Prevalence and Risk Factors of Anxiety, Depression, and Post-Traumatic Stress Disorder in Critical Care Survivors (중환자실 퇴원 환자의 불안, 우울, 외상 후 스트레스 장애 유병률 및 위험요인)

  • Purpose : This study aimed to investigate the prevalence and risk factors of mental health problems in patients discharged from the intensive care unit (ICU). Methods : This was a secondary analysis study using data from a multicenter prospective cohort of post-ICU patients. We analyzed data of 311 patients enrolled in the primary cohort study who responded to the mental health questionnaire three months after the discharge. Anxiety and depression were measured on the Hospital Anxiety-Depression Scale, and post-traumatic stress disorder (PTSD) was measured on the Posttraumatic Diagnostic Scale. Results : The prevalence of anxiety, depression, and PTSD in patients at three months after ICU discharge were 25.7%, 17.4%, and 18.0%, respectively, and 7.7% of them experienced all three problems. Unemployment (OR=1.99, p=.033) and unplanned ICU admission (OR=2.28, p=.017) were risk factors for depression, while women gender (OR=2.34, p=.009), comorbid diseases (OR=2.88, p=.004), non-surgical ICUs (trauma ICU: OR=7.31, p=.002, medical ICU: OR=3.72, p=.007, neurological ICU: OR=2.95, p=.019) and delirium (OR=2.89, p=.009) were risk factors for PTSD. Conclusion : ICU nurses should proactively monitor risk factors for post-ICU mental health problems. In particular, guidelines on the detection and management of delirium in critically ill patients should be observed.

Endless Management of Mind and Body for Healing: The Process of Pursuing Complementary and Alternative Medicine in Patients with Breast Cancer (유방암 환자의 보완대체요법 추구 과정: 치유를 향한 끝없는 심신 다스리기)

  • Purpose: The purpose of the study was to explore and describe the experience of pursuing complementary and alternative medicine (CAM) in breast cancer patients. Methods: Ten women with breast cancer participated in the study. Data were collected through individual in-depth unstructured and individualized interviews with each participant from February to July, 2015. Theoretical sampling was used upto the point of theoretical saturation. Data were analyzed using Corbin & Strauss's grounded theory methodology. Results: Through open coding, 22 sub-categories, and 13 categories were identified. Analysis revealed that the core category was 'endless management of mind and body for healing', which consisted of four phases; exploring, applying, grasping, and integrating. Through this process, the participants utilized various action/interactional strategies such as 'dealing with information', 'trying out blindly', 'enduring volitionally', 'experiencing effects on mind and body,' and 'grasping one's own way.' The consequences of these strategies were integrating the habit of health remedy into one's life, becoming a main agent for change, and tolerating one's weak body. Conclusion: In-depth understanding of the CAM pursuing experiences of patients with breast cancer would guide clinical nurses and policy makers to develop effective interventions and policies for better supporting them with regard to the usage of CAM.

The Experience of People with HIV/AIDS : A Phenomenological Study (HIV 감염자의 생활 경험에 관한 현상학적 연구)

  • The purpose of this phenomenological study was to explore and describe the experience of people with HIV/AIDS. Three men and three women with HIV/AIDS participated in the study. They were asked open-ended and descriptive questions in order for them to talk about their experience in their own terms. Interviews were audiotaped and transcribed to maintain data integrity and to reduce perceptual bias. The transcripts were analyzed by the Colaizzi method. Member checks from the participants were used to validate the emergent themes. Seven themes emerged from the analysis. 1. People with HIV/AIDS have a strong impact and emotional reaction when receiving positive HIV test results. 2. All people with HIV/AIDS attempt to conceal HIV infection because of fear of stigma or rejection. 3. After the initial brief traumatic reaction to HIV/AIDS diagnosis, all participants feel lonely because they had no one to express or share their own experiences. 4. People with HIV/AIDS reflect on their life, think about the death and dying process and they become depressed. 5. Most participants feel that the family members who know his/her diagnosis are a big source of support. 6. According to the normativeness of infection route of HIV, the degree of guilty feeling differs among participants. 7. Although all participants think medication is a lifeline, the adherence to medication is not maintained because of side effects, denial and concealment of HIV/AIDS. The results of this study may help nurses and other health care workers to implement more efficient nursing strategies for people with HIV/AIDS by more deeply understanding their experience.

