Arthroscopic Excision of Heterotopic Ossification in the Supraspinatus Muscle

  • Heterotopic ossification is formation of bone in atypical extra-skeletal tissues and usually occurs spontaneously or following neurologic injury with unknown cause. We report a 46-year-old female with right shoulder pain and restricted range of motion (ROM) for 3 months without history of trauma. Magnetic resonance imaging (MRI) showed a lesion within the rotator cuff supraglenoid. Excisional biopsy from a previous institution revealed a heterotopic ossificans (HO). Following repeat MRI and bone scan, histopathology from arthroscopic resection confirmed an HO. The patient demonstrated improved pain and ROM at follow-up. Idiopathic HO rarely occurs in the shoulder joint, and resection of HO should be delayed until maturation of the lesion to avoid recurrence. The current case showed that arthroscopic HO resection provides an excellent surgical view to ensure complete lesion removal and minimize soft tissue damage at the supraglenoid area. Furthermore, the minimally invasive procedure of arthroscopy may reduce rehabilitation time and facilitate early return to work.

  • Kangwon province has been suffering from the various types of disasters and these disasters stand in the way to sustainable development. The heavy rainfall by typhoon RUSA in 2002 and HAEMI in 2003 damaged to property and bodily injury, and gave us a lesson. Even if the refuge activities plan by local government plan for disaster were established with rainfall event, it could not its role in practice. Beside it, the potential disaster risk remains still in existence because the disaster restoration work aims to original state. Therefore, in this study we focused on the point at issue of disaster countermeasure plan by local government and its integrated plan considering local characteristics.

Alternative Prevention on Human Error of Fatal Injuries by the Folk Lifts (지게차 사망재해의 인적오류에 대한 대안)

  • According to the statistics, occupational fatal injuries by the fork lifts were about 30 per year in whole industrial. Fork lifts are widely used in various parts of industries to improve the efficiency of the work. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by the fork lifts. Fatal injury analysis were conducted with several accident cases by the fork lifts. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the fork lift showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. First, fork lifts were the highest of the machines caused the accidents. In order to prevent fatal injuries by the fork lifts, the tarket was manufacturing industry. Second, the order of the cause of cognitive engineering agenda in the manufacture industrial was visibility, responsibility and affordance, and revision of acts was proposed. Third, there was not a lots of different points of human error between occurrence types and business sizes. Forth, number of fatalities by the attacker was more than by the inducer.

Business Cycle and Occupational Accidents in Korea

  • Background: Occupational accidents occur for a variety of reasons, such as unsafe behaviors of workers and insufficient safety equipment at the workplace, but there are also various economic and social factors that can impact working conditions and working environment. This study analyzed the relationship between changes in economic factors and the occurrence of occupational accidents in Korea. Methods: Multilinear regression analysis was used as the analysis model. The general to specific method was also used, which consecutively removes statistically insignificant variables from a general model that includes dependent variables and lagged variables of dependent variables. Results: The frequency of occupational accidents was found to have a statistically significant relationship to economic indicators. The monthly number of cases of occupational injury and disease and fatal occupational injuries were found to be closely related to manufacturing capacity utilization, differences in the production index in the services sector, and commencements of building construction. The increase in equipment investment indicators was found to reduce fatal occupational injuries. Conclusion: The results of this study may be used to develop occupational accident trends or leading indicators, which in turn can be used by organizations that manage and monitor occupational accidents toward taking administrative action designed to reduce occupational accidents. The results also imply that short-term and mid- to long-term economic and social changes that can impact workers, workplaces and working conditions, and workplace organizations must be taken into account if more effective government policies are to be established and implemented toward further prevention of occupational accidents.

