임플란트 보철물에 저작력 등 외부 하중이 작용하면 내부 반응으로 응력이 발생되는데 지지골에 나타난 응력은 골재생 및 흡수 파괴, 임플란트에 나타난 응력은 임플란트 자체의 파절이나 나사의 풀림현상 및 파절, 상부 구조물에 나타난 응력은 보철물의 파절 등을 예견하는 지침이 될 수 있을 것이다. 지대주의 형상과 재질에 따라 연결방법과 보철방법이 달라지고 임플란트 내부의 하중전달 기전이 변하게 되고 이에 따라 악골에 발생하는 응력분포 역시 달라질 수 있다. 본 연구에서는 하악 제 1대구치 부위에 이중나사 구조를 갖고 원추형 내측연결 임플란트 시스템인 GSII$^{(R)}$ (Osstem, Korea)임플란트를 이용해 지대주의 종류를 티타늄 소재의 2-piece Transfer$^{TM}$ abutment (GST), 금합금 소재의 2-piece GoldCast$^{TM}$ abutment(GSG), 외부 연결형태를 가진 3-piece Convertible$^{TM}$ abutment (GSC) 로 분류하여 이에 따른 응력분포 양상을 비교 분석하여 보았다. 결과 하중조건에 관계없이 응력은 주로 지대주와 고정체가 접촉하는 경부에 집중되었다. 또한 하중조건에 관계없이 임플란트의 고정체 상부와 접촉하는 치밀골에 높은 응력이 나타나고 해면골에는 아주 작은 응력이 나타났다. 축하중보다는 중심축을 벗어난 하중조건에서 더 높은 응력이 발생되었고 수직하중보다 경사하중에서 더 높은 응력이 발생되었다. 전체에 걸친 최대응력은 GSG에서는 지대주, 치관 및 고정체에 고르게 분포되었고 GST는 주로 고정체와 지대주 나사에, GSC는 고정체와 지대주에 집중되었다. 세 지대주 간 골내의 최대응력에는 유의한 차이가 없었고 GSG가 전체 구성부의 응력분포에 있어 유리한 것으로 나타났다.
A total of 605 implant fixture prosthesis delivered by 3 clinics and 2 laboratories were examined in this study, The object of this study was to determine the proper length of screw head. The depth of access hole were measured and compared to the type of fixture, abutment, gold screw and prosthesis. The results were as follows : 1 The average number of fixtures per patient were 2.97. 2. The number of fixture installed in the upper posterior area are 327(55.56 %), the upper posterior area 171 (28.25%). 3. The depth of access hole is 4.23 mm in shallow area, and 5.46 mm in deep area and the differences were 1.23 mm. 4. The average depth of the aceess hole of the UCLA abutment were 5.02 mm. 5. The number of 4-5 mm access hole depth were 60(22.39%) in abutment screw level and the number of 4-5 mm depth in fixture level were 101 (29.19%). 6. In the shape of screw head, hexed type were 576(95.21%), slotted type were 29(4.79%).
순수형 보강토교대는 상부구조의 하중을 보강토체 상단에 직접기초 형식으로 지지하는 교대이다. 교대 자체의 변형을 최소화하기 위해 비신장성 보강재인 메탈스트립을 사용하여야 한다. 순수형 보강토교대의 적용조건 도출을 위한 매개변수해석은 Zevogolis(2007)에 의해 수행되었다. 그 결과, 최상단 보강재의 인발 안전율이 가장 작게 산정되는 것으로 나타났다. 따라서 최상단 보강재의 인발 안전율이 가장 중요한 설계인자로 판단된다. 본 연구에서는 교대의 형상에 따른 최상단 보강재의 인발 안전율 변화를 검토하였다. 교대 길이와 교대 뒷굽 길이, 교대 높이를 변수로 하여 매개변수해석을 수행하였다. 매개변수해석 결과, 교대 길이와 교대 뒷굽 길이가 증가함에 따라 인발 안전율이 증가하는 것으로 나타났다. 이는 교대 길이가 증가함에 따라 교대의 접지면적이 증가하게 되었으며, 그로 인해 상부구조의 하중이 분산되었기 때문이다. 교대 길이 1.2m에서와 교대 뒷굽 길이 0.9m 지점에서 인발 안전율이 수렴하는 것으로 나타났다. 이는 접지면적 증가에 따라 보강재의 유효길이가 감소하였기 때문이다. 그러나, 교대 길이와 교대 뒷굽 길이가 과도하게 증가될 경우 상부구조의 연장이 증가하게 된다. 그리고 교대 높이가 과도하게 증가할 경우 교대 뒤채움부 토공량이 증가하게 된다. 이는 보강토옹벽에 상부하중으로 작용하게 된다. 따라서 이에 대한 면밀한 검토가 필요하다고 판단된다.
