흡수성차단막과 골이식재가 성견 치주질환 치조골재생에 미치는 영향 (THE EFFECTS OF RESORBABLE MEMBRANE IN CONJUNCTION WITH OSSEOUS GRAFTS ON THE PERIODONTAL HEALING IN DOGS)

  • There has been many attempts to develop a method that can regenerate periodontal tissues that were lost due to periodontal diseasd, but none of them was completely successful. This study was designed to investigate the healing and regeneration of periodontal tissue when bone substitutes such as porous replamineform hydroxyapatite and porous resorbable calcium carbonate were used in combination with oxidized cellulose membrane and collagen absorbable hemostat, compared to a control where only oxidized cellulose membrane or collagen absorbable hemostat were used. Chronic periodontitis was induced on mandibular premolars of and adult dog by placing orthodontic elastic ligatures for 10 weeks. After flap operation, the control group were received oxidized cellulose membrane (control- I )or collagen absorbable hemostat (control- II) only, while one experimental group was given either porous replamineform hydroxyapatite or porous resorbable calcium carbonate in addition to oxidized cellulose membrane (Experimental I-A, I-B), and another experimental group was treated by using either porous replamineform hydroxyapatite or porous resorbable calcium carbonate in addition to collagen absorbable hemostat. (Experimental II-A, II-B) After 56 weeks, healing was histologically analyzed with the following results. 1. Apical migration of junctional epithelium was observed only in areas coronal to the notch for both control and experimental group. 2. Inflammatory cell infiltration was not observed in any groups. 3. Oxidized cellulose membrane and collagen absorbable hemostat were completely resorbed. 4. Newly-formed cementum was observed up to the level where junctional epithelium was located, for both control and experimental groups. 5. Bone formation was limited of the middle portion of the notch in the control group, where as experimental groups showed bone formation up to the level of implant materials coronal to the notch. 6. Minute resorption of apically located portions of implanted materials was observed in experimental group I-B and II-B only.

흡수성 치주조직 재생 차폐막에 대한 비교연구 (Comparative study on absorbable periodontal tissue regeneration barrier membranes)

  • 흡수성 치주조직 재생유도재(국내 4개, 국외 2개) 총 6개 제품을 비교 연구하였다. 이종소재 차폐막의 경우, 콜라겐 제품은 우수한 기계적 강도를 보였으나 연신율이 작았다. 돼지심막은 우수한 기계적 물성에도 불구하고 두꺼운 두께로 사용에 제약이 되었다. 전기방사법으로 제조한 독일 PLLA/콜라겐 하이브리드형 복합 차폐막은 강도값과 흡수도는 낮았지만, 합성 및 천연고분자를 적절히 혼합함으로써 원하는 특성의 기계적 물성과 생체적합성 구현이 가능하였다. 본 연구에서 DA02(PLLA/젤라틴) 하이브리드형 복합고분자 소재는 기계적 물성과 생체적합성이 적절하여 100 % 수입에 의존하는 하이브리드형 흡수성 치주조직 재생 차폐막에 적용이 가능하였다.

노령견의 하악골절 불유합 1례에서 골유도재생술의 임상적 효과 (Clinical Effect of Guide Bone Regeneration of Mandibular Nonunion in a Geriatric Dog)

  • 13년령의 4.2 kg 암컷 푸들이 하악의 병적 골절 후 지역병원에서 받은 양측성 하악 수술의 실패로 내원하였다. 수술은 2.0-mm miniplates/screws, 돼지해면질골 및 흡수성 막을 사용해 시행되었다. 수술 후 1일, 1, 4, 8, 12주에 예후를 평가하였다; 치아 부정교합, 골절 불유합, 또는 연부조직 감염의 징후는 보이지 않았다. 그러나 1년 후 장기간 검진에서 콜라겐 막을 사용하지 않았던 왼쪽 하악골절부위에 불유합이 관찰되었다. 따라서 노령견에서 하악골 불유합의 치유에 돼지해면골과 흡수성 막을 함께 사용하는 것은 효과적이라 생각된다.

The Role of Collagen Membrane as a Scaffold of Etchant for Regional Acceleratory Phenomenon

  • Purpose: The goal of this research is to find the role of collagen membrane, which can reduce physical damage, as a scaffold for possible alternative to the corticotomy which causes Regional Acceleratory Phenomenon (RAP). Materials and Methods: The experiments were carried out on 12 New Zealand white rabbits, approximately 3.5 kg in bodyweight. We made an incision on the skin of the mandibular border and applied 37% phosphoric acid and collagen membrane to the mandibular bone surface of the first group (experimental group), and only phosphoric acid to the second group (control group). After 3 days, 1 week, and 2 weeks, 4 rabbits each were sacrificed and specimens were obtained. Each specimen was stained by H&E and Tartrate-resistant acid phosphatase (TRAP), and histological changes were observed by light microscope. Results: The demineralization of the experimental group was weak compared to the control group. It also showed a gradual increase of demineralization (after 3 days, 1 week, and 2 weeks) and the control group showed more extensive demineralization than the experimental group. Conclusion: This study demonstrates the amount of demineralization as a result of using phosphoric acid, and as time went by, demineralization increased. The absorbable collagen membrane was used as a scaffold to increase bone demineralization effect and prevent dispersion to adjacent tissues, but rather the amount of bone demineralization decreased. Therefore, the role of collagen membrane as a scaffold for RAP was weak.

