV) and pneumococcal conjugate vaccine (PCV). PPSVs have been used for more than 30 years, and PCVs for about 10 years. There have been many reports concerning the evaluation of the vaccines' efficacies in preventing pneumococcal diseases such as meningitis, pneumonia, and otitis media and bacteremia, but the clinical trials had been performed with different conditions, such as diverse vaccine valencies, age groups, races, target outcomes, immunological cut-off values, and follow-up periods. PPSV is recommended for elderly people and chronic disease patients such as asthma, diabetes mellitus, chronic renal failure, and hyposplenic patients. According to the data from several systemic reviews and population-based surveillances, PPSV is effective for pneumococcal pneumonia and vaccine-type bacteremia among healthy adults. Until now, however, there is insufficient evidence of the effectiveness of PPSV among high-risk adults. PCV is very effective in preventing vaccine-type invasive pneumococcal disease (IPD) among children, but its efficacy for pneumonia is very low among children. The incidence of vaccine-related or non-vaccine-type IPDs is increasing after the introduction of 7-valent PCV (PCV7) as a routine immunization for children. Recently, 10- and 13-valent PCVs have been used for children, instead of PCV7. Therefore, continuous surveillance for serotype change among pneumococcal diseases is necessary to evaluate the vaccines' efficacy.
