상한병(傷寒病)의 개념(槪念)과 논치근거(論治根據) 확립을 위한 구조주의적 분석 (Structuralistic Analysis for Establishment of Concept and Variables of Shanghan Diseases)

  • In order to grasp the clear concepts and variables to treat Shanghan diseases which has complex meaning by each medical literature, several concepts and analytic method of structuralism from early to late stage encompassing Saussure and Derrida were used. Main concepts are langue and parole, signifiant and signifie, syntagme and paradigme, denotation and connotation, synchronie and diachronie, identity of structure and differance etc. and methods are substituting these concepts to historical Shanghan theories from Zhongjing to Ming-Qing dynasty and comparisons of synchronie about their era. Essential qualities of Shanghan diseases are pathologic phenomena under the order of unification of nature and man formed through concrescence between individual human body and geo-climatico-socio-cultural environmental conditions, neither injuries by cold pathogen nor five types of exopathogenic febrile diseases. The former environmental elements can be inferred from general traits of desires and public pathological aspects of social members, and the latter personal factors from corresponding features to those pathogenic variables. In addition, the concepts of Shanghan disease are added successively up to now via Jin-Yuan's four great masters and Wenbing masters, and thus the new concepts of denotation became another connotation obtaining new signifiant. In this way, for the treatment of Shanghan diseases, new theories should be made for ranging prescription over the wenbing field; reflecting not only climatical variables but also each patient's physio-pathological features and sociocultural variables. Thereby an appropriate and reasonable therapeutical systems can be designed and can guide research direction hereafter.

일제강점기의 한의학 교재 중 하나인 『한방의학강습서(漢方醫學講習書)』 - 서문과 발문(跋文)의 번역과 인용서적의 분석을 중심으로 (A Lecture Book on Traditional Korean Medicine in the Period of Japanese Occupation, 『Eihak Gangseupseo(醫學講習書)』 - focused on its preface, epilogue and reference books)

  • In July 2008, I obtained a copy of "Hanbang Eihak Gangseupseo" through Uibangseowon. It was recorded that the book was compiled by Seong Ju-bong and reviewed by Ji Seok-young. According to previous studies, this book was the lecture book that was used in teaching Traditional Korean Medicine in Daejeon, Chungcheongnam-do. This book gave insight to the system and curriculum of the school for Traditional Korean Medicine in Daejeon, Chungcheongnam-do. It also exhibited the academic characteristics of Traditional Korean Medicine in the Period of Japanese Occupation and the medical viewpoint of Seong Ju-bong. The summary is as follows: First, an independent School for Traditional Korean Medicine was run in Daejeon, Chungcheongnam-do, with reasonable curriculums and systematic textbooks. Second, the medical viewpoint and treatment methods of Huang Yuan-Yu of Qing Dynasty was actively introduced. Then it was reorganized for the society and stimulated the progress of Traditional Korean Medicine. Third, while absorbing Chinese Medicine, it still inherited our heritage of Traditional Korean Medicine Especially, Seong Ju-bong's original opinions and clinical experiences are shown in surgery, gynecology and pediatrics. Fourth, in a break from the past, when Chinese culture could not be introduced due to diplomatic problems between Joseon and Qing, efforts were made to overcome limitations of lagging behind by adopting and educating Warm Disease study. Fifth, while working side by side with Ji Seok-young who introduced the modern Western Medicine through vaccination, it still searched for a traditional Korean medical treatment for chickenpox. I hope that the report of my findings through reading "Eihak Gangseupseo" could make up for the fact that the medical history during the colonial period is scarce.

『의림촬요(醫林撮要)』의 의안(醫案)에 대한 연구 (A Study on the Yi'an (醫案) of Uirimchalyo (醫林撮要))

  • Uirimchalyo (醫林撮要), one of the most important books in the history of Korean Medicine (KM), has not been researched within the framework provided by Yi'an (醫案), an East Asian tradition of describing clinical encounters with the therapies employed. In modern times, this practice of Yi'an might be similar to the outline of a singular "case study". The authors designed the study to analyze the basic information of Yi'an within the Uirimchalyo and to contribute to the foundations of employing Yi'an in Korean Medicine. A standard was established, and the 123 Yi'ans were extracted, most of which were taken from the chapter, Historic Doctors (歷代醫學姓氏). Using this information and in comparison with other medical books such as Euibangyoochui (醫方類聚), the authors learned that the Yi'ans from the Jinyuansidajia (金元四大家), including those of four eminent clinicians in Jin (金) and Yuan (元) Dynasty, are excluded from Uirimchalyo. The authors identified that Yi'ans from other medical books, are cited in the Uirimchalyo but with different format, not with the traditionally understood form of Yi'an. This study of the Uirimchalyo Yi'an resulted in three important understandings of Yi'an. First, the mere number of Uirimchalyo Yi'an is meaningful in that it raised the genre of Yi'an to the a level not previously recognized. Second, in the history of Korean Medicine, Yi'an is first systematized in the Uirimchalyo at the chapter of Historic Doctors. Third, Uirimchalyo raised the concept of usefulness of Yi'an, to the practice of Korean Medicine.