Bone formation around rhBMP-2-coated implants in rabbit sinuses with or without absorbable collagen sponge grafting

  • Purpose: The purpose of this study was to evaluate bone formation around recombinant human bone morphogenetic protein (rhBMP-2)-coated implants placed with or without absorbable collagen sponge (ACS) in rabbit maxillary sinuses. Methods: The Schneiderian membrane was elevated and an implant was placed in 24 sinuses in 12 rabbits. The space created beneath the elevated membrane was filled with either blood (n=6) or ACS (n=6). In the rabbits in which this space was filled with blood, rhBMP-2-coated and non-coated implants were alternately placed on different sides. The resulting groups were referred to as the BC and BN groups, respectively. The AC and AN groups were produced in ACS-grafted rabbits in the same manner. Radiographic and histomorphometric analyses were performed after eight weeks of healing. Results: In micro-computed tomography analysis, the total augmented volume and new bone volume were significantly greater in the ACS-grafted sinuses than in the blood-filled sinuses (P<0.05). The histometric analysis showed that the areas of new bone and bone-to-implant contact were significantly larger in the AC group than in the AN group (P<0.05). In contrast, none of the parameters differed significantly between the BC and BN groups. Conclusions: The results of this pilot study indicate that the insertion of ACS after elevating the Schneiderian membrane, simultaneously with implant placement, can significantly increase the volume of the augmentation. However, in the present study, the rhBMP-2 coating exhibited limited effectiveness in enhancing the quantity and quality of regenerated bone.

Distinctive bone regeneration of calvarial defects using biphasic calcium phosphate supplemented ultraviolet-crosslinked collagen membrane

  • Purpose: To overcome several drawbacks of chemically-crosslinked collagen membranes, modification processes such as ultraviolet (UV) crosslinking and the addition of biphasic calcium phosphate (BCP) to collagen membranes have been introduced. This study evaluated the efficacy and biocompatibility of BCP-supplemented UV-crosslinked collagen membrane for guided bone regeneration (GBR) in a rabbit calvarial model. Methods: Four circular bone defects (diameter, 8 mm) were created in the calvarium of 10 rabbits. Each defect was randomly allocated to one of the following groups: 1) the sham control group (spontaneous healing); 2) the M group (defect coverage with a BCP-supplemented UV-crosslinked collagen membrane and no graft material); 3) the BG (defects filled with BCP particles without membrane coverage); and 4) the BG+M group (defects filled with BCP particles and covered with a BCP-supplemented UV-crosslinked collagen membrane in a conventional GBR procedure). At 2 and 8 weeks, rabbits were sacrificed, and experimental defects were investigated histologically and by micro-computed tomography (micro-CT). Results: In both micro-CT and histometric analyses, the BG and BG+M groups at both 2 and 8 weeks showed significantly higher new bone formation than the control group. On micro-CT, the new bone volume of the BG+M group (48.39±5.47 ㎣) was larger than that of the BG group (38.71±2.24 ㎣, P=0.032) at 8 weeks. Histologically, greater new bone area was observed in the BG+M group than in the BG or M groups. BCP-supplemented UV-crosslinked collagen membrane did not cause an abnormal cellular reaction and was stable until 8 weeks. Conclusions: Enhanced new bone formation in GBR can be achieved by simultaneously using bone graft material and a BCP-supplemented UV-crosslinked collagen membrane, which showed high biocompatibility and resistance to degradation, making it a biocompatible alternative to chemically-crosslinked collagen membranes.

인간 치주인대 유래 섬유모세포에 대한 흡수성 교원질 차폐막의 생체적합성 (BIOCOMPATIBILITY OF ABSORBABLE COLLAGEN MEMBRANES IN HUMAN PDL-DERIVED FIBROBLASTS IN VITRO)

