Difference of Improvement according to Hospitalization Time after Traffic Accident: A Retrospective Study (교통사고 후 입원 시기에 따른 호전도 차이에 대한 후향적 연구)

  • Objectives The purpose of this study is to find a relation between hospitalization time and the overall outcome of treatment, and suggest an optimal hospitalization date. Methods We analyzed the medical records of patients who received admission treatment at Mokhuri Neck and Back Hospital in April, 2018. Results By analyzing the difference in visual analog scale (VAS) scores according to the hospitalization date, it was shown in this study that when hospitalization was held out by the fourth day of accident there was a significant difference in the improvement of symptoms than any other day. There were no statistically significant differences in initial VAS scores and admission periods. Conclusions It is the most effective for a traffic accident patient to receive hospitalization within the first four days of accident for the most effective reduction in overall pain.

  • 1. An analysis of "GuGeupGanIBang(救急簡易方)" through Gwakran(霍亂) showed that the reference books are not concretely cited, but the symptoms are described in detail ed to let the common people understand and deal with Gwakran(霍亂) easily. It has the most detailed among Korean annotation emergency treatment books. It focuses on moxibustion and hot medicinal compress rather than acupuncture. The Korean annotation is translated in the order of the Korean language, so anyone can execute the treatments. The annotation used a lot of active verbs, so it is more of a book for the patient, not the doctor. So we can understand that this book is focused on the practical tips at an actual emergency situation. 2. "GuGeupGanIBang(救急簡易方)" is the most complete emergency treatment book and a Hangul medical book, which is scarce. It was published shortly after the invention of Hangul. It is an important material showing how terms related to medicine such as disease, prescriptions, symptoms were understood and expressed. As it is important in both medical book presswork study and Korean language study, it is designated as treasure. 3. The existing "GuGeupGanIBang(救急簡易方)" seem to be a reissued version. It is also not complete and scattered in several places. Volumes 1,2,3,6 and 7 survived, and volumes 4,5 and 8 have not been found yet.

A Bibliographic Study on the Therapeutic Effects of Achyranthis Radix in Arthritis (우슬(牛膝)의 관절염(關節炎) 치료(治療) 효능(效能)에 관(關)한 서지학적(書誌學的) 고찰(考察))

  • According to herbalogical bibligraphy and literature, therapeutic effects of Achyranthis Radix on arthritis was as follows, 1. Achyranthis Radix of Amaranthaceae is divided into five species-Achyranthes japonica, A. bidentata, A. longifolia, A. fauriei, Cyathula capitata, C. officialis and the characteristic, taste, channel entry, effects and main treatments were alike. 2. Winefrying stood for repairing treatement method for Achyranthis Radix Before repairing treatment method, Achyranthis Radix had three tastes(bitter, sour, and sweet) and calm and not poisoning characteristic. After repairing treatment method, the bitter taste was disappeared, and calm and not poisoning characteristic was changed into warm characteristic. 3. Effects of Achyranthis Radix were quicking the blood and dispelling stasis, liver-kidney supplement and strengthening musculo-skeletal system. Main treatments were relief of lumbar and knee joint pain, static menstrural block and wind-cold- damp impediment. 4. Contraindication of Achyranthis Radix was sympthom caused by spleen-kidney yang vacuity, upper burner disease and lower burner hemorrhage etc. Being used in pregnant woman, it could incur abortion. 5. Contraindication of Achyranthis Radix was beef, milk and mutton. It's fear was Radix Cynanchi Stauntonii and Semen Plantaginis. It's aversion was the firefly. Herba Taraxaci, Carapax Amydae, Carapax Testudinis and Radix Cynanchi Stauntonii. From above results, I suppose Achyranthis Radix has enough herbalogical foundation and could be used to treat arthritis and it is necessary to make a profound study of it.

