지팡이 손잡이 형태가 편마비 환자의 균형 회복에 미치는 영향 (The Effects The Type of Canes-Handle Affects in Recovering-Balance of Hemiplegic patients)

  • Purpose: We compared T-type and I-type canes on postural balance in 28 hemiplegic patients. Methods: Subjects were allocated randomly into two groups: a T-shape cane group (n=14) and an I-shape cane group (n=14). Before the test, subjects were trained by a physical therapist to walk with a cane for 6 weeks. The Main Outcome Measures were measured as maximal sway velocity, sway path, sway area, and partial weight bearing using a Balance Performance Monitor (BPM) and ambulation velocity using a 'Timed up and go test'. We also measured the maximal ambulation velocity. Results: The distribution of weight bearing on the affected side without the cane was 35% in the I-shape cane group and 36% in the T-shape cane group. After training, weight bearing on the affected side increased by 45% in the I-shape cane group and 40% in the T-shape cane group. With the cane held in the hand, weight bearing on the affected side in the T-shape cane group decreased by 3%. Conclusion: The I-shaped cane increased static standing balance, including hemiplegic side weight bearing. Therefore, I-shape canes can improve the balance of hemiplegic patients.

무릎 체중부하 촬영 보조장치 개발 (Development of Knee Weight-bearing Radiography Assist Device)

  • The knee arthritis patients are generally performed standing position radiography. However, patients with pain caused by degenerative diseases or injuries in the knee have difficulty maintaining a standing position. Therefore, this study aims to develop a knee joint standing weight bearing projection assist device to solve these problems when patients undergo knee radiography due to various diseases. The design of the knee joint weight-bearing radiography assist device is carried out with 3D design and drawing production to secure basic data, electric support and frame manufacture. A fixed device for maintaining the patient's knee standing weight-bearing projection, an electric digital height device, a digital protractor, and a safety device were designed. The arm support is made of PVC with a cushion to relieve the impact and make it easier to remove debris. The digital electric device can be moved up and down according to the patient's height, and a remote control is attached. The safety device is made to be adjustable in size by attaching metal hooks on both sides of the frame and to shield the scrotum. A digital protractor was attached to the side frame to set the desired knee angle. When a self-made assist device was used to perform a knee joint standing weight bearing projection, it helped maintain arm support and lower extremities position. In addition, the height could be adjusted using an assist device during standing projections, which helped the patient maintain the position.

초고속 원심분리 회전축계의 최적설계 (An Optimum Design of a Rotor-Bearing Spindle System for a Ultra Centrifuge)

  • This paper presents an optimum design of a rotor-bearing spindle system for a ultra centrifuge (80,000 RPM) supported by ball bearings with nonlinear stiffness characteristics. To obtain the nonlinear bearing stiffnesses, a ball bearing is modeled in five degrees of freedom and is analyzed quasi-statically. The dynamic behaviors of the nonlinear rotor-bearing system are analyzed by using a transfer-matrix method iteratively. For optimization. we use the cost function that simultaneously minimizes the weight of a rotor and maximizes the separation margins to yield the critical speeds as far from the operating speed as possible. Augmented Lagrange Multiplier (ALM) method is employed for the nonlinear optimization problem. The result shows that the rotor-bearing spindle system is optimized to obtain 9.5% weight reduction and 21% separation margin.

비대칭적 입식자세에서 상지 조립 작업 시 체간 및 둔부 근육의 좌우 근활성도 비교 (Comparison Between Right and Left Muscle Activities of Hip and Trunk During Manual Task in Asymmetric Weight-Bearing Posture)