The development and effectiveness of web-based continuing nurse education program (웹기반 간호사 보수교육 시스템의 개발 및 효과)

  • This research aims to produce and implement web-based continuing nurse education programs in response to nurses' educational needs, and to verify them, thus preparing for the times that the program will be offered via web. This research designed, produced and implemented 'emergency nursing' and 'medical fee schedule management' subjects based on Jung, In-seong's(1997) web-based instructional system design, and then compared the learning achievements of web-based learning group of 38 people with those of face-to-face learning group of 39 people. The questionnaire have been developed by these researchers to measure pre-learning knowledge on 'emergency nursing' and 'medical fee schedule management.' Data collected for this research have been given statistical analysis, using SPSS 10.0 for Windows Program. As a result of giving Mann-Whitney test, with respect to pre-learning prior knowledge level, there was no significant difference between the web-based learning group and the face-to-face learning group(Z=-.092, p=.926), while after completing learning, there was a significant difference in the learning achievements between the web-based learning group and the face-to-face learning group(Z=-2.406, p=.008). That is, this research revealed this: the web-based learning group and the face-to face learning group with both having no significant difference in the pre-learning level, after receiving the continuing education each with different methods(face-to-face education and web-based education), showed that the web-based learning groups attained higher learning achievements than the face-to-face learning groups. This result proves the effect of the web-based education to be no worse or even better than that of the face-to-face education, provided that choices of appropriate themes and quality courses composition, as well as systematic design development effective implementation are guaranteed.

The Effects of a Comprehensive Education Program on Knowledge, Self-Efficacy, and Coping Style among Newly Diagnosed Patients with Breast Cancer (통합적 교육프로그램이 진단 직후 유방암 환자의 지식, 자기효능감 및 대응방식에 미치는 효과)

  • Purpose: The purpose of the study was to identify the effect of a comprehensive education program on knowledge, self-efficacy for self-management, and coping style among newly diagnosed patients with breast cancer. Methods: This study used a one group pretest-posttest design. Data were collected from April to September, 2010 at a cancer-specific hospital in Goyang, Korea. One hundred forty three women received a comprehensive education program consisting of 3 sessions composed of 8 themes was conducted by several experts (nurses, doctors, dietitians and clinical psychologists) for 3.5 hours a day for 3 days (1 day before and 2 days after the operation). Results: This study showed that knowledge and self-efficacy for self-management significantly increased after education (p<.001). In terms of coping style, however, only the subscale, 'anxious preoccupation', significantly improved (p=.001) among 5 sub-scales of coping style after education. Conclusion: A comprehensive education program for newly diagnosed patients with breast cancer was effective for increasing the knowledge and self-efficacy for self-management of breast cancer, in addition to decreasing the coping style of 'anxious preoccupation'. However, it did not improve other coping style. Therefore, other strategies for improving coping style are needed.

A basic study for establishment of specialized dental hygienist system around the duties of dental hygienist (전문치과위생사제도 확립을 위한 기초 조사 연구)

  • With these changes in the environment of dental service, dental hygienists are suggested to change their roles to cope with the changes. Hygienists are putting forth many efforts to promote smooth and efficient dental treatments, and as a practical measure. As a part to cope with such changes. Korea Dental Hygienists Association(KDHA) has prepared the system of Specialized Dental Hygienist and put the system into enforcement through an affiliated organization, Korean Academy of Dental Hygiene. The purpose of this study is to investigate the specialized nurse system in our country's medical environment and the specialized dental hygienist systems in other countries as similar cases comparable to the specialized dental hygienist system in our country and present basic data for the establishment of specialized dental hygienist system. For this study, a survey of dental specialists, such as dentists, dental hygienists and nurses, and patients, has been conducted qualitatively through person-to-person depth interview. The interview questions were related to the need for a specialized dental hygienist system, educational programs, functions and roles, and issues that must be solved for establishment of the system. Based on the interview results, the following conclusions were derived. The specialized dental hygienist system must encourage dental hygienists to possess advanced abilities in clinical practices, present systematic and developmental directions in educational programs, and stimulate specialized dental hygienists to actively work as manager and supervisor, medical health service providers, educators, and researchers. Lastly, for issues that must be solved for the establishment of the system, the duties and jobs of specialized dental hygienists must be defined more concretely, which must be acknowledged by people working in related occupations, citizens and the government. Furthermore, we need to examine the scope of duties of dental hygienists and enact laws and systems to protect the scope. These roles will lead dental hygienists to lay the foundation that allow them to make enthusiastic activities as an oral hygienist and clinician as well as show the way how to act as an educator, a researcher, a manager and an administrator.