A case report of Cardiac chest pain with dizziness and headache treated by Oriental Medicine (현훈(眩暈), 두통(頭痛)을 동반한 심장성(心臟性) 흉통(胸痛) 환자 1례의 한방치료에 의한 증례보고)

  • Chest pain is classified into two major categories of cardiac chest pain and non-cardiac chest pain. Cardiac chest pain is caused by cardiovascular disease, for example, myocardial infarction, angina pectoris, valvular heart disease, cardiac enlargement or hypertrophy, dissecting aortic aneurysm, pericarditis, myocarditis, etc. When the chest pain is not attributed to heart disease, it is termed non-cardiac chest pain. Non-cardiac chest pain is caused by pulmonary, gastrointestinal, musculoskeletal disease, psychiatric factor, etc. In tills case, we treated a 54-year old female patient who was diagnosed with dilated cardiomyopathy and suspicious sick sinus syndrome. She complained of chest pain, exertional dyspnea, dizziness and headache. For treatment, we made use of Yugultangami(六鬱湯加味) and Daejobwan(大造丸). Before and after treatment, we measured Heart rate variability(HRV). In result, the clinical symptoms were improved and there was a significant increase in assessmeut by Heart rate variability(HRV). Tills result suggests that Yugultaugami aud Daejowhan have a good effect on cardiac chest pain.

A Trend Analysis of Case Reports and Case Series on Hwa-byung in Korea

  • Objectives: This study aimed to provide a basis for treatment of Hwa-byung, a mental disorder frequently found among Koreans, by analysing published case reports and case series. Methods: In June 2015, a literature search for case reports and case series describing interventions for Hwa-byung was conducted in 7 databases for research publications in Korea, using Korean and English terms. Results: Thirty-two papers, reporting 439 cases of Hwa-byung, met the inclusion criteria. Since the first case report on Hwa-byung in 1996, case reports were continuously published until 2012. A total of 22 interventions were described, of which herbal medicine (71.9%), acupuncture (65.6%), and moxibustion (34.4%) were the most frequently described interventions. Twenty-five papers reported patterns, for which liver qi depression (28%) was most frequently used. A total of 48 outcome measures were used in 32 papers, of which Beck Depression Index (46.9%) and Symptom change (43.8%) were used in at least 10 papers. Conclusions: Since only 32 case reports/case series have been published on Hwa-byung in Korea, a strong basis for recommending standardized treatments is lacking. Therefore, a sufficient number of clinical studies, in particular randomized clinical trials (RCTs), are warranted to provide a clinical basis for treatment of Hwa-byung.

A Case Report of Cyclophosphamide plus Doxorubicin-induced Side effects in Patient with Both Sides Breast Cancer Treated with Integrative Medicine Therapy Including Pharmacopuncture. (유방암 환자의 cyclophosphamide, doxorubicin 유발 부작용에 대한 약침을 포함한 통합 암 치료 1례)

  • Objectives: To observe the mitigating effects of a Traditional Korean Medicine(TKM) treatment program especially including pharmacopuncture, with Cyclophosphamide and doxorubicin chemotherapy on a both sides breast cancer patient. Methods: AA 74 year-old female patient diagnosed with both sides breast cancer (Right) pT1bpN0M0, (Left) pT1cpN1Mx was admitted to hospital of Dong-eui university in May of 2017. She received Cyclophosphamide and Doxorubicin from May $31^{st}$ to August $2^{nd}$, 2017 followed by TKM treatment consisting of herbal medicine, acupuncture, moxibustion and pharmacopuncture (Trionycis Carapax, Non-toxic Bee Venom, and Cultivated Wild Ginseng Extract) for a period of almost 4 months, from May $13^{th}$ to August $19^{th}$, 2017. Symptoms were evaluated by the grade of chief complaints refer to Common Terminology Criteria for Adverse Events (CTCAE) and Eastern Cooperative Oncology Group (ECOG). Results: TKM including pharmacopuncture alleviated chemotherapy-induced nausea, fatigue, joint pain, diarrhea, insomnia. Conclusions: This case study potentiates TKM with pharmacopuncture's significant efficacy in aiding breast cancer patients suffering from Cyclophosphamide plus Doxorubicin induced adverse effects. Further research should take place for clear understanding of the exact amount of dosage and safety. Moreover it must be accompanied by long-term follow up researches.