  • The purpose of this study was to compare the electromyographic(EMG) activities of trunk and hip muscles between right and left sides while subjects performed prolonged manual task in asymmetric and symmetric weight-bearing posture. Fifteen healthy male college students were recruited for this study. The subjects were asked to perform bimanual upper extremity task for 6 minutes in two different standing postures. In the symmetric weight-bearing posture, the subjects were standing with evenly distributed body weights to both legs. In the asymmetric weight-bearing posture, the subjects distributed about 90% of their body weight onto their preferred(supporting) leg and 10% of their body weight onto the opposite leg while they were standing. EMG activities of the right and left internal oblique, erector spinae, gluteus maximus, and gluteus medius were measured and normalized as % MVIC. Then the EMG data were statistically analyzed using paired t-tests. The EMG activities of all measured muscles were not significantly different between the right and left side in the symmetrical weight-bearing posture(p>0.05). However, the EMG of the supporting side internal oblique was significantly lower than the opposite side(p<0.05), and the EMG of the erector spinae, gluteus maximus, and gluteus medius were significantly greater on the supporting side(p<0.05). The results of this study support that unbalanced use of right and left muscle possibly causes the changes in muscle length which results in asymmetry of trunk and hip muscles. Furthermore, the uneven weight support onto right and left legs will cause a distortion of viscoelastic ligaments around hip and sacroiliac joints in the long run. Further studies to determine the effect of various manual tasks on the trunk and hip muscles as well as the effect of asymmetrical weight-bearing standing posture on hip and back muscle fatigue may be required.

전자체중계를 이용한 환측 체중부하훈련이 편마비 환자의 제중지지율과 보행에 미치는 효과 (Effects of continuous involved weight bearing training on symmetrical weight supporting rate improvement and gait characteristics of patients with Hemiplegia)

  • The purpose of this study was to investigate the effect of the involved lower limb weight bearing training on symmetrical weight supporting rate improvement and gait characteristics of patients with hemiplegia including their gait velocity, cadence, stride length, step length of the non affected side, step length of the affected side, foot angle of the non affected side, foot angle of the affected side, base of support, and so on. The subject of the study was 28 men and women patients with hemiplegia from 22 to 77 age, among patients who doctor diagnosed as hemiplegia due to stroke or traumatic brain injury, were possible to do independent gait more than 45m without others assistance, the flexion contracture of hip pint was less than $15^{\circ}$, did not have contracture for knee pint and one more than $5^{\circ}$ for ankle joint, did not have contraindication for exercise or gait did not show visual defect due to brain injury. The patients, the subject of the study, were classified into 14 patients of treatment. group applying continuous involved weight bearing with general therapeutic exercise and 14 patients of control group applying general therapeutic exercise, and then analyzed their gait before and after exercise. Temporal distance gait analysis(Boening, 1977) was used to analyze their gait, and ink foot-print was applied as one of measurement methods. However, it was statistically signifiant in the change rate(%) of gait characteristics, and treatment group's patients with hemiplegia had been highly changed in gait characteristics in comparison with control group. From the above-mentioned results, could find that continuous involved weight bearing training for patients with hemiplegia was effective to improve their gait ability and it could increase the ability in comparison with general exercise.

기능적 체중지지훈련이 뇌졸중 환자의 균형 및 보행에 미치는 영향 (The Effects of Functional Weight Bearing Exercise on Balance and Gait in Stroke)

  • Purpose : This study investigated to find the therapeutical effects of functional weight bearing exercise on the balance and gait in stroke. Methods : The subjects of this study were 13 hemiplegia was exercised using functional weight support exercise for 5 weeks, all of whom agreed to participate in the study. All subjects were measured to see their balance and gait with a Pro-3 balance system and Gait analysis. In order to assure the statistical significance of the results, we used for SPSS 12.0 for windows. Results : The results of this study were as follows : 1) There were statistically significant difference in medial-lateral stability and overall stability index. 2) There were statistically significant in distance and gait velocity index. Conclusion : According the results of this study, functional weight bearing exercise is effect on the balance and gait for hemiplegia.

  • Background: The purpose of this study was to investigate differences in balancing ability and weight bearing between paretic and non-paretic. Methods: Subjects of the study were consisted of 30 acute hemiplegia patients(paretic 15, non-paretic 15) who were receiving rehabilitation therapy in hospital. Weight shifting was quantitatively evaluated to determine the ability of patients to and during conscious weight shifting on the Gaitview. Independent t-test was utilized to detect the mean difference between the groups. Results: 15 patients with stroke were able to shift more weight onto the paretic leg. And pressure ratio of right hemiplegia was significantly difference on the nonparetic leg, pressure ratio of left hemiplegia was significantly difference onto the paretic leg. Conclusions: Patients with left hemiplegia who was cortical lesions in the right hemisphere were able to shift more body weight onto their paretic leg. These patients should be encouraged to practice shifting their weight towards their non-paretic leg to improve their balance.

