  • The purpose of this study is to explore qualitatively what kind of coping strategy the Korean older adults use when they are depressed and why. Participants were users, older than 60, of an elderly welfare center in Seoul, and answers of 34 respondents who experienced depression were included in the analysis. The collected data were analyzed through content analysis. The results of the analysis showed that strategies to cope with depression used by participants were composed of 6 domains and 11 sub-domains: health behavior (medical approach / exercise and diet), family and social contact (social interaction / going out and going on a trip / communication with family), Religious activities (Religious activities), lifelong education (hobbies / educational activities), productive activities (labor), health risk behaviors (drinking and gambling / resignation). The depression coping strategies of the older adults and their characteristics are as follows. First, older adults used diverse problem-focused coping strategies to cope with depression. Second, older adults considered various coping strategies together and used them simultaneously. Third, the facility space for older adults functions as a shelter. Fourth, although there were cases where medical approach was used, the intention to reuse was very low. Through this study, the following suggestions were made to help older adults cope with depression more successfully. First, access to various coping strategies should be promoted. Second, more places where older adults can spend their time comfortably need to be provided. Third, resistance to mental health care should be resolved.

  • Objectives: The number of elderly patients with systemic basal disease requiring invasive dental treatment has increased. Appropriate prediction of surgical invasiveness and combined use of psychosedation are thought to contribute to safe whole-body management. Dexmedetomidine (DEX) exhibits analgesic and anti-anxiolytic properties and causes mild respiratory depression. Studies regarding DEX use in elderly non-intubated patients are scarce. We aimed to use retrospective data to determine an effective dose of DEX to induce adequate sedation in elderly patients undergoing invasive dental surgery under local anesthesia. Materials and Methods: One hundred two patients aged 70 to 96 years were presumably appropriately controlled with sedation. DEX was administered at an initial loading dose of 2.0 to $3.1{\mu}g/kg/hr$ for 10 minutes. We divided the patients into five groups by age and compared their blood pressures and heart rates. Results: In all five groups, blood pressure decreased suddenly at approximately 15 and 20 minutes after DEX administration. A marked decrease in blood pressure was noted in patients aged 75 to 79 years. Conclusion: For elderly patients aged 75 years and above, the initial loading dose of DEX needs to be reduced to lower than half that required for young and middle-age adults.

  • Background: Deficits of both ankle dorsiflexion range of motion (DFROM) and dynamic balance are shown in persons with chronic ankle instability and the elderly, with the risk of falls. Objects: This study aims to investigate the relationship between DFROM and dynamic balance in elderly subjects and young adults. Methods: Fifty-nine subjects were divided into three groups: ankle stability young group (SY), ankle instability young group (IY) and ankle stability older group (SO). We recruited three old subjects with ankle instability, but excluded them during a pilot testing due to the safety issue. DFROM was measured by weight bearing lunge test (WBLT) and dynamic balance was measured via star excursion balance test (SEBT) in anteromedial, medial, and posteromedial directions. The group differences in WBLT and SEBT and each group's correlation between WBLT and SEBT were detected using the R statistical software package. Results: The dorsiflexion range of motion was significantly different between the SY, IY, and SO groups. The SO group showed the highest DFROM and IY group showed the lowest DFROM (SY: $45.88{\pm}.66^{\circ}$, IY: $39.53{\pm}1.63^{\circ}$, SO: $47.94{\pm}.50^{\circ}$; p<.001). However, the SO group showed the lowest dynamic balance score for all SEBT directions (SY: $87.24{\pm}2.05cm$, IY: $83.20{\pm}1.30cm$, SO: $77.23{\pm}2.07cm$; p<.05) and there was no relationship between the dorsiflexion range of motion and dynamic balance in any group. Conclusion: Our findings suggest that ankle DFROM is not a crucial factor for dynamic stability regardless of aging and ankle instability. Other factors such as muscle strength or movement coordination should be considered for training dynamic balance. Therefore, we need to establish the rehabilitation process by measuring and treating ROM, balance, and muscle strength when treating young adults with and without ankle instability as well as elderly people.

  • This study was to investigate the effects of percussion program on depression and self-esteem for the elderly lived in residential care. 12 older adults participated in the 40-min. percussion program for 4 weeks(10 sessions). The percussion program consisted of three steps; 1) introduction for creating rapport and relationship, 2) process for mood induction and increasing interaction, and ending for increasing self-achievement and confirming the changed mood. Korean Geriatric Depression Scale (KGDS) and Self-Esteem Scale (SES) were administered before the first session and after the completion of the program. Results showed statistical significance (p<.05) in KGDS (p=.020) and SES (p=.025). Findings imply that percussion program can induce positive changes in depression and self-esteem for elderly in the residential care, and more depressed elderly group may have better outcome in percussion playing. Future studies should consider various levels of depression among elderly, and design the program to be more sensitive to the level of functioning.

  • Physiological changes that affect nutrient absorption and utilization occur in older adults, and the collection of nutritional information is an important part of examining the nutrition and health issue. A nutritional survey using the 24-hour recall method was performed to identify nutritional quality and leading patterns of food group intake for protein, vitamin C, P and Fe were over 1, whereas those for vitamin A, $B_1$, $B_2$, niacin and Ca were below 1. The MAR(Mean Adequacy Ratio) was relatively low(0.75). The percentage of energy from carbohydrate, protein, and fat was 65.1, 15.6, and 19.5, respectively, thus the quantities of energy source were ideal. The Older subjects revealed poorer nutrition quality than the younger subjects did. The food group intake pattern of the elderly was not diverse, only 9% of them consumed all food groups in a day. These results suggest that nutritional guidelines for older Koreans should focus on the maintenance of adequate energy intake. In addition, selection of foods with high protein and calcium, such as dairy food, should be emphasized, particularly in the older elderly. Nutrition programs should target individuals at risk of not receiving enough nutrients, like the oldest elderly, and persons with lower education.

  • Objective: With the proportion of the elderly population in Korea reaching 17.5% in 2022 and projected to increase to 20.6% by 2025, 30.1% by 2035, and 43% by 2050, the accelerated aging of the population is raising societal concerns about elderly care. Maintaining mobility is crucial for a healthy and independent old age. Design: Cross-sectional study Methods: This study investigates the effects of dual-task performance on gait variables and cognitive function in older adults. This cross-sectional study involved 60 older adults aged 65 and above, categorized into a dementia group (Korean version of Montreal Cognitive Assessment (MoCA-K) score ≤ 22) and a control group (MoCA-K score ≥ 23). Cognitive and gait functions were assessed using the GAITRite system (GAITRite system, CIR Systems Inc., USA), measuring gait variables (speed, stride length, etc.) before and after dual-task performance. The assessments were conducted under a single-blind condition, and data were analyzed using SPSS (ver. 25.0, SPSS Inc., USA). Results: The dementia group scored lower on cognitive assessments compared to the control group (p<0.05). In dual-task performance evaluations, the dementia group exhibited longer total task times and lower accuracy than the control group (p<0.001), while reaction times were longer but not statistically significant. GAITRite system analysis revealed that the dementia group had reduced gait speed and stride length compared to the control group (p<0.05). However, the difference in gait time was not statistically significant. The study results indicate that older adults with dementia show significant differences in cognitive function and gait performance, with notable impacts under dual-task conditions. Conclusions: These findings underscore the effect of cognitive decline on gait and provide valuable insights for predicting gait and cognitive function deterioration in dementia, which can aid in developing fall prevention strategies.
