한국형 호스피스 케어 개발을 위한 기초 조사 연구 (The National Hospice Care Service Development in Korea)

  • The urgent needs to establish hospice care systems in Korea arise from the following reasons: 0) a drastic increase in chronically ill patients with the increase of aged population: (2) rapid changes in living environment from the traditional habitation (e. g., Many Koreans living in apartment complexes, which is the most popular form of modern residence in recent years, prefer to die in the hospital.): the overall increase in patients with advanced cancer: (4) recent trends in early discharge of terminally ill patients from the limited hospital facilities to accomodate other medical insurance beneficiaries; (5) easy acceptance of euthanasia owing to the recent social atmosphere that belittles the dignity of human life; (6) medical and nursing care of AIDS patient in terminal stage; (7) and the problem associated with inhumane medical care system, overtreatment, and groundless fears against narcotics. Terminally ill patients were used to be treated in the hospital in the past. In these days, however, they are forced to have home cares with little assistance from the qualified medical personnel because of insufficient hospital facilities, which are even short for the need of emergency patients and provide priority cares to medical insurance beneficiaries with other acute problems. And yet, neither are there any administrative organizations nor systematic medical studies that deal with the level of terminally ill patient's need, their family's problems and resources of hospice care systems in Korea. Thus, most patients are not able to get appropriate medical care at the terminal stage of their lives. The objective of this study is to make comprehensive database for various hospice care organization currently in operation, link them through medical information system, and develop an easily accessible hospice care model that meets the need of most Korean people. Our survey results may be summarized as follows: Nationally there are 40 organizations that provide partial or full hospice care. However, these organizations are not linked to any formal medical service network. Furthermore, the objective of hospice care, care principles, personnel with appropriate training, educational programs, standard for care, costs, consulting service to patients' family members, the extent of medical care from professional staff members, status of hospice facility, and management of those institutions are neither clearly defined nor organized compared to the international hospice care standards. The surveys on patients of terminal stage. grouped in hospice and non-hospice care patients. reveal what they want visiting nursing care to help their pain control. psychological. social and spiritual demands. While the more than 90% of hospice care patients want to reduce their pains. the non-hospice care patients. in addition to their desire for pain control. demanded more psychological. social and spiritual helps as well. The results of this research could be utilized to 0) define the standard of hospice care. (2) provide the guidance for hospice medical care costs. (3) establish the database of hospice care systems. (4) develop softwares. (5) build communication network through Medinet. and (6) provide an organized visiting home nursing care system. These information should be a valuable resource to many medical staffs who are involved in cancer therapy. nursing care. and social welfare programs.

건강타운 이용노인의 삶의 질 변화 결정요인 (Factors Determining the Quality of Life of Elders Using the Health Town)

  • 이 연구는 지역사회 노인 여가시설의 이용에 따른 삶의 질 변화에 영향을 미치는 요인들을 실증적으로 분석하기 위한 것이다. 이을 위해 광주광역시 노인건강타운을 이용하는 노인들을 대상으로 건강타운의 시설환경, 직원과 강사의 역량, 프로그램 만족도 등의 요인이 이용노인들의 삶의 질 변화에 영향을 미치는지를 규명하여, 이용노인들의 삶의질 변화에 기여할수 있는 다각적이고 실천적 대안을 제공하고자 하였다. 조사방법은 광주광역시 노인건강타운을 이용하는 만60세 이상의 1,038명을 대상으로 설문지에 의한 자기기입방식과 조사원의 면접조사를 병행하여 실시하였다. 조사기간은2010년 12월 10일부터 12월15일까지였다. 분석결과, 노인들의 삶의 질 변화에 영향을 미치는 변인으로 학력(B=-.074, p<0.05), 강사만족도(B=.172, p<0.001), 직원만족도(B=.104, p<0.01), 시설 및 환경만족도(B=.217, p<0.001)였다. 즉, 학력이 낮은 노인일수록, 강사만족도가 높을수록, 직원만족도가 높을수록, 시설 및 환경만족도가 높을수록 삶의 질 변화에 정(+)적인 영향을 미치는 것으로 분석되었다. 특히, 시설 및 환경만족도의 영향력이 상대적으로 높게 나타나 노인들의 삶의 질 변화를 위해서는 지역사회 노인이용 문화여가시설의 현대화와 쾌적한 환경조성이 병행되어야 함을 시사하고 있다.

율동적 동작(Dance movement) 훈련이 노년기 여성의 생리, 심리적 변수에 미치는 영향 (Effect of Korean Traditional Dance Movement Training on Psychophysiological Variables in Korean Elderly Women)

