뇌실주위 배아종 - 증례보고 - (Periventricular Germinoma - Case Report -)

  • Germinomas of the central nervous system are rare embryonal tumors(accounting for less than 1% of intracranial neoplasms) that may be located in the pineal region, in the floor of the third ventricle, or in the suprasellar area. We report a case of germinoma developed in periventricular deep white matter without pineal region tumors or suprasellar masses. The 19-year-old male patient presented with slowly progressing headache, dizziness, photophobia, and dysarthria. Initial brain MRI revealed a irregular and dense enhancement from lateral ventricles to 4th ventricle. The stereotactic biopsy of tumor and histologic examination revealed the germinoma. Craniospinal axis radiation therapy was performed. After radiation therapy patient was improved and no neurologic sequelae was seen at discharge. Periventricular germinomas without pineal or suprasellar lesion are very rare. The radiation therapy, as in our case, is beneficial as with other intracranial germinomas. Stereotactic biopsy of periventricular germinoma provides precise pathologic diagnosis and thus allows more specific management.

  • Progressive multifocal leukoencephalopathy(PML) is a fatal demyelinating disease that occurs in immunocompromised hosts. We report a case of PML that developed in patient with T cell lymphoma of head and neck. During chemotherapy for lymphoma, she was confused and had memory impairment. A magnetic resonance imaging of the brain revealed confluent signal change at white matter of the frontal lobe, insula, and anterior limb and genu of internal capsule. The lesion was confirmed with brain biopsy and the histopathological finding was compatible with PML.

  • Primary subacute pyogenic osteomyelitis, or Brodie's abscess has received much attention since its initial documentation in the literature in 1832 by Sir Benjamin Brodie. Brodie's abscess is a localized form of chronic osteomyelitis that occurs most often in the metaphyseal area of the long bones of the lower extremities of young adults, Intermittent pain of long duration is the presenting complaint, along with local tenderness over the affected area. Laboratory evaluation is unrevealing, with a normal white blood cell count and differential count. The erythrocyte sedimentation rate may also be normal. Roentgenogram shows a markedly varied appearance and an abscess may be easily mistaken for various neoplasm. The most common organism cultured from abscess is Staphylococcus species. Treatment includes curettage of the lesion and administration of antibiotics. We present a case report (with a 1-year follow-up period), demonstrating the successful surgical treatment of Brodie's abscess of the distal metaphysis of the left tibia caused by Salmonella cholerasuis in a 33-year-0ld male who had no hemoglobinopathy.

Isolation and Identification of Colletorichum musae from Imported Bananas

  • Colletotrichum musae was isolated from dark-brown anthracnose lesions on commercial banana (Musa sapientum L.) to establish the causal agent of the symptom. The fungus grew fast and produced white aerial mycelium on PDA. Acervuli developed abundantly on culture plates after incubation for 10 days at $25^{\circ}C$. Pinkish conidial masses were produced on the acervuli, which mostly coalesced together, Conidia were aseptate, hyaline, straight, ellipsoid to globose, and 14.5$\times$6.9 $\mu\textrm{m}$ in size. Black, clavate, round, or irregular-shaped appressoria measuring 8.8$\times$6.8 $\mu\textrm{m}$ were readily formed from germ tubes. Setae-like structures were not found either on the lesion or on the cultures. Sclerotia were also absent. Among the media, PDA medium was the best for mycelial growth. The optimum temperature for mycelial growth was $28^{\circ}C$, while the optimum pH ranged from pH 5.5 to 6.5. The isolates of C musae caused black necrotic lesions on banana fruits by needle-wound inoculation, and orange-colored spore masses were produced on the lesions. The fungus also caused discoloration on apple fruits inoculated.

  • A sclerotinia rot of Leonurus sibiricus caused by Sclerotinia sclerotio겨m occurred severely in farmer's fields at Seosang-myon, Hamyang-gun, Gyeongnam Province, Korea in 2001. The typical symptoms of the disease were that leaves, stems or collar of the infected plants at first became darker green and then watery soft rotted, and white fluffy mycelia formed on the lesion developed into black sclerotia later, Sclerotia on the plant and PDA medium were globose to cylindrical or irregular in shape, and 0.8~10.3$\times$0.8~6.4 m in size. Aphothecia with numerous asci were cup-shape, and 0.6~1.2 cm in diameter, Asci with 8 spores were cylindrical, and 87~246$\times$4.6~21.6 $\mu\textrm{m}$ in size. Ascospores of one cell were hyaline, ellipsoid to ovoid in shape, and 10.2~14.6$\times$4.7~7.2 $\mu\textrm{m}$ in size. The range of temperature for mycelial growth was from 5 to 3$0^{\circ}C$, and the optimum was $25^{\circ}C$. This is the first report on the sclerotinia rot of L. sibiricus caused by Sclerotinia sclerotioum in Korea.

Evaluation of Watermelon Germplasm for Resistance to Phytophthora Blight Caused by Phytophthora capsici

  • This study was conducted to determine the Phytophthora rot resistance of 514 accessions of watermelon germplasm, Citrullus lanatus var lanatus. About 46% of the 514 accessions tested were collections from Uzbekistan, Turkey, China, U.S.A., and Ukraine. Phytophthora capsici was inoculated to 45-day-old watermelon seedlings by drenching with 5 ml of sporangial suspension ($10^6$ sporangia/ml). At 7 days after inoculation, 21 accessions showed no disease symptoms while 291 accessions of susceptible watermelon germplasm showed more than 60.1% disease severity. A total of 510 accessions of watermelon germplasm showed significant disease symptoms and were rated as susceptible to highly susceptible 35 days after inoculation. The highly susceptible watermelon germplasm exhibited white fungal hyphae on the lesion or damping off with water-soaked and browning symptoms. One accession (IT032840) showed moderate resistance and two accessions (IT185446 and IT187904) were resistant to P. capsici. Results suggest that these two resistant germplasm can be used as a rootstock and as a source of resistance in breeding resistant watermelon varieties against Phytophthora.

