Diagnostic Strategy of Primary Site in Metastatic Bone Tumor (전이성 골종양에서 원발병소의 진단)

  • We carried out a prospective study of the effectiveness of a diagnostic strategy in thirty consecutively seen patients who had skeletal metastasis. The diagnostic strategy consisted of the recording of a medical history, physical examination, routine laboratory analysis, plain radiography of the involved bone and chest, whole-body technetium-99m-phosphonate bone scintigraphy, abdominal ultrasound, computed tomography of the chest, abdomen and pelvis, fiberbronchoscopy and fibergastroscopy. After this evaluation, a biopsy of the most accessible osseous lesion was done in twenty four patients. On the basis of the our diagnostic strategy, we were able to identify the primary site of the malignant tumor in nineteen patients(63%). The laboratory values were non-specific in all patients. The history and physical examination revealed the occult primary site of the malignant tumor in one patient(3.3%) who had carcinoma of the breast. Plain radiographs of the chest established the diagnosis of carcinoma of the lung in three patients(9.9%). Computed tomography of the chest identified an additional three primary carcinoma of the lung(9.9%). Fiberbronchoscopy identified an additional one primary carcinoma of the lung(3.3%). Abdominal ultrasound established the diagnosis in three patients(9.9%). Computed tomography of the abdomen and pelvis established the diagnosis in four patients(13.2%). Fibergastroscopy established the diagnosis in two patients(6.6%). Examination of the biopsy tissue established the diagnosis in one patient(3.3%). So we recommend to perform plain radiographs of chest, abdominal ultrasound, chest C-T, abdomino-pelvic C-T, fiber-bronchoscopy, fibergastroscopy sequentially.

Evaluation of Image Quality and Radiation Dose for Filtered Back-Projection and Iterative Reconstruction Algorithm in Abdominal Computed Tomography Protocol (복부 CT 프로토콜에서 필터 보정 역투영법과 반복적 재구성기법에 따른 화질 및 선량에 관한 연구)

  • In Computed Tomography, abdominal examination showed the highest proportion of use, and effort of reducing the radiation dose is required. Recently introduced Iterative Reconstruction(IR) is repetitive reconstruction technique of Computed Tomography. SIEMENS' IR, ADMIRE and GE's IR, ASIR-V, were used in this examination. Noise, % Contrast, and High contrast resolution were measured by using ACR phantom for image quality evaluation. In addition, CTDIvol and DLP displayed in the CT device were used for dose evaluation. When FBP and IR were compared, stage 2 to stage 5 of ADMIRE and 10, 30, 50, 70, and 90% of ASIR-V were applied, noise could be reduced from a minimum of 0.46 to a maximum of 2.38 in ADMIRE compared to FBP, and noise from a minimum of 0.51 to a maximum of 2.5 in ASIR-V compared to FBP. Also, % Contrast and High contrast resolution of FBP and IR were no statistical difference. When IR was used for abdominal CT examination, the radiation dose of ADMIRE is reduced by 25.39% compared to the radiation dose of FBP. Also, the radiation dose of ASIR-V is reduced by 16.61% compared to the radiation dose of FBP. In conclusion, it is believed that if IR is applied during abdominal CT examination, the radiation dose can be reduced without deteriorating the image quality.

  • The association between heavy metals in the blood and obesity has been examined in many studies. However, inconsistencies have been observed in the results from these studies. The present study was conducted using data from 119,181 participants of the Korea National Health and Nutrition Examination Survey (KNHANES) for 11 years in 2005 and between 2008 and 2017. The subjects with missing heavy metal blood tests, health interview data, and health examination data were excluded from the study. The study population comprised 1,844 individuals (972 men, 872 women) who were eligible for inclusion. It was found that obesity and abdominal obesity were associated with an increase in both blood mercury (P<0.001) and alanine aminotransferase (ALT) (P<0.001). After adjusting the confounding factors, those with concurrent high levels of ALT and the highest tertile of mercury showed an increased risk of obesity (odds ratio 4.46, 95%, confidence interval 2.23~8.90, P<0.001) as well as abdominal obesity (odds ratio 5.36, 95%, confidence interval 2.57~11.17, P<0.001). The interrelationship of mercury and ALT with the parameters of body mass index (P for interaction=0.009) and waist circumference (P for interaction=0.012), respectively, have been observed to be significant, suggesting that the reciprocal relationship could contribute to obesity and abdominal obesity.