Bilateral Skin Temperature Change of the Anterior Thigh Following Unilateral Isokinetic Exercise (등속성 운동 후 양측 대퇴부의 피부 온도 변화)

  • The purpose of this study was to measure and compare the skin temperature over the exercised muscle and corresponding non-exercised muscle after unilateral isokinetic exercise using digital thermography. Thirty-two young healthy volunteers with no history of knee injury were tested. After isokinetic exercise at 60 degree per second angular velocity using the right leg in a climatic chamber at ambient temperature of $23-26^{\circ}C$, skin temperature of the anterior thigh was tested. After exercise, the skin temperature of both the right and left leg had fallen significantly. The skin temperature of the exercised leg fell less than that of the non-exercised leg. The fall in skin temperature after work was not due to increased evaporative cooling, but was the result of segmental vasoconstriction probably caused reflexly in the spinal cord by non-thermal afferents from exercising muscle or moving tissues. The effect of thermoregulatory vasodilation was reduced by reflex vasoconstriction caused by non-thermal factors such as catecholamine.

Clinical-Case Study on a Patient with Organic Mental Disorder after Head Trauma Caused by Traffic Accident (교통사고로 인한 외상 후 기질성 정신장애 환자 치험 1례)

  • Objectives: This study evaluated the effect of oriental treatments and rehabilitation on a patient with organic mental disorder after head trauma caused by traffic accident. Methods: We treated the patient with acupuncture, herb-medication, oriental psychotherapy, and rehabilitation. MMSE-K, BDI, K-MBI, FIM were evaluated before and after treatment. Results: The results indicated that MMSE-K was maintained at 27 points; BDI was reduced from 16 points to 5 points; K-MBI was increased from 75 points to 99 points; and FIM was increased from 86 points to 115 points. Furthermore, the treatment resulted in reduced mental symptoms and improved work capacity. Conclusions: Oriental medical treatment has potential validity in improving symptoms of organic mental disorder.

  • Thrombotic microangiopathy (TMA) is a microvascular thrombotic lesion caused by endothelial injury and subsequent formation of platelet rich thrombus. TMA is first described as a classical pathologic feature of HUS/TTP. Renal biopsy finding of TMA represents kidney involvement of HUS/TTP as well as other diseases such as malignant hypertension, drug toxicity, eclampsia, pre-eclampsia, and several systemic infections. Autoimmune diseases and transplant kidney sometime also have TMA. It is needed to consider a complete autoimmune work-up of patients presenting with TMA including tests for ANA, ANCA, and ADAMTS13 inhibitory antibodies, because there are several reports of association with TMA in patients of SLE, anti-phospholipid syndrome, and ANCA-associated vasculitis.

  • The primary purpose of this study was to analysis quantitatively the degree of injury and/or the progress of treatment for physical impairment. This study provided a more detailed study that evaluates all test parameters including maximum grip strength, duration of operation, average grip strength, acceleration work, dynamic endurance time and percent change in static endurance. Also, a complete database management system is developed and used to store related training, evaluation, and personal information. In addition, the system is developed a grip sensor using load.cell transducer (${\leq}60kg$). The system will be efficient to operate and convenient to use, furthermore, it can be helped for understanding and analysis the progress of a patient during a doctor's rehabilitation program.

The Axillary Approach to Raising the Latissimus Dorsi Free Flap for Facial Re-Animation: A Descriptive Surgical Technique

  • The latissimus dorsi flap is popular due to the versatile nature of its applications. When used as a pedicled flap it provides a robust solution when soft tissue coverage is required following breast, thoracic and head and neck surgery. Its utilization as a free flap is extensive due to the muscle's size, constant anatomy, large caliber of the pedicle and the fact it can be used for functional muscle transfers. In facial palsy it provides the surgeon with a long neurovascular pedicle that is invaluable in situations where commonly used facial vessels are not available, in congenital cases or where previous free functional muscle transfers have been attempted, or patients where a one-stage procedure is indicated and a long nerve is required to reach the contra-lateral side. Although some facial palsy surgeons use the trans-axillary approach, an operative guide of raising the flap by this method has not been provided. A clear guide of raising the flap with the patient in the supine position is described in detail and offers the benefits of reducing the risk of potential brachial plexus injury and allows two surgical teams to work synchronously to reduce operative time.
