Background: Breast cancer is the second most common cancer in women in Cambodia, a low income country in South-East Asia. The Sihanouk Hospital Centre of Hope (SHCH) is a charity hospital set up by an international non-governmental organisation, HOPE Worldwide. In 2008, SHCH partnered with AmeriCares, a global health organisation to set up and deliver a breast cancer programme to provide education, diagnosis and treatment for women with breast cancer. The objective of this study is to characterise the presentation, diagnosis, treatment and outcomes of women treated under this program. Materials and Methods: A total of 215 women newly diagnosed with breast cancer from 1 March 2008 until 31 March 2011 were studied. Age at diagnosis, tumour size, histological type, tumour grade, ER, lymph node involvement, treatment modalities (surgery, radiotherapy, chemotherapy, hormone therapy) were recorded. Data on mortality at 3 years were obtained whenever possible. Results: The median age was 47 years old. Some 77.8% were diagnosed with stage 3 and 4 lesions, and 78.5% underwent mastectomy, of which 28.4% the intent was palliative. Of those whose ER status were known, only 48.3% were ER positive. Only 6 patients could afford chemotherapy while only 1 patient had radiotherapy. Hormone therapy was provided free for those who were ER positive. The overall survival rate at 3 years was 39.1%. Conclusions: Breast cancer presents at a late stage, and because treatment is suboptimal, survival is poor in Cambodia. A more aggressive approach to early detection and treatment needs to be developed to improve outcome from this potentially curable disease.
This study describes an efficient protocol for Agrobacterium tumefaciens-mediated transformation of two subgroups of genotype AAA bananas (Musa acuminata cv. Pei Chiao and Musa acuminata cv. Gros Michel). Instead of using suspension cells, cauliflower-like bud clumps, also known as multiple bud clumps (MBC), were induced from sucker buds on MS medium containing $N^6$-Benzylaminopurine (BA), Thidiazuron (TDZ), and Paclobutrazol (PP333). Bud slices were co-cultivated with A. tumefaciens C58C1 or EHA105 that carry a plasmid containing Arabidopsis root-type ferredoxin gene (Atfd3) and a plant ferredoxin-like protein (pflp) gene, respectively. These two strains showed differences in transformation efficiency. The EHA105 strain was more sensitive in Pei Chiao, 51.3% bud slices were pflp-transformed, and 12.6% slices were Atfd3-transformed. Gros Michel was susceptible to C58C1 and the transformation efficiency is 4.4% for pflp and 13.1% for Atfd3. Additionally, gene integration of the putative pflp was confirmed by Southern blot. Resulting from the pathogen inoculation assay, we found that the pflp transgenic banana exhibited resistance to Fusarium oxysporum f. sp. cubense tropical race 4. This protocol is highly advantageous to banana cultivars that have difficulties in setting up suspension cultures for the purpose of quality improvement through genetic transformation. In addition, this protocol would save at least 6 months in obtaining explants for transformation and reduce labor for weekly subculture in embryogenic cell suspension culture systems.
The yeast gene THR1 encodes the homoserine kinase (EC 2.7.1.39: HKase) which catalyses the first step of the threonine specific arm at the end of the common pathway for methionine and threonine biosynthesis. A recombinant plasmid pMC3 (12.6 kilobase pairs, vector YCp50) has been cloned into E. coli HB101 from a yeast genomic library through its complementing activity of a thr1 mutation in a yeast recipient strain M39-1D. When subcloned into pMC32 (8.6kbp, vector YRp7) and pMC35 (8.3 kbp, vector YIp5), the HindIII fragment (2.7 kbp) of pMC3 insery was positive in the thrI complementing activity in both yeast and E. coli auxotrophic strains. The linearized pMC35 was introduced into the original recipient yeast strain and the mitotically stable chromosomal integrant was identified among the transformants. Through the tetrad analysis, the integration site of the pMC35 was localized to the region of THR1 structural gene at an expected genetic distance of approximately 11.1 cM from the ARG4 locus on the right arm of the yeast chromosome VIII. When episomically introduced into the auxotrophic cells and cultured in Thr omission liquid medium, the cloned gene overexpressed the HKase in the order of thirteen to fifteenfold, as compared with a wildtype. HKase levels are repressed by addition of threonine at the amount of 300 mg/l and 1, 190 mg/l for pMC32 and pMC3, respectively. Data from genetic analysis and HKase response thus support that the cloned HindIII yeast DNA fragment contains the yeast thr1 structural gene, along with necessary regulatory components for control of its proper expression.

