Stem cell niche as a prognostic factor in leukemia

  • Despite high interests on microenvironmental regulation of leukemic cells, little is known for bone marrow (BM) niche in leukemia patients. Our recent study on BMs of acute myeloid leukemia (AML) patients showed that the mesenchymal stromal cells (MSCs) are altered during leukemic conditions in a clinical course-dependent manner. Leukemic blasts caused reprogramming of transcriptomes in MSCs and remodeling of niche cross-talk, selectively suppressing normal primitive hematopoietic cells while supporting leukemogenesis and chemo-resistance. Notably, differences in BM stromal remodeling were correlated to heterogeneity in subsequent clinical courses of AML, i.e., low numbers of mesenchymal progenitors at initial diagnosis were correlated to complete remission for 5-8 years, and high contents of mesenchymal progenitor or MSCs correlated to early or late relapse, respectively. Thus, stromal remodeling by leukemic cell is an intrinsic part of leukemogenesis that can contribute to the clonal dominance of leukemic cells over normal hematopoietic cells, and can serve as a biomarker for prediction of prognosis.   • Application of microarray technologies which monitor simultaneously the expression pattern of thousands of individual genes in different biological systems results in a tremendous increase of the amount of available gene expression data and have provided new insights into gene expression during drug development, within disease processes, and across species. There is a great need of data mining methods allowing straightforward interpretation, visualization and analysis of the relevant information contained in gene expression profiles. Specially, classifying biological samples into known classes or phenotypes is an important practical application for microarray gene expression profiles. Gene expression profiles obtained from tissue samples of patients thus allowcancer classification. In this research, molecular classification of microarray gene expression data is applied for multi-class cancer using computational biology such gene selection, principal component analysis and fuzzy clustering. The proposed method was applied to microarray data from leukemia patients; specifically, it was used to interpret the gene expression pattern and analyze the leukemia subtype whose expression profiles correlated with four cases of acute leukemia gene expression. A basic understanding of the microarray data analysis is also introduced.

CROX (Cluster Regulation of RUNX) as a Potential Novel Therapeutic Approach

  • Comprehensive inhibition of RUNX1, RUNX2, and RUNX3 led to marked cell suppression compared with inhibition of RUNX1 alone, clarifying that the RUNX family members are important for proliferation and maintenance of diverse cancers, and "cluster regulation of RUNX (CROX)" is a very effective strategy to suppress cancer cells. Recent studies reported by us and other groups suggested that wild-type RUNX1 is needed for survival and proliferation of certain types of leukemia, lung cancer, gastric cancer, etc. and for their one of metastatic target sites such as born marrow endothelial niche, suggesting that RUNX1 often functions oncogenic manners in cancer cells. In this review, we describe the significance and paradoxical requirement of RUNX1 tumor suppressor in leukemia and even solid cancers based on recent our findings such as "genetic compensation of RUNX family transcription factors (the compensation mechanism for the total level of RUNX family protein expression)", "RUNX1 inhibition-induced inhibitory effects on leukemia cells and on solid cancers through p53 activation", and "autonomous feedback loop of RUNX1-p53-CBFB in acute myeloid leukemia cells". Taken together, these findings identify a crucial role for the RUNX cluster in the maintenance and progression of cancers and suggest that modulation of the RUNX cluster using the pyrrole-imidazole polyamide gene-switch technology is a potential novel therapeutic approach to control cancers.

방사선 전신조사 (Total Body Irradiation in Leukemia - Preliminary Report -)

  • 1987년 8월부터 1988년 월까지 가톨릭의대 부속 성모병원 방사선과에서는 20명의 급성 백혈병 환자와 만성과립성 백혈병 환자를 6MV선형가속기를 사용하여 전신조사 하였다. 20명의 환자 중 8명은 총 방사선량 1200cGy책 1회 방사선 조사를 받았다. 나머지 2명 중 1명은 4회 분할 조사로 총 850cGy를 받았으며, 다른 1명은 2회 분할 조사하여 총 1100cGy를 받았다. 이식편은 20명중 17명이 동종 이식편을 받았고, 2명이 자가 이식편을 받았으며, 오직 1명만이 부적합 이식편을 받았다. 현재 13멍이 생존해 있고, 7명이 사망했으며, 전신조사에 의한 합병증은 설사, 점막염증, 구토와 구역, 피부발진 등의 순으로 나타났다.

Outcome of allogeneic hematopoietic stem cell transplantation for childhood acute lymphoblastic leukemia in second complete remission: a single institution study

  • Purpose: The survival rate for childhood acute lymphoblastic leukemia (ALL) has improved significantly. However, overall prognosis for the 20 to 25% of patients who relapse is poor, and allogeneic hematopoietic stem cell transplantation (HSCT) offers the best chance for cure. In this study, we identified significant prognostic variables by analyzing the outcomes of allogeneic HSCT in ALL patients in second complete remission (CR). Methods: Fifty-three ALL patients (42 men, 79%) who received HSCT in second CR from August 1991 to February 2009 were included (26 sibling donor HSCTs, 49%; 42 bone marrow transplantations, 79%). Study endpoints included cumulative incidence of acute and chronic graft-versus-host disease (GVHD), relapse, 1-year transplant-related mortality (TRM), disease-free survival (DFS), and overall survival (OS). Results: Cumulative incidences of acute GVHD (grade 2 or above) and chronic GVHD were 45.3% and 28.5%, respectively. The estimated 5-year DFS and OS for the cohort was $45.2{\pm}6.8%$ and $48.3{\pm}7%$, respectively. Only donor type, i.e., sibling versus unrelated, showed significant correlation with DFS in multivariate analysis ($p$=0.010). The rates of relapse and 1 year TRM were $28.9{\pm}6.4%$ and $26.4{\pm}6.1%$, respectively, and unrelated donor HSCT ($p$=0.002) and HLA mismatch ($p$=0.022) were significantly correlated with increased TRM in univariate analysis. Conclusion: In this single institution study spanning more than 17 years, sibling donor HSCT was the only factor predicting a favorable result in multivariate analysis, possibly due to increased TRM resulting from unrelated donor HSCT.

안면마비로 초기 발현된 소아 급성골수성백혈병: 조혈모세포이식으로 성공적으로 치료한 3예 (Facial palsy as the presenting symptom of acute myeloid leukemia in children: Three cases with stem cell transplantations)

  • 안면 마비가 백혈병의 최초증상으로 발현되는 경우는 매우 드물고, 이들 대부분은 두개내 방사선 치료를 받는다. 이에 저자들을 백혈병의 최초증상으로 안면마비가 발현된 3례의 환자들을 보고하고자 한다. 3례 모두 두개내 방사선 치료없이 관해유도가 된 후 조혈모세포이식을 시행하였다. 2례는 동종이식 후 각각 52개월, 62개월까지 무사건 생존 중이고, 나머지 1례는 자가이식 후 재발의 최초증상으로 안면마비가 나타났고, 타인 조혈모세포이식 후 59개월째 무사건 생존 중이다. 2례에서 여전히 안면마비가 남아있긴 하지만, 두개내 방사선 치료 없이도 동종 조혈모세포이식으로 안면마비가 발현된 백혈병을 치료하여 이를 보고하고자 한다.
