  • With the Westernization and opening of our society, adolescents' smoking is increasing and being popularized. Many adolescents start smoking at an early age out of curiosity and venturesomeness, and earlier start of smoking makes it more difficult to quit smoking. Adolescents' habitual smoking not only becomes a gateway to all kinds of substance abuse but also causes various health problems including upper respiratory infection, immature lung development, reduced maximum vital capacity, and lung cancer. Therefore, it is quite important to prevent adolescents from smoking. The lowering of adolescents' smoking rate cannot be achieved only through social restrictions such as stereotyped education on the harms of smoking and ID checking. In order to lower adolescents' smoking rate substantially, each area of society should develop standardized programs and make related efforts. As adolescents' smoking is highly influenced by home environment or school life, it is necessary to make efforts in effective education and social reinforcement in school, to establish related norms, and to execute preventive education using peer groups. When these efforts are spread throughout society in cooperation with homes and communities, they will be helpful to protect adolescents' health and improve their quality of life.

청소년의 가정 역동적 환경, 성격 및 흡연이 비행행동에 미치는 영향 (The Influence of a Family Dynamic Environment, Personality, and Smoking on Delinquent Behavior Among Korean Adolescents)

  • Purpose: The purpose of this study was to determine the important factors on juvenile delinquency and to examine relationships between sex, age, the family environment, the personality of adolescents, smoking, and juvenile delinquency. Methods: Data collection was done through questionnaire surveys. The subjects for this study consisted of 1,948 adolescents delinquents : 784, students : 1,164 in Korea, using proportional stratified random sampling method. Statistical methods employed were chi-square, t-test and path analysis. Results: The results of this study were as follows : 1. The delinquent adolescents were reared in a more dysfunctional family environment, and had a higher maladaptive personality than the other student adolescents. 2. The delinquent adolescents showed the larger amount of smoking than the student adolescents. The cause of increased smoking tendency among delinquents simply were 'for social relation', 'for diversion', 'for nicotine addiction', whereas 'for tension relieving' among non-delinquent adolescents. 3. The most powerful contributors on delinquent behavior were antisocial personality tendencies, smoking, sex, strength of parent- child relationships, and the age of the adolescents in this order. Conclusions: Our cross-sectional findings indicate that smoking was one of the most powerful contributing variables to delinquent behavior, but family environment, personality, sex and age of adolescents were also proved to be strong exogenous variables to smoking in adolescents.

청소년 흡연 행위 및 흡연량 관련 요인 (Factors Associated with Smoking Behaviors and Amount of Tobacco Consumption among Korean Adolescents)

  • Purpose: Smoking among adolescents is a critical healthcare concern that needs to be tackled with respect to not only intrapersonal and interpersonal factors but also socio-cultural factors. This study was to identify the smoking behaviors and amount of tobacco consumption among adolescents, and to investigate the factors associated with the behaviors and amount. Methods: A cross-sectional study was conducted based on the theory of triadic influence using a nationally representative secondary data set, the 2018 Korea Youth Risk Behavior Survey (N=60,040). The analysis was performed by dividing smoking behaviors into lifetime, current, and daily smoking, and smoking amount into light, moderate, and heavy smoking. Descriptive statistics and multinomial logistic regression analyses were conducted. Results: Overall, 8.2%, 3.2% and 3.4% of the adolescents were involved in lifetime, current, and daily smoking, respectively. In addition, 5.3%, 0.8%, and 0.6% of the adolescents were involved in light, moderate, and heavy smoking, respectively. The factor associated with smoking behaviors and the amount of tobacco consumed were gender, academic achievement, depression, living with family, close friend's smoking, violent victimization, household economic status, and school level (all ps<.05). Conclusion: A considerable number of adolescents are engaged in risky smoking behaviors and consume large amounts of tobacco. Adolescents who said yes to peer smoking, violent victimization, not living with their family, and depressive emotions were more likely to engage in risky smoking behaviors and consume large amounts of tobacco. Tailored interventions to decrease smoking should be planed and provided, while considering the school and home environment and individual needs of adolescents.