마취 전문서 "마약고(麻藥考)"의 처방과 그 원류에 대한 연구 (A Study on Herbal Formulas and their origin in Mayaku-ku(麻藥考))

  • Objective : This article shows that while Seishu Hanaoka(華岡靑洲) is known to have developed an effective anesthetic formula composed of traditional herbs and performed the world first partial mastectomy under a general anesthesia in 1804, anesthetic formulas very similar to those developed by him were widely recorded and deemed used in Japan and Northeast Asia before his invention. The origin of the formulas will be tracked down to compare with the several formulas broadly administered in the region. Methods : Historical literature analysis was adopted to achieve the objective. 1. Mayaku-ku (麻藥考): this book is the main medical classic by Nakagawa Syutei(中川修亭) that introduces Seishu Hanaoka, his anesthetic formulas and mastectomy. 2. Northeast medical classics: Seuideukhyobang (世醫得效方) in 1337, Uibangryuchui (醫方類聚), Uihui(宜彙) and so on. Result : Herbs such as aconitum and datura were applied as a anesthetic agent early on before the Chinese Yuan dynasty. In Korea as well, some old medical books documented such use of those herbs and relevant formulas. Conclusion : Formulas that counted as invented and employed by Seishu Hanaoka as anesthetics, in fact, had been widely known and used in the region before his era. We should pay due attention to his creativity that combined a western surgical intervention and traditional anesthetic agents and successfully performed a newly introduced surgical practice in Japan. The point is that Hanaoka took note of anesthetic herbs or formulas traditionally inherited in North-east Asian medicine and successfully applied them to the surgical procedures for breast cancer, or mastectomy and mammotomy. This history alerts us to neglected or forgotten potentials of traditional medicine in anesthetic treatment and more.

명당장부도(明堂臟腑圖)에서 기원한 간장도(肝臟圖) 29종의 비교 (Comparison of 29 Diagrams of Liver originated from Mingtangzangfutu(明堂臟腑圖))

  • Objectives : The goal of this paper is to research what affected diagrams of liver originated from Mingtangzangfutu while they were changed. Methods : Diagrams of liver in Traditional Chinese and Japanese medical books had been gathered as many as possible. After they were divided into ones which were originated from Mingtangzangfutu and the others, diagrams originated from Mingtangzangfutu, were again classified into 4 filiations according to Huang Longxiang(黃龍祥)'s standard. Apart from his classification, they were analysed by 4 form factors, e.g. shape, number, veins and petiole of leafs. Results : Reliability of this methods had been confirmed by the correspondence with Huang's 14 standard books, before 29 diagrams of liver were derived from 33 books including 13 books that Huang had already suggested. The lobes of liver in most of diagrams resembled 7 lanceolate or long oval leafs, or a maple leaf with 6 indentations, 4 left lobes and right 3 ones. In most of diagrams, veins of leafs were described variously, and petioles of leafs were drawn more shortly or longer. Analysing 29 diagrams by 4 form factors proved that Huang's classification had crossing features among 4 filiations. This phenomenon implied the painters or doctors drawing diagrams might not convince if the shape of liver was correct or not. Although veins and petiole of leafs in diagrams of liver could be compared with tissues in liver in modern human anatomy, diagrams were not been affected by dissection from Yuan(元) to middle Qing(淸) dynasty. Conclusions : Lingshu(靈樞), Nanjing(難經) and Wang Bing would had constantly acted on 29 diagrams of liver, since diagram of liver originated from Mingtangzangfutu, appeared first in Zhenjiujuying(鍼灸聚英). But they did not come from dissection during same periods.