되먹임과 보행보조도구의 형태가 30%체중지지의 정확성에 미치는 영향 (Effect of Feedback Methods and Ambulatory Assistive Aids on Accuracy of Partial Weight Bearing)

  • 이 연구는 건강한 성인을 대상으로 30%체중지지 수행력이 되먹임 형태와 보행보조도구에 따라 차이가 있는지를 알아보고, 이를 통해 효과적인 체중지지 훈련방법을 제안하기 위해 실시되었다. 20명의 건강한 성인이 참여하였으며, 겨드랑이 목발과 바퀴보행기를 이용하여 3점 보행을 하는 동안 오른쪽 발에 체중의 30%만 부하하도록 하였다. 훈련을 위해 동적 되먹임과 정적 되먹임을 제공하였다. 훈련 직후와 훈련 3일 후, 보행 동안 %체중지지를 측정하기 위해 힘판을 이용하였고 보행관련 변수를 측정하기 GAITRite를 사용하였다. 30%체중지지의 정확성은 겨드랑이 목발을 이용하고 동적 되먹임을 받았을 때가 가장 정확하였다(p>0.05). 보행보조도구와 되먹임 형태에 따라 %체중지지는 유의한 차이를 보였으며(p<0.05), 훈련 직후와 훈련 3일후의 %체중지지는 유의한 차이를 보이지 않았다(p>0.05). 또한 보행보조도구에 따라 보행속도와 입각기비율이 유의한 차이를 보였다(p<0.05). 따라서 효과적인 체중지지 훈련을 위해서는 가능한 경우 겨드랑이 목발을 이용하여 동적 되먹임을 통한 %체중지지를 교육하는 것이 필요할 것으로 생각된다.

The Relationship Between Asymmetrical Weight Bearing and Bone Mineral Density in Chronic Hemiplegic Limbs

  • Hemiplegia-induced immobilizatoin and reduction of mechanical loading in chronic stroke limbs are common cause of disuse osteoporosis. The purpose of this study was to investigate the effects of asymmetrical weight bearing on the loss of bone mineral in the individual with chronic stroke. Sixteen hemiplegic patients with strokes were evaluated. The measurements of bone mineral density (BMD) were evaluated with the quantitative ultrasound system on the calcaneus region of the paretic and non-paretic side. Plantar pressure was measured using the Mat-Scan system. The paretic side showed significantly smaller values in the T-score of BMD, and peak value of plantar pressure, which included forefoot, midfoot, and hindfoot, than the non-paretic side (p<.05). Results from the pearson correlation analysis showed statistically significant correlation between the BMD difference and the peak-pressure difference of midfoot pressure (p<.05). This finding indicated that BMD loss depended on decrease of body weight born on the paretic leg.

마비측에 적용한 외측 쐐기 깔창이 뇌졸증 환자의 체중부하율과 균형, 보행에 미치는 영향 (The Effects of Lateral Wedged Insole to the Shoe of the Affected Side on Weight Bearing, Balance and Gait with Stroke)

  • Purpose : The study was to evaluate the weight distribution, balance and gait function of stroke patients wearing lateral wedged insole to the shoe of the affected side. Methods : 27 patients with stroke (15 men, 12 women) participated in this study. Participants performed weight distribution, dynamic balance and gait ability with or without wedged insole on affected side in a random order. The balancia was used to evaluate the weight distribution. Deviation from the center line was analyzed by Dartfish during sit to stand to evaluate dynamic balance. The functional walk ability evaluated by 10 m walking velocity. Results : The asymmetry index of weight bearing improved significantly with wedged insole of affected side(p<.05). During sit to stand, center of gravity significantly moved from non-affected side to more mid line of body(p<.05). Improvement were shown in walking speed after wearing the wedged insole(p<.05). Conclusion : Wedged insole applied on affected side have a beneficial effect on weight distribution, dynamic balance and walking speed with stroke.