One of the most common problems of implant prosthesis is the screw loosening of abutment screws. This brings on discomfort in mastication, inflammation in the peri-implant tissue due to poor oral hygiene and fracture of prosthesis or loss of osseointegration. To prevent screw loosening, appropriate implantation to direct the occlusal force to the long axis of the implant, accurate design of the superstructure, decrease of the occlusal table, and adequate torque on the abutment screw are necessary. In this study the screw loosening torque was evaluated in implants with dimples or flutes in the internal surface of abutment screw holes. The abutments were fastened with slot type and hexagonal type abutment screws and were sealed with vinyl poly siloxane impression and bite registration material respectively. The screw loosening torque was evaluated after 1,800 and 12,600 times loading under a loading machine. The results were as follows. 1. The flute form group showed significantly higher loosening torque compared to the dimple form group and the group with no inner surface treatment (p<0.05). 2. There was no statistical difference in loosening torque according to the sealing materials. 3. The loosening torque according to the types of abutment screw showed no significant difference. 4. The loosening torque was significantly higher after 1800 times loading compared to 12600 times loading(p<0.05). From the above results. it is thought that formation of a flute in the internal surface of the screw hole decreases the chance of screw loosening, but the sealing materials and types of abutment screw did not show significant difference in prevention of screw loosening.
PURPOSE: The aim of this study was to evaluate the influence of the oxide layer removal process in the Co-Cr-Mo (CCM) abutment after casting procedure on the prosthesis settlement and screw stability. MATERIALS AND METHODS: CCM abutments of four different interface conditions (CCM-M; machined, CCM-O; oxide layer formed, CCM-B; blasted, CCM-P; polished after blasted) and gold abutment (Gold-C; Cast with type III Gold alloy) were used. The initial settling values of abutments were evaluated according to the difference of implant-abutment length when the tightening torques were applied at 5 Ncm and 30 Ncm, and the settling values of abutments caused by loading were evaluated according to the difference of implant-abutment length before and after loading with 250 N, 100000 cycle. The loss ratios of removal torque for abutment screws were evaluated according to the difference in value of removal torques under 30 Ncm tightening torque applied before and after cyclic loading. RESULTS: The CCM-P and CCM-B group showed a higher initial settling value compared with the Gold-C group (P<.05), while the Gold-C group showed the highest settling values caused by loading (P<.05) and no significant differences were observed for between CCM groups (P>.05). The loss ratio of removal torque values for the CCM-B, CCM-P groups did not differ significantly from that of the Gold-C group (P>.05). CONCLUSION: Even though the oxide layer was removed by different methods, CCM abutment with internal conical connection structure showed lower abutment settling and similar screw loosening after cyclic loading compared with gold abutment.
Purpose : This study was to assess the loading distributing characteristics of implant systems with internal connection or external connection under vertical and inclined loading using finite element analysis. Materials and methods : Two finite element models were designed according to type of internal connection or external connection The crown for mandibular first molar was made using cemented abutment. Each three-dimensional finite element model was created with the physical properties of the implant and surrounding bone This study simulated loads of 200N at the central fossa in a vertical direction (loading condition A), 200N at the centric cusp tip in a 15$^{\circ}$ inward inclined direction (loading condition B), or 200N at the centric cusp tip in a 30$^{\circ}$ outward inclined direction (loading condition C) respectively. Von Mises stresses were recorded and compared in the supporting bone, fixture, abutment and abutment screw. Results : 1. In comparison with the whole stress or the model 1 and model 2, the stress pattern was shown through th contact of the abutment and the implant fixture in the model 1, while the stress pattern was shown through the abutment screw mainly in the model 2. 2. Without regard to the loading condition, greater stress was taken at the cortical bone, and lower stress was taken at the cancellous bone. The stress taken at the cortical bone was greater at the model 1 than at the model 2, but the stress taken at the cortical bone was much less than the stress taken at the abutment, the implant fixture, and the abutment screw in case of both model 1 and model 2. 3. Without regard to the loading condition, the stress pattern of the abutment was greater at the model 1 than at the model 2. 4. In comparison with the stress distribution of model 1 and model 2, the maximum stress was taken at the abutment in the model 1. while the maximum stress was taken at the abutment screw in the model 2. 5. The magnitude of the maximum stress taken at the supporting bone, the implant fixture, the abutment, and the abutment screw was greater in the order of loading condition A, B and C. Conclusion : The stress distribution pattern of the internal connection system was mostly distributed widely to the lower part along the inner surface of the implant fixture contacting the abutment core through its contact portion because of the intimate contact of the abutment and the implant fixture and so the less stress was taken at the abutment screw, while the abutment screw can be the weakest portion clinically because the greater stress was taken at the abutment screw in case of the external connection system, and therefore the further clinical study about this problem is needed.