청소년의 흡연 유혹에 관한 연구 (Survey on Adolescents' Knowledge and Attitudes as related to Temptation toward Cigarette Smoking)

  • Purpose: The purpose of this study was to identify the relationship of knowledge and attitude to temptation toward cigarette smoking in adolescents. Methods: A survey was conducted using a 52-item questionnaire in 2 high school settings. Descriptive statistics, t-tests, ANOVA, post hoc analysis and Pearson correlation coefficients were used with SPSS WIN version 14.0 to analyze the data. Results: There were significant differences in attitude toward smoking and smoking temptation depending on gender, academic performance, religion, current smoking status, parental responses toward smoking, initiation of smoking, and the number of friends who smoke (p<.001). The level of smoking knowledge was not related to any of general characteristics as well as attitude toward smoking and smoking temptation. However, negative attitude toward smoking was moderately correlated with reduced smoking temptation (r=-.608, p<.001). Conclusion: Adolescents' attitude toward smoking and coping ability against smoking temptation were influenced by various factors related to smoking behavior and social situations, rather than by the level of smoking knowledge. In order to develop an effective smoking prevention and cessation program for adolescents, various strategies to change attitudes toward smoking should be included.

고등학교 남학생의 흡연 및 금연 경험 (The Experiences of Smoking and Non-smoking in Male Adolescents)

  • 본 연구는 16명의 고등학교 남학생을 대상으로 흡연 및 금연에 대한 경험을 심층적으로 이해하고자 시행되었다. 포커스 그룹 인터뷰를 통해 자료를 수집한 후, 질적 주제 분석 방법을 이용하여 자료를 분석한 결과 6가지 주제를 도출하였다. '호기심에서 시작한 어른 놀이'는 참여자들의 첫 흡연 과정을 나타낸다. '흡연 초기 담배에 대한 거부감'은 기대와는 다른 첫 흡연 경험 후 참여자들의 심리적 변화를 말하며, '다시 시작된 흡연'은 또래 그룹과 어울리고, 담배의 맛을 시험하기 위해 재흡연을 하는 과정을 설명한다. '금연을 어렵게 하는 근묵자흑의 환경'은 담배에 쉽게 노출되는 청소년의 모습을 나타낸다. '금연과 흡연 사이의 갈등'은 참여자들의 반복되는 금연 시도 이유와 재흡연의 과정을 설명한다. 마지막으로 '비흡연 청소년의 흡연하지 않는 이유'는 흡연 청소년과 비슷한 환경 속에서도 흡연을 하지 않는 특별한 이유를 설명하고 있다. 고등학교 남학생의 흡연 및 금연 경험은 여학생이나 어른들과는 다른 독특한 차이가 있었다. 따라서 본 연구결과를 바탕으로 향후 고등학교 남학생의 흡연 예방 및 금연 교육을 기획하고 실행하여야 하며, 이는 궁극적으로 우리나라 남학생 청소년의 건강을 증진시킬 수 있을 것이다.

흡연 청소년의 흡연 빈도와 자살 시도에 영향을 미치는 요인 (Factors Associated with Smoking Frequency and Suicidal Attempt in Korean Adolescent Smokers)

  • Objectives : Among adolescents, the proportion of daily smoking tends to increase with age, and does not return to the previous level. Therefore, we examined the factors associated with smoking frequency in a nationwide sample of Korean adolescents to help prevent the progression of smoking. Methods : Among 127,804 adolescents who participated in the 2016 and 2017 Korean Youth Risk Behavior Web-Based Survey, data from 7,781 adolescents who replied that they currently smoked were analyzed. According to the frequency of cigarette smoking, participants were classified into daily and non-daily smokers. Participants' socio-demographic characteristics, smoking behavior, and psychological problems including perceived stress, depressive mood, and suicidality were assessed. Results : There were 3,627(47.4%) daily smokers and 4,154(52.6%) non-daily smokers. Low parental educational level, and early smoking experience were positively associated with daily smoking. High academic achievement, and high socio-economic status were negatively associated with daily smoking. Daily smoking was positively associated with lower happiness, lower sleep satisfaction, and severe stress during the past 12 months. Factors affecting suicide attempts of smokers were sex, residential type, socioeconomic status, and early smoking experience. Conclusion : The results indicated that socially disadvantaged families, and smoking experience with young age influenced daily smoking among Korean adolescents. Daily smoking was associated with psychological problems including lower happiness, lower sleep satisfaction, and severe stress. Early smoking experience influenced suicidal attempts among Korean adolescent smokers. Early intervention for smoking cessation in this high-risk group may contribute to prevention of these psychiatric complications of smoking.

건강신념모델을 이용한 청소년 흡연 행위 예측 (Predicting Adolescents' Smoking Behavior Using Health Belief Model)

  • This study was conducted to explain adolescents' smoking behavior using Health Belief Model. A cross-sectional study design was used to analyze characteristics and factors that influence smoking behavior among Korean adolescents. A total of 1.535 adolescents attending 21 high schools in one district of Seoul participated in the study. The results indicated that perceived susceptibility, severity, benefits, and barriers were significant in predicting smoking behavior of adolescents. Likewise, the degree of juvenile delinquency, gender, the amount of pocket money, and having smoking parent(s) and friend(s) significantly affect smoking behavior of adolescents. Based on the study results, it is recommended to incorporate HBM components in smoking cessation programs, and to include parents and friends in smoking prevention and cessation programs for high school students in Korea.