백비심에서 자연 발생한 신장의 칸디다증 (Renal Candidiasis in a Masked Palm civet (Paguma larvata))

  • A case of renal candidiasis is reported in a 4-year-old male Masked Palm civet (Paguma larvata) On necropsy, the kidneys were bilaterally swollen, pale and had numerous 1 to 3 mm diameter white foci throughout the parenchyma on cut section. The urinary bladder was filled with opaque and milky exudate. Histologically, severe infiltration of neutrophils and macrophages and necrosis were noted in the interstitial areas of both cortex and medulla and in the lumens of renal tubules and collecting duct often resulting in cystic dilation of the tubules. PAS-positive fungal yeasts or pseudohyphae were often associated with the lesion. Candida albicans was isolated from the kidney and urinary bladder.

Monocrotaline을 투여한 백서 폐의 초기 혈관병변에 관한 주사전자현미경적 관찰 (Scanning Electron Microscopic Observation on Early Vascular Lesion in Rat Lung Administered with Monocrotaline)

  • An experimental study was performed to observe the early effects of monocrotaline on pulmonary vascular system by means of light microscopy and scanning electron microscopy, attempting to expore the mechanism behind the process of pulmonary hypertension. Experimental animal(Sprague-Dawley male rats ; 150-200g B. W.) were intra-peritoneal administered with 100mg/kg B. W. monocrotaline. Authors observed light microscopically various gradational increase of wall thickness in pulmonary muscular and non-muscular arteries in duration from 2 weeks to 5 weeks after monocrotaline administration and the changes were more sever in the latter than the former. The scanning electron microscopy shows severe and diffuse endothelical cell swelling, microvilli and microbleb formation since 1 hour after monocrotaline administration and during the course, after 5 hours the severity of endothelial cell damage was prominent with presence of fibrin, webs, platelet thrombi and white cell adherence. It was concluded that the monocrotaline primarily induced severe and diffuse endothelial cell damage of pulmonary arteries and laterly added the participation of platelets, which attributed to the pathogenesis of monocrotaline induced pulmonary vascular lesions in relation to pulmonary hypertension.

인두암과 식도암의 새로운 진단내시경 (New Diagnostic Techniques in Cancer of the Pharynx and Esophagus)

  • The diagnosis and treatment of early gastrointestinal cancers is the gastroenterologists' mission because of national cancer screening program in South Korea. The detection of early cancers is emphasized, because these were previously treated with surgical treatment can be currently cured with endoscopic treatment. Gastroenterologists who achieved at least on some level can make an exact diagnosis regardless of what type of endoscopy, but generally, there are some required conditions for an optimal diagnosis. First, clinically important lesions have to be detected easily. Second, the border and morphology of lesions have to be characterized easily. Third, lesions have to be diagnosed exactly. Precancers and early cancers are often subtle and can pose a challenge to gastroenterologists to visualize using standard white light endoscopy. The use of dye solutions aids the diagnosis of early gastrointestinal cancers, however, it is a quite cumbersome to use dye solutions all the time and the solution often bothers the exact observation by pooling into the depression or ulceration of the lesion. To overcome this weakness, newer endoscopes are now developed so called "image enhanced endoscopy" using optical and/or electronic methods such as narrow band imaging (NBI), autofluorescence imaging (AFI), i-scan, flexible spectral imaging color enhancement (FICE) and confocal endomicroscopy (CLE).

Brain Abscesses Associated with Asymptomatic Pulmonary Arteriovenous Fistulas

  • Brain abscess commonly occurs secondary to an adjacent infection (mostly in the middle ear or paranasal sinuses) or due to hematogenous spread from a distant infection or trauma. Pulmonary arteriovenous fistulas (AVFs) are abnormal direct communications between the pulmonary artery and vein. We present two cases of brain abscess associated with asymptomatic pulmonary AVF. A 65-year-old woman was admitted with a headache and cognitive impairment that aggravated 10 days prior. An magnetic resonance (MR) imaging revealed a brain abscess with severe edema in the right frontal lobe. We performed a craniotomy and abscess removal. Bacteriological culture proved negative. Her chest computed tomography (CT) showed multiple AVFs. Therapeutic embolization of multiple pulmonary AVFs was performed and antibiotics were administered for 8 weeks. A 45-year-old woman presented with a 7-day history of progressive left hemiparesis. She had no remarkable past medical history or family history. On admission, blood examination showed a white blood cell count of 6290 cells/uL and a high sensitive C-reactive protein of 2.62 mg/L. CT and MR imaging with MR spectroscopy revealed an enhancing lesion involving the right motor and sensory cortex with marked perilesional edema that suggested a brain abscess. A chest CT revealed a pulmonary AVF in the right upper lung. The pulmonary AVF was obliterated with embolization. There needs to consider pulmonary AVF as an etiology of cerebral abscess when routine investigations fail to detect a source.
