Study on Consequent Body Fat and Serum Lipid Metabolism after Cocoon Hydrolysate, Green Tea Leaves and Dietary Fiber Supplementation (누에고치 유래 실크 펩타이드와 녹차잎 및 식이 섬유소 보충이 체지방 및 혈청 지질 대사에 미치는 영향에 관한 연구)

  • This study is to investigate consequent nutrient intake status, Influences of body mass index (BMI) and serum lipid composition, and fat distribution on the cocoon hydrolysate, green tea leaves and dietary fiber supplementation. During 2 months of this research (April to May, 2002), 47 women aged 20 yr-30 yr (average age 26.2 yr) were selected as subjects. Nutrient intake was investigated by questionnaire and 24-hr recall method. Antropometric assessments of the subjects were investigated by SBIA method (Segmental bioimpedance assay, Inbody 3.0). The results were as follows: mean body weight was 60.7 kg, mean body height 161.7 cm and mean BMI 23.4. Status of energy intakes significantly decreased (p<0.01) and dietary fiber intakes significantly increased (p<0.001) after supplementation. BMI and WHR (waist-hip ratio) significantly decreased (p<0.01) and body fat significantly decreased after supplementation (p<0.001). Total cholesterol and LDL-cholesterol significantly decreased after supplementation (p<0.05). HDL-cholesterol was negatively correlated with BMI and WHR (p<0.01). LDL/HDL ratio was positively correlated with BMI (p<0.01) and WHR (p<0.05). Above results of this study show that low-molecule peptide, green tea leaves and dietary fiber supplementation-added routine diet improves lessening body fat distribution, total cholesterol, LDL-cholesterol. Especially, decrease of abdominal fat and WHR were notable. That meant decrease of risk factors.

A Distribution of Waist-hip Ratio Associated with the Blood Pressure in Middle-aged Men (중년 남성에서 혈압과 관련한 허리-엉덩이 둘레비 산출)

  • Backgroud: Excess abdominal fat, expressed as an increased ratio of waist to hip circumferences (WHR), is independently associated with higher levels of blood pressure. Although a WHR greater than 1.0 in men has been shown to predict complications from obesity, the WHR has not been evaluated in all ethnic groups. Methods: In order to ascertain the association between WHR and classification of blood pressure and to investigate the critical value of WHR as a predictive factor of hypertension in Korean middle-aged men, we compared the mean of WHRs according to the classification of blood pressure in Seoul Cohort participants. Results: Through a surrey of direct measurement of waist and hip girth, 452 subjects were recruited from the cohort. The mean of WHR was 0.88 and its standard deviation was 0.04. The mean of WHRs was higher in the systolic blood pressure group (above 140 mmHg), diastolic blood pressure group (above 90 mmHg), and hypertension group than in the systolic blood pressure group (below 140 mmHg), diastolic blood pressure group (below 90 mmHg), and normotensive group, respectively. And WHR of above 0.89 was associated with hypertension (z-value =6.66). Conclusion: It is necessary for Korean males with WHR greater than 0.89 to recommend the primary prevention and early defection of hypertension.

Factors Affecting Metabolic Syndrome in a Rural Community (한 농촌지역 주민들의 대사증후군 관련요인)

  • Objectives: This study set out to investigate the relationship among the factors of metabolic syndrome diagnosis criteria, their risk factors including general characteristics, and the distribution of the diagnosis criteria and risk among the adult residents of a rural community. Methods: Among 1,968 residents, those who had three or more of the risk factors of metabolic syndrome, which include blood pressure, blood glucose, triglyceride, abdominal obesity, and HDL-C, were categorized as the metabolic syndrome group. And their correlations were analyzed. Results: As for the risk ratio with five factors of the metabolic syndrome diagnosis criteria, it was high according to age and smoking. In addition, the results show that body fat percentage, hs-CRP, insulin, BMI, PP2, total cholesterol, and W/Ht also had much impact on increasing the risk ratio of the metabolic syndrome diagnosis criteria. It turned out that metabolic syndrome was affected by the body mass index(BMI), insulin, waist to height ratio(W/Ht), and hs-CRP. It was 2.51 times crude odds ratio that BMI over the 25kg/m2 in the ratio of the fact of metabolic syndrome and adjusted for sex odds ratio 2.50times and W/Ht was 3.31times, adjusted for sex odds ratio 3.25 times. Conclusion: BMI, W/Ht and smoking of the general characteristics seem to have close relationships with high correlations between the metabolic syndrome diagnosis criteria and the risk factors. Thus there is an urgent need to evaluate them and take interventions and monitoring measures for the clustering of risk factors.