청소년들의 흡연경험 및 흡연빈도 증가에 영향을 미치는 요인 (Factors Affecting Cigarette Use and an Increase in Smoking Frequency among Adolescents in South Korea)

  • Purpose: Because it is important to prevent adolescents from becoming involved in smoking, this study was done to explore important factors influencing cigarette use and the increase in smoking frequency. Method: For this study the Korea Youth Panel Survey (KYPS) was analyzed. Because the KYPS is longitudinal, a fixed effect regression method was used to control for the effects of time-independent factors. More specifically, a logistic regression was used to explore factors affecting cigarette use, and a Poisson regression was used to explore smoking frequency. Result: As the adolescents got older, the number of male adolescents who tried smoking increased, while the number of female adolescents who tried smoking decreased. Also, the frequency of cigarette use among male and female smokers increased over time. Significant factors affecting cigarette use were friends who smoked, delinquent behavior, and loneliness at schools. Important factors affecting the increase in smoking frequency were grade (e.g., the 2nd- and 3rd-year of middle school), friends who smoked, delinquent behavior, monthly pocket money, expectation for the highest level of education, and attack tendency. Conclusions: To solve the problems linked to adolescent smoking, it is critical to develop intervention programs that target specific homogeneous subgroups of smokers, and that take into consideration gender difference in smoking and factors affecting levels of smoking behavior.

포커스 그룹 인터뷰를 통한 학교기반 청소년 금연 프로그램 효과성 제고 방안 (Ways to Improve Effectiveness of School-Based Smoking Cessation Intervention for Adolescents: Implications from Focus Group Interviews)

  • Objective: The objective of this study is to explore factors that contribute toward improving the effectiveness of school-based smoking cessation programs for Korean adolescents. Methods: Thematic analysis was done on the feedback gathered from a focus group interview of 5 instructors with ample service experience at smoking cessation programs provided in school settings. Results: This study has identified 9 categories of initiatives that may contribute to improving the quality of existing programs. These nine categories were grouped into the following 4 themes. 1) Improvement in the structure of intervention: the programs should be restructured to allow more instruction time and better instruction environment; 2) Improvement in the content of intervention: the programs should develop differentiated contents considering the characteristics of adolescence; 3) Importance of rapport, adolescents' interests and compensation: the program should be more interesting to adolescents and provide rewards based on the rapport between instructors and adolescents; 4) Cooperation among smoking cessation instructor, school, and community: instructors, teachers at schools, and communities should cooperate actively with one another. Conclusions: The school-based smoking cessation programs will benefit from implementing the findings of this study in improving its effectiveness in smoking cession in adolescents.

Impact of Conventional and Electronic Cigarette Use on the Adolescents' Experience of Periodontal Disease Symptoms

  • Background: Smoking in adolescence leads to an intensified addiction to nicotine when physical and mental growth has not yet been completed. With the advent of e-cigarettes, the rate of e-cigarette use among Korean adolescents has been steadily increasing. To date, studies on e-cigarettes and oral health, especially on the relationship between smoking styles and oral health in adolescents, are limited. Therefore, this study aimed to identify the risk factors for oral health problems caused by the repeated use of conventional cigarettes and e-cigarettes. Methods: This explanatory research study compared the adolescents' experiences of periodontal disease symptoms according to smoking type through a secondary analysis of the original data from the 15th Adolescent Health Behavior Survey (2019). Cross-analysis was performed to compare the smoking patterns according to the adolescents' general characteristics. Finally, a binary logistic regression analysis was performed to determine how smoking characteristics affect the adolescents' experience of periodontal disease symptoms. Results: In terms of patients' general characteristics, significant differences were observed in sex, school level, grades, household economic status, type of residence, and father's education level between adolescents who smoked conventional cigarettes alone and those who smoked both conventional cigarettes and e-cigarettes (p<0.05). After checking the factors affecting the smoking pattern and the experience of periodontal disease symptoms in adolescents, it was found that the duplicate smoking group was more likely to experience periodontal disease symptoms (odds ratio, 1.20) than the group that smoked conventional cigarettes alone (p<0.05). Conclusion: Duplicate smokers experienced more symptoms of periodontal disease than those who smoked cigarettes alone. Based on the findings of this study, smoking cessation counseling according to the smoking type and differentiated education for oral health promotion should be provided.