신안해저 인양 침몰선의 복원 연구 (A Study on the Restoration of Shinan Shipwreck)

  • This study focused on the reconstructional point of Shinan ship-wreck that was excavated between $1976\~1984$. The wreck, which might be sunk in the beginning of the 14th century, is regarded as a vessel of Yuan dynasty, China. This paper tried to find out some structural characteristics and principal dimensions for restoration. The Shinan shipwreck's structural characteristics are summarized as follow, 1) The Shinan shipwreck is formed V-shaped cross section with bar keel, 2) The vessel is divided 8 holds by 7 bulkheads. 3) The ship has flat type stem and transome stern. 4) A rabbeted clinker -built is basically adopted on planking joint. 5) A wooden sheathing, which means a sort of protecting board against marine insects, is covered outside of the main hull, 6) For making an watertight structure, oakum and lime mixtured t'ung-oil are used along the seam of planking and bulkhead. 7) A V-shaped deep water-way exists at both deck side. 8) The shipwreck is believed to have 2 masts at least. 9) The shiptimbers are classified as Chinese Red Pine(Pinus Massonina) which is mainly grown in the southern part of China. Considering as mentioned above the structural characteristics, Shinan ship-wreck could be classified as Chinese Fu-chuan type(복선형) of sea-going ship. The Shinan ship's principal dimensions which are calculated on the basis of Chinese traditional shipbuilding custom, are as follow, Length overall(L.O.A). : 34.80m Length water line(L.W.L) : 24.90m Breadth(B.max.) : 11m Breadth(B) : 10m Depth at keel line(H) : 3.75m Draft(D). : 3.15m Freeboard(F) : 0.65m Ratio, length/breadth(L/B). : 2.26 Ration, breadth/depth(B/D) : 3.5 Height of stem : 7m Height of stern : 10m Displacement : ab.340ton.

조선통신사(朝鮮通信使) 의학필담록(醫學筆談錄) 내용 분석 - 의서(醫書) 관련 내용을 중심으로 - (A Content Analysis of the Medical Conversation Records from the Visits of Joseontongshinsa - Focused on the Medical Classics -)

  • Objectives : The purpose of this study is to find the differences of the viewpoints about medicine between Korean and Japanese doctors through analysing the contents of the medical conversation records from the visits of Joseontongshinsa. Methods : The conversations were classified according to the topics and analysed with emphasis on the subject of medical classics. Results : Japanese doctors were skeptical about the application of Huangdineijing(黃帝內經) and the theory of Five Circulation and Six Qi(五運六氣) to clinical treatment while they respected Shanghanlun(傷寒論). They were interested in the bibliography of medical classics, and considered it was important to study the original texts rather than the new editions. The doctors of Joseon valued Huangdineijing highly and accepted the theory of Five Circulation and Six Qi. They mainly used the treatments in Yixuezhengzhuan(醫學正傳), Yixuerumen(醫學入門), Shoushibaoyuan(壽世保元) as therapeutic methods. Conclusions : The conversation records reflect the trend of Korean Medicine in the Joseon Dynasty that Huangdineijing had been mainly studied and the Jin-Yuan(金元) Medicine had been accepted, and the trend of Japanese Medicine in the mid-Edo period that Koho school(古方派) had predominated.

중국 전통 귀족 여성 예복인 직거심의(直裾深衣)의 패턴 및 3D 복원 연구 (A study on pattern and 3D restoration of Chinese traditional women's robe, straight Ju(直裾深衣))

  • This study analyzed the style, dimensions, fabric patterns, colors, and fabrics of a traditional Chinese women's dress from the Zhou Dynasty, and reconstructed it in the form of a virtual garment using 3D CLO. Based on ancient flat image data and three-dimensional portrait data, who wore them, how they were worn, and how they were coordinated was analyzed. In order to analyze the size and pattern of the straight Ju Chines dress, data from the excavation report and the tomb owner's anthropometric measurements were combined to infer the wearing condition and organize the sculptural features. Dimensional analysis was carried out using a well-preserved small-scale woven cotton cloth as a restoration model, and the horizontal and vertical dimensions were reasonably estimated using the shape proportioning method. The analysis of the colors and patterns of the fabrics was based on the colors and patterns of the fabrics excavated from Masan Tomb No. 1 during the Eastern Zhou, Qin, and Han periods. Finally, a virtual model was created using data from the excavation report and the age and height information of the owner of the excavated robe, and the pose and size of the virtual model were determined using 3D CLO. Based on the previous research data, the garment was virtually sewn and simulated. The shape, pressure, and strain of the garment in different postures was also compared. Through the research direction of pattern and 3D restoration, this research maximizes the restoration of Chinese traditional women's dress and presents it in a more intuitive, comprehensive, and vivid way.