YangDongChangHwaHuRok(兩東唱和後錄)is a book that organized the conversation between Joseon physician Ki-DuMun(奇斗文) and Japanese Acupuncture specialist Murakami Tanio(村上溪南) and others when they visited the quarters where the Choson Delegation(朝鮮通信使) were staying in, while the Choson Delegation party were visiting Japan in 1711. YangDongChangHwaHuRok(兩東唱和後錄) was discovered in the process of gathering medicine-related historical documents of the Choson Dynasty that is spread out all over the Japanese region, with the help from Japanese and Chinese scholars. This was part of the Co-Examination-Research-Process of korean medical documents that are located in the East Asia traditional medical region, which was promoted by the Korean Institute of Oriental Medicine in 2003. This paper has analyzed in-depth, with an inner-medical view, the academic exchange content of YangDongChangHwaHuRok(兩東唱和後錄) by following the order of the main text. With this examination, we were able discover new facts about the interest in Primary Symptom(是動病) and Secondary Symptom(所生病) of 17th century doctors, a new hypothesis on the diffusion of HeoYim(許任)'s ChimGuKyungHeomBang(鍼灸經驗方) in Japan, the existence of a new acupuncture tool called ChongChim(腫鍼), and the influence of the 18th comtemporay famous traditional medical books (it is called UiHakYipMun(醫學入門) and ShinEungKyung(神應經) on the 18th century East Asian medical circle. We were also able to ascertain the specific medical content that was diffused to the Japanese medical circle by the Korean medical circle.

Business process engineering (BPE) is a top-down management approach for increasing efficiency and productivity through radical and fundamental changes to the business processes of the organization. BPE requires firms to initially develop a model of the existing business processes of the firm to distinguish functional tasks from processes used for coordinating inputs, activities and outputs. The model is used for understanding the business processes in the organization and to simulate the effect of changes to the processes. The model can also be used to justify business processes, which involves assessing whether the business process provides value to the customer in its current configuration. Justification requires a careful examination of the key business processes used by the firm to identify systemic shortcomings in the process and to create a new business process to produce greater efficiency. BPE also considers automating as many business processes as possible to increase operational efficiency and the integration of business process tasks. The construction industry has been slow to adopt BPE because of its project approach in which a major firm contracts with various functional service providers and regards each project as unique. The industry focuses on functional task efficiency rather than business process efficiency. There is no formal methodology or criteria for determining whether a business process is effective for a construction firm in its current configuration. The use of performance measures such as costs, task duration times or other metrics can be useful in evaluating the effectiveness of an existing business process and for modeling the possible outcome of a fundamental and radical change to the process.

Emergency pancreaticoduodenectomy (EPD) is a rarely performed operation. It is important to know the indications and outcomes of EPD to have a better understanding of its application in clinical practice. A review of eight consecutive cases of EPD was done. Between January 2003 and December 2021, 8 out of 370 patients (2.2%) in a single center received pancreaticoduodenectomy as emergency. There were six males and two females with a median age of 45.5 years. The indications were trauma in three patients, bleeding tumors in two patients, and one patient each in obstructing duodenal tumor, postoperative complication and post-endoscopic retrograde cholangiopancreatography (ERCP) complication. The median operative time and blood loss were 427.5 minutes and 1,825 mL, respectively. There was no operative mortality. Seven patients (87.5%) had postoperative complications. Three patients (37.5%) developed postoperative grade B pancreatic fistula. The median postoperative hospital stay was 23.5 days. Five patients were still alive while three patients survived for 13, 31, and 42 months after the operation. The causes of death were recurrent tumors in two patients, and sepsis in one patient. According to this case series, EPD is associated with increased morbidity and pancreatic fistula, but is still deserved in life-threatening situations and long-term survival is possible after EPD.

Purpose: The incidence of deep vein thrombosis (DVT) following total hip arthroplasty (THA) without chemoprophylaxis could be as high as 50% in Caucasians. However, according to several subsequent studies, the incidence of venous thromboembolic events (VTE) in Asians was much lower. The routine use of chemoprophylaxis, which could potentially cause increased bleeding, infection, and wound complications, has been questioned in low-incidence populations. The objective of this study is to determine the incidence of VTE after primary THA without chemoprophylaxis in an Asian population using a fast-track rehabilitation protocol and to verify the safety profile for use of 'mechanical prophylaxis alone' in patients with standard risk of VTE. Materials and Methods: This is a retrospective cohort study of 542 Hong Kong Chinese patients who underwent primary THA without chemoprophylaxis. All patients received intermittent pneumatic compression and graduated compression stockings as mechanical prophylaxis. Multimodal pain management was applied in order to facilitate early mobilisation. Routine duplex ultrasonography was performed between the fourth and seventh postoperative day for detection of proximal DVT. Results: All patients were Chinese (mean age, 63.0±11.9 years). Six patients developed proximal DVT (incidence rate, 1.1%). None of the patients had symptomatic or fatal pulmonary embolism. Conclusion: The incidence of VTE after primary THA without chemical prophylaxis can be low in Asian populations when following a fast-track rehabilitation protocol. Mechanical prophylaxis alone can be regarded as a reasonably safe practice in terms of a balanced benefit-to-risk ratio for Asian patients with standard risk of VTE.