Body Type Characteristics and Classification of Men in Their 20s for Development of Fitted Sloper (피티드 원형 개발을 위한 20대 남성의 체형 특성 및 유형화)

  • This study is a preliminary study to suggest a distinguished fitted sloper according to body types. To this end, this study classified body types of men in their 20s in an attempt to examine the characteristics of each type. A total of 842 adult men of age 20 to 29 were studied by using the direct measurement data taken in the 6th Size Korea. The subjects' body types were classified based on 48 measurements. We extracted a total of six factors that characterized the body types of adult men in their 20s: horizontal size, vertical length, shoulder development, trunk area length, hip area length and shoulder slope. Result of the cluster analysis, the body shapes of men in their 20s were classified into four types. We developed the overlapped silhouettes for each of the four body types and compared the front and side shapes to clearly identify the differences of each body types. The findings of this study indicated that Korean adult men in their 20s can be classified into the big inverted triangular type that represents healthy and muscular men, the small inverted triangular type that represents short and skinny men, the triangular type that represents men with abdominal obesity, and the rectangular type that represents men that are fat overall, with a fairly even distribution of each body type.

A Clinical Study on the Effect of 62 cases through Obesity Clinic in Dae-Gu Hanny University (대구한의대 비만클리닉 내원 환자 62명에 대한 임상적 관찰)

  • Objectives : The purposes of this study are to evaluate the effect of treatment in obesity clinic in Dae-Gu Hanny University and to improve the oriental medical methods of treatment in obesity patients. Methods : 62 patients were classified into four groups by Body Mass Index(BMI) : normal, overweight, 1st obesity and 2nd obesity groups. Weight, BMI, body fat rate and abdominal fat rate were compared in each group by t-test. Results : In distribution of sex, male were 10(16.1%), female were 52(83.9%). Mean age was $31.0{\pm}2.59$ and the patients in twenties and thirties were the most. Mean weight was $71.80{\pm}4.08kg$ and 1st obesity group(23) was the most. Mean BMI was $27.11{\pm}1.20(kg/m^2)$. BMI Changes in obesity groups treated by obesity clinic were $0.95{\pm}0.68kg/m^2$ in normal group, $1.45{\pm}0.38kg/m^2$ in overweight group, $2.67{\pm}0.48kg/m^2$ in 1st obesity group and $4.01{\pm}1.10kg/m^2$ in 2nd obesity group. The changes in all groups were significant. The more the obesity degree was. the more weight loss was. Conclusions : The weight and BMI in patients were almost decreased significantly when they treated. It resulted that the effect of oriental medical treatment in obesity was positive. This study is the effect of treatment in obesity by several medical methods and it is thought that the study of the effects of treatment in obesity by each medical methods that were used in this study is necessary.

A Retrospective Review about Korean Medicine Treatment for Obesity and Change of Body Composition on Climacteric Women in a Korean Medicine Hospital (일개 한방병원에 내원한 갱년기 여성의 한의 비만 치료와 체성분 변화에 대한 후향적 분석)