  • Purpose: This study is designed to evaluate biocompatibility of three types of absorbable collagen GBR membrane in vitro. Material and Method: The human PDL fibroblasts culture was obtained through typical way and the cells used in the experiment was forth passage. The membranes examined were Experimental group A, B, C. All the 3-experimental groups were made of bovine pericardium and the membranes were excised into 5$\times$5mm respectively. The samples of the membranes were fixed on the 24-well plate with the double-sided adhesive tape. Then, 2ml of cell suspension which included $2{\times}10^4$cells was inoculated into the 24-well plate, and the cells were cultured for 1 week. Cellular viability and the alkaline phosphatase activity were measured with ELISA. The membranes in the culture were processed to examine with SEM. Results: The survival rate was highest in control and Experimental group A is the next, group B and group C in order of the value. The values are analyzed for statistical difference using Wilcoxon test. All the values of experimental groups are significantly lower than those of control, and the vaules among the experimental groups significantly differ from each other. Alkaline phosphatase level was identical order with the viable cell rate. SEM examination revealed that the PDL fibroblasts adherent on culture dish (control) and group A were spindle-shaped, but on group B and C, the cells were round-shaped without processes.

Dehydrothermal Treatment로 제작한 흡수성 콜라겐 골유도재생술 차단막 (Absorbable Guided Bone Regeneration Membrane Fabricated from Dehydrothermal Treated Porcine Collagen)

  • Purpose: Collagen membranes are used extensively as bioabsorbable barriers in guided bone regeneration. However, collagen has different effects on tissue restoration depending on the type, structure, degree of cross-linking and chemical treatment. The purpose of this study was to evaluate the inflammatory reaction, bone formation, and degradation of dehydrothermal treated porcine type I atelocollagen (CollaGuide$^{(R)}$) compared to of the non-crosslinked porcine type I, III collagen (BioGide$^{(R)}$) and the glutaldehyde cross-linked bovine type I collagen (BioMend$^{(R)}$) in surgically created bone defects in rat mandible. Methods: Bone defect model was based upon 3 mm sized full-thickness transcortical bone defects in the mandibular ramus of Sprague-Dawley rats. The defects were covered bucolingually with CollaGuide$^{(R)}$, BioMend$^{(R)}$, or BioGide$^{(R)}$ (n=12). For control, the defects were not covered by any membrane. Lymphocyte, multinucleated giant cell infiltration, bone formation over the defect area and membrane absorption were evaluated at 4 weeks postimplantation. For comparison of the membrane effect over the bone augmentation, rats received a bone graft plus different covering of membrane. A $3{\times}4$ mm sized block graft was harvested from the mandibular angle and was laid and stabilized with a microscrew on the naturally existing curvature of mandibular inferior border. After 10 weeks postimplantation, same histologic analysis were done. Results: In the defect model at 4 weeks post-implantation, the amount of new bone formed in defects was similar for all types of membrane. Bio-Gide$^{(R)}$ membranes induced significantly greater inflammatory response and membrane resorption than other two membranes; characterized by lymphocytes and multinucleated giant cells. At 10 weeks postoperatively, all membranes were completely resorbed. Conclusion: Dehydrotheramal treated cross-linked collagen was safe and effective in guiding bone regeneration in alveolar ridge defects and bone augmentation in rats, similar to BioGide$^{(R)}$ and BioMend$^{(R)}$, thus, could be clinically useful.

Biological effects of a porcine-derived collagen membrane on intrabony defects

  • Purpose: To prolong the degradation time of collagen membranes, various cross-linking techniques have been developed. For cross-linking, chemicals such as formaldehyde and glutaraldehyde are added to collagen membranes, but these chemicals could adversely affect surrounding tissues. The aim of this study is to evaluate the ability of porous non-chemical cross-linking porcine-derived collagen nanofibrous membrane to enhance bone and associated tissue regeneration in one-wall intrabony defects in beagle dogs. Methods: The second and third mandibular premolars and the first molars of 2 adult beagles were extracted bilaterally and the extraction sites were allowed to heal for 10 weeks. One-wall intrabony defects were prepared bilaterally on the mesial and distal side of the fourth mandibular premolars. Among eight defects, four defects were not covered with membrane as controls and the other four defects were covered with membrane as the experimental group. The animals were sacrificed 10 weeks after surgery. Results: Wound healing was generally uneventful. For all parameters evaluating bone regeneration, the experimental group showed significantly superior results compared to the control. In new bone height (NBh), the experimental group exhibited a greater mean value than the control ($3.04{\pm}0.23\;mm/1.57{\pm}0.59$, P=0.003). Also, in new bone area (NBa) and new bone volume (NBv), the experimental group showed superior results compared to the control (NBa, $34.48{\pm}10.21%$ vs. $5.09{\pm}5.76%$, P=0.014; and NBv, $28.04{\pm}12.96$ vs. $1.55{\pm}0.57$, P=0.041). On the other hand, for parameters evaluating periodontal tissue regeneration, including junctional epithelium migration and new cementum height, there were no statistically significant differences between two groups. Conclusions: Within the limitations of this study, this collagen membrane enhanced bone regeneration at one-wall intrabony defects. On the other hand, no influence of this membrane on periodontal tissue regeneration could be ascertained in this study.