  • Regular long term dance movement could be one of ways to induce improvement of psychophysiological variables, resulting in improvement of quality of life. However, there have been few studies to evaluate the effect of dance movement training on both physiological and psychological variables in the elderly. This study was focused to determine the effect of Korean traditional dance movement training on psychophysiological variables-body weight, body fat, lean body mass, muscle strength, muscle endurance, flexibility, agility, resting heart rate and blood pressure, depression and life satisfaction-in Korean elderly women. Thirty four subjects, aged between 65 and 75years who have normal cognition, sensory function, cerebellum function, cardiovascular function, participated in this study. Seventeen experimental group subjects were selected from E-elderly university in Kyung Gi province, and Seventeen control group subjects were selected from N-welfare facility in Seoul City. Seventeen experimental group subjects participated for 12weeks dance movement program. Korean traditional dance movement program was developed on the basis of Korean traditional dance and music by the author. The program consisted of approximately 50minutes of dance, 3times a week for 12weeks. During 50minutes workout, there were 15minutes of warm-up dancing, 25minutes of conditioning dance and 10minutes of cool-down dancing. The intensity for the conditioning phase was at between 60% and 65% of age-adjusted maximum heart rates. The body weight, body fat, lean body mass, muscle strength(grip strength, leg strength), muscle endurance, flexibility, agility, resting heart rate and blood pressure, depression and life satis-faction were measured prior to and following the experimental treatment. The participants in dance movement were interviewed focusing on subjective feeling following 12 week's regular dance movement. Data were analyzed with mean, standard deviation, percentage of change, X²-test, t-test, and ANCOVA test using SPSS PC/sup +/ program. Subjective feeling was categorized into cognitopsy-chological and physiological responses. Results were obtained as follows : 1) The body weight (F=15.52, p=.000), body fat (F=18.33, p=.000) and lean body mass (F=7.28, p=.011) of the experimental group were significantly lower than those of the control group following the dance movement training. 2) The leg strength (F=30.96, p=.000), muscle endurance (F=9.06, p=.005), agility(F=44.92, 000), flexibility(F=6.84, p=.014) of the experimental group were significantly higher than those of the control group following the dance movement training. There was no significant difference of grip strength(F=.43, p=.515) between experimental and control groups. 3) The heart rate(F=26.96, p=.000), systolic (F=10.40, p=.000) and diastolic(F=3.99, p=.005) blood pressure at rest of the experimental group were significantly lower than those of the control group following the dance movement training. 4) No significant difference of score of depression (F=3.49, p=.071) was observed between experimental and control groups. 5) Score of life satisfaction of experimental group was remarkably higher than that of control group following 12weeks of dance movement training (p<0.05). 6) Thematic responses about the dance movement following the training were positive. "I feel good" was the most frequent among cognitopsychological responses and "I feel lightness of body" was the most frequent among physiological responses. The results suggest that Korean traditional dance movement training can improve psychophysiological variables of Korean elderly.

일 도시 노인복지시설 이용자들의 불면증 및 우울증에 대한 연구 (A Study of Insomnia and Depression of Elderly Welfare Facility Users in a City)

  • 연구목적 본 연구는 일지역사회의 노인 불면 및 우울 증상과 인구사회학적 변수들과의 상관관계에 대해 분석하고, 이들 변수들이 우울증에 미치는 영향을 확인하고자 하였다. 방 법 광명시에 거주 중인 60세 이상의 노인을 대상으로 불면 및 우울 정도를 평가하고 그와 관련되는 인구사회학적 요인에 대하여 조사하였다. 2016년 4월 20일부터 2016년 12월 1일까지의 기간 동안 ${\bigcirc}{\bigcirc}{\bigcirc}$병원 정신건강의학과 위탁 광명시 정신건강증진센터에서 광명시에 위치한 복지관과 경로당을 방문하여 경기도 광명시에 거주하는 노인 총 837명을 대상으로 설문조사와 면담을 시행하였다. 인구사회학적 항목과 불면 증상, 우울 증상의 연관성을 살펴보기 위해 ISI와 S-GDS를 사용하여 구조화된 면담을 시행하였다. 결 과 연령(70대 미만, 70대, 80대, 90대 이상), 의료보장형태(건강보험, 의료보호), 주거 형태(자가, 비자가), 결혼 상태(미혼, 기혼, 이혼 및 사별)에 따른 S-GDS 평균 점수에서 유의한 차이가 나타났다. 학력에 대해서는 초졸 또는 무학인 경우와 대졸 이상인 경우를 비교했을 때 평균에 유의한 차이가 나타났다(F=3.227, p=0.012). S-GDS 10점 이상과 미만군으로 나누어 인구사회학적 인자들을 비교한 결과, 연령, 의료보장형태, 주거 형태, 동거 가족의 수에서 유의한 차이가 나타났다(p<0.05). ISI로 측정한 불면 증상은 인구사회학적 항목과는 유의한 관련성이 나타나지 않았으나(p>0.05), 우울 증상과는 유의한 연관성이 나타났다(p<0.05). 불면 증상이 심할수록 우울 증상이 나타날 확률이 더 높았으며 또한 우울 증상을 보이는 노인들의 경우 더 심한 불면 증상을 갖는 것으로 나타났다. 결 론 노인 우울증은 인구사회학적 요인 중 연령, 의료보장형태, 주거 형태, 결혼 상태, 학력 및 동거 가족의 수와 유의한 연관성을 갖는다. 또한 노인 우울증의 주요 증상인 불면 증상은 우울 증상의 심각도와 진단을 예측할 수 있는 중요한 의미를 갖는 것으로 나타났다.