Diagnostic laparoscopy in a leopard cat (Prionailurus bengalensis) with intercostal abdominal hernia and hepatic lipidosis

  • Intercostal abdominal hernia in the 11th intercostal space was identified in a leopard cat. Although mild leukopenia was found in laboratory examinations, no remarkable abnormality was revealed in medical imaging. To investigate abdominal organs, diagnostic laparoscopy was performed after hernia repair. In laparoscopic view, closure of the herniation site and a lesion with whitish discoloration in the liver (left medial lobe) were observed. Subsequently, laparoscopic liver biopsy was performed against the affected hepatic tissue. Histologically, the sample was diagnosed as mild hepatic lipidosis. Laparoscopy is considered useful for abdominal visceral examination and liver biopsy in a leopard cat patient.

Forecasting obesity prevalence in Korean adults for the years 2020 and 2030 by the analysis of contributing factors

  • BACKGROUND/OBJECTIVES: There are few studies that forecast the future prevalence of obesity based on the predicted prevalence model including contributing factors. The present study aimed to identify factors associated with obesity and construct forecasting models including significant contributing factors to estimate the 2020 and 2030 prevalence of obesity and abdominal obesity. SUBJECTS/METHODS: Panel data from the Korea National Health and Nutrition Examination Survey and national statistics from the Korean Statistical Information Service were used for the analysis. The study subjects were 17,685 male and 24,899 female adults aged 19 years or older. The outcome variables were the prevalence of obesity (body mass index ${\geq}25kg/m^2$) and abdominal obesity (waist circumference ${\geq}90cm$ for men and ${\geq}85cm$ for women). Stepwise logistic regression analysis was used to select significant variables from potential exposures. RESULTS: The survey year, age, marital status, job status, income status, smoking, alcohol consumption, sleep duration, psychological factors, dietary intake, and fertility rate were found to contribute to the prevalence of obesity and abdominal obesity. Based on the forecasting models including these variables, the 2020 and 2030 estimates for obesity prevalence were 47% and 62% for men and 32% and 37% for women, respectively. CONCLUSIONS: The present study suggested an increased prevalence of obesity and abdominal obesity in 2020 and 2030. Lifestyle factors were found to be significantly associated with the increasing trend in obesity prevalence and, therefore, they may require modification to prevent the rising trend.

FAST Reappraisal: Cross-sectional Study (혈역학적으로 안정된 복부둔상환자에서 FAST의 유용성 평가)

  • Purpose: Focused Assessment with Sonography for Trauma (FAST) provides an important initial screening examination in adult trauma patients. However, due to its low sensitivity, FAST is not a replacement for computed tomography (CT) in hemodynamically stable trauma patients. The aim of this study was to determine the test characteristics of FAST in adult, hemodynamically stable, blunt abdominal trauma patients by using a critical action as a reference standard. Methods: The medical records for FAST examination at a single hospital from January 2009 to February 2011 were retrospectively reviewed. The inclusion criterion was isolated, hemodynamically stable, blunt abdominal trauma. Hemodynamically unstable patients or patients with penetrating injuries were excluded. The reference standard was the presence of a critical action, which was defined as one of the following: 1) operative intervention for a finding discovered on CT, 2) interventional radiology for bleeding, 3) transfusion of 2 or more packed RBCs, or 4) death at the emergency department. Results: There were 230 patients who met the inclusion criterion. There were 20 true positive, 206 true negative, 0 false positive, and 4 false negative results. The sensitivity and the specificity were 83% and 100%, respectively. Conclusion: Despite its low sensitivity for detecting any abnormal finding discovered on CT, negative FAST could aid to exclude critical action in hemodynamically stable, blunt abdominal trauma patients.

