Individual Doses to the Public after the Fukushima Nuclear Accident

  • Background: International organizations such as the World Health Organization (WHO) and the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) reported public exposure doses due to radionuclides released in the Fukushima nuclear accident a few years after the event. However, the reported doses were generally overestimated due to conservative assumptions such as a longer stay in deliberate areas designated for evacuation than the actual stay. After these reports had been published, more realistic dose values were reported by Japanese scientists. Materials and Methods: The present paper reviews those reports, including the most recently published articles; and summarizes estimated effective doses (external and internal) and issues related to their estimation. Results and Discussion: External dose estimation can be categorized as taking two approaches-estimation from ambient dose rate and peoples' behavior patterns-and measurements using personal dosimeters. The former approach was useful for estimating external doses in an early stage after the accident. The first 4-month doses were less than 2 mSv for most (94%) study subjects. Later on, individual doses came to be monitored by personal dosimeter measurements. On the basis of these measurements, the estimated median annual external dose was reported to be < 1 mSv in 2011 for 22 municipalities of Fukushima Prefecture. Internal dose estimation also can be categorized as taking two approaches: estimation from whole-body counting and estimation from monitoring of environmental samples such as radioactivity concentrations in food and drinking water. According to results by the former approach, committed effective dose due to 134Cs and 137Cs could be less than 0.1 mSv for most residents including those from evacuated areas. Conclusion: Realistic doses estimated by Japanese scientists indicated that the doses reported by WHO and UNSCEAR were generally overestimated. Average values for the first-year effective doses for residents in two affected areas (Namie Town and Iitate Village) were not likely to reach 10 mSv, the lower end of the doses estimated by WHO.

몬테카를로 방법을 이용한 중재방사선시술자에 대한 선량평가 (Assessment of Occupational Dose to the Staff of Interventional Radiology Using Monte Carlo Simulations)

  • 중재방사선을 이용한 의료적 시술이나 진단은 꾸준히 증가하고 있다. 특히 환자에 근접하여 이루어지는 중재방사선시술의 특성상 시술자에 대한 직무피폭의 관리 및 감시가 중요하다. 개인선량계를 통해 측정되는 방사선 방호 실용량인 심부선량은 중재방사선시술의 경우 균질한 방사선장에 의해 전신에 고르게 노출되는 경우가 아니므로 유효선량을 항상 대표할 수는 없다. 따라서 본 연구에서는 C-arm을 이용한 대표적인 중재방사선시술에 대해 수학적 모의피폭체와 몬테카를로 방법을 이용한 계산과 개인선량계를 이용한 실측을 통해 개인선량당량과 장기별 선량을 평가하고자 하였다. 주요 장기별 선량평가 결과는 개인선량계로 측정된 선량 값보다 낮았으나, 갑상선과 같은 장기는 전신 연조직 선량보다 상당히 높은 것으로 평가되었다. 중재방사선시술자에 대한 적절한 방사선방호를 위해 납치마의 착용과 같은 전신 방호와 더불어 갑상선 방호와 같은 추가적인 방호조치가 고려되어야 할 것이다.

컴퓨터 방사선영상에서 고 관전압 기법을 이용한 안구 입사표면선량 감소에 관한 연구 (A Study on Reducing of Entrance Surface Dose with the Eye in the Computed Radiography by Using High Kilo Voltage Peak Technique)

  • The purpose of this study was to minimize of entrance surface dose (ESD) at the eye using high kVp technique in the computed radiography. We used REX-650R (Listem, Korea) general X-ray unit, and external detector with ESD dosimeter of Piranha 657 (RTI Electronics, Sweden). We used head of the whole body phantom. The total 64 images of X-ray anterior-posterior of skull were acquired using the film/screen (F/S) method and the digital of computed radiography method. The three radiology professor of more 10 years of clinical career evaluated a X-rays images in the same space by 5-point scale. The external detector was performed measurement of ESD of three times by same condition on the eye of the head phantom. The good image quality in the F/S method (90 kVp, 2.5 mAs) showed at the minimized ESD of 0.310 ${\pm}$ 0.001 mGy. the good image quality in the computed radiography method (90 kVp, 2.0 mAs) showed at the minimized ESD of 0.180 ${\pm}$ 0.002 mGy (P = 0.002). Finally the radiation dose could reduced about 50% in the computed radiography method more than the F/S method. In addition the eye entrance surface dose using high kVp technique with the computed radiography was reduced 92% more than conventional technique (F/S method).

방사성 콜로이드를 이용한 감시림프절 생검 병리처리과정에서 방사선 피폭의 정량적 평가 (Quantitative Assessment of the Radiation Exposure during Pathologic Process in the Sentinel Iymph Node Biopsy using Radioactive Colloid)