This paper presents a parametric study on the behavior of integral abutment PSC beam bridge. An integral abutment bridge is a simple span or multiple span continuous deck type bridge having the deck integral with the abutment wall. The rational structural model and design load combinations accounting for each construction stage are proposed. It can be used for defining the effect of earth pressure and temperature change in the design process including for determining maximum flexural responses. The bending moment at each response location due to the design load combination is investigated according to the change of flexural rigidity of piles and abutment height. The flexural responses of proposed model are computed for the cases of applying the Rankine passive earth pressure and the earth pressure based on the soil-structure interaction respectively, and the results are discussed.
This paper presents an analytical investigation on the behavior of simple span integral abutment bridge. An integral abutment bridge is a simple span or multiple span continuous deck type bridge having the deck integral with the abutment wall. Although the temperature variation and earth pressure are the major attributor to the total stress in integral abutment bridge, the superstructure has been designed by modeling it as a simple or continuous beam In order to investigate the effect of temperature change and earth pressure on the superstructure of integral bridge, the simple span integral bridge is modeled as a plane frame element. Performing frame analysis, the variations of bending moment and axial force of superstructure due to the various loading combination are investigated with respect to the flexural rigidity of piles, and the bending moment and axial force obtained by frame analysis are compared with the maximum bending moment obtained by conventional design method and initial prestressing force respectively.
목적: 내부 연결형과 외부 연결형의 임플란트에서 직선형과 경사형 지르코니아 지대주의 파절강도를 비교하였다. 재료 및 방법: 내부육각 연결형 임플란트 20개와 외부육각 연결형 임플란트 20개에, 기성 직선형 지르코니아 지대주와, $17^{\circ}$ 경사형 지르코니아 지대주를 10개씩 체결하였다. 시편은 연결 방식과 지대주 각도에 따라 4개의 군으로 나누었다: 내부 연결형/ 직선형 지대주, INS군; 내부 연결형/ 경사형 지대주, INA 군; 외부 연결형/ 직선형 지대주, EXS 군; 외부 연결형/ 경사형 지대주, EXA 군. 모든 시편은 만능시험기에서 1 mm/min의 crosshead speed로 $30^{\circ}$ 하중을 가했다. 지대주의 파절강도를 측정하고, 2-way ANOVA와 independent t-test로 통계처리 하였다(${\alpha}=.05$). 결과: 각 군의 평균파절강도는 다음과 같다: INS군, 955.91 N; INA군, 933.65 N; EXS군, 1267.20 N; EXA군, 1405.93 N. 외부 연결형이 내부 연결형에 비해 파절강도가 높았다(P < .001). 내부연결형(P = .747)과 외부 연결형(P = .222)에서 지대주 각도에 따른 파절강도는 차이가 없었다. 또한 내부 연결형 지대주는 육각 부위에서 수평 파절이 일어난 반면, 외부 연결형 지대주는 설측 치경부에서 파절되었다. 결론: 외부 연결형 지르코니아 지대주가 내부 연결형 지대주에 비해 파절 강도가 높았고, 직선형과 경사형 지르코니아 지대주의 파절강도는 차이가 없었다.
In this study design and construction technique for joint-less integral abutment for short to mid span bridges was developed. Expansion of superstructure due to thermal effect was absorbed in the flexible pile-type abutment in stead of expansion joint in the conventional bridges. Design method for pile subject to vertical and horizontal force was proposed. Backfill, approach slab and details of its connection joint with pavement was also proposed.