Advanced Abdominal MRI Techniques and Problem-Solving Strategies (복부 자기공명영상 고급 기법과 문제 해결 전략)

  • MRI plays an important role in abdominal imaging because of its ability to detect and characterize focal lesions. However, MRI examinations have several challenges, such as comparatively long scan times and motion management through breath-holding maneuvers. Techniques for reducing scan time with acceptable image quality, such as parallel imaging, compressed sensing, and cutting-edge deep learning techniques, have been developed to enable problem-solving strategies. Additionally, free-breathing techniques for dynamic contrast-enhanced imaging, such as extra-dimensional-volumetric interpolated breath-hold examination, golden-angle radial sparse parallel, and liver acceleration volume acquisition Star, can help patients with severe dyspnea or those under sedation to undergo abdominal MRI. We aimed to present various advanced abdominal MRI techniques for reducing the scan time while maintaining image quality and free-breathing techniques for dynamic imaging and illustrate cases using the techniques mentioned above. A review of these advanced techniques can assist in the appropriate interpretation of sequences.

Fatty Acids Intake and Its Association with Abdominal Obesity and Hyperglycemia in Korean Adults: Korea National Health and Nutrition Survey, 1998~2007 (한국 성인의 지방산 섭취와 복부비만 및 고혈당과의 관련성 -1998~2007 국민건강영양조사 자료에 근거하여-)

  • Abdominal obesity is an important factor of metabolic syndrome and is known as an independent risk factor of cardiovascular disease. The purpose of this study was to examine the fatty acid intake pattern as well as the association between fatty acid intake and abdominal obesity and hyperglycemia in Korean adults. This study was based on data from the Korea National Health and Nutrition Examination Surveys (KNHANES) I, II, III, and IV. After excluding subjects who had missing data for dietary intakes, anthropometric measurments, and laboratory tests, a total of 12,320 subjects aged 20~64y (5,266 men and 7,054 women) were included in the study. Trends in individual consumption of dietary fatty acids by year were analyzed by One-way ANOVA test, and the association between fatty acid intake and abdominal obesity and hyperglycemia was analyzed by logistic regression analysis after adjusting for age, energy intake, marital status, job, education level, alcohol intake, and smoking status. In men, intakes of TFA, SFA, MUFA, FUFA, and n-6 PUFA significantly increased with year. On the other hand, intakes of TFA, SFA and FUFA significantly increased with year in women. Logistic regression analysis showed that the risk of abdominal obesity was significantly associated with intakes of PUFA, LNA and n-3 PUFA in women. However, there was no association with any fatty acid intakes in men. The risk of hyperglycemia was significantly and positively associated with intakes of TFA, SFA, PUFA, and LNA in men, but, there was no significant association with any fatty acid intakes in women. These results show that abdominal obesity and hyperglycemia are significantly associated with individual intake of fatty acids, suggesting that fatty acid type may be associated with risk of abdominal obesity and hyperglycemia. Futher, the macronutrients of measl must be properly balanced.

  • Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRl) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a $3.8{\times}3.0$ cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This finding was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.

A Case of Fetal Mummification and Lithopedion from a Bitch with Prolonged Gestation over about One Month (장기임신 개에서 발생한 태아미이라변성 및 석아의 1례)

  • A six-year old bitch pregnant with prolonged gestation over about one month was ovariohysterectomied. The bitch was proved to be normal by physical and biochemical examination and had not a purulent vaginal discharge. A large firm mass was palpated in left caudoventral abdomen. Radiography identified the mass as a fetus. The abdominal ultrasono-graphy identified the fetus was dead. Caesarian section through the median raphe over linea alba was attempted. Adhensions were found between the uterus, stomach, spleen, urinary bladder, and abdominal viscera. Two fragments of bone were found in the abdominal cavity because of rupture of left uterine horn. Radiography and ultrasonography were proved to be of use to diagnose prolonged fetal mummification. Ovario-hysterectomy was considered to be choice of treatment to remove the prolonged mummified fetus.
