동서의 피부 질환 명칭에 대한 소고 (A Study of the Term 'Dermatology' in Oriental Medicine)

  • Objectives: In order to establish a base for proper treatment and management of patients with dermal problems through correct diagnosis, I considered the naming rule for dermatology in Oriental Medicine, referring to the dermatology literature compared to western medicine. In addition, this paper examines the characteristic classification of dermatology. Methods: I examined the naming rule of dermatology in Oriental Medicine and then compared the disease names in Oriental and Western medicine and the characteristic classification of dermatology referred to the records. Results: The dermal diseases have been named according to their colors and morphologies, causes, progress of symptoms, recurrent sites, the character of distribution, recurrent seasons, ages, the character of patients' jobs and locations. Sometimes some have been named by referring to their main morphologies, sites, causes, colors and seasons synthetically. However it was found some names for dermal diseases, even though the same diseases, had been named differently according to for example: historical times, condition of locations and the quality of doctors whose process of naming developed and changed over time. The relationship between Oriental and Western medicine of each name for dermal diseases is basically divided into 5 types: same names - same diseases; same names but different diseases; same diseases but different names; one disease with multiple names; and one name with multiple diseases. Considering the methods of classification, these were generally achieved according to their places of origin. It is a method unique to Oriental medicine that we classified some dermal diseases into 疥, 癬, 瘡, 風, 丹, 疱, 疹, 癰, 痘, 疽 and so on and it is very easy to diagnose which part they belong to. This was classified by putting first the causes of diseases; for instance: viruses, bacteria, fungi. Sometimes, however there was a problem, connected to the classification of morphology. Conclusions: I suggest that we need to unify and refine dermatological terms in Oriental Medicine in order to establish a base for proper treatment and management of patients with dermal problems through correct diagnoses.

폐주기(肺主氣)에 관한 고찰 (Study on the Pye(Fei)/Lungs Control Ki(Qi))

  • The Pye is one of the organs in oriental medicine, which has functions of controlling Ki. The Lungs is also a organ that has a function of respiration, in western medicne. Ki means the air in the first notion. Therefore, the functions of the Lungs and Pye are no differences. However, Ki doesn't mean the air only. Respiration in western medicne has two meanings. One of them is a external respiration, ang the other is a cellular respiration. In that process, the Lungs supervise oxygen and oxidation of protein, carbohydrate and fat and prduce ATP as energy. Two of Functions in Ki are Choodong(推動;Tuidong) and Onhoo(溫煦;Wenxu). Choodong means making energy ang Onhoo means making heat, that indicate a similarity functions of Pye and Lungs from the view of making energy.

  • Review of literature on languor and fatigue. According to comparative studies of Oriental and Western medical literature on languor and fatigue of treatment, following results were obtained. 1. Treatment of languor and fatigue in Western medicine were firstly treatment of an organic disease, secondly inhalation of massage, bathing, sleeping, balance 02 of a motion, dosage, supply of a nutrition and the others. 2. On the treatment of languor and fatigue in oriental medicine, the methods of treatment were an warming and supplement the vital energy, tranquilization and nourishment, and the prescriptions were Samultang, Sagunja tang, palmultang, Yoogkunjatang, Gongjindan, SSanghwatang, Dogsamtang, yoogmijihwangwon, sungyangikgitang, Samchulgeonbitang and the others.

뇌졸중 후 우울증 : 한방 치료에 대한 국내외 최신 문헌 고찰 (Poststroke Depression : A Review of the Latest Oriental Medicine Articles)

  • Objectives : This study reviews the latest articles in Korea and other countries that studied oriental medicine treatment on poststroke depression. Methods : Korean articles were retrieved from the 9 major Korean web article search engines. Foreign articles were retrieved from PubMed. Article published date was from 2000 up to September 2012. There were no restrictions on the types of publication, but articles not available in full text were excluded. The methodological quality was assessed according to Cochrane's assessment of risk of bias and Newcastle-Ottawa quality assessment scale. Results : Twenty-two articles were included in this study. Eleven articles were published in Korea, the rest were published in China. Nine articles were randomized controlled trials (RCT), one article was a non-randomized study (NRS), four articles were case reports, three articles were cross-sectional studies, two articles were comparative studies. In RCT articles, risk of selection bias and performance bias were generally high, risk of detection bias was unclear. The NRS article took four stars in Newcastle-Ottawa quality assessment. Comparison Hamilton rating scale for depression score between oriental medicine treated group and western medicine treated group revealed that there was no remarkable difference in mean score changes after treatment on PSD. Conclusions : The results of this study suggest that oriental medicine treatment is as effective as western medicine treatment for PSD. In the future, more rigorous oriental medicine treatment studies should be conducted.

양.한방 물리치료의 차이점 연구 (Comparative Study of Physical Therapy between the Oriental and the Western Medicine)

  • The objective of this study was to investigate characteristics in physical therapy according to the oriental and the western medicine. Questionnaires were referred to 101 chiefs of physical therapy departments of 66 hospitals of western medicine and 35 hospitals of oriental medicine. The results were as follows ; 1) For therapeutic members, significant indicators related to difference of the two groups were number of therapist, kind of therapist, programmer of physical therapy and referer to physical therapy. 2) For therapeutic environment, the size of therapeutic room and the respective department. 3) The two groups regarding whether the treatment was carried out or not in 14 cases of treatment (42.4 % ), and whether the cost of treatment was requested by medical insurance or not in 23 cases of treatment (70.0%).