  • Objectives: This study is aimed to describe the effects of using a traditional Korean herbal medicine, Jaeumkunbi-tang, on acute dizziness and gait disturbance that arose during treatment for trigeminal neuralgia. Methods: We closely observed one female patient who had been hospitalized complaining of trigeminal neuralgia and acute onset of dizziness. The patient was treated using Korean medical treatments, such as Oyaksungi-san, Jaeumkunbi-tang, acupuncture, and moxibustion. We evaluated the patient's condition using the visual analog scale (VAS) at the Department of Hepato-Hemopoietic System, Kyung Hee University Korean Medicine Hospital in June 2019 for 12 days. Results: The patient's dizziness was decreased after treating with Jaeumkunbi-tang for five days as measured by the VAS score. Conclusions: Jaeumkunbi-tang appears to be effective for controlling dizziness.

  • Objective: To describe the effects of traditional Korean medicine on dizziness and gait disturbance arising after removal of a vestibular schwannoma. Methods: The patient was treated using Korean medical treatments, such as herbal medicines (Jaeumkunbi-tang-gagambang and Yookmijihwang-Insamyangyoung-tang-gagambang), acupuncture, and moxibustion. We measured the state and progress of this case with the Korean Vestibular Disorder Activities of Daily Living Scale (K-VADL), Berg Balance Scale (BBS), and a visual analogue scale (VAS). Results: After treatment, the K-VADL score decreased from 167 to 74 and the VAS score decreased from 10 to 5.7, while the BBS score increased from 3 to 42. Conclusion: Jaeumkunbi-tang-gagambang (滋陰健脾湯) and Yookmijihwang-Insamyangyoung-tang-gagambang(六味地黃湯 合 人蔘養榮湯 加減方) appear to be effective for controlling dizziness and gait disturbances occurring after removal of vestibular schwannomas.

  • Fever in cancer patients is often due to the following causes: evil qi and toxity stagnancy, disorders of qi and blood, deficiencies of zang and fu organs, and the disorder of yin and yang. The treatments given to cancer patients with a fever are according to five: (a) Excessive inner heat and toxicants: remove heat and the toxicant, induce purgation. We use Cheng-Qi-Tang plus Qing-Wen-Bai-Du-Yin. (b) Tangle of damp and heat, and qi stagnancy: remove damp and heat, smooth the qi channel. We use Gan-Lu-Xiao-Du-Dan or San-Ren-Tang. (c) Obvious blood and heat stagnancy: remove heat and blood stasis. We use Xue-Fu- Zhu-Yu-Tang. (d) Deficiency of spleen qi, inner heat caused by a yin deficiency: nourish spleen qi and yin to remove the inner heat. We use Bu-Zhong-Yi-Qi-Tang or Xiao-Jian-Zhong-Tang. (e) Prominent yin deficiency and hectic fever: replenish yin and remove inner heat. We use Qing-Hao-Bie-Jia-Tang or Chai- Qian-Mei-Lian-San. The pathogenesis of fever in cancer patients is complicated. We can see both deficiency and excess in one differentiation. Therefore, we must make sure of it, then we can get the most effective treatment.

Two Clinical Cases of Active Release Technique with Oriental Medicine Treatments for Sequlae of Tibial Plateau Fracture (경골 고평부 골절 후유증에 능동이완기법을 병행한 한방치료 호전 2예)

  • The purpose of this study is to evaluate the effect of active release technique on sequelae of tibial plateau fracture. Two patients with sequelae of tibial plateau fracture were treated with active release technique. Visual analog scale (VAS), Lysholm knee scoring scale, range of motion were used to measure changes during treatment. After treatment, visual analog scale, Lysholm knee scoring scale, range of motion were improved significantly. Active release technique showed significant improvement to the sequlae of tibial plateau fracture that were showing no signs of improvement for twelve and eighteen weeks. This clinical trial showed that active release technique has meaningful effect on sequlae of tibial plateau fracture and more research should be followed.
