A Case Report of Lung Cancer in a Horse Trainer Caused by Exposure to Respirable Crystalline Silica: An Exposure Assessment

  • Here, we present a case of lung cancer in a 48-year-old male horse trainer. To the best of our knowledge, this is the first such case report to include an exposure assessment of respirable crystalline silica (RCS) as a quartz. The trainer had no family history of lung cancer. Although he had a 15 pack/year cigarette-smoking history, he had stopped smoking 12 years prior to his diagnosis. For the past 23 years, he had performed longeing, and trained 7-12 horses per day on longeing arena surfaces covered by recycled sands, the same surfaces used in race tracks. We investigated his workplace RCS exposure, and found it to be the likely cause of his lung cancer. The 8-hour time weight average range of RCS was 0.020 to $0.086mg/m^3$ in the longeing arena. Horse trainers are exposed to RCS from the sand in longeing arenas, and the exposure level is high enough to have epidemiological ramifications for the occupational risk of lung cancer.

An Analysis on Working Attitude of Workers in Integrated Social Welfare Facilities through Mediation Effects of Relationship Factors (관계요인의 매개효과를 통한 종합사회복지관 종사자의 직무태도에 관한 실증적 분석)

  • This study positively analysed the effects of service qualities of workers in integrated social welfare facilities on their working attitudes, targeting those who work for the welfare facilities. It identified the effects of service qualities such as tangibility, reliability, responsiveness, assurance, and sympathy on relationship factors such as relationship closeness through customization, organizational immersion and job satisfaction and presents suggestions to improve working attitudes of the workers. The results of the study are presented as follows: Reliability, responsiveness and assurance, service qualities of the workers, had effects on working attitudes. Relationship closeness through customization had an effect on organizational immersion and job satisfaction. Therefore, reliability, responsiveness and assurance that had effects on working attitudes of the workers were very significant factors and relationship closeness through customization were significant factors to improve working attitudes of the workers. This study presents political suggestions on the relations between service qualities of the workers in the welfare facilities and their working attitudes as alternatives.

Malondialdehyde and 3-Nitrotyrosine in Exhaled Breath Condensate in Retired Elderly Coal Miners with Chronic Obstructive Pulmonary Disease

  • Background: Chronic obstructive pulmonary disease (COPD) is an important cause of occupational mortality in miners exposed to coal mine dust. Although the inflammatory mediators involved in COPD have not been defined, many studies have shown that inflammatory mediators such as reactive oxygen and nitrogen species are involved in orchestrating the complex inflammatory process in COPD. Methods: To investigate the relevance of exhaled biomarkers of oxidative and nitrosative stress in participants with COPD, we determined the levels of hydrogen peroxide, malondialdehyde (MDA), and 3-nitrotyrosine (3-NT) in exhaled breath condensate (EBC) in 90 retired elderly coal miners (53 non-COPD and 37 COPD participants). Results: Mean levels of MDA (4.64 nMvs. 6.46 nM, p = 0.005) and 3-NT (3.51 nMvs. 5.50 nM, p = 0.039) in EBC were significantly higher in participants with COPD. The median level of MDA did show statistical difference among the COPD severities (p = 0.017), and the area under the receiver operating characteristic curve forMDA (0.67) for the diagnostic discrimination of COPD indicated the biomarker. The optimal cutoff values were 5.34 nM (64.9% sensitivity and 64.2% specificity) and 5.58 nM (62.2% sensitivity and 62.3% specificity) forMDA and 3-NT, respectively. The results suggest that high levels ofMDA and 3-NT in EBC are associated with COPD in retired elderly miners. Conclusion: These results showed that the elevated levels of EBC MDA and EBC 3-NT in individuals with COPD are biomarkers of oxidative or nitrosative stress.

The Effects of Housing Poverty on the Depression of the Elderly: The Mediating Effect of Social Service (노년기 주거빈곤이 우울에 미치는 영향: 사회서비스의 매개효과)

  • The study looked into the effect of housing poverty on the depression level for the elderly in depth. In this study, we defined housing poverty as sub-minimum standard housing conditions, excess housing expenditure and housing instability. In order to verify the correlation of two variables, a mediating model structured by social welfare service was used which gave out the 4th Korea welfare panel data. When it came to our research methods, structured equation analysis was applied to verify the mediating effect and theoretical background. The results revealed that housing poverty of the elderly directly affected their depression level. Also the satisfaction of social service showed a partial mediating effect between housing poverty and depression level. But the mediating effect of social service experience between housing poverty and depression level was not statistically significant. The outcome of this study indicated the practical and social intervention to promote a mental health of the elderly by improving residential environment.

A Study on the Classification of Blind Spots in Administrative Welfare Service for the Elderly (노인 행정복지서비스 사각지대의 유형 구분에 관한 연구)

  • In south korea, the elderly population has been continuously increasing, so blind spots in administrative welfare services for the elderly are expected to increase. therefore, this study aimed to analyze the types of blind spots in administrative welfare services for the elderly in gijang-gun, Busan, where the elderly population is increasing. using a network analysis, the demand and service supply ranges were derived. after that, in comparison to their sizes, the blind spots were divided into three types: i) blind spots in service supply, ii) blind spots in mobility constraint, and iii) dual blind spots. As a result, in gijang-gun, the blind spots in mobility constraint were 21.5%, the dual blind spots were 9.5%, and the blind spots in service supply were 3.2%. Based on these results, this study suggested policy directions for resolving the blind spots in administrative welfare services for the elderly by type.