${\ll}$Gup Yu Bang${\gg}$ is the first korean book that specialized in pediatrics. It was written by Jo Jeong-jun in 1869 and contains 87 medical records. Medical records is one of the best way to develop one’s abilities of curing a disease without clinical practice and understanding medical condition of the time. These days we are fully aware of the importance of medical records, but there is no sufficient paper or study on medical records. On this study, we investigate 87 medical record of ${\ll}$Gup Yu Bang${\gg}$ by statistical methods. It was analyzed on the base of items such as age, sex, disease, method of medical treatment, treating prescription, modified medicinal substance, progress of disease. ${\ll}$Gup Yu Bang${\gg}$ contain 87 medical records and among of them, 84 records were Jo Jeong-jun own writings of clinical case. Systemic division of disease showed the highest in digestive disease and most frequent case was symptom of vomiting and diarrhea. The distribution of age was showed the highest in the age group under 5 years and the number of male case was more than female case. It showed that he considered observation of the patient's expression in diagnosis methods and have used 87 kinds of treating prescription and the external treatment, acupuncture, moxa. It showed that he quoted a lot of treating prescription from ${\ll}$Dong eui bo gam${\gg}$, ${\ll}$Yi-Hak-Yip-Mun${\gg}$ and ${\ll}$Xiao Er Yao Zheang Zhi Jue${\gg}$. The further study on medical records in traditional medical literatures, would reveal the developmental progress of Korean pediatrics and inform more actual proof on medical condition of the time.
Background: The survival outcomes for women presenting with early breast cancer are influenced by treatment decisions. In Malaysia, survival outcome is generally poor due to late presentation. Of those who present early, many refuse treatment for complementary therapy. Objective: This study aimed to explore the decision making experiences of women with early breast cancer. Materials and Methods: A qualitative study using individual in-depth interviews was conducted to capture the decision making process of women with early breast cancer in Malaysia. We used purposive sampling to recruit women yet to undergo surgical treatment. A total of eight participants consented and were interviewed using a semi-structured interview guide. These women were recruited from a period of one week after they were informed of their diagnoses. A topic guide, based on the Ottawa decision support framework (ODSF), was used to facilitate the interviews, which were audio recorded, transcribed and analysed using a thematic approach. Results: We identified four phases in the decision-making process of women with early breast cancer: discovery (pre-diagnosis); confirmatory ('receiving bad news'); deliberation; and decision (making a decision). These phases ranged from when women first discovered abnormalities in their breasts to them making final surgical treatment decisions. Information was vital in guiding these women. Support from family members, friends, healthcare professionals as well as survivors also has an influencing role. However, the final say on treatment decision was from themselves. Conclusions: The treatment decision for women with early breast cancer in Malaysia is a result of information they gather on their decision making journey. This journey starts with diagnosis. The women's spouses, friends, family members and healthcare professionals play different roles as information providers and supporters at different stages of treatment decisions. However, the final treatment decision is influenced mainly by women's own experiences, knowledge and understanding.

Objectives : ${\ulcorner}HuaTaHyunMunNeJoDo{\lrcorner}$ is a medical literature dealing deeply with the theory of internal organs thought to be the core of theories in Chinese medicine. The aim of this study was to explorer on the formation and the organization of ${\ulcorner}HuaTaHyunMunNeJoDo{\lrcorner}$. Methods : We investigated the process of the formation of ${\ulcorner}HuaTaHyunMunNeJoDo{\lrcorner}$ and analyzed the organization of ${\ulcorner}HuaTaHyunMunNeJoDo{\lrcorner}$ on the medical-historical points of view. Results : As a result from studies. Main contents include 11 pieces of pictures about the positions of the regions of acupuncture and moxibustion as well as the internal organs, in addition to descriptions in detail regarding each disease in the internal organs, problems occurred among them, obstinate diseases, etc. Conclusions : A published book in Ming Dynasty and another transcribed by someone in Qing Dynasty as the xylographic books of this literature in China, which turned out to books referred considerably to contents of YangGae's ${\ulcorner}JonJinDo{\lrcorner}$, a famous doctor in Northern Song Dynasty in China. Moreover, the main contents of ${\ulcorner}HuaTaHyunMunNeJoDo{\lrcorner}$ greatly affected the theories of the internal organs for the following generations as they were quoted in Yicheon's ${\ulcorner}UiHakYipMun{\lrcorner}$ in Ming Dynasty in China.