Intentions to Care for New Influenza A(H1N1) Patients and Influencing Factors: An application of theory of planned behavior (계획된 행위이론을 적용한 간호사의 신종인플루엔자 A 환자 간호의도와 영향 요인)

  • Purpose: The purpose of this study was to explore the intentions and influencing factors of intentions to care for New Influenza A ($H_1N_1$) patient Methods: This study involved a descriptive design using self-administered questionnaire. Intentions to care for H1N1 patient was evaluated by prediction tool, based on the Theory of Planned Behavior (TPB). The data were analyzed by SPSS 17.0 using descriptive statistics, t-test, ANOVA with a Scheffe test, Pearson's correlation and multiple regression analysis. Results: The level of intention was high, attitude was negative, subjective norm was high, and perceived behavioral control was moderate. The general regression model with intention as a dependent variable was statistically significant (F=39.31, p<.001). 28.1% of variance in intention was explained by subjective norm (t=8.75, p<.001), and perceived behavioral control (t=4.28, p<.001). Among the predictors, subjective norm had the greatest effect on intention (${\beta}=.44$). The nurse with the higher subjective norm and more positive perceived behavioral control reported the higher intention. Conclusion: The findings of this study suggested that the various aspects of nurse's characteristics should be considered when establishing strategies to improve the nurse's intention for care of infectious disease.

Evaluation of the Standardized Patients(SP) Managed Instruction for a Clinical Maternity Nursing Course (표준화 환자를 활용한 모성간호학 실습 학습방법 효과 분석)

  • Purpose: The purpose of this study was to investigate the learning effect of a standardized patient managed instruction program for a clinical maternity nursing course. Methods: The SP managed instruction was made for clinical cases in intrapartum and postpartum nursing care. The SP managed instruction was evaluated by using a quasi-experimental, nonequivalent control group post-test design with separate classes of students attending clinical maternity nursing classes at a nursing school in Incheon. Control groups were taught by traditional lecture/model method and experimental groups were taught by SP managed instruction. Data was collected from September 2009 to February 2010. Results: 1. There was no significant difference between the experimental group and control group in identifying relevant data (z=-1.418, p=.156) and necessary nursing skills performance (z=-.643, p=.520) for intrapartum nursing care. In addition, there was no significant difference between the two groups in identifying relevant data (z=-.264, p=.782) and necessary nursing skill (z=-.342, p=.732) for postpartum nursing care. 2. In clinical nursing skill performances the total score was statistically significantly higher in the experimental group than the control group (intrapartum care z=-4.181, p=.000, postpartum care z=-4.279, p=.000). 3. There was a statistically significant difference between the two groups (intrapartum care z=-3.731, p=.000, postpartum care z=-4.066, p=.000). Conclusion: In conclusion, the SP methodis effective in teaching clinical maternity nursing for student nurses and it is necessary to develop SP instruction courses for many areas in the clinical nursing education field.

An Analysis of the Work of Visiting Health Care Providers In Public Health Centers (보건소 방문보건인력의 방문보건 업무수행 실태 조사)

  • The purpose of this study was to analyze the work of visiting health care providers in public health centers. A descriptive analysis of self-records for work data from 875 visiting health care providers working in 242 public health centers in South Korea was done. The results of the study are as follows: 1. The number of households for each visiting health care provider ranged from under 100 households to over 500 households. 2. Low performance was found for several items on the work list for visiting health care providers. 3. There were significant differences in the work performance of visiting health care providers between nurses and nursing assistants. 4. There were significant differences in the work performance of visiting health care providers according to region. In conclusion. work performance of visiting health care providers was low and differed according to type of occupation and region. This study suggest the need for further studies which analyse the quality of visiting health care providers and services, and the visiting health care system.

Prevalence, Cause of Insomnia and Drug Medication of Newly Admitted Patients to a University Hospital (일개(一個) 대학병원(大學病院) 초입원환자(初入院患者)의 불면증(不眠症) 유병률(有病率), 원인(原因) 및 약물처방(藥物處方)에 관한 연구(硏究))

  • The purpose of this study was to investigate the prevalence, primary causes, and management of insomnia newly admitted patients in a university hospital. Subjects consisted of 168 adult patients (95 men and 73 women, 88 medical and 94 surgical patients) newly admitted to Gyeongsang National University Hospital from September 7 through September 27, 1996. Sleep patterns of all subjects in the usual nights before admission(UN), the previous night to admission(PN), the night on admission(ON), and the 5th night after admission(5N) were investigated using the Korean version of the St. Mary's Hospital Sleep Questionaire. In addition, all insomnia patients and their doctors and nurses in charge were interviewed by psychiatric residents. Additionally, their medical records were reviewed. Prevalence of insomnia were 22.6% in the UN, 42.9% in PN, 51.8% in ON, and 43.5% in 5N. The prevalence of insomnia was significantly increased immediately before and after admission. There were no significant differences in the prevalence of insomnia by age and sex. The most ammon primary causes of insomnia were somatic symptoms and psychological factors in PN, somatic symptoms and noise in ON and 5n. Only 17 (10.1%) of insomnia patients took medicstions for insomnia control(analgesics in 15, hypnotics in 2). These results shorred that the prevalence of insomnia was significantly increased on hospitalization due to somatic symptoms, environmental factors, and psychological factors, but nearly none were adequately managed.