  • Nowadays we can take a physical examination during pregnancy by various and advanced technology instruments. But there were no instruments in old times, so the ancient doctors examined symptoms and pulse of a pregnant woman. Therefore it is very important to comprehend the pulse in Oriental medicine. Fetus and mother are connected with the umbilical cord, and they interact with each other. Especially, symptoms and pulse of mother change during the formation of fetus. On account of being easy to study the changes of pulse from pregnant woman, it is important to know the formation of fetus. In Western medicine, the Buddhist scriptures and Oriental medicine mentioned about the formation of fetus. Because of different viewpoints, the choice of words and formative methods are differ from field to field. Western medicine calls the formation of fetus 'development', the Buddhist scriptures call it 'Ru-tai', and oriental medicine calls it 'Yang-tai'. In spite of the different viewpoints, they have supplementary explanations to study fetus. As a result, we can understand the reasons why pregnancy pulse cannot help changing from month to month.

수근관증후군 환자에 대한 한방과 양방치료효과의 비교 (Comparison of Therapeutic Effect on Carpal Tunnel Syndrome between Oriental and Western Medicine)

  • Objectives : This study was to assess the effectiveness of Oriental medical treatment on carpal tunnel syndrome, comparing its outcome with that of Western medical treatment. Methods : After being diagnosed with carpal tunnel syndrome by electromyography, subjects were enrolled in Kyung Hee Medical Center from March 2006 to January 2007. We prescribed Kejibokryung-hwan to the Oriental medical therapy group (OM group) and NSAIDS to the Western medical therapy group (WM group). Effectiveness was assessed by degree of pain using visual analog scale (VAS) before and after 3 weeks' treatment. Adverse effects were also monitored. Results : There were 21 patients in the OM group and 19 in the WM group. No statistical significant difference was detected at the baseline assessment. After 3 weeks of medication, pain was reduced about 26% in the OM group and 46% in the WM group. These findings might be explained by that more than half of the WM group received local steroid injection, which has been known to have more rapid analgesic effect that oral medication. Although pain reduction rate was higher in the WM group than in the OM group, we suggest that Oriental medical treatment is still effective, faking into consideration the fact that completely recovered cases were found only in the OM group. No adverse effect was found in either of the groups. Conclusion : This work could help us to understand the effectiveness of Oriental medical treatment on carpal tunnel syndrome.

조헌영의 『동양의학총서(東洋醫學叢書)』에 대한 의사학적 연구 (A Historical Study on Cho Heon-yeong's 『Eastern Medicine Series (東洋醫學叢書)』)

  • This study is about Cho Heon-yeong's 『Eastern Medicine Series (東洋醫學叢書)』. It consists of 5 clinical books, 『Therapy for People (民衆醫術理療法)』, 『Tuberculosis Eastern Medical Treatment (肺病漢方治療法)』, 『Neurasthenia Treatment (神經衰弱症治療法)』, 『Gastrointestinal Disease Treatment (胃腸病治療法)』 and 『Gynecological Treatment (婦人病治療法)』, which were published between 1935-1941. This series mainly succeeded 『Donguibogam (東醫寶鑑)』, and was influenced by 『Kyeongakjeonseo (景岳全書)』, and 『Hwangjenegyeong (黃帝內經)』. Cho Heon-yeong's medical philosophy appears in two ways. First, he emphasized invigoration in treatment for the people who lacked nutrition and medical care at the time. Second, eclecticism of Korean Medicine and Western medicine is specifically revealed through this series. He aimed for a comprehensive medicine that consists mainly of Korean medicine and includes only a part of Western medicine.

위장관 질환의 연구전략수립을 위한 최근 5년간 한의약논문의 체계적 고찰 (Systemic Analysis for Research Strategy Establishment of Gastrointestinal Disease with Oriental Medicine in Korea)

  • To research the trends of study and the guidelines of investigation and management about gastrointestinal (GI) disease in oriental medicine, we reviewed and analysed articles published in Korea from 2004 to 2009. The western medicine in medical treatments of GI tract is notorious for their side effects. But, in oriental medicine, we couldn't find any side effects. Although we treat or manage clinical practices of the GI disease every day, however, we do not have abundant evidences about scientific mechanisms of those treated with oriental medicine therapy. Therefore, we suggest that the upward of qualities of life and the improvement of health-care system through the development of oriental medicine therapies in Korea will be need.

한방치료에 따른 천식환자의 삶의 질에 대한 임상적 평가 (Clinical Study of Quality-of-Life in Asthmatic Patients managed by Oriental Medical Treatments)

  • Objective : Asthma is a lung disorder characterized by periodic attacks of wheezing alternating with periods of relatively normal breathing. And quality of life in asthmatic patients shows a tendency to decrease. So we had a clinical study whether if oriental medical treatment could improve their quality of life or not. Methods : The 15 cases were new patients that visited at division of respiratory system, department of internal medicine in Kyunghee medical center from the 1 st of July, 2000 to the 1 st of August. All of them were diagnosed as asthma and treated by western medical therapy. We used a Quality of Life Questionnaire for adult Korean Asthmatics(QLQAKA) and evaluated the results after two times answers, at an interval of two weeks. Results : The mean of the first answer was 2.84 marks and 3.36 ones after two weeks. If there is a difference of over 0.5 points between the two, we can admit significant effect. After the asthmatic patients treated by oriental medical therapy, they gained a difference of 0.52 points, 10.4%. Conclusions : We suggest that oriental medical therapy can improve the quality of life of asthmatic patients arid we can alternate side effects of western medical therapy with oriental one. if they carry out two different treatments at the same time.