  • Objectives: This study aimed to identify the current use of Korean medicine for obesity and its effect for women in climacteric period. Methods: We studied women aged 45-55 who visited Daejeon Korean medicine Hospital of Daejeon University to lose body weight from January 1, 2021 to December 31, 2022 via an analysis of the medical records. The treatment duration was continuous for more than 2 weeks, and a body composition was measured by Inbody 770 at 2 to 4 weeks after the first visit. Results: 28 patients were finally selected and their average age was 49.32±3.38 years. Based on the body mass index (BMI) classification, 19 were in the 1st obesity group, 5 in the 2nd obesity group, 3 in the overweight group and 1 in the normal group. Patients usually complained comcomitant symptoms, and the symptoms of menopausal disorder was the most frequent. The average treatment duration was 3.68±0.67 weeks and the average treatment frequency was 3.93±0.98 times. All patients took herbal medicines Gambi-tang and 23 took modified fasting therapy including Gamro-su. 14 were treated by whole body far-infrared therapy and 6 were gotten partial obesity treatment. Among patients treated for accompanying symptoms, menopausal disorders were the most common (35.71%), and herbal medicine such as Gamisoyou-san, Hominis Placenta Pharmacopuncture, moxibustion, and cupping were used. After treatment, on average, body composition changed significantly; body weight 3.28±1.82 kg, BMI 1.36±1.04 kg/m2, body fat 1.70±1.67 kg, skeletal muscle mass 0.81±0.91 kg, abdominal circumference 2.04±2.6 cm, and visceral fat area 8.91±12.83 cm2. Conclusions: We analyzed general characteristics, BMI distribution, types of Korean medicine treatment and change of body composition. This study could be used as reference to provide practical data of treatment for obese women in climacteric period.

The Role of Adipokines in the Pathogenesis of Non-alcoholic Fatty Liver Disease in Obese Children; the Relationship between Body Fat Distribution and Insulin Resistance (비만아의 비알코올성 지방간 발병에 있어 Adipokine과 체지방분포 및 인슐린 저항성과의 연관성에 대한 연구)

  • Purpose: This study was conducted to evaluate the role of adiponectin, leptin, and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) in the pathogenesis of non-alcoholic fatty liver disease (NAFLD) in obese children, and to elucidatethe relationship between these adipokines and insulin resistance. Methods: A total of 61 obese children (M : F=42 : 19, mean age 11.2${\pm}$1.3 years) admitted to our facility between March 2004 and June 2005 were included in this study. Patients were divided into three groups based on their NAFLD status obese children without fatty liver (N=23); obese children with simple steatosis (N=20); and obese children with non-alcoholic steatohepatitis (NASH) (N=18). The serum levels of adiponectin, leptin, and TNF-${\alpha}$ were measured, and insulin resistance determined by homeostasis model assessment (HOMA-IR) was calculated to estimate insulin resistance. In addition, the VSR (visceralsubcutaneous fat ratio) was estimated using abdominal computed tomography. Results: There was no difference in serum TNF-${\alpha}$ and leptin levels observed between the 3 groups (22.13${\pm}$6.37 vs. 21.35${\pm}$6.95 vs. 25.17${\pm}$9.30; p=0.342 & 20.29${\pm}$8.57 vs. 16.42${\pm}$6.85 vs. 20.10${\pm}$7.86; p=0.330). However, the serum adiponectin level was significantly lower in children with non-alcoholic steatohepatitis (NASH) than in the other two groups (6.08${\pm}$1.38 in children without steatosis vs. 5.69${\pm}$0.79 in simple steatosis vs. 4.93${\pm}$1.75 in NASH; p=0.026). In addition, the VSR was significantly increased in the NASH group (0.31${\pm}$0.08 vs. 0.32${\pm}$0.11 vs. 0.47${\pm}$0.14; p=0.001), and HOMA-IR revealed a significant difference among the three groups (4.77${\pm}$3.67 vs. 6.89${\pm}$7.05 vs. 10.42${\pm}$6.73; p=0.000). However, there was no significant correlation observed between the adiponectin levels and the HOMA-IR or the VSR (r=-0.117; p=0.450 & r=-0.106; p=0.499). Conclusion: Insulin resistance may affect the development of hepatic steatosis and steatohepatitis in children, and the results of this study suggest that, of several adipokines evaluated, adiponectin is important in the progression of steatosis to steatohepatitis in obese children.