A Study of Activities and Demand on Welfare Nursing in Korea (사회복지분야의 간호활동실태 및 간호수요에 관한 연구)

  • Social security is concerned with ensuring all citizens maintain basic needs, community health nursing maintains and promotes health for all community members. Lately, This new area of community health nursing, concern social welfare has increased. The objectives of this study are, first, to analysis the activities of nurses at community social welfare institutes, second, to estimate nursing demand for social welfare areas. The study methods used were as literature review, an analysis of statistical data and case study etc. The analytical framework also included a demand analysis of nursing manpower in community social welfare areas. The major results are as follows; 1. Employees which work at social welfare institutes number 55,464, nursing manpower (including nurse aids) number 1,458 and this is 3% of the total employees. Within nursing manpower, nurses number 780, nurse aids number 670. 2. The rates of nurses among total employees were high in institutes for mental disorders and institutes for the age. 3. The salary level of nurses was lower than average and the rates of retirement showed a roughly middle level in welfare institutes. 4. The satisfaction level of nursing services was high, and it is the trend that nurses substitute for nurse aids which retire at social welfare institutes. 5. Nurse demand that follows legal criteria is 2,221, but only 35% are working. It is therefore insufficient from the minimum of 733 to a maximum 1433. 6. The sufficiency rates by institution were high at institutes for vagrants, aged and the handicapped. In conclusion, the conditions are of nurses which are working in with the social welfare institutes are poor. Also the number of nurses compared rates of demand were in surplus. But, the basic direction of welfare policy is universal-preventive and provision of the family and of community centered service, and nursing service demand in the social welfare institute will increase continuously, we predict. Therefore, we will need a positive plan such as the development of an inservice education program and the construction of an information collection system etc.

The effects which work events were experienced by employees on the service quality in local healthy family support center: mediating effect of affective reaction and job satisfaction (건강가정지원센터 종사자가 경험하는 작업사건이 서비스 질에 미치는 영향: 정서반응과 직무만족의 매개효과)

  • The purposes of this study were to examine effects which work events and affective reaction were experienced by employees on job satisfaction and service quality in local healthy family support center for family health enhancement. For the purpose, survey were conducted with employees working in 56 local healthy family support centers in Seoul and Gyeonggi region, and final data were resulted from analysis of 319 examines of those employees. The results of this study were as follows. First, work events had a significant effect on the affective reaction. Second, affective reaction had a significant effect on the job satisfaction. Third, job satisfaction had a significant effect on the service quality. Fourth, affective reaction worked as mediators between the work events and job satisfaction, affective reaction and job satisfaction worked as double mediators between work events and service quality. Based on those findings, we can propose practical and political implications towards improving service quality considering the work experience of personnel in the healthy family support center.

A Study on the Utilization of the Volunteer Service in a Rural Community (지역 사회 자원 봉사 실태 연구-강원지역-)

  • The purpose of this study was to investigate the activities of woman volunteers. The sample consisted of 626 woman volunteers working in 18 districts in Kangwon province. The questionnaires consists of three parts: The first part of the questionnaire addresses demographic information. The second part of the questionnaire asks the feelings of subjects concerning volunteer activities (15 items). The third part of the questionnaire was to explore their perspectives of volunteer activities. An SAS was used for frequency. F-test, and a Pearson Correlation. The results of this study were as follows: 1. Most of the subjects were in their 40's(37.7%) and 10.7% were under age 20. About two thirds(69.8%) were housewives, the Buddhists were the highest religious groups (34.9%). Most of the subjects(36.7%) worked at Red-Cross. 2. Most of the subjects(56.8%) have nuclear families. 3. The subjects worked most frequently was at the Social Welfare Center, followed by homes, and welfare centers. 4. The most frequently used direct service was conversation(38.2%) followed by assistance with the eating process. 5. About thirty-nine percent of the subjects had less than one year of experience, and the majority of subjects had volunteer service once per week. 6. There was a significant relationship among age, number of family, duration of volunteer service, and experience. Integrated service will be needed to connect home health nursing with welfare service.

  • The purpose of this study was to explore the experiences and future challenges in providing of social service based on agro-healing. The study conducted was a general qualitative research which involved the collection and analysis of experiences shared by four representatives of participating in Gyeonggi-do community service investment pilot project. The data were gathered through individual in-depth interviews, and the findings were thoroughly analyzed to draw meaningful insights. The study revealed three main topics of discussion: "knowing the necessity of entering social services," "thresholds to be crossed to enter social services," and "future tasks". Based on the these results, implications for the development of community social services related to agro-healing were suggested, and the limitations were also discussed.

  • The importance of voucher service evaluation is increasing with the expansion of voucher, but the previous studies on performance evaluation could not reflect the characteristics of voucher. But there is no study on the factors affecting voucher performance. Thus, this study focuses on the empirical analysis of voucher program performance and analysed the factors affecting voucher satisfaction. Data used in this study is collected form 70 social service agencies that deliver voucher service and 1445 service users in Community-Development voucher service in Community Service Investment Program. The methods applied in this study are Hierarchial Linear Model(HLM) using service user variables(level 1) and service agency variables(level 2). These variables are extracted from the previous studies on performance of social welfare service and voucher service. The findings show that user's satisfaction is influenced by level 1 variables such as sex, need and also influenced by level 2 variables of service type and area* organization type. On these analysis, this study gives policy implication.