"삼국사기(三國史記)"에 기록된 의약내용(醫藥內容) 분석 (Study on Medical Records In ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$)

  • We tried to observe the features of ancient medical practice by analysing the records related to medicine in the book, ${\ulcorner}$the Historical Records of the Three Kingdom${\lrcorner}$ of which content includes the features of medicine in mythology, plague, delivery of twins, drugs, medical system, shamanism, constitutional medicine, psychiatry, forensic medicine, deformity, a spa, medical phrase, health and welfare work, religion, death. physiological anatomy, Taoist medicine, acupuncture, the occult af of transformation and etc. Our initial concern was about where to draw line as of medical field and we defined medicine in more broad meaning. The book ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$ describes the world of mythology by way of medicine which is not clearly a conventional one. There appears records of birth of multiple offsprings 7 times in which cases are of triplets or more. Delivering multiple offsprings were rare phenomenon though such fertility was highly admired. This shows one aspect of ancient country having more population meant more power of the nation. Of those medical records conveyed in that book includes stories of childbirth such as giving birth to a son after praying, giving birth to Kim Yoo-shin after 20 months after mother's dream of conception, and a song longing for getting a laudable child. Plagues were prevalent throughout winter to spring season and one can observe various symptoms of plagues in the record. Of these epidemic diseases, cold type might have been more common than the heat one. Appearance of epidemic diseases frequently coincided with that of natural disasters that this suggests a linkage between plague and underlying doctrine on five elements' motion and six kinds of natural factors. There exists only a few names of diseases such as epidemic disease, wind disease, and syndrome characterized by dyspnea. Otherwise there appeared only afflictions that were not specified therefore it remains cluless to keep track of certain diseases of prevalence. Since this ${\ulcorner}$Historical Records of the Three Kingdoms'${\lrcorner}$ wasn't any sort of medical book, words and terms used were not technical kind and most were the ones used generally among lay people. Therefore any mechanisms of the diseases were hardly mentioned. Some of medicinal substances such as Calculus Bovis, Radix Ginseng, Gaboderma Luciderm, magnetitum were also in use in those days. 53 kinds of dietary supplies appears in the records and some of these might have been used as medicinal purpose. Records concerning dicipline of one's body includes activities such as hunting, archery, horseback riding etc. In Shilla dynasty there were positions such as professor of medicine, Naekongbong(內供奉), Kongbong's doctor(供奉醫師), Kongbong's diviner(供奉卜師). As an educational facility, medical school was built at the first year of King Hyoso's reign and it's curricula included various subjects as ${\ulcorner}$Shin Nong's Herbal classic${\lrcorner}$, ${\ulcorner}$Kabeul classic of acupuncture and moxbustion${\lrcorner}$, ${\ulcorner}$The Plain Questions of the Yellow Emperor's Classic of Internal Medicine${\lrcorner}$, ${\ulcorner}$Classic of Acupuncturer${\lrcorner}$, ${\ulcorner}$The Pulse Classic${\lrcorner}$, ${\ulcorner}$Classic of Channels and Acupuncture Points${\lrcorner}$ and ${\ulcorner}$Difficult Classic${\lrcorner}$. There were 2 medical professors who were in charge of education. To establish pharmacopoeia, 2 Shaji(舍知), 6 Sha(史), 2 Jongshaji(從舍知) were appointed. In Baekje dynasty, Department of Herb was maintained. Doing praying for the sake of health, doing phrenology also can be extended to medical arena. Those who survived over 100 years of age appear 3 times in the record, while 98 appears once. The earliest psychiatrist Nokjin differentiated symptoms to apply either therapies using acupuncture and drug or psychotherapy. There appears a case of rape, a case of burying alive with the dead, 8 cases of suicide that can characterize a prototype of forensic medicine. Deformity-related records include phrases as follow: 'there seems protrudent bone behind the head', 'a body which has two heads, two trunks, four arms.', 'a body equipped with two heads' In those times spa can be said to be used as a place for he리ing, convalescence, and relaxation seeing the records describing a person pretended illness and went to spa to enjoy with his friends. Priest doctors and millitary surgeons were in charge of the medical sevice in the period of the Three Kingdoms by the record written by Mookhoja(墨胡子) and Hoonkyeom(訓謙). Poor diet and regimen makes people more vulnerable to diseases. So there existed charity services for those poor people who couldn't live with one's own capacity such as single parents, orphans, the aged people no one to take care and those who are ill. The cause of affliction was frequently coined with human relation. There appeared the phenomenon of releasing prisoners and allowing people to become priests at the time of king's suffering. Besides, as a healing procedure, sutra-chanting was peformed. There appears 10 cases of death related records which varies from death by drowning, or by freezing, death from animals, death from war, death from wightloss and killing oneself at the moment of spouse's death and etc. There also exist certain records which suggest the knowledge of physiology and anatomy in those times. Since the taoist books such as ${\ulcorner}$Book of the Way and Its Power(老子道德經)${\lrcorner}$ were introduced in the period of Three Kingdoms, it can be considered that medicine was also influenced by taoism. Records of higher level of acupuncture, records which links the medicine and occult art of transformation existed. Although limited, we could figure out the medical state of ancient society.