원대(元代)의 왕역(王繹)·예찬(倪瓚) 합작 <양죽서소상(楊竹西小像)> 연구 (A Study of Portrait of Yang Zhuxi housed in the Palace Museum in Beijing)

  • <양죽서소상>은 1363년이라는 분명한 연대를 지닌 원대의 희소한 초상화라는 점에서 유의미하다. 또 최초의 초상화 이론인 "사상비결(寫像秘訣)"의 저자이자 중요한 초상화가인 왕역(王繹)의 유일한 현존 작이라는 점, 대표적 산수화가인 예찬(倪瓚)과의 합작이라는 점, 그리고 산수인물화 형식 초상화의 이른 사례라는 점에서도 주목된다. 그림의 주인공은 본래 원대의 고관대작 양우로 알려져 있었으나, 이는 명대 이일화(李日華)의 오인에서 비롯된 것이다. 실제 피사인물은 송강지역의 은거 문인으로 강남의 문인 사회에서 명망이 높았던 양겸이다. <양죽서소상>은 소식(蘇軾)과 같은 은자의 도상으로 묘사되었는데, 이는 은거 문인 양겸을 표현하는데 있어 적절한 선택이었다. 또 예찬이 그린 바위와 소나무는 군자, 문인이라는 전통적 상징성과 맞물려 작품의 의미를 강화시켜 주었다. 양식적으로는 백묘법을 사용하였다는 점에서 동시기의 여타 초상화들과 차별화된다. 그러나 안면 표현의 경우 채색이 배제되었음에도 오히려 뛰어난 사실성을 보여준다. 반면 신체의 표현에 있어서는 단아하고 절제된 서예적인 선묘를 구사함으로써 작품의 격조를 높이고자 하였다. <양죽서소상>은 기능이라는 측면에서 보았을 때, 제의적 성격이 강한 여타 초상화들과 달리 감상적 성격이 강하다. 피사인물 자신과 친구들의 열람, 감상 목적으로 제작된 것으로 서로간의 교유적인 측면에서도 의미가 컸다. 그러나 여기에는 단순한 감상의 차원을 넘어서서 피사인물 자신과 친구들의 심회를 담고자 하는 의도가 컸다.

사총혈가(四總穴歌)에 관(關)한 연구(硏究) (The study of clinical usefulness of Si-Zong-Sue-Ge(四總穴歌))

  • Ge-Fu(歌賦) means prose and poetry individually, and they both have a meaning of all the rhythmical poetrical compositions making it ease for people to remember the content. All the Ge-Fus used in oriental medicine are made in Yuan(元), Mine(明)and Qing(靑)dynasty, and they have been largely used in most of all the fields of medicine such as Ben-Cao(本草), Tang-Ye(湯液), Zhen-Jiu(鍼灸), Zhen-Duan(診斷). Zhen-Jiu-Ge-Fu(鍼灸歌賦) has about 90 poetries and 10 proses and they contain the names of meridian; courses of meridian streams; accurate positions of acupuncture points; functions; effects; meanings of the name of acupuncture points; usages and effects of special points; manipulations of reinforcing and reducing method; contraindications of acupuncturing; principles of selections and orders of acupuncture points in therapy; and eight diadgoses. Zhen-Jiu-Ge-Fu is subdivided into Jing-Xue(經穴歌), Zhi-Jiu-Ge(刺灸歌), Shu-Xue-Ge(輸穴歌), Zhi-Liao-Ge(治療歌). And In Zhi-Liao-Ge, the most brief and essential Ge-Fu-Si-Zong-Sue-Ge- contains theraputic designs using far apart acpuncture points from the right painful areas in the body. In this study, the author opinionated the Si-Zong-Sue-Ge can be the prototype of the distant needling; the research on this can open the importance of Ge-Fus. On conclusion, 1. "Upper and lower Abdomen - Zu-San-Li($S_{36}$) (肚腹三里留)" means when there are problems and disorders in upper and lower abdomen, distinctively, such as gastric pain, maldigestion, flatulence, abdominal pain, constipation, diarrhea, vomiting, menstrual disorer, knee pain and tonic functioning, 족삼리 can be a right choice for distant needling point for treating. 2. "Face and Eye-He-Gu($Li_4$) (面目合谷收)" means when there are problems and disorders in facial, eye, ear, nose, throat, mouth regions, distinctively, such as facial edema, toothache, headache, sore throat, rhinorrhea, frontal headache, abdominal pain, dizziness, He-Gu can be a right choice for distant needling point for treating. 3. "Upper and lower back - Wei-Zhong($B_{40}$) (腰背委中求)" means when there are problems and disorders in upper and lower back, distinctively, such as upper back pain, lumbargo, hamstring muscle pain, popliteal region pain, lower extremity compartment syndrom, Inguinal region pain, muscle twitch, vomiting and diarrhea, hemorrhoidal bleeding, skin rash, Wei-Zhong can be a right choice for distant needling point for treating. 4. "Head and neck - Lie-Que($L_7$) (頭項審列缺)" means when there are problems and disorders in capital and nuchal area, distinctively, such as migraine, frontal headache, rhinorrhea, asthmatic dyspnea, aphasia, coughing, neck stiffness, occipital headache, upper extremity pain, Lie-Que can be a right choice for distant needling point for treating.