  • 감시 림프절 생검은 유방암 수술에서 림프절 전이 상태를 알기 위한 표준 시술이다. 환자는 방사성 콜로이드를 주사 받은 후 수술을 받게 된다. 이 과정에서 검사를 하는 핵의학과, 수술장, 유방암 검체를 다루는 병리과의 관계자는 미량이나마 환자와 검체에 의해서 방사선 피폭을 받을 수 있다. 이 연구의 목적은 감시 림프절 생검 과정, 특히 병리처리 과정에서 받는 방사선피폭을 정량하여 그 안전성을 확인하고 병리 시설과 폐기물에 대해서도 방사선 관련 안전성을 확인하는 것이다. 대상 및 방법 : 감시림프절 생검은 방사성 콜로이드를 이용하여 일반적인 임상적 방법으로 시행되었다. 병리기사, 핵의학 기사 및 핵의학 의사의 피폭량을 열형광선량계를 이용하여 1달간 측정하였다. 또한 작업과정중의 잔존 방사능량, 흡수선량, 작업시간, 작업거리, 조직폐기물 및 병리검사실의 공간선량을 측정하였다. 결과 전신 및 손의 피폭량은 병리기사에서 각각 0.21 및 0.85 uSv/study이었고 핵의학과 의사 및 핵의학과 기사의 전신피폭량은 각각 0.2 및 2.3 uSv/study 이었다. 일반인 기준(1000 uSv/year)으로 병리기사는 년간 약 1100건 감시림프절 관련 검체 처리를 할 수 있었다. 각 과정의 잔존방사성 및 피폭거리, 시간으로 측정한 피폭량은 수술의사는 전신/손의 피폭량이 건당 2.47/22.4 uSv 이었고 수술장간호사는 건당 0.22/0 uSv 이었다. 병리실의 공간선량률은 0.02-0.03 mR/hr로 방사성 관리구역의 설정 기준에 도달하지 않았다. 폐기되는 검체 조직의 방사능은 거의 측정되지 않아 100 Bq/g에 훨씬 미치지 않았다. 결론: 방사성동위원소를 이용한 감시림프절 검사에 관계된 병리처리과정은 방사선안전측면에서 일반적으로 안전하며 별도의 안전관리나 시설 없이 이루어 질 수 있다.

X-선(線) 조사(照射)가 Ehrlich 암세포(癌細胞)의 용적(容積), 단백양(蛋白量) 및 수종(數種) Sulfhydryl 기(基)에 미치는 영향(影響)에 관(關)하여 (Effect of X-Irradiation on the Levels of some Sulfhydryl Groups, Protein and Cell Volume of Ehrlich Ascites Tumour Cells)

  • It is well known that a number of -SH and -SS containing substances afford a certain measure of protection against radiation effects in many biological systems, and it is conceivable that inherent -SH levels in Ehrlich ascites tumour (ELD)cells may be of decisive improtance with respect to the development of cellular radiation injury. So far, little effort has been directed to elucidate the changes in levels of different -SH and -SS groups in ELD cells when the tumour-bearing whole animal was subjected to the sublethal dose of X-irradiation. The present study was designed to bring some lights in the possible changes of and relationship between various sulfhydryl levels, such as P-SH, NP-SH and NP-SS, as well as the content of protein and cell volume of ELD cells, after subjecting the ELD mice to 1,200 r of X-irradiation. The animals used in this experiment were all mixed bred mice of $20{\sim}25\;gm$ in body weight (approximately 2 months old) irrespective of sex. 12 mice in one experiment were inoculated intraperitoneally with 0.2 ml of ascites tumour cells $(2{\times}10^6\;cells)$, and on the 7th day of the tumour growth, they were X-irradiated with 1,200 r, using the conventional X-ray machine under the following conditions: 200 Kv at 15 mA, 0.5 mm Cu filter, target-skin distance: 50 cm. Radiation dose was measured with the the Philip integrating dosimeter. At 24, 36, 48 and 60 hours after the X-irradiation, the mice were killed by cervical dislocation, and the tumours were taken out. Freshly withdrawn ascites tumours were placed in ice, and immediately the cell concentration was measured with the Coulter Cell Counter (Model B), and the hematocrit of the tumour cells were also determined. Cell volume was thus calculated by the cell concentration and hematocrit value. P-SH content of ELD cells was measured potentiometrically according to the method of Calcutt & Doxey, and NP-SH and NP-SS contents were measured spectrophotometrically by the method described by Ellman. Protein content of ELD cells was determined with the Folin phenol reagent by Lowry et al. Altogether, 48 experimental mice were used, and 12 mice with the only exception of X-irradiation were used as the control. Results obtained indicate that the contents of all the cellular sulfhydryl groups as well as cell volume and protein content of the ELD cells increase significantly as time progresses after the sub-lethal X-ray dose of 1,200 r was given and that all the increase is in a lineal fashion. The regression lines of the relative values, (i. e., taking each control value as 1) of all the values obtained, and the regression lines of cell volume, protein and NP-SH are identical, whereas those of NP-SS and P-SH appear to be widely seperated. However, the difference of those two lines (NP-SS & P-SH) were found to be not significant statistically (p>0.05). Therefore, it can be concluded from the above results that all the values examined increase in a lineal fashion with no statistically significant difference among them. Also, with the radiation dose of 1,200 r, the ELD cell becomes enlarged and swollen progressively up to 60 hours post-irradiation and it becomes more than two times of the original normal size at 60 hours after the irradiation, and up to this stage, it seems apparent that the cell division has been slow due to the X-irradiation applied in this experiment. It is well understandable that the contents of NP-SH, NP-SS, P-SH and protein of the ELD cells increase in parallel with the increase of the cell volume by the X-ray does used, but it also seems interesting to note that all the cellular substances tested show no appreciable difference in the pattern of increase.
